The way the surgical orthodontic team approach has been developed over the years is described in its historical context. Originally orthodontists and surgeons worked more or less separately from each other. Nowadays a team of specialists including psychologists takes care of the patient. The importance to balance the indications for surgical orthodontic treatment against the contra-indications is emphasized.
The results of a retrospective study of the complications that occurred in 410 Le Fort I osteotomies performed in the Department of Oral and Maxillofacial Surgery of the Free University Hospital, Amsterdam, the Netherlands, are presented. The complication rate was 9.0%. In the discussion, our results are compared with the literature.--Suggestions are made on how to avoid these complications.

Following surgical procedures in 25 patients, a study was made of the stability of the maxilla after superior repositioning. In cases of open bite, as well as in cases of absolute maxillary hyperplasia or relative mandibular deficiency, a slight overcorrection that led to a stable occlusion was observed. The use of miniplates for stabilization of the maxilla is discussed briefly.
A review of the literature on reverse smoking is presented, followed by a report of a case of reverse smoking in a 59-year-old woman, living in the Netherlands. This is the first report of a case of reverse smoking in the Netherlands so far. The clinical and histological findings are in accordance with those found in the literature. The patient refused to give up her habit. During the 4 years follow-up period, no clinical changes were noticed.

A case of a benign osteoblastoma or osteoid osteoma in the maxilla of a 20-year-old female, treated by a conservative surgical approach, has been reported. It has been suggested, as has been done by others in the past, to consider the benign osteoblastoma and the osteoid osteoma of the jaw bones as one entity of osteoblastic derivation rather than using arbitrary distinguishing factors such as size. Apart from the relation between osteoblastoma and osteoid osteoma, the relation between osteoblastoma and cementoblastoma is discussed, the most important distinguishing factor apparently being the true attachment to the root cementum of a tooth. In the list of differential diagnoses, the cementifying or ossifying fibroma should also be taken into account.
