Intra-articular pressure changes in rheumatoid and normal peripheral joints.

OBJECTIVE: To investigate the intraarticular pressure (IAP) dynamics of a spectrum of joints in rheumatoid and normal subjects in order to determine whether a reperfusion event is likely to occur at these sites. METHODS: IAP was measured in the metacarpophalangeal (MCP) (n = 8), wrist (n = 8), ankle (n = 4), and elbow joints (n = 4) of rheumatoid subjects, in addition to the MCP (n = 8), wrist (n = 6), and ankle joints (n = 1) of normal healthy controls, using the hand held portable 295-1 Intra-Compartmental Pressure Monitor System (Stryker, UK). RESULTS: Resting IAP was positive in all rheumatoid joints, and subatmospheric or weakly atmospheric in normal subjects (p < 0.01). Exercise produced an increase in IAP in rheumatoid subjects only (p < 0.01). The addition of saline to normal joints mimicked the IAP changes seen in the rheumatoid group. CONCLUSION: These observations suggest that increased resting IAP is a marker for chronic joint inflammation. The IAP increase seen in the rheumatoid group during exercise supports the concept of hypoxic reperfusion mediated joint injury.

Epidemiological studies of coccidiosis in the domesticated fowl (Gallus gallus): I. The fate of ingested oocysts of Eimeria tenella during the prepatent period in susceptible chicks.

A small proportion of sporulated and unsporulated coccidial oocysts (Eimeria tenella) inoculated per os into domesticated fowl were recovered intact within about one day, well within the usual 5-day ¿prepatent period.¿ Recovery rates of sporulated oocysts were ca. 2-9% from chicks inoculated at all ages tested from 2 to 28 days. From chicks within the same age range, the recovery rates for unsporulated oocysts were ca. 15-21%. The higher recovery rates of unsporulated oocysts indicated that they were more resistant to disruption in the gizzard than sporulated oocysts. Up to 83% of unsporulated oocysts that survived passage through the host's gut were able subsequently to sporulate. The first oocysts recovered, either sporulated or unsporulated, appeared at virtually the same time in all the chicks tested, despite the oldest birds having guts twice as long as the youngest. Moreover, the lengths of oocyst recovery periods in the oldest birds were 10-25% of those in the youngest.

Epidemiological studies of coccidiosis in the domesticated fowl (Gallus gallus): II. Physical condition and survival of Eimeria acervulina oocysts in poultry-house litter.

The survival of coccidial oocysts in poultry-house litter has been examined. The experimental design ensured that viable oocysts were not continuously replenished by multiplication in chicks. Hence, reliable oocyst survival data were obtained, which is not possible in a commercial house where there is a continuously changing population of coccidia. Litter was seeded directly from infected chicks with unsporulated oocysts of Eimeria acervulina. Whilst sporulation proceeded rapidly, oocysts simultaneously began to deteriorate after only 24 h. After five days, most oocysts were disintegrating. Viable oocysts were, however, detected (using sentinel birds) up to day 23, although subsequently the viability of the oocyst population began to wane. Few viable oocysts were detectable after 38 days, despite some intact sporulated oocysts being evident up to 54 days. The physical condition of oocysts is therefore not a reliable indication of their viability. The surface temperature of the litter was 25-28 degrees C throughout and the moisture content varied from 31.0 to 62.1%; these conditions were not limiting factors for oocyst sporulation.

Residents' breast examination performance. How often? How well?

The American Cancer Society recommends annual clinical breast examination (CBE) for women with average risk of breast cancer beginning at age 40. The purpose of the current chart review study was to determine the adequacy of CBE performed by house officers in a community teaching hospital. A chart audit of 92 women aged 50 or older who were consecutively admitted to the medical service was used as a baseline measure of CBE documentation. Two subsequent series of 100 women each were evaluated. The first series, which also involved women admitted to the medical service, followed increased emphasis on the importance of CBE during twice daily conferences with supervisor staff. The second series involved women admitted to the obstetrics/gynecology service. In that series, house staff used two history/physical examination forms: one with a slot dedicated to CBE and one without a dedicated slot. Results indicate that supervisors' stressing the importance of CBE resulted in only slight improvement in house officers' performance, even in the presence of risk factors for breast cancer. In the third chart audit, CBE was documented by 88% of house staff who used a form with a dedicated slot for CBE; there were no CBE documentations among staff who used a form without a dedicated slot. The dedicated form was more successful in increasing CBE performance and documentation than verbal emphasis about the importance of breast cancer screening.

Hostility in asymptomatic men with angiographically confirmed coronary artery disease.

The association of hostility and coronary artery disease was evaluated in a case-control study of aircrew members who had been referred for coronary angiography on the basis of noninvasive tests or risk factor status. The asymptomatic status of the sample and the structured nature of the referral process minimize the methodologic problems normally associated with studies of patients undergoing angiography. Cases (n = 24) had some angiographic evidence of coronary artery disease, whereas controls (n = 25) were found to have no evidence of occlusion. An interaction was observed between smoking history and a measurement of hostility based on observations of the respondent's behavior during a standard interview. Among nonsmokers, cases had higher hostility scores than did controls (p = 0.004). This association was not present among smokers. Self-reported hostility did not discriminate cases from controls. These findings support the notion that hostility plays a role in the pathogenesis of coronary atherosclerosis and point to the potential importance of interactions between hostility and other risk factors.

Effects of coronary angioplasty, coronary bypass surgery, and medical therapy on employment in patients with coronary artery disease. A prospective comparison study.

OBJECTIVE: To compare return-to-work rates after coronary angioplasty, coronary bypass surgery, and medical therapy in patients with coronary disease. DESIGN: Prospective cohort study. SETTING: Tertiary care referral center. PATIENTS: Between March 1986 and June 1990, we enrolled 1252 patients who were younger than 65 years, who had not had previous coronary revascularization, and who were employed. All patients were followed for 1 year. MAIN OUTCOME MEASURE: One-year employment status. RESULTS: After 1 year, 84% of patients who had coronary angioplasty were still working compared with 79% of patients who had bypass surgery and with 76% of patients who received medicine. After adjusting for the more favorable baseline characteristics of patients who had angioplasty (less severe coronary artery disease, better left ventricular function, and less functional impairment), however, no significant differences were noted in 1-year employment rates among the three groups. These adjusted 1-year return-to-work rates were 84% for angioplasty, 80% for surgery, and 79% for medicine (P > 0.05). In a random subset of 72 patients, 23 patients who had angioplasty returned to work after a median of 18 days (mean, 27 days) compared with 54 days (mean, 67 days) for 24 patients having bypass surgery and with 14 days (mean, 45 days) for 25 patients receiving medicine (P = 0.002). CONCLUSIONS: Patients who had coronary angioplasty were able to return to work earlier than those who had bypass surgery, but by 1 year no significant difference was noted in employment rates. Neither revascularization strategy improved employment rates when compared with initial treatment using medical therapy.

Safety of the attenuated anticoccidial vaccine 'Paracox' in broiler chickens isolated from extraneous coccidial infection.

A trial was carried out in 1-day-old broiler chicks raised for 54 days in floorpens under simulated commercial conditions, but isolated from all extraneous virulent coccidial infections, to demonstrate the safety of 'Paracox' attenuated vaccine administered at the recommended dose when chicks were 7 days old. The vaccine did not adversely affect the water consumption, faecal moisture or litter condition of the broilers. Recycling of the attenuated coccidia occurred in vaccinated birds, oocysts being present in the litter between 5 and 33 days after vaccination; a single peak of oocysts was detectable 5 or 12 days after vaccination. Unvaccinated medicated control birds did not produce oocysts, indicating freedom from both between-pen contamination by the vaccine and invasion of the chicken-house by extraneous coccidial infection. A small proportion (4%) of vaccinated birds had mild coccidial lesions when sampled at 26, 33 or 40 days after vaccination. Despite this, the vaccinated birds performed better than control (unvaccinated, nicarbazin-treated) birds and there was no post-vaccinal check in their weight gain. Their mean finishing weight was 10.4% greater, their food conversion ratio 7.2% lower and their mortality 44.4% lower than the unvaccinated control birds, reflecting the safety of 'Paracox' vaccine.

Personality measures as predictors of smoking initiation and cessation in the UNC Alumni Heart Study.

MMPI data collected from a sample of college men and women during 1964-1967 were used to predict smoking initiation and cessation over a 20-year follow-up period. People who subsequently began smoking were more rebellious, impulsive, sensation seeking, and hostile; were less likely to present a positive self-image; and were socially extraverted while in college. People who continued to smoke 20 years later were more hostile and sensation seeking. The personality variables that predicted smoking initiation and cessation were the same for men and women. Discussion centers on the potential role of hostility as a predictor of smoking cessation.

A short MMPI scale to identify people likely to begin smoking.

An item analysis of the Minnesota Multiphasic Personality Inventory (MMPI) was conducted to develop questionnaires that best discriminated (a) individuals who ever smoked versus those who never smoked and (b) ex-smokers from current smokers. The analyses were based on a sample of subjects who took the MMPI during 1964-1967 and reported their smoking status during 1987. Although no items distinguished individuals who quit versus continued to smoke, several items discriminated people who ever smoked from nonsmokers. These items formed two factors: Extraversion and Unconventionalism. These results provide support for the importance of extraversion, impulsivity, and antisocial tendencies as personality predictors of smoking initiation. The utility of this MMPI scale is discussed.

Welfare of competition horses.

In the large majority of cases and circumstances, horses benefit from their association with man. However, abuse of horses can occur, due to neglect or through the pressures of competition. The welfare of all animals, including competition horses, has become increasingly topical over the past ten years. Equestrian sport is coming under closer public scrutiny due to reports of apparent abuse. The bodies responsible for regulating these sports strenuously endeavour to protect the welfare of horses which compete under their rules and regulations. The Fédération Equestre Internationale (FEI: International Equestrian Federation) is the sole authority for all international events in dressage, show-jumping, three-day event, driving, endurance riding and vaulting. The FEI rules illustrate the ways in which the welfare of competing horses is safeguarded.

Comments on a process for identifying stages of dementia in residents of nursing facilities.

This paper gives a description of how items of the Global Deterioration Scale's Brief Cognitive Rating Scale and Functional Assessment Staging can be verified by reviewing specific sections and items of the Minimum Data Set for Nursing Facility Resident Assessment and Care Screening which is completed annually and updated every three months or when significant changes in health occur. A likely outcome of such comparison is improved understanding of the cognitive and functional status of residents with dementia and other medical conditions.

Cardiovascular and emotional responses in women: the role of hostility and harassment.

The relation of hostility and harassment to cardiovascular and emotional responses was examined by having 51 women (ages 18-26) high and low in hostility complete a task with or without harassment. Harassed high hostile Ss showed greater systolic blood pressure (SBP) increases during task and recovery periods than did harassed low hostile Ss and nonharassed Ss. Harassed low hostile Ss evidenced greater SBP increases during task and recovery periods than did nonharassed Ss. Among high hostile women, cardiovascular elevations during the task were associated with self-reported levels of negative affect. Antagonistic hostility, relative to neurotic hostility, was positively associated with harassment-induced SBP changes. These results support the hypothesis that hostile people exhibit excessive behaviorally induced cardiovascular responses to interpersonally challenging tasks that evoke anger-related emotional states.

Ceftriaxone-induced acute pancreatitis.

OBJECTIVE: To report a case probable ceftriaxone-induced acute pancreatitis. CASE SUMMARY: A patient with a history of short-bowel syndrome on home total parenteral nutrition developed fever, chills, and right flank pain. She was diagnosed with gram-negative catheter sepsis and prescribed antibiotic therapy to be administered for four weeks. After completion of the first week of therapy, the antibiotic regimen was changed to intravenous injections of ceftriaxone to be given daily at home. Prior to discharge the patient developed acute abdominal pain, leukocytosis, jaundice, and markedly elevated lipase and amylase concentrations consistent with acute pancreatitis. The patient's condition improved upon discontinuation of the ceftriaxone and the remainder of her stay was uneventful. DISCUSSION: There is only one other case report in the literature of probable ceftriaxone-induced pancreatitis. Multiple other medications have been implicated in causing acute pancreatitis. The exact mechanism of this uncommon adverse effect of ceftriaxone is unknown. CONCLUSIONS: There was a temporal relationship between the development of this patient's signs and symptoms and the administration of ceftriaxone. We could not identify any other factors that may have been responsible for the development of her acute pancreatitis. Ceftriaxone should be considered as a possible etiologic agent in patients who present with acute abdominal pain and elevated lipase and amylase concentrations.

Case study training for seeing school through adolescents' eyes.

The first of two years of training in case study activities, adapted from the program originally developed by Fritz Redl, Daniel A. Prescott, and others at the Universities of Chicago and Maryland in the 1940s, is described. Excerpts from the case study of a 16-year-old female provide examples of activities, such as: (1) identifying recurring patterns of behavior, (2) selecting a pattern of behavior for analysis by the multiple hypotheses method, and (3) summarizing the case study record. The significance of this process is that high school personnel can learn to see what adolescents are up against in and out of school.

Hostility during late adolescence predicts coronary risk factors at mid-life.

Hostility, as measured by the Cook-Medley Hostility Scale of the Minnesota Multiphasic Personality Inventory, has been found to predict higher rates of both coronary heart disease and all-cause mortality. To evaluate one mechanism whereby hostility might contribute to health problems, the authors used regression models to determine whether hostility measured in college (1964-1966) predicted coronary risk factors assessed 21-23 years later (1987-1990) in 4,710 men and women. Of this group, 828 had lipids measured (1988-1991). Persons with higher hostility scores in college were significantly more likely at follow-up to consume more caffeine (r = 0.043), to have a larger body mass index (r = 0.055), to have higher lipid ratios (r = 0.092), and to be current smokers (r = 0.069) than those with lower hostility scores during college. Cross-sectional analyses found significant associations of contemporaneous hostility scores with the same four risk factors, as well as with alcohol consumption and hypertension (rs ranging from 0.043 to 0.117). These associations are large enough to have possible public health significance. We conclude that hostility may contribute to health problems through its influences on several coronary risk factors across the adult life span.

Differences between the anticoccidial potencies of monensin in maize-based or wheat-based chicken diets.

Experiments were carried out to investigate why the anticoccidial ionophore monensin is more potent against the coccidium Eimeria tenella in chickens fed on a maize-based diet (as in the USA) than in chickens fed on a wheat-based diet (as in the UK). The explanation seems to be that the pathogenicity of E. tenella is lower in maize-fed chickens than in wheat-fed chickens, whether monensin is present in the diet or not. Possible reasons for this are suggested. The better survival of maize-fed birds may be partly due to protective effects of the higher concentrations of vitamins A and E derived from their diet. Furthermore, the higher concentrations of niacin and riboflavin in wheat than in maize may enhance coccidial pathogenicity. These opposing factors might combine to cause equivalent infection levels to produce more severe coccidiosis in chickens fed on a wheat-based diet than on a maize-based diet.

Triaxial force transducer for investigating stresses at the stump/socket interface.

Although normal pressures at the stump socket interface of the lower-limb amputee have been investigated, little is known about the shear stresses that also occur. Studies suggest that the combination of both shear and normal stresses significantly exacerbates discomfort and vascular and tissue damage. A means of simultaneously measuring normal and shear stresses will aid in the investigation and improvement of prosthetic fit. A miniature triaxial force transducer (4.9 x 16 mm diameter) has been developed which can be recessed into the socket wall. The principle of operation, construction, performance and limitations of the device are described. Preliminary measurements of the interface stress variations over the gait cycle in a supra-condylar PTB socket are presented. These show clear differences in the stress patterns present when two different prosthetic feet are used.
