Heterogeneity of extractable gut glucagon-like immunoreactivity (GLI) and its lipolytic activity.

Gut glucagon-like immunoreactivity (GLI) was extracted from the mucosa of the canine intestine and was separated into two peaks by gel filtration; Peak I (7,000 daltons) and Peak II (3,500 daltons). These two peaks were purified by means of affinity chromatography using an anti-glucagon antibody as a ligand substance. These purified gut GLIs were not composed of single component, when applied to ion exchange chromatography. These partially purified Peak I and Peak II showed lipolytic activities, though very weak in potency, in vitro on the rat fat tissue at pharmacologically high concentration (5 ng eq/ml in final). It was concluded that gut GLI, even though purified, showed heterogeneity and that gut GLI revealed lipolytic activity although weak in potency compared with pancreatic glucagon.

Effect of exercise on alimentary lipemia in healthy men.

To investigate the effect of exercise on alimentary lipemia, 6 healthy volunteers were exmained for their plasma parameters successively for 6 hr after ingestion of emulsified butter (1 g/kg body weight) with and without subsequent intermittent loading of exercise (720 kilopound meters per min for 2 min, 6 times at hourly intervals) using a bicycle ergometer. No apparent differences were found between the fat loading test and the fat and exercise loading test in the 6 hr levels of plasma chylimicron-triglyceride, lipoprotein-triglyceride and endogenous lipolytic activity measured by the usual lipoprotein lipase assay method, although the increase in plasma free fatty acids in the fat and exercise loading test was significantly greater than that in the single fat loading test. It is concluded that the intermittent muscle exercise adopted in this study has no significant inffuence on the triglyceride elevation after the fat loading in man.

The thyroid reserve in patients with breast cancer.

Patients with breast cancer were studied with regard to changes in the levels of serum thyroxine (T4) and serum triidothyronine (T3) after intramuscular administration of TSH for three days to investigate the thyroid reserve in these patients. The changes were examined also in normal subjects, in patients with Hashimoto's thyroiditis and primary hypothyroidism. In primary hypothyroidism no increase was seen in serum T4 and T3 levels after TSH administration, and the thyroid reserve was considered to be absent. In Hashimoto's thyroiditis the serum T3 levels after TSH administration were the same as in normal subjects, but the serum T4 levels were significantly lower than normal. This suggests that in patients with Hashimoto's thyroiditis the thyroid reserve is decreased and is in a state of so-called low thyroid reserve. In patients with breast cancer, however, the levels of serum T4 and T3 showed no difference as compared with those in normal subjects after TSH administration, and this implies that the thyroid reserve is well maintained.

Studies on copolymers of collagen and a synthetic polymer. First report--experimental study on biocompatibility of laminar copolymers of collagen and a synthetic polymer.

To develop biomaterials for semiparmanent substitution, we are studying copolymers of collagen and a synthetic polymer. In this study, in order to test tissue compatibility, the laminar copolymers were prepared from bovine collagen treated with proctase to remove telopeptides and synthetic polymers such as polyethylene, polyvinylalcohol, silicon gum etc., by the OKAMURA-HINO metthod, namely polymerization achieved by the introduction of cross-lincages, applying plasma discharge and gamma-irradiation. These copolymers were implanted into dorsal subcutaneous tissue in rabbits and removed after 1, 2, 3, 4 and 6 weeks to be examined by light microscope and electron microscope. One or two weeks after implantation, we observed that a large number of fibroblasts gathered on the surface of the grafted collagen. After 3 or 4 weeks, bridges of collagen fibrils were observed between the copolymers and rabbit tissue. These copolymers were firmly adhered to the tissues and separable only with difficulty. These results showed that the copolymers maintain the capacity for extreme strong bonding for at least 6 weeks and possess high tissue compatibility. From that we concluded copolymers are useful as a biomedical material.

Dietary intake and hyperlipidemia in controlled diabetic outpatients.

Statistical analyses were performed in order to determine the effects of the control of the diabetes as well as the food intake on serum lipid levels of 73 diabetic outpatients. They had had elevated fasting blood glucose levels, mostly complicated by various grades of hyperlipidemia, before the initiation of treatment. Hyperlipidemia was found to be ameliorated in nearly half of those patients after the start of diabetic treatment. However, the elevation of serum triglyceride persisted in 30 per cent of controlled diabetics, and no differences were found in occurrence of hypertriglyceridemia between diet-treatment group, sulfonylurea group, biguanide group, combined group and insulin group. Estimation of diet intake revealed that the controlled hypertriglyceridemic patients consumed slightly (but significantly) greater amounts of sucrose, alcohol, and total calories than the controlled normotriglyceridemic patients. In addition to such inadequate diet consumptions, the tendency to be overweight and the subtle increment of fasting blood glucose levels were also shown to have contributed to hypertriglyceridemia. It is thus concluded that the lipid disorder in controlled diabetic outpatients is the result of multifactorial influences and that well-conducted diet therapy and stricter regulation of blood glucose are essential in the management of posttreatment hyperlipidemia.

A study on the pathogenesis of postmastectomy lymphedema.

In order to study the causes of postmastectomy lymphedema, venography, lymphangiography and RISA absorption test were performed on the patients following radical mastectomy. Venous obstruction was a rare cause of postmastectomy lymphedema. Edema patterns in arm lymphangiography were observed in the patients with lymphedema and even in some of the non-edematous upper extremities. The finding of axillary lymphangiography was correlated with the degree of edema of the upper extremity. The absence of adequate lymphatic pathway across the axilla (type 2, collateral and type 3, blocking) was thought to provide the pathologic base for the postmastectomy lymphedema. RISA absorption test also provided precise information of the lymphatic flow in the extremity. It was lower in the patients with edema, as compared with the patients without edema.

Significant prognostic factors in breast cancer of Japanese women: special reference to nuclear grade and sinus histiocytosis.

Based on clinicopathological observations of 91 female patients with breast cancer in Miyagi Prefecture, TNM stage, histologic type, grade of infiltrative growth tendency (INV), vessel involvement, lymph node metastasis, nuclear grade and sinus histiocytosis were assessed in relation to prognosis. Follow-up studies indicated that 73.6% of the patients survived more than five years after operation. Among the factors submitted to histologic evaluation, nuclear grade, INF, vessel involvement and lymph node metastasis were apparently correlated to prognosis, while sinus histiocytosis was not significantly related to it, except for patients with positive nodes.

Biological characteristics of breast cancer in obesity.

Biological characteristics of breast cancer in obese patients were investigated in comparison with non-obese patients. 134 patients with breast cancer were divided into three categories; thin, normal and obese based upon the height and weight. Five-year survival rate was 55.6% in the obese patients, whereas it was 79.9% in the non-obese patients including thin and normal subjects. A cause for unfavorable prognosis in the obese patients with breast cancer was clinically and histologically investigated. It was found that in the obese patients primary tumor, regional lymph node engorgement and clinical stage were relatively advanced, scirrhous carcinoma was predominant ( 46%), tumor margin was more infiltrattive (gamma type 64%), the incidence of vascular involvement was high (73%), lymph node metastasis was also frequent (58%) and nuclear grade was low.

Evidence for a familial form of hypertriglyceridemia as disorders coupled with insulin resistance.

To evaluate the role of insulin in familial hypertriglyceridemia, 34 relatives of the pedigrees of 3 index cases of endogenous hypertriglyceridemia and hepatic steatosis as well as 9 spouses were examined for plasma lipids and responses of blood glucose and plasma insulin during oral glucose tolerance tests. The combined disorders of hypertriglyceridemia and hyperinsulinemia plus glucose intolerance--insulin resistance--were most commonly found among the relatives, which were often accompanied by an impaired liver function. Some relatives showed hyperinsulinemia without hypertriglyceridemia. Obesity was frequent, but its incidence was similar to the controls. Thus, the observed form of familial hypertriglyceridemia was apparently coupled with insulin resistance; and hyperinsulinemia, or insulin resistance by itself, might be a basic genetical trait in this form of lipid disorder.
