Effects of changes in articulator settings on generated occlusal tracings. Part I: Condylar inclination and progressive side shift settings.

An occlusal scribing device attached to a fully adjustable articulator was used to generate occlusal tracings. The tracings were analyzed to determine the effect changes in progressive side shift and condylar inclination settings had on molar occlusal morphology. The progressive side shift adjustment affected the cusp height and ridge and groove position, primarily in the nonworking excursion. The condylar inclination adjustment affected the cusp heights in the protrusive and nonworking excursion without affecting the working excursion.

Effects of changes in articulator settings on generated occlusal tracings. Part II: Immediate side shift, intercondylar distance, and rear and top wall settings.

An occlusal scribing device attached to a fully adjustable articulator was used to generate occlusal tracings. The tracings were analyzed to determine the effect changes in immediate side shift, intercondylar distance, and rear and top wall settings had on molar occlusal morphology. When ranked from greatest to least effect, a 0.2 mm change in the immediate side shift setting had the greatest effect on the occlusion, whereas a 5-degree change in the rear and top wall settings had the least effect.

Peel bond strengths of five impression material tray adhesives.

The peel bond strengths of five impression materials and their tray adhesives to a perforated and non-perforated test surface were measured and the results were compared using a one-way ANOVA and Student Newman Keuls Multiple Range Test at p less than or equal to .05 level of significance. The auto-mix addition reaction silicone material Extrude produced the greatest peel bond strengths on both the non-perforated test surface (10.88 kg/cm2), and on the perforated test surface (15.88 kg/cm2). These bond strengths were greater than those obtained from the polysulphide and polyether materials used in the study. Using the perforated test surface significantly increased the peel bond strength for all the materials tested. It was concluded that in order to achieve the maximum adhesive bond, impression trays should be perforated. The greatest peel bond strengths were obtained using Extrude medium-body impression material.

Potential errors when using a computerized pantograph.

This study determined the effect on articulator settings of using an arbitrary hinge axis as compared with the terminal hinge axis in orienting a computerized pantograph on a patient. The effect of an error in measuring the reference plane angle or clutch angle was also determined. Two subjects were pantographed 30 times by using the terminal hinge axis and 30 times by using the arbitrary hinge axis. Statistically the articulator settings obtained by using the arbitrary hinge axis and the terminal hinge axis were different, but the difference was small. One subject was pantographed on 18 occasions by using different reference plane angle and clutch angle settings. Analysis showed that 1-degree change in the reference plane angle setting produced approximately 0.9-degree change in the protrusive and orbiting path articulator settings. A 1-degree change in the clutch angle setting produced approximately 0.9-degree change in the progressive side shift settings. It was concluded that the terminal hinge axis should be used as the posterior reference point and the reference plane angle and clutch angle should be carefully recorded.

Resin-bonded bridges for the pediatric patient.

The resin-bonded bridge is a relatively new clinical technique that has had favorable clinical reports. Recently, new materials and techniques have been developed which may improve the retention of resin-bonded bridges. This article reviews recent developments concerning resin-bonded bridges, present a case report, and give practical suggestions regarding the fabrication of resin-bonded bridges.

Evaluation of a digitizer and computer system designed to analyze articulator-generated occlusal tracings.

Occlusal tracings generated on a Denar fully adjustable articulator were photographed, projected, digitized, and stored in a computer data file. These steps were accomplished with sufficient accuracy for clinical analysis. This study demonstrated that appropriately referenced tracing data can be transferred accurately into a machine-readable form. Studies of pantographic tracings for mandibular movements are facilitated by this convenient and accurate method of handling data.

A comparison of articulator settings obtained by using an electronic pantograph and lateral interocclusal recordings.

Articulator settings were obtained for two subjects 10 times in a 2-week period by using both a lateral interocclusal record technique and a Pantronic pantograph. The ORB and PSS articulator settings obtained from the two techniques were compared and the following conclusions drawn. 1. Over a 2-week period, the variance in the Pantronic pantograph ORB and PSS articulator settings was less than the variance in the setting obtained by using lateral interocclusal records. For the Pantronic pantograph technique, 95% of any ORB settings were within 2.5 degrees of the Pantronic pantograph mean and 95% of any PSS settings were within 1.9 degrees of the Pantronic pantograph mean. For the interocclusal record technique, 95% of the ORB settings made with interocclusal records were within 11.9 degrees of the mean for one subject and 13.4 degrees of the mean interocclusal records setting for the other subject. For the PSS settings, 95% of the interocclusal record settings were within 10.1 degrees of the mean setting for one subject and 11.6 degrees of the mean interoccusal record setting for the other subject. 2. In adjusting the ORB and PSS articulator settings, the maximum potential occlusal errors that may be produced by using the Pantronic pantograph are smaller than the maximum errors that may be produced in using the lateral interocclusal record technique.

Percutaneous angioplasty in clinical management of renovascular hypertension: initial and long-term results.

Between January 1980 and July 1983, percutaneous transluminal angioplasty was attempted on 137 stenotic renal arteries in 100 patients. At termination of follow-up studies (3-39 months, mean of 16 months), 70% of those treated for hypertension had benefited from the procedure. Stenosis secondary to fibromuscular dysplasia responded better than stenosis from arteriosclerosis (85% and 65% of the patients, respectively). Benefit was minimal for those with stenosis of the renal artery ostium or renal insufficiency. Determining levels of renal vein renin before angioplasty is helpful in selecting patients; following angioplasty, this has considerable significance in predicting the success of the procedure.

The computed tomography-guided adrenal biopsy. An alternative to surgery in adrenal mass diagnosis.

A series of 16 patients with adrenal masses were biopsied percutaneously under computed tomography (CT) guidance with 18- to 22-gauge modified Chiba needles. Adrenal adenomas, cysts, metastases, melanoma, and adrenal hemorrhage were identified. Of nine oncologic patients, four had adrenal metastases, while five had other nonmalignant adrenal masses. Thus, an adrenal mass in an oncologic patient is not always metastases. No complications occurred. The diagnostic evaluation of an adrenal mass in selected cases should include CT-guided percutaneous aspiration as a safe and reliable alternative to open surgical biopsy. CT-guided biopsy can be performed as an outpatient procedure, avoiding the cost of hospitalization and the morbidity of surgery.

Percutaneous drainage of multiseptated hepatic abscess.

Thirteen patients with multiseptated, hepatic abscesses were initially treated by percutaneous drainage using computed tomographic guidance. One of these 13 patients required surgery because of failure of percutaneous drainage due to the viscosity of the debris within the multiseptated cavities. Another patient required a second percutaneous drainage procedure due to recurrent hepatic fluid collection. Eleven of the 12 successful drainages were accomplished with one catheter because there was communication between the multiseptated "pockets." Our approach to hepatic abscess drainage is discussed with specific attention to the multiseptated abscess. The multiseptated abscess should not discourage an attempt at percutaneous catheter drainage.

Varicoceles: a coaxial coil occlusion system.

A modified transfemoral technique for testicular venography and occlusion of the testicular vein (TV) was used to treat 30 patients with a total of 43 varicoceles (17 left-sided and 13 bilateral). Occlusion was satisfactory in all left varicoceles and 90% of right varicoceles. This coaxial technique facilitates subselective catheterization of the TV and its collaterals and enables coils to be placed deep within these vessels, either alone or in combination with detachable balloons.

Biopsy of the right adrenal gland by the transhepatic approach.

A transhepatic computed-tomographic-guided biopsy of a right adrenal mass is described. This method is simpler to perform than the usual posterior biopsy carried out with the patient prone and is less likely to cause a complicating pneumothorax. In seven of eight patients with right adrenal masses, adrenal tissue was obtained and an accurate diagnosis was possible. No complications resulted.
