Pulmonary emboli associated with coagulum pyelolithotomy.
Coagulum pyelolithotomy has become a popular means to extract renal calculi in selected circumstances. Serious complications with this procedure have not been previously reported. A case report of an operative death due to a pulmonary embolus is presented, because of which we designed a canine laboratory study to determine the relationship between coagulum injection into the renal pelvis and thrombo-embolic phenomena. Positive lung scans were detected in a large number of animals related to the volume of the coagulum used and the concentration of the thrombin injected. Judicious use of this procedure with suggested cautions is proposed.

Flutamide therapy for carcinoma of the prostate.
In our three-year experience with the use of flutamide for the treatment of carcinoma of the prostate, 20 patients with previously untreated stage D carcinoma of the prostate were enrolled in a 12-week double-blind study. Seven patients received 1.5 gm of flutamide daily, seven patients received 0.75 gm of flutamide daily, and six patients received 1.0 mg of diethylstilbestrol daily. Two patients receiving flutamide did not complete the 12-week study. Six patients receiving flutamide and three patients receiving diethylstilbestrol had objective evidence of improvement. Of the patients receiving flutamide, 50% were alive at one year, and 43% were alive at two years. Three of the original 14 patients started on flutamide are alive at 36, 33, and 24 months. In addition, we continued clinical evaluation of flutamide as an open study, enrolling six other patients (four with stage D and two with stage C carcinoma of the prostate). All except one of these patients had received previous hormonal therapy. At present, four are still being followed up at 13, 15, 20 and 24 months, demonstrating that flutamide can give positive results to some patients whose condition is deteriorating despite diethylstilbestrol therapy.

Renal carcinoma presenting with pulmonary emboli: a review of two cases.
Two patients with renal carcinoma presenting with pulmonary emboli are discussed. Pulmonary emboliform renal carcinoma may be more common than previously supposed. Evaulation for such tumours should be done in patients with pulmonary emboli of obscure origin.

Bilateral synchronous transitional ureteral carcinoma: two additional cases.
Bilateral synchronous transitional ureteral carcinoma has previously been reported only 11 times. Herein, two additional cases are reported. Analysis shows these tumors to be of relatively low grade and low stage. It is necessary to fully evaluate the opposite side in a patient presenting with unilateral ureteral tumor. If bilateral disease is present, a conservative surgical procedure should be done.
