Comparative investigation of the respiratory and cardiovascular effect of mepindolol, propranolol and pindolol in asthmatic patients.

In a controlled double-blind crossover trial of 5 mg mepindolol versus 80 mg propranolol, 15 mg pindolol or placebo given as single oral doses in 16 asthmatic patients, respiratory function and cardiovascular parameters were measured simultaneously. The doses chosen of the 3 active drugs were almost equipotent as shown by the similarity in the percentage fall in pulse rates at 2 hr after ingestion, although it was observed that propranolol had a different time course of effect from the 2 other beta blockers. The effect on respiratory function was marked for propranolol, with statistically significant falls in FEV1 and FEV3 at all times; mild for pindolol with lesser falls which were statistically significant only at nearly 3 hr post-ingestion; and intermediate for mepindolol. Mepindolol gave rise to a rapid and statistically significant fall in lying diastolic blood pressure compared to control values; the reductions in diastolic B. P. for propranolol or pindolol were not statistically significant compared to pretreatment control values.

Future research--potentially rewarding areas for investigation.

Further research is still needed for many aspects of hormone replacement therapy in post-menopausal women. The type of regimen to be used, and in it the type of oestrogen, the type of progestogen, the route of administration, and the dose all require further evaluation. Careful investigation will then be needed to determine the effect of the chosen regimen on the presenting symptoms, and also long-term effect on coagulation factors, carbohydrate and lipid metabolism, osteoporosis and endometrial carcinoma. At present there are no definite criteria as to which patients should be chosen for therapy or the duration for which treatment should be given.

Toxoplasmal and toxocaral infections: a clinical investigation into their relationship.

An investigation was carried out to determine whether a relationship existed between infections with Toxoplasma gondii and Toxocara canis. Antibodies were sought by the toxoplasma dye test and the toxocara skin sensitivity test. Sixty-seven patients were examined in the United Kingdom; 38 had positive toxoplasma dye tests, two of these being toxocara-positive; and 29 had negative dye tests, six of them being toxocara-positive. From the total of eight toxocarapositive patients antibodies to toxoplasma were detected in two, an incidence no greater than that expected for a normal population.Sera from 60 toxocara-positive African patients were examined; 20 possessed antibodies to Toxoplasma. Analysis according to age group or geographical location indicated that this incidence was no greater than that expected for patients without evidence of toxocaral infection. This study therefore showed no causal or clinically important relationship between toxoplasmal and toxocaral infections.
