Many different activities have been defined for the effective use of clinical audit. Proposes a method of linking the practice and outcome of audit with the Trust business people, setting out criteria methodology and background following a two-year research study.
Four years of war in Bosnia and Herzegovina have severely damaged the health care system and destroyed basic infrastructures. Yet in spite of these conditions and the reappearance of several "dormant" diseases, no major fatal disease outbreaks have occurred. Morbidity and mortality due to infectious diseases have remained surprisingly low. This article explores some of the reasons why this potentially enormous public health disaster was avoided. Public health interventions can generally be considered the outcome of two components: knowledge and action. War situations inhibit thorough or ideal data collection and therefore the balance between knowledge and action must be tipped towards the latter. In Bosnia and Herzegovina, the WHO/national health monitoring system maintained a sufficient level of surveillance so as to provide early detection of significant threats to health. As the lead health agency in the conflict, WHO applied several health monitoring strategies. Major fatal epidemics were avoided because health workers have become increasingly scientific in predicting epidemics in war situations and formulating the steps necessary to limit them. Some of the preventive interventions applied in Bosnia and Herzegovina to reduce the impact of infectious diseases are discussed.

Improving the nation's health is a major challenge. It has a long history within the specialty of public health medicine, with is commitment to knowledge and actions. But achieving a balance between these activities has been a source of considerable tension within the specialty. The commissioning function, with its related dimensions of purchasing and contracting, provides the best chance for decades of aligning public health knowledge with the action that can produce health improvement. The concept of public health management seeks to bridge knowledge and action, drawing on a range of skills. But there is a long way to go before the necessary skills and capacities are demonstrated in practice. The organizational and personal development agenda remains very large. Yet, what is occurring in the NHS has not been achieved anywhere in the world. Describes one practical approach to strategic health development and commissioning. But it will require sustained commitment if it is to succeed.

The first 1000 cases notified to the National Childhood Encephalopathy Study were analysed. The diagnoses included encephalitis/encephalopathy, prolonged convulsions, infantile spasms, and Reye's syndrome. Eighty-eight of the children had had a recent infectious disease, including 19 with pertussis. Only 35 of the notified children (3.5%) had received pertussis antigen within seven days before becoming ill. Of 1955 control children matched for age, sex, and area of residence, 34 (1.7%) had been immunised with pertussis vaccine within the seven days before the date on which they became of the same age as the corresponding notified child. The relative risk of a notified child having had pertussis immunisation within that time interval was 2.4 (p less than 0.001). Of the 35 notified children, 32 had no previous neurological abnormality. A year later two had died, nine had developmental retardation, and 21 were normal. A significance association was shown between serious neurological illness and pertussis vaccine, though cases were few and most children recovered completely.
