[Study on the epidemiology and determinants of drug-resistant tuberculosis in northern rural area of Jiangsu province].

OBJECTIVE: To understand the determinants and epidemiology of drug-resistant tuberculosis (TB) in rural area. METHODS: All the diagnosed TB patients in a county with directly observed treatment (DOTS) short-course program in 2002 and a sample of patients in another county without DOTS program located in northern Jiangsu province were surveyed with questionnaires. Drug susceptibility testing (DST) for positive cultures were performed by standardized proportion method. Univariable analysis and multivariate nonconditional logistic regression modeling were applied for data analysis. RESULTS: Among the 152 patients with DST results, 32.9% of the cases showed resistance to at least one of the first-line anti-tuberculosis drugs with 26.3% to isoniazid, 18.4% to rifampin and 17.1% to both isoniazid and rifampin respectively. Previous treatments for TB and residence in the county without DOTS program were independent risk factors for isoniazid and rifampin resistance. TB patients showing indifferent to their health and delayed health seeking for more than 1 month were more likely to have rifampin resistance. Independent predictors of multidrug-resistant TB would include delayed health seeking for more than 1 month (OR = 4.66, 95% CI: 1.26 - 17.24), residing in the county without a DOTS program (OR = 3.01, 95% CI: 1.10 - 8.22), indifference to their health condition (OR = 5.13, 95% CI: 1.06 - 24.90) and suffering from chronic diseases (OR = 0.22, 95% CI: 0.05 - 0.87). CONCLUSION: Drug-resistant TB was quite serious in this rural areas, mainly associated with man-made factors but partly due to the availability of the transmission.

[Application of random amplification polymorphic DNA in the genotyping of Neisseria gonorrhoeae].

OBJECTIVE: To set up random amplified polymorphic DNAs (RAPD) method in genotyping Neisseria gonorrhoeae on DNA level, and to explore its use to trace the source of infection. METHODS: Four different pretreatments were used to extract the Neisseria gonorrhoeae genomic DNA with its advantages and disadvantages compared. Arbitrary sequence was then used to amplify the genomic DNA of Neisseria gonorrhoeae and RAPD fingerprint maps was applied to distinct the Neisseria gonorrhoeae strains. Finally, RAPD fingerprint of Neisseria gonorrhoeae strain between patient and his/her sexual partner was compared. RESULTS: Cetyltrimethylammonium bromide method was classical in extracting genomic DNA, and could get integrated genomic DNA and good fingerprint maps, since main segments were common to all the Neisseria gonorrhoeae but some were different among strains so that the fingerprint of different Neisseria gonorrhoeae were distinctive. However, fingerprint maps of Neisseria gonorrhoeae collected from sex partners were quite similar. CONCLUSION: Based on genomic levels, effective fingerprint maps could be identified and to classify the Neisseria gonorrhoeae into different genotypes. RAPD fingerprint maps could be used to trace the source of infection.

[The discovery of naked cluster particles of Parachlamydia and its developmental mechanism].

OBJECTIVE: To study the survival and developmental morphology of Parachlamydia (BN9) within Acanthamoeba. METHODS: The morphology of BN9 within Acanthamoeba was studied by inverted phase contrast microscope, electron microscope, Gimenez and AO-staining with amoebal co-culture. RESULTS: The endosomal maturation-blocked were formed after the egress of BN9. Two developmental stages-elementary and reticulate bodies, were both observed within the vacuoles. The reticulate bodies, multiplicated by binary fission, were located mainly within the vacuoles, while the elementary bodies can also be located in the plasma individually. The naked cluster particles were observed after the trophozoites cytolysis with Gimenez-staining. The light infectious trophozoites could encyst, and elementary bodies could survive within the mature cysts. CONCLUSION: The egress of BN9 could form the endosomal maturation-blocked, which was presented in two developmental stages-elementary and reticulate bodies. It exhibited the cytolysin activity that could lyse the infectious trophozoites and were expelled in the vesicles. A few light infected amoeba could encyst with survival elementary bodies in the plasma.

[A new statistical method on familial correlation dealing with family data from case-control studies].

OBJECTIVE: This paper presents a statistical method of familial correlation on family data from case-control studies. METHODS: Marginal mean models of the probands and the relatives conditional on the proband's disease status, as well as the marginal association model of the relatives were modeled integrately. Conditional odds-ratio and marginal odds-ratio were used to measure the familial correlation. RESULTS: The parameter's interpretation in the model was in accordance with sample characteristics. This method is more efficient due to making fully use of information of the probands and relatives. In addition, the method has all advantages of GEE2. CONCLUSION: The method in this paper efficiently and conveniently analyzes the family data from case-control studies to estimate the familial correlation on disease.

[A study on the relationship of birth order hepatocellular carcinoma].

OBJECTIVE: To explore the association of hepatocellular carcinoma (HCC) with environmental factors through analyzing birth order data from pedigree sibships. METHODS: A birth order study was conducted based on the methods of Greenwood and Haldane for 100 probands and 22 affected siblings from 100 pedigrees in a cohort which was followed up for 8 years in Haimen, Jiangsu, China. RESULTS: The findings from the Greenwood method suggested that there was a tendency for cases of HCC to concentrate at lower birth orders of 1 to 3. With the effects of hepatitis B virus on the birth orders being controlled, the same tendency was also observed. The results from Haldane method showed that difference between actual value and expected value of 6A was more than the double standard deviation. Sigma 6A(actual value) = 1,806, Sigma 6A(expected value) = 1,988, the absolute value of (Sigma 6A(actual value) - Sigma 6A(expected value))/the square root of Sigma V6A = 2.1657 (0.02 < P < 0.05). The actual value of 6A was lower than the expected value of 6A suggesting that individuals at lower birth orders were at a higher risk of suffering from HCC. In addition, through data from ecologic research, there was a decreasing tendency of mortality of HCC within 10 years after the prevalence of tap water in Haimen city. The correlation coefficient by Spearman test was -0.818 (P < 0.01). The contribution of the quality of drinking water in the locality to the onset of HCC was found among people born in earlier period. CONCLUSIONS: There was a tendency that the onset of HCC cases concentrating was at lower birth orders in this research, which was inconsistent with several reports of foreign authors' which showed a reverse tendency. According to the ecological results, it was suggested that long-term drinking of ditch-water was responsible for the tendency of cases of HCC concentrating at lower birth orders. There were also certain effects of environmental factors exerted on the onset of HCC being discovered.

[Principal component analysis on ultrasound indexes of schistosomiasis and the assessment on prevalence rate].

OBJECTIVE: To explore the synthetical index for diagnosing schistosomiasis with ultrasound and to assess the prevalence rate with the index. METHODS: Ultrasound indexes of schistosomiasis Japonicum were analyzed by principal component analysis, and the synthetical indexes were assessed by ROC curve. RESULTS: Among the abnormal rates of the 6 indexes, the lowest was 1.6% comparing with the highest of 59.5%. Significant difference was noficed among the abnormal rates (chi(2) = 631.1, P < 0.01). The individual correlation of the six indexes to each other as will as with age distribution was significant (P < 0.05). The three principal components reflected the degree of pathological changes on liver and spleen. The first principal component was the factor reflecting the degree of liver pathological changes, and the second and third principal components reflected the degree of pathological changes on spleen. The synthetical index D(1) = 0.047X(1) + 0.428X(2) + 1.247X(3) + 0.095X(4) + 0.002X(5) + 0.213X(6) - 12.837 was found by adding the three weight principal components, and it's area under the ROC curve was 0.957. When -1.70 was taken as the critical value, the abnormal rate of population was 66.3%, close to the resident's actual prevalence rate 66.9%. CONCLUSION: Ultrasonography was considered as a method which could rapidly assessing the resident's prevalence rate in the endemic areas of schisitosomiasis Japonicum, and could also provide powerful information for development of strategy on chemotherapy.

[Estimating family correlation of quantitative traits using generalized estimating equation].

OBJECTIVE: To study the method for measuring familial correlations of quantitative trait and analyzing family data set of body height. METHODS: Generalized estimating equation 2 (GEE2) was employed to estimating both regression coefficients and the familial correlation. Analyses was carried out on software MAREG. A example from height pedigrees illustrated the method. RESULTS: GEE2 provided robust estimations of regression coefficients and familial correlations simultaneously. In body height the correlations between parents and offspring (r = 0.459) and between siblings (r = 0.671) were significantly higher than those between two parents (r = 0.184) after adjusting gender, residence and birth age. Of the same types of relative pairs, the correlation between pairs with individuals of the same gender (eg. father-son r = 0.603, mother-daughter r = 0.456, male sibling r = 0.947, female sibling r = 0.681) was higher than those individuals of different gender (eg father-daughter r = 0.431, mother-son r = 0.364, sibling with different gender r = 0.530). CONCLUSION: GEE2 should be considered a standard method for the investigation of familial aggregation due to its flexibility and robustness.

[National surveillance of schistosomiasis from 2000 to 2002].

OBJECTIVE: To identify the trends of endemic situation at the twenty-one national surveillance sites from 2000 to 2002. METHODS: According to the protocol of the national surveillance system, longitudinal surveillance of endemic situation of schistosomiasis was carried out at the twenty-one surveillance sites since 2000. RESULTS: Relative indexes of endemic situation of schistosomiasis decreased in two surveillance sites annually. The infection rate of Schistosoma japonicum in human population decreased in most surveillance sites. The infection rate of cattle decreased in eight surveillance sites. However, acute cases were found in two sites each year. The density of live snails and infected snails was higher compared with that in 2000 among eight sites and infected snails were found in inner embankment in Junshan surveillance sites. Hunan Province, in 2002 at first time. The snail ridden areas increased significantly in Fengjin sites, Shanghai, where schistosomiasis was eliminated years ago. CONCLUSION: Current control strategies have some impact on the morbidity of schistosomiasis. However, these strategies should be lasted for longer time and surveillance on snails should be enhanced.

[Study on the intention of the administrative model of anti-schistosomiasis institutions at county level].

OBJECTIVE: To investigate current status of schistosomiasis control system at the county level and make recommendations to improve the infrastructure adapting to the prevalent condition of the disease. METHODS: Through pertinently sampling and semi-quantitative questionnaire among the professional workers, experts and administrators in schistosomiasis control, the primary weaknesses involved in the management system and human resources were analyzed. RESULTS: 380 questionnaires were distributed, 33 out of 40 were returned from experts/administrators and 285 out of 340 were received from the professionals. In general, the participants acknowledged the effectiveness of the mechanism on anti-schistosomiasis campaign during the past 40 years. It was found that lack of funds for schistosomiasis control and the poor administrative model of anti-schistosomiasis activities were the main barriers to the control. About half of the participants agreed that the policy about merging the county level anti-schistosomiasis facilities into the Center for Disease Prevention and Control (CDPC) system. The study also showed an inbalanced distribution of the manpower for schistosomiasis control across the endemic regions. The dropout rate of human resources was high due to the poor management and welfare. CONCLUSION: Reforms on the management model should be performed and multiform compensation system should be built in accordance with the prevalence and trends of schistosomiasis.

Relationship between the transmission of schistosomiasis japonica and the construction of the Three Gorge Reservoir.

OBJECTIVE: To study the relationship between the changes of environment and the transmission of Schistosomiasis japonica after the construction of the Three Gorge Reservoir. METHODS: On the basis of the predictive data on changes of water level and sediment in the middle and lower reaches of the Yangtze River after the dam construction provided by the Yangtze River Committee on Water Conservancy, corresponding data were collected and analyzed. In order to understand the effects of changes of water level and sediment on Oncomelania hupensis, the intermediate host of Schistosoma japonicum, field surveys and ecologically intimated experiments were done. The prevalence status and factors related to transmission were investigated by epidemiology, medical geography and public health. RESULTS: Positive impact on control of schistosomiasis is that floods in the Yangtze River will occur less and will decrease the dispersal of snails and the chances of infection for humans and animals as well. Snail habitats' beach will be reduced in the flushed area along the Yangtze River. Exploitation of beach and elimination of snails will benefit from the beach's uplift in flushed area. The floods ahead of time in autumn will interfere with the development of snails. Negative impact is that the flushed beaches and migratory settlements at certain altitude may become snail habitats. So the reservoir area will be a potential transmission area of schistosomiasis due to dispersal of infectious resources and Oncomelania snails. The uprising of the water body level may cause the expending of the snail habitats in some places of Jianghan plain during the transition of spring to summer. The destruction of the embankments of the Dongting Lake will be put off by its delayed flush, which causes the beach in a situation of growing reed-grass or grass for a long time that benefits the reproduction of the snails. The chances for schistosome infection to humans and animals will be on the increase, because the water regression occurs ahead of time in the Dongting Lake beach in autumn. The distribution of snails will not be affected by the changes of water level in the lower reaches of the Yangtze River after the dam is built, but the flushed section in Anhui in the recent 80 years are suitable for the snails' development. The environmental changes due to dam construction will not obviously affect the development of snails and the transmission of schistosomiasis in the Poyang Lake area. CONCLUSION: The construction of the huge dam and the formation of the Three Gorge Reservoir on the Yangtze River may have both positive and negative impact on schistosomiasis transmission and control. Great effort must be made to decrease the negative impact to avoid or, at least, to decrease S. japonicum transmission.
