A third-year family medicine clerkship based in an academic family practice center.

In this paper, the authors describe the planning, curriculum, and evaluation of a five-week family medicine clinical clerkship for third-year students based in an academic family practice center. The program is an outpatient experience utilizing several innovative techniques: (a) problem-based learning that focuses on patient management tutorials, (b) consultation sessions with individuals offering specialized expertise, (c) supervised patient care in a family practice center and with a nursing home inpatient teaching service, and (d) workshops on various topics, such as office-surgical techniques, practice management, and alternative methods of health care. Evaluation of the course has demonstrated the merits of careful planning with student input, the need for flexibility, and the value of focusing on concepts in ambulatory care.

On the molecular basis of T helper cell function. II. B-lymphocyte promotor factors: I-A-restricted production and their apparent antigen-independent, direct interaction with B cells.

The differentiation of Ig+ B cells into plaque-forming cells is dependent on antigen and factors produced by T cells and/or macrophages. We describe here the production of T-cell factors termed lymphocyte promotor factors (LPF). A foetal calf serum-specific T-cell line and its clones synthesize LPF, which is defined as factors that polyclonally stimulate normal spenic T cells to differentiate into cytotoxic T lymphocytes (T-LPF) and normal splenic B cells to differentiate into plaque-forming cells into (PFC) (B-LPF) in the apparent absence of specific antigen. The proliferation of and the B-LPF production of all T-cell clones tested were foetal calf serum-specific and I-Ab-restricted. Some of these clones produced only T-LPF, some clones produced only B-LPF, and some clones produced both T-LPF and B-LPF. B-LPF stimulate the polyclonal differentiation of Ig+ B cells into PFC without the apparent need for helper T cells, is different from T-LPF, and induces almost exclusively IgM PFC. The B-LPF described in the present paper are compared with previously described T-cell factors, which stimulate antigen-specific B-cell responses or bystander B-cell responses. The conclusion is that B-LPF are probably different from B-cell growth factors, T-cell replacing factors, allogeneic effector factors, and interleukin 2.

Assay procedures for the determination of biogenic amines and their metabolites in rat hypothalamus using ion-pairing reversed-phase high-performance liquid chromatography.

Procedures are described for the simultaneous determination, by high-performance liquid chromatography of adrenaline, noradrenaline, dopamine and 5-hydroxytryptamine using laurylsulphate as pairing ion and for the separate simultaneous determination of vanillylmandelic acid, dihydroxyphenylacetic acid, 3-methoxy-4-hydroxyphenylethylene glycol, 5-hydroxyindoleacetic acid and homovanillic acid using tetraethylammonium as pairing ion. Sample preparation consists of protein precipitation only and octadecylsilane coated silica is used for both sets of determinations as is electrochemical detection. The chromatographic basis of each separation is discussed in the light of modern ideas of ion exchange and desolvation to enable the method to be modified chromatographically if other compounds are to be resolved or the assay method modified. The quantitative aspects of the methods are detailed and applied to amine and metabolite levels in rat hypothalamus. The values determined together with their sample variation and estimated limits of detection are quoted.

Comparison of the immunogenic effects of covalent-bonded immune complexes in mice and sheep.

Covalent antigen-antibody complexes containing the protein antigen ovo-transferrin primed both mice and sheep to give an enhanced antibody response to a subsequent single injection of soluble ovo-transferrin. Complexes prepared using horse, sheep or rabbit antibody had a priming effect in mice, although rabbit antibody-antigen complexes were the most effective. In sheep, only rabbit antibody-antigen complexes significantly enhanced antibody levels.

The analysis of tricyclic antidepressant drugs at therapeutic blood levels by reversed-phase high-performance liquid chromatography using pairing and organic counter-ions.

A serum assay method incorporating two internal standards suitable for routine clinical analysis of tricyclic antidepressant drugs at the therapeutic level is described. The method involves extraction and chromatography using a reversed-phase high-performance liquid chromatography system incorporating pairing and organic counter-ions. The quantitative characteristics of the method are reported and results are compared with consensus values for four commonly encountered drugs, amitriptyline, nortriptyline, imipramine and desipramine. The method is shown to be applicable to more recently developed antidepressants such as nomifensine and maprotiline and the flexibility of the chromatography is discussed. Sample chromatograms obtained from patient samples in the presence and absence of potentially interfering drugs and metabolites are discussed.

A chromatographic quality control procedure based on HPLC for 5-hydroxymethylfurfural in autoclaved D-glucose infusion fluids.

A specific qualitative chromatographic assay method is described for 5-hydroxymethylfurfuraldehyde (5HMF) in degraded D-glucose solutions incorporating an internal standard. Comparison is made between the chromatographic method and the official direct ultraviolet absorption measurements at 284 nm for solutions of D-glucose degraded to different extents. Hospital samples of autoclaved Dextrose Injection, 5% w/v, are also examined by both methods and it is demonstrated that quite different results can be obtained depending upon the conditions of measurement. It is demonstrated that the direct absorption method does not provide an adequate limit test for compounds related to 5HMF.

Immunoregulatory effects of covalent antigen-antibody complexes. IV. Priming and tolerance in T-dependent responses.

Stable, covalently bonded, monomeric complexes of rabbit anti-NAP (4-azido-2-nitrophenyl) antibodies and NAP-bovine pancreatic ribonuclease (RNase), when injected into mice, prime the subsequent response to a soluble challenge of RNase. This effect is shown to be dependent on an intact Fc portion of the rabbit antibody and not simply due to foreign determinants recognized on the latter. A study of the kinetics of elimination of radioiodinated complexes from the serum indicates that the generation of a primary anti-rabbit IgG response and subsequent clearance of the complex leads to priming of the anti-RNase response. If mice are previously rendered tolerant to rabbit IgG or the complexes are ultracentrifuged, the priming to RNase is often abolished and tolerance may be induced.
