Thorn-induced granulomata of bone.

A case of thorn-induced granuloma of bone is presented and discussed in relation to the previous eight cases reported. The condition affects the exposed areas, usually the hands, of children. It may be as long as nine months from the initial episode of injury or inflammation to presentation as pseudo-tumour. The radiographic appearance are those of a chronic granuloma with longitudinal cavitation and adjacent perioseal reaction and as such can be differentiated from Ewing's sarcoma, osteoid osteoma, tuberculosis dactylitis, benign osteoblastoma, and stress fracture.

Excision arthroplasty of the elbow in rheumatoid disease.

The results of ten excision arthroplasties of the elbow for rheumatoid disease are described. The operation may afford good relief of pain and a useful increase both of hinge motion and of forearm rotation. Instability is not a serious problem unless the patient has to bear weight on crutches. To increase stability after arthroplasty it appears that Kirschner wire fixation is advisable as well as a plaster cast. For advanced rheumatoid disease unilateral excision arthroplasty has a definite value, especially for patients confined to a wheelchair.

The control of pain after Keller's operation by the instillation of local anaesthetic before closure. A prospective controlled trial.

With the aim of controlling pain after Keller's operation, a solution of a long-acting local anaesthetic was instilled into the field before closure in a series of thirty patients. In a controlled prospective trial this simple technique by itself proved to be significantly better than either foot block at the ankle or the usual analgesics and was free from local or systemic complications.

Rheumatoid arthritis of the elbow. Pattern of joint involvement, and results of synovectomy with excision of the radial head.

Thirty-two rheumatoid elbow joints in 25 patients which had undergone synovectomy and excision of the radial head have been studied from the point of view of pattern of joint involvement and the efficacy of surgery. In this group of patients there was a significant trend towards affection of proximal joints occurring earlier in the course of the disease than distal joints. There was a high incidence of involvement of wrist and knee. There was a positive relationship between dominance and the elbow affected. Pain diminished in all cases after surgery, and there was a significant improvement in hinge movement but no significant improvement in range of forearm rotation though it was less painful. Single lateral approach gave satisfactory exposure and is favoured. Complciations of the procedure were few.
