[An experimental substantiation of nitric-oxide containing gas flow in the treatment of eye traumas].

The hypothesis on the biostimulating effect of exogenous nitric oxide (NO) was made use of to develop a new method to stimulate the healing of wounds through treating them by a NO saturated gas flow. The above gas flow is generated by air-plasma unit "Plazon". The experimental and clinical studies confirmed that the NO-therapy is a highly effective treatment method for different lesions of the skin and soft tissues. We tried to use the above method in ophthalmology. A comprehensive experimental study was carried out to assess the impact of the NO-containing gas flow on the eyeball structures. An optimal mode was designed, which does not exert any influence on the intraocular pressure, Ph of the lachrymal fluid, antioxidative activity and on the proteinase-inhibitor balance in tears; no morphological changes occurred in the ocular tissue structures. The mentioned morphological and biochemical studies confirmed that the application of the NO-containing gas flow speeds up the healing, process of both an experimental cornea erosion and penetrating corneal wounds. Optimal modes of NO-therapy were defined for both types of lesions.

Natural complex of cytokines is a potent stimulant to posttraumatic regeneration in rabbit cornea.

We elaborated an original technique based on local application of natural complex of cytokines (NCC) secreted by autologous peripheral leukocytes. In this study, we evaluated NCC influence on the healing of penetrating corneal wounds. NCC was derived from supernatants of PHA-stimulated rabbit peripheral leukocytes. Biological tests revealed the presence of IL-1, TNF, IL-6, MIF, and LIF in the complex. Chinchilla rabbits with standard penetrating corneal wounds received daily NCC instillation. The controls were instilled with cultural media 199 with antibiotics. At 24 hours, 3, 7, 14 and 30 days post-wounding, the rabbits were euthanized. We performed morphometry of corneal cross-sections stained with hematoxylin and eosin. In NCC-treated animals, we observed more vigorous migration and activation of neutrophils and macrophages followed by augmented resorption of fibrin. The later post-injury period (14-30 days) was marked with complete healing of the endothelial defect (in the controls, the mass of proliferating cells projected into the anterior eye chamber) and mature scar tissue with a higher content of fibrous component. NCC-treated eye scars were 1.6-fold thinner than the controls'. Local application of NCC promotes effective healing of posttraumatic cornea. It regulates all stages of regeneration and prevents rude scarring.

[Reconstruction of the anterior chamber in posttraumatic epithelial ingrowths and epithelial anterior chamber cysts].

The pathology of penetrating injuries of the cornea and corneal-scleral area with the ingrowth of epithelium and formation of anterior chamber cysts is characterized by polymorphism and can be defined by two main kinds: 1) plane ingrowth of epithelium along the posterior surface of the cornea involving the iris and the lens, and partially or totally obliterating the anterior chamber; 2) formation of cysts with a locked cavity and clear content. In 42 patients with epithelial ingrowths as a result of penetrating injuries of the cornea and corneal-scleral area, the following kinds of interventions were used: 1) transplantation of posterior layers of the cornea with dissection of epithelial growth; 2) dissection of anterior chamber cysts; 3) cystectomy; 4) dissection of the cite of epithelial ingrowth by means of corneal transplantation with removal of the cyst or reconstruction of the anterior chamber. Immediate and remote results of surgical treatment, complications in various forms of the mentioned pathology are described.

[The indications for and sequence of vitrectomy and scleroplastic operations in posttraumatic retinal detachment and vitreous body pathology (1)].

The paper presents a clinical analysis of surgical treatment of 378 patients (426 operations) with posttraumatic retinal detachment. A broadened variant of a classification of traumatic retinal detachment signs in associated pathology of the retina and vitreous body is proposed. Recommendations are worked out concerning the approach of surgical treatment of traumatic detachment with determination of indications and the sequence of vitrectomy and scleroplastic operations. It is advisable to assess the effectiveness of surgical intervention in this pathology in more delayed terms.

[Comparison of the effectiveness of a mobile prosthesis created by the implantation of Hydron and cartilage after enucleation of the bulb].

The authors submit experience with the use of embedded mobile implants of the eyeball made from hydrogel of methylmetacrylate ("HYDRON" SPOFA) and from costal cartilage of different shapes. From the aspect of cosmetic effectiveness similar results were obtained with the implant "HYDRON" SPOFA and costal cartilge in the shape of a flat cylinder. Spherical implants "HYDRON" SPOFA were expelled in 16.8%, while flat cylindrical implants from costal cartilage were not expelled. Other shapes of implants from costal cartilage are less suitable. Both mentioned implants produce a satisfactory cosmetic effect and ensure a stable mobile base for cosmetic prostheses of the eye.

[The principles of the primary surgical treatment of fragmentation eye wounds].

On the grounds of the analysis of results after treatment of 280 patients (25 women, 255 men), aged from 9 to 62 years, with fragmentation wounds of the eye a system of primary surgical treatment of such patients is worked out. The analysis of personal investigations carried out on the basis of studying variants of intraocular pathology in penetrating fragmentation injuries of the eye, succession and volume of surgical interventions allowed to work out a tactics of treatment for such patients using primary surgical treatment of the wound. removal of a foreign body, operations of phacoemulsification, phacofragmentation, vitrectomy and operations on the retina depending on the localization of the fragment, the size of the penetrating wound and associated changes in the eye. As a result of primary surgical treatment, foreign bodies were removed in 235 patients, reconstructive interventions were made in 208 patients.
