Refractive development of the human eye.

A complete refractive investigation was performed on 148 normal eyes of 79 patients ranging from premature newborns to 36-year-old adults. Cycloplegic refraction, keratometry, and axial length measurements were performed. From these data, we then calculated the refractive power of the lens. The change with respect to age in these measurements was subjected to cross-sectional analysis. The full-term newborn eye had a mean axial length of 16.8 mm, a mean keratometric power of 51.2 diopters (spherical equivalent), and a mean lens power of 34.4 D. The adult values for these measurements were 23.6 mm, 43.5 D, and 18.8 D, respectively. This information concerning the expected change with age in the refractive components should aid in the refractive management of pediatric patients requiring cataract extraction.

Characteristics and correlates of MMPI change within an adult psychiatric inpatient setting.

This study investigated characteristics and correlates of MMPI change scores among 188 male and female inpatients who were assessed at admission and upon completion of a 9-week residential program. Predictor variables included admission scores from the standard 13 MMPI scales, the Internal-External Locus of Control Scale, the trait component of the State-Trait Anxiety Inventory, and Shipley-Hartford Verbal IQ scores, as well as demographic variables including factors that involved psychiatric history. Outcome variables consisted of change scores based on raw score values for each of the 13 MMPI scales. Results of a canonical correlation indicated a significant relationship between predictor and outcome variables, which cumulatively accounted for 27% of the variance of outcome change scores. Findings were discussed in terms of implications for research and theory in the areas of the measurement of psychological change through use of the MMPI.

Tumor seeding in ocular fine needle aspiration biopsy.

The purpose of this study was to investigate the occurrence of tumor cell seeding in needle tracks following fine needle aspiration biopsy of intraocular tumors. Sixteen biopsies were performed in four enucleated globes (3 with retinoblastoma and 1 with choroidal melanoma). The biopsies were done immediately after surgery and the needle tracks from the pars plana region were serially sectioned for histological examination. Clusters of tumor cells were seen within six of the 11 needle tracks that could be histologically identified.

Nonmedical costs to patients and their families associated with outpatient chemotherapy.

One hundred thirty-nine patients receiving outpatient chemotherapy kept diaries of nonmedical expenses resulting from their disease and its treatment. Diaries were kept for both treatment and nontreatment weeks. Results showed that the mean cost to patients and their families for treatment weeks was $72.81, and for nontreatment weeks it was $45.88. Approximately 45% of these costs were out-of-pocket expenses, and 55% were wages lost. Transportation and food were the largest out-of-pocket expenses. Patients living at greater distance from treatment had higher out-of-pocket costs, and younger patients reported more wages lost. Fourteen percent of the patients were estimated to be spending more than 50% of their weekly incomes on nonmedical expenses, and these patients were found largely in the lower-income categories. A method is proposed for using these data to estimate total nonmedical expenses for different treatment regimens, and also for estimating cancer patients' total nonmedical costs nationally.

The bleeding time is longer than normal in hemophilia.

Bleeding times were performed on 71 hemophiliacs using the Simplate II device. Eight patients receiving Indocin or Motrin for hemophilic arthropathy were evaluated separately from the remaining 63 who had a mean bleeding time of 7.65 +/- 3.20 min (1 SD) compared to the control group of 5.35 +/- 1.49 min (p less than 0.005). No difference was found when 26 mild hemophiliacs who had received less than 10,000 U of clotting factor concentrate the previous year and no infusions in at least 3 mo were compared with 28 severe hemophiliacs who had received greater than 20,000 U of clotting factor concentrate the previous year and had been infused within 1 mo of testing. Ten patients (16%) had bleeding times greater than 10 min. Bleeding times remained prolonged on repeat evaluations in 7 of these patients, 3 of whom had mild disease and all of whom had normal platelet aggregations in response to arachidonic acid. We conclude that the bleeding time is longer than normal in hemophilia. This abnormality is not related to disease severity, recent transfusions, or the use of nonsteroidal antiinflammatory drugs.

Intra-abdominal abscess and fungemia caused by candida krusei.

A 73-year-old man with chronic lymphocytic leukemia experienced an intra-abdominal abscess caused by Candida krusei. Treatment with drainage alone led to dissemination of the infection and resultant C krusei fungemia. The fungemia responded to therapy with amphotericin B, but the patient died of multisystem failure. We stress the rarity of intra-abdominal infection and fungemia with this organism and the necessity for treatment of candidal intra-abdominal infection with antifungal agents. X

Skin infiltration in Hodgkin's disease.

Hodgkin's disease involving the skin is an unusual occurrence and often portends a poor prognosis. We present a case of Hodgkin's disease and extensive skin infiltration with a favorable outcome. A variety of therapeutic modalities, including a mixture of mechlorethamine hydrochloride, vincristine sulfate, prednisone, and procarbazine; electron-beam radiation; topical mechlorethamine hydrochloride; and intravenous vinblastine sulfate were employed with moderate success.

Septicemia due to Streptococcus equisimilis in a child with acute lymphoblastic leukemia.

A 4-year-old boy with acute lymphoblastic leukemia in relapse with documented septicemia due to group C Streptococci (Streptococcus equisimilis) is described. The patient responded well to therapy with appropriate cytotoxic and antimicrobial agents. There is a general lack of recognition of the pathogenicity of group C Streptococci in man. The potential opportunistic nature of these organisms in immunocompromised hosts and the need for early recognition and appropriate treatment of such infection is emphasized.

The use of fluorescein angiography to study oxygen toxicity.

This study was designed to test whether oxygen toxicity results in retinal damage that could be documented by the technique of fluorescein angiography. Three squirrel monkeys lived in 100% oxygen environments for periods ranging from 50 to 117 hours. Two of the monkeys incurred damage as evidenced by early multifocal leakage and subsequent accumulation of dye in the retina. All monkeys survived the exposures and were followed for several weeks thereafter. The pathology appeared to be reversible since later angiograms were either normal, or approaching normality, when the animals were sacrificed. Subsequent histopathologic examination revealed no abnormalities.
