Leukocyte and bone marrow effects of a thiomorpholine quinazosin antihypertensive agent.

PD-88823, a thiomorpholine analog of prazosin, induced a consistent dose-related suppression of granulopoiesis with subsequent neutropenia and leukopenia in rats and dogs. Rats treated at 600 mg kg-1 day-1 had neutrophil counts reduced by 44% in males and 30% in females after 13 weeks. A 4-week observation period after drug treatment resulted in a rebound in neutrophil counts to 123 and 215% of control values in males and females, respectively. White blood cell count reductions were less evident in dogs, probably because of the lower doses. In both species, the extent of bone marrow suppression was related to duration of treatment. No other hematologic changes were manifest in either species. The mechanism for bone marrow depression and subsequent granulocytopenia was not established. The lack of reported bone marrow effects by quinazosin analogs suggests that the thiomorpholine group of PD-88823 is involved in toxicity. This correlation may be important to safety considerations for future drug design.

Epidemiology of neural tube defects in Utah, 1940-1979.

The prevalence and distribution of 991 cases of neural tube defects who were born in Utah in 1940-1979 are analyzed. The average prevalence at birth is 1.00 per 1000 births (live and stillbirths), which is similar to other states in the western United States. A decline in prevalence is observed during the past 10 years, although there is not a long-term secular trend for the whole time period. Examination of hospital records shows that 17.9% of neural tube defects were not reported on birth or fetal death certificates. The male/female sex ratio is 0.67. No seasonality is found for spina bifida cases; however, a bimodal distribution, with a substantial deficit in May, is observed for anencephaly. A significant negative correlation is found between neural tube defect prevalence and the percentage of individuals in each Utah county who are members of the Mormon Church. No association is found between levels of fallout exposure in Utah and regional variation in neural tube defects, and time-space clustering analysis demonstrates no significant clusters of neural tube defects in Utah.

Analytical methods for quantitation of methylxanthines.

The methylxanthines can be determined in foods and biological systems by the chromatographic methods of TLC, GC, and HPLC. Ultraviolet spectroscopy following a separation procedure is also still used. More recently immunoassay methods have been developed. There is no single best method; the analyst must balance the features of each assay with the final requirements for data precision and reproducibility.

Pins, wires, and staples in foot surgery.

Steinmann's pins, Kirschner wires, staples, and suture wire (commonly referred to as monofilament wire or flexible wire) are implantable materials used in all phases of podiatric surgery. Their main purpose is for temporary fixation of fractures, fusions, bone grafts, and osteotomies and for stabilization of soft-tissue corrections.

Phenotypic heterogeneity in neural tube defects: a clue to causal heterogeneity.

We report here retrospective data on 991 liveborn and stillborn infants with neural tube defects (NTDs) born to Utah residents from January 1, 1940 to December 31, 1979. Data were obtained from multiple sources including approximately 1.25 million vital statistics records and several hundred physician and hospital charts. Causal heterogeneity among NTD patients is presumed because 6% of our cases have other congenital anomalies not part of the NTD field defect. A significant association of NTDs with oral clefts is noted. Sex ratios and empiric recurrence risks for isolated NTDs and NTDs associated with other major malformations are also calculated.

Epidemiology and genetics of neural tube defects: an application of the Utah Genealogical Data Base.

The distribution and prevalence of births with neural tube defects in Utah from 1940 to 1979 are analyzed with regard to prevalence rates, secondary sex ratios, seasonality, yearly rates, and time-space clustering. The overall prevalence rate of 1.00 per thousand live births is comparable to that of other populations in the western United States. Analysis of sex ratios indicates a substantially higher proportion of females than males. No significant secular trends or time-space clustering are observed. No seasonality is seen for spina bifida; however, the anencephaly cases are delivered more frequently in the early spring and fall months. Following linkage of the neural tube defect cases to the Utah Genealogical Data Base, application of the genealogical index method shows substantial familial clustering of the disease. The average inbreeding coefficient of the neural tube defect cases is not elevated over that of matched controls. The empirical recurrence risk for the disease is calculated to be 3%, and the heritability estimate is 70%. Likelihood analysis of pedigrees containing spina bifida occulta and spina bifida cystica indicates that they may segregate as an autosomal dominant trait with a penetrance of 75%.

Functional responses of the rat hepatic endoplasmic reticulum to treatment proposed as a model for cholestasis.

Concurrent treatments of cobalt chloride (CoCl2) and phenobarbital (PB), alone or in combination with lithocholic acid (LCA), were administered to rats for 7 days to assess whether or not a hypoactive hypertrophic smooth endoplasmic reticulum (HHSER) could be induced, as well as investigating the potential role of HHSER in the pathogenesis of cholestasis. LCA given alone slightly reduced hepatic triglycerides, significantly elevated plasma triglycerides and decreased microsomal glucose-6-phosphatase (G6P-ase) activity. PB administered alone significantly increased hepatic phospholipids and microsomal protein, phospholipid and cytochrome P-450 contents, as well as microsomal aminopyrine-N-demethylase (APDM-ase) activity. Functional indicators of liver impairment were associated primarily with CoCl2 treatment, whether given alone or in combination with PB + LCA. These signs included significantly reduced hepatic triglycerides, and increased plasma triglycerides associated with enhanced release of hepatic VLDL-triglycerides, as well as significantly decreased microsomal G6P-ase activity and/or reduced APDM-ase activity and cytochrome P-450 content. Elevated plasma bilirubin levels, and aspartate and alanine aminotransferase activities were also evident with concurrent CoCl2 + PB + LCA treatments. Combined CoCl2 + PB treatments, with or without LCA, caused significant increases in microsomal protein and phospholipid, and decreased activity of the rough endoplasmic reticulum (RER) marker G6P-ase, but no changes in cytochrome P-450 levels and no marked alterations in the activity of the SER marker APDM-ase. The data indicated that simultaneous CoCl2 and PB treatments, whether given alone or in combination with LCA, caused a functional impairment of the RER, and did not induce HHSER membranes.

High performance liquid chromatographic analysis of cholesterol in milk.

A method was developed for analysis of cholesterol in whole milk powder and commercial liquid milk products. High performance liquid chromatography employed a nonaqueous reversed phase chromatographic system. Sample preparation was by saponification with methanolic potassium hydroxide followed by extraction with ether. Potential interfering substances were removed prior to injection by a simple clean-up procedure using a silica gel Sep-pakTM. The chromatographic system included a carbon-18 column with a hexane/isopropanol mobile phase and detection at 205 nm. Method validation and recovery studies using carbon-14 labeled cholesterol indicated excellent performance.

The HPLC analysis of thiamine in milk chocolate.

An HPLC method for the analysis of thiamin in milk chocolate and various chocolate products is described. Samples are extracted with H2SO4, treated with enzymes and interferences removed with a small cleanup column. The separation is effected on a monolayer reversed phase column with a post column thiochrome reaction and fluorescence detection. The method is accurate and precise.

Interaction of hemolytic anemia and genotype on hemolysis-induced gallstone formation in mice.

We previously reported that nb/nb mice with hereditary hemolytic anemia spontaneously developed calcium bilirubinate pigment gallstones. To assess the extent to which gallstone formation and bile composition is gene dependent, we transferred the hemolytic process by transplanting bone marrow from nb/nb mice into a nonhemolytic, but histocompatible genotype, W/Wv mice. Hematologic parameters of transplanted W/Wv mice were nearly identical to those of nb/nb mice. Like nb/nb mice, the percentage of transplanted mice with gallstones increased with the duration of the hemolysis and occurred twice as often in female mice as in male mice (37% vs. 19%; p less than 0.05). However, the rate of gallstone formation in transplanted mice was one-third less than that in nb/nb mice (3.6% per month vs. 5.5%; p less than 0.05). Analysis of hepatic bile revealed that (a) marrow-transplanted mice had higher concentrations of unconjugated bilirubin due to hemolysis (p less than 0.05) and of total bile acids determined by the W/Wv genotype (p less than 0.001) than their respective nb/nb counterparts and (b) transplanted mice with stones had a significantly lower proportion of cholic acid (p less than 0.005) and higher proportion of keto-bile acids (p less than 0.005) than transplanted mice without stones, suggesting that the cholic acid concentration may retard stone formation. These data indicate that the hemolytic process is the primary determinant of pigment gallstone formation in these mice and is influenced by the following factors: (a) duration of the hemolytic process, (b) gender, and (c) the genotype that regulates the composition of biliary components like bile acids.

Carbohydrate composition of candy bars.

The data in Table 1 show a wide variation among products and when added to other available data provide a more complete picture of the carbohydrate composition of candy bars. Interestingly, the sucrose content of the bars in Table 1 ranges from a low of 19.12% to a high of 56.06%. Extensive use of corn syrup is clearly evident in ingrediated bar types (categories e and f). Although the higher saccharides of corn syrup were not quantitated, we think that this article adds to the knowledge of the carbohydrate composition of the best-selling candy bars, since previously only a carbohydrate by difference value was available. This report provides the health professional with an update on carbohydrate composition of a large number of candy bars.

Spinal dysraphia as an autosomal dominant defect in four families.

Four families were selected randomly on the basis of the occurrence of spina-bifida cystica and/or spina bifida occulta in one or more family members. Sixty-three relatives were studied clinically and roentgenologically; their roentgenograms were evaluated blindly. Twenty-eight were clinically and roentgenologically normal; 35 were diagnosed as having spina bifida occulta (SBO), spina bifida cystica (SBC), vertebral anomalies, and/or external defects usually interpreted as evidence for SBO. Excluding one proband we found the frequency of SBO to be 19/51 (37%) and the frequency of all types of spinal/vertebral defects (excluding five probands) to be 30/58 (52%). The distribution of these defects in the four families was analyzed using likelihood methods corrected for random ascertainment. The log likelihood values for sporadic, recessive, and dominant models were -26.69, -20.95, and -18.90, respectively, indicating a higher likelihood of autosomal dominant inheritance than sporadic occurrence or recessive inheritance. The penetrance probability in this dominant model, estimated by maximum likelihood, is 0.749 +/- 0.100. Further examination of these data suggest that SBO and SBC represent different expressions of the same dominant gene in these kindreds.
