Increased Genetic Risk for β-Cell Failure Is Associated With β-Cell Function Decline in People With Prediabetes.

Partitioned polygenic scores (pPS) have been developed to capture pathophysiologic processes underlying type 2 diabetes (T2D). We investigated the association of T2D pPS with diabetes-related traits and T2D incidence in the Diabetes Prevention Program. We generated five T2D pPS (β-cell, proinsulin, liver/lipid, obesity, lipodystrophy) in 2,647 participants randomized to intensive lifestyle, metformin, or placebo arms. Associations were tested with general linear models and Cox regression with adjustment for age, sex, and principal components. Sensitivity analyses included adjustment for BMI. Higher β-cell pPS was associated with lower insulinogenic index and corrected insulin response at 1-year follow-up with adjustment for baseline measures (effect per pPS SD -0.04, P = 9.6 × 10-7, and -8.45 μU/mg, P = 5.6 × 10-6, respectively) and with increased diabetes incidence with adjustment for BMI at nominal significance (hazard ratio 1.10 per SD, P = 0.035). The liver/lipid pPS was associated with reduced 1-year baseline-adjusted triglyceride levels (effect per SD -4.37, P = 0.001). There was no significant interaction between T2D pPS and randomized groups. The remaining pPS were associated with baseline measures only. We conclude that despite interventions for diabetes prevention, participants with a high genetic burden of the β-cell cluster pPS had worsening in measures of β-cell function.

Experimental demonstration of quantum entanglement between frequency-nondegenerate optical twin beams.

The quantum entanglement of amplitude and phase quadratures between two intense optical beams with a total intensity of 22 mW and a frequency difference of 1 nm, which are produced from an optical parametric oscillator operating above threshold, is experimentally demonstrated with two sets of unbalanced Mach-Zehnder interferometers. The measured quantum correlations of intensity and phase are in reasonable agreement with the results calculated based on a semiclassical analysis of the noise characteristics given by Fabre et al. [J. Phys. (France) 50, 1209 (1989)].

Understanding the basic case-mix adjustment for the composite rate.

In April 2005, Medicare began adjusting payments to dialysis providers for composite-rate services for a limited set of patient characteristics, including age, body surface area, and low body mass index. We present analyses intended to help the end-stage renal disease community understand the empirical reasons behind the new composite-rate basic case-mix adjustment. The U-shaped relationship between age and composite-rate cost that is reflected in the basic case-mix adjustment has generated significant discussion within the end-stage renal disease community. Whereas greater costs among older patients are consistent with conventional wisdom, greater costs among younger patients are caused in part by more skipped sessions and a greater incidence of certain costly comorbidities. Longer treatment times for patients with a greater body surface area combined with the largely fixed cost structure of dialysis facilities explains much of the greater cost for larger patients. The basic case-mix adjustment reflects an initial and partial adjustment for the cost of providing composite-rate services.

Simultaneous experimental generation of vacuum squeezing and bright amplitude squeezing from a frequency doubler.

Both vacuum-squeezed and bright amplitude-squeezed states of light are experimentally generated from a frequency doubler with a semimonolithic Fabry-Perot configuration consisting of a type II nonlinear crystal and a concave mirror. Vacuum squeezing of 3.2 +/- 0.1 dB and amplitude squeezing of 1.3 +/- 0.2 dB are obtained simultaneously at a pump power of 8 mW. The two quadrature-squeezed optical fields that are generated are of identical frequency at 1080-nm wavelength and orthogonal polarization. Optimizing the input pump power by 19 mW yields as much as 5.0 +/- 0.2 dB of maximum vacuum squeezing. The advantages of the system are its simplicity and multiple utility.

Experimental demonstration of unconditional entanglement swapping for continuous variables.

The unconditional entanglement swapping for continuous variables is experimentally demonstrated. Two initial entangled states are produced from two nondegenerate optical parametric amplifiers operating at de-amplification. Through implementing the direct measurement of the Bell-state between two optical beams from each amplifier the remaining two optical beams, which have never directly interacted with each other, are entangled. The quantum correlation degrees of 1.23 and 1.12 dB below the shot noise limit for the amplitude and phase quadratures resulting from the entanglement swapping are measured straightly.

Quantum dense coding exploiting a bright Einstein-Podolsky-Rosen beam.

Highly efficient quantum dense coding for continuous variables has been experimentally accomplished by means of exploiting a bright Einstein-Podolsky-Rosen (EPR) beam with anticorrelation of amplitude quadratures and correlation of phase quadratures. Two bits of classical information are encoded on two quadratures of a half of a bright EPR beam, then are simultaneously decoded by the other half of the EPR beam with the sensitivities beyond that of the shot noise limit. A high degree of immunity to unauthorized eavesdropping has been experimentally demonstrated.

[A long-term observation of laser in situ keratomileusis for myopia].

OBJECTIVE: To evaluate the long-term results of laser in situ keratomileusis (LASIK) for the correction of myopia. METHODS: A total of 1 285 eyes with preoperative refraction of -1.50 - -25.00 diopters were treated with LASIK. Based on the preoperative refraction, the patients were divided into 3 groups (-1.50 - -6.00 D, -6.12 - -12.00 D, -12.12 - -25.00 D). The mean follow-up was 26.7 months and the results were concluded and analyzed. RESULTS: At two years, respectively 93.4%, 86.7% and 65.5% in the above three groups were within +/- 1.00 D, 84.4%, 79.7%, 42.1% were within +/- 0.50 D; the uncorrected visual acuity was >/= 0.5 in 99.6%, 93.8% and 62.6% of the patients, and 90.4%, 68.6%, and 27.2% had vision of >/= 1.0. Thirty-nine eyes with regression or under correction, and 2 eyes with epithelial ingrowth had been retreated with LASIK. CONCLUSIONS: LASIK is effective and safe for the correction of mild, moderate, high and super-high myopia. The results of follow-up for two years are stable.

Differences between regressive eyes and non-regressive eyes after LASIK for myopia in the time course of corneal changes assessed with the Orbscan.

The aim of this study was to compare regressive eyes with non-regressive eyes after laser in situ keratomileusis (LASIK) for myopia with regard to the time course of biomechanical changes of the cornea and to evaluate the possible roles of these changes in refractive regression following surgery. 15 eyes of 9 patients with refractive regression and 15 eyes of 11 patients with no refractive regression after LASIK for myopia were enrolled in this study. The time courses of central corneal thickness (CCT) and anterior/posterior corneal curvature radiuses (CCR) were measured with Orbscan slit scanning before and 1 week, 1 month and 3 months after LASIK for myopia. The regressive and non-regressive eyes had similar time courses of CCT before and after LASIK, whereas the 2 groups differed in the time courses of corneal shifting movements. In the regression group, the posterior CCR after LASIK was relatively stable (all p > 0.05). By contrast, in the non-regression group, the postoperative posterior CCR at week 1 had a significantly steeper curvature than that at month 3 (p = 0.000). The anterior CCR in the regression group at week 1 had a significantly flatter curvature than that at month 3 (p = 0.002) postoperatively. In contrast, there was no significant change of anterior CCR postoperatively (all p > 0.05) in the non-regression group. Taken together, these data suggest that refractive regression after LASIK might be mainly induced by corneal protrusion rather than central corneal thickening.
