[Enhanced effect of hepatic arterial infused anticancer drugs on rabbit VX-2 carcinoma by means of combination with Lipiodol. Adriamycin versus Lipiodol adriamycin sandwich method and adriamycin suspended in a Lipiodol].

To increase drug delivery to a hepatic malignant tumor, various tissue contents and serum levels of adriamycin (ADR) were evaluated in comparison with different intraarterial injection methods of oily or aqueous solution. VX-2 tumor was implanted in the rabbit liver. ADR was infused through the hepatic artery by the following three methods: Group 1: ADR with physiological saline; Group 2: ADR administered between lipiodol injection; and Group 3: urographin-dissolved ADR suspended in lipiodol. ADR concentrations of the tumor, liver, heart, kidney or bone marrow were compared between these three groups one hour after ADR administration. Tumor uptake and ADR concentration ratio of tumor to liver were higher in Group 3 than in the other two groups. ADR concentration ratio of tumor to liver was higher in group 2 than in group 1. Cardiac and renal uptake in group 3 was less than in group 1 and 2, and so infusion of ADR suspended in a lipiodol may serve to reduce side effects of ADR, especially cardiotoxicity.

[Surgical treatment for intraductal proliferative lesions].

Intraductal proliferative lesions of the breast including benign, borderline and malignant diseases continue to be one of the most difficult diagnostic and therapeutic problems to the pathologist and surgeon. The optimal management should be done according to the different characteristics of the diseases. However, it is difficult to obtain the precise information of the individual disease before surgery. Selected ductolobular segmentectomy of the breast was performed in 73 patients who had been preoperatively diagnosed as having intraductal papilloma due to the clinical examinations. The specimen obtained from 25 patients were subjected to 3-dimensional (3-D) reconstruction analysis of their duct architectures. As the result, intraductal papilloma was divided into central papilloma and peripheral one. It revealed that none of the nine patients originating in the central duct had carcinoma and 6 of the 16 (37.5%) patients with peripheral papilloma coexisted with carcinoma in the terminal ductal-lobular units (TDLUs). This procedure is consisted of the total excision of affected ductolobular system. The favorable results is obtained in the cosmesis and breast feeding, and there is no recurrent case for the follow up period from 1 to 13 years.

[Flow cytometric DNA analysis in colorectal cancer].

Flow cytometric DNA analysis for assessing malignant potential of colorectal carcinoma was investigated by paraffin-embedded materials. Preservation time of paraffin blocks and formalin fixation time of surgical specimens within 14 days do not influence the nuclear DNA content. There was seen a good correlation between the DNA contents of paraffin-embedded and fresh materials obtained from the same surgical specimens. Using deparaffinized tumor specimens, the nuclear DNA content was measured by flow cytometry in 144 patients with primary colorectal cancer, who had been treated from 1971 to 1985. Forty-four percent of colorectal cancer were diploid and 56% were aneuploid. There was no significant correlation between ploidy pattern and clinicopathological factors. However, the patients with aneuploid tumor had a significantly worse survival than those with diploid tumor (Generalized Wilcoxon test, p less than 0.001). The patients with aneuploid seemed to have an unfavorable survival than those with diploid in the same stage, and had a significantly worse survival in each group of negative nodes, P0 and H0. It is concluded, therefore, that the nuclear DNA content of colorectal cancer may be an important prognostic factor, being independent of pathological stage.

[Small thymolipoma in association with myasthenia gravis].

A 40-year-old woman was admitted with diplopia and muscle weakness. A diagnosis of myasthenia gravis was made. Chest radiographs and chest CT showed no mediastinal abnormality. Extended thymectomy was performed. Small tumor (2 X 1 X 0.5 cm) was found within the thymic tissue. Histological examination revealed that the tumor was composed with mature adipose tissue, which expanded and compressed the thymic tissue. The histological appearances were considered to be a typical thymolipoma. This case is the sixth in the world literature in which a thymolipoma is associated with symptoms of myasthenia gravis, and the size of tumor is smallest in the reported cases.

[Acute promyelocytic leukemia with a history of RAEB in transformation and the 15/17 translocation].

A 34-year old female was admitted to our clinic because of fever and general fatigue on March 26, 1987. On admission, peripheral blood (PB) revealed pancytopenia. Bone marrow smears revealed 9. 0% of promyelocytic cells with or without Auer rods. Diagnosis of RAEB in transformation was made. Chromosome study of the bone marrow cells showed t(15; 17) in 3 out of 20 cells analysed. After 3 months, the leukemic cells were observed in PB and increased in number. Then the patient showed bleeding tendency and fibrin degradation products (FDP) increased up to 40 micrograms/ml. And the leukemic cells were over 30% in PB at the end of July, 1987. The diagnosis of APL with DIC was made. To our knowledge, this is the first case of APL with a history of MDS with t(15; 17).

[Flow cytometric DNA measurements of thyroid carcinomas].

Flow cytometric DNA analysis has been performed using paraffin-embedded tissue blocks from 121 patients with thyroid carcinoma. The incidence of aneuploidy found in thyroid carcinoma was 9 (7.4%), while 112 (92.6%) were diploid. Aneuploid specimens consisted of 6 papillary, 1 follicular, 1 medullary and 1 undifferentiated carcinoma. Of 4 undifferentiated carcinoma patients who subsequently died, only 1 was aneuploid. The 5- and 7-year survival rate for diploid and aneuploid patients were 94% vs 37%, and 91% vs 37%, with significant differences (p less than 0.01). DNA measurements have been found useful in predicting the survival of aneuploid patients, though there remains some discrepancy between the DNA content and the clinical findings in cases of thyroid carcinoma.

[The use of MCNU to a patient of primary myelofibrosis complicated with pericardial effusion and proteinuria].

A case of primary myelofibrosis complicated with pericardial effusion and proteinuria is described. A 66-year-old female was admitted to our hospital because of abdominal fullness and shortness of breath. On admission, hepatosplenomegaly and pericardial effusion were observed. Blood examination revealed leukoerythroblastic anemia and thrombocytosis with tear drop cells and giant platelets. Bone marrow aspiration was dry tap and its biopsy showed remarkable myelofibrosis. Urinalysis indicated severe proteinuria. Although neutrophilic alkaline phosphatase score was low, no signs of acute blastic crisis of chronic myelogenous leukemia was found. The diagnosis of an atypical type of primary myelofibrosis was obtained. Administration of MCNU was started in August 1987. Hepatosplenomegaly, pericardial effusion and proteinuria were gradually improved after the administration. The etiology of the pericardial effusion and proteinuria were not obvious, however, these facts suggest that these abnormal findings might be related to PMF itself and MCNU was effective to PNF.

[Immune parameters of peripheral blood in chemoembolization with microencapsulated anticancer drugs].

To investigate possible immunological consequences after arterial infusion of microencapsulated anticancer drug (chemoembolization), cellular immune parameters in peripheral blood were monitored for 4 weeks after chemoembolization in 20 patients with urological malignancies including 7 renal cell carcinomas, 2 renal pelvic carcinomas, 8 bladder carcinomas and 3 prostatic carcinomas. Natural killer (NK) cell activity and OKT4/8 ratio were augmented in 67% and 83% of the cases after chemoembolization respectively. Mature NK cell population (Leu7-OKT 16+) in these cases also showed a trend of continuous increase for more than 4 weeks. On the other hand, a small number of the patients showed an augmentation of OKT3+ cells, Ia-1+ cells or response to phytohemagglutinin. Histopathological examination of regional lymph nodes in 9 patients who underwent chemoembolization for renal cell carcinoma showed stimulating reactions in 8, while lymphocyte depletion in 1. These findings suggest that inhibition of micrometastasis by the enhanced natural killer cell activity could be anticipated in more than half of cases after chemoembolization.

[Bone marrow aplasia without blast crisis in a case of CML of 10-year survival].

A case of chronic myelogenous leukemia (CML) of 10-year survival in described. A 44-year old male was admitted to our hospital because of general malaise, abdominal fullness and fever in February, 1977. On physical examination, giant splenomegaly and hepatomegaly were detected. Peripheral blood examination revealed leukocytosis without hiatus leukemia , normochromic macrocytic anemia and thrombocytosis. NAP rate and score were 16% and 22. Cytogenetic analysis of PB without stimulator revealed 46, XY, Ph1. Then he was diagnosed as having a typical type of Ph1-positive CML. He had been successfully treated over 9 years by intermittent administration of busulfan. However, anemia suddenly progressed in February, 1986 followed by leukopenia and thrombocytopenia. Hemorrhage was not detected by the examination. Though he had been received blood transfusion, the anemia progressed rapidly. He was died of cachexia on 4th of August, 1987. The postmortem examination revealed bone marrow aplasia with no signs of blast crisis nor myelofibrosis. Secondary hemochromatosis was seen in the liver, spleen, pancreas and some other organs.

[DNA analysis by flow cytometry in breast carcinoma].

Using deparaffinized tumor specimens, a flow cytometric DNA analysis was performed in 63 primary breast cancers from patients who were treated from 1976 through 1982. Fourty-three percent of the cases were found to be aneuploid, and 57% were diploid. No trend to increased aneuploid proportion with a more advanced disease was noted. There was no correlation between the level of ploidy and the clinicopathological factors. Aneuploid tumors seem to have a poorer survival, but no significant difference was found between the two patterns of ploidy. Patients with a low S-phase fraction (SPF) had a significantly longer survival than those with a high SPF. A DNA analysis of the tumor cells in breast cancer is felt to be use for a prognostic evaluation.

Preferential expression of the T-cell receptor V beta 3 gene by Mlsc reactive T cells.

The precursor frequency of T cells specific for any given foreign antigen is, in general, extremely low. Prominent exceptions to this rule are the T cells that are specific for foreign major histocompatibility complex (MHC) products or for products of the minor lymphocyte stimulatory (Mls) genes in the mouse which are present at high frequencies. Here, we report a striking overlap or cross-reactivity between the T cells specific for the protein antigen pigeon cytochrome c in association with Ek alpha Ek beta and the set of T cells specific for Mlsc products. In addition, we demonstrate that the basis for this overlap is the predominant expression of one T-cell receptor (TCR) V beta gene, V beta 3, by T cells that recognize Mlsc products. These results indicate the importance of specific TCR alpha beta dimers in the recognition of Mlsc products and that positive or negative selection of T cells specific for Mls self-determinants may selectively alter the repertoire of T cells available for MHC-restricted recognition of foreign antigens.

T cell recognition of Mlsc. I. Influence of MHC gene products in Mlsc-specific T cell recognition.

Monospecific T cell clones have been proven to be powerful tools for the characterization of T cell recognition in many Ag-specific as well as allo-specific T cell responses. In this report, in order to elucidate the mechanism of T cell recognition of minor stimulating locus Ag (Mlsc) determinants, Mlsc-specific cloned T cells were employed together with primary T cell responses to clarify the role of MHC-gene products in Mlsc-specific T cell recognition. The results indicated that T cells recognize Mlsc determinants in conjunction with I-region MHC gene products. Moreover, certain MHC haplotypes (e.g., H-2a and H-2k) appear to function efficiently in the "presentation" of Mlsc, whereas other haplotypes (e.g., H-2b and H-2q) function poorly if at all in presenting Mlsc. Experiments with the use of stimulators derived from F1 hybrids between the low stimulatory H-2b, Mlsc strain, C3H.SW, and a panel of Mlsb, H-2-different or intra-H-2 recombinant strains strongly suggested that expression of E alpha E beta molecules on stimulators plays a critical role for Mlsc stimulation. The functional importance of the E alpha E beta product in Mlsc recognition was further demonstrated by the ability of anti-E alpha monoclonal antibody to inhibit the response of cloned Mlsc-specific T cells. Inhibition of the same Mlsc-specific response by anti-A beta k antibody suggests that the A beta product may also play a role in T cell responses to Mlsc.

The expression of Mlsc determinants on Mlsa, Mlsb, and Mlsx prototypic strains.

In the mouse system, specific determinants other than major histocompatibility complex (MHC) gene products are capable of inducing strong primary proliferative responses in naive T cells. These determinants are encoded by at least two gene loci designated as minor lymphocyte stimulatory (Mls) loci. In order to elucidate the biological role of the Mls system, an effort has been initiated to clarify the fundamental immunogenetic characteristics of the Mls system. In this report, we describe the unexpected finding that Mlsc determinants are expressed on splenocytes of strains including those which have been used as prototypic examples of three other Mls types: Mlsa (DBA/2, DBA/1), Mlsb (BALB/c), and Mlsx (PL/J). The expression of Mlsc by these strains was demonstrated both by the response patterns of unprimed T cells from MHC-identical inbred or F1 hybrid strains and by the responses of a panel of Mls-specific T-cell clones. The experimental results reported here also suggest that the expression of Mls determinants may be influenced by multiple other genes, including MHC-linked genes.

Apocrine carcinoma of the breast--a case report.

A case of apocrine carcinoma of the breast is reported herein. Apocrine carcinoma is a rare tumor characteristically composed of large cells with eosinophilic cytoplasm. This case involves a 34-year old woman who underwent a modified radical mastectomy and is now doing well with no evidence of recurrence, 10 months after her surgery.

Infantile acute leukemia with 11q23 chromosome abnormality and lineage infidelity.

An infant case of acute leukemia (AL) showed lineage infidelity and a chromosome rearrangement involving 11q23. This case was morphologically diagnosed as ALL-L2 according to the FAB classification. However, the blast cells were highly positive for monoclonal antimyeloid antigens (Mol and TG-8) and lymphoid markers (B1, J5 and TdT). These immunologic findings indicated that the blast cells had characteristics of lymphoid B-cell and myeloid lines. In the literature, so-called "11q23 chromosome abnormalities," commonly observed in acute nonlymphoblastic leukemia (ANLL) and in a subtype of acute lymphoblastic leukemia (ALL) with t(4;11) were observed in both lymphoid and myeloid acute leukemias, with some of them recently reported to have lineage infidelity. These unique characteristics may indicate the possibility that the latter is a variation of the former, and support the hypothesis that the chromosomal rearrangement at 11q23 occurs at a multipotent stem-cell level.
