Mammographic density as a surrogate marker for the effects of hormone therapy on risk of breast cancer.

BACKGROUND: Some types of hormone therapy increase both risk of breast cancer and mammographic density, a risk factor for the disease, suggesting that mammographic density may be a surrogate marker for the effects of hormones on risk of breast cancer. This research was undertaken to determine whether the effect of hormone therapy on breast cancer risk is mediated by its effect on mammographic density. METHODS: Individually matched cases and controls from three nested case-control studies in breast screening populations were studied. Cases had developed invasive breast cancer at least 12 months after the initial screen. Information was collected on hormone use and other risk factors at the time of the baseline mammogram, and percent density was measured by a computer-assisted method. RESULTS: There were 1,748 postmenopausal women, of whom 426 (24.4%) were using hormones at the time of their initial screening mammogram. Current use of hormone therapy was associated with an increased risk of breast cancer (odds ratio, 1.26; 95% confidence interval, 1.0-1.6) that was little changed by adjustment for percent density in the baseline mammogram (odds ratio, 1.19; 95% confidence interval, 0.9-1.5). Percent density in the baseline mammogram was among cases greater in current users of hormones that in never-users (difference = 5.0%, P < 0.001), but the difference was smaller and nonsignificant in controls (difference = 1.6%, P = 0.3). CONCLUSION: Although the effects of hormone therapy on mammographic density were greater in cases than controls, we did not find evidence that these effects were causally related to risk of breast cancer.

Simultaneous determination of protocatechuic acid, syringin, chlorogenic acid, caffeic acid, liriodendrin and isofraxidin in Acanthopanax senticosus Harms by HPLC-DAD.

A high performance liquid chromatography (HPLC) method was developed for the first time to quantify simultaneously the six major active ingredients in Acanthopanax senticosus (Rupr. et Maxim.) Harms, namely protocatechuic acid, syringin, chlorogenic acid, caffeic acid, liriodendrin and isofraxidin. The analysis was performed by a reverse phase gradient elution with an aqueous mobile phase (containing 0.05% phosphoric acid) modified by acetonitrile and diode-array multiple-wavelength UV detector (DAD). Six regression equations showed good linear relationships between the peak area of each marker and concentration. The recoveries of the markers listed above were 92.3%, 93.9%, 90.3%, 93.1%, 94.3% and 90.7%, respectively. The relative standard deviation of intra-day and inter-day were less than 2.7% and 3.1%, respectively. This method was validated for specificity, accuracy, precision and limits of quantification. Medicinal materials of ten commercial brands were analyzed and found to contain different amounts of the six bioactive markers. The method developed can be used for the quality control of Acanthopanax senticosus (Rupr. et Maxim.) Harms.

Neonatal loss of gamma-aminobutyric acid pathway expression after human perinatal brain injury.

OBJECT: Perinatal brain injury leads to chronic neurological deficits in children. Damage to the premature brain produces white matter lesions (WMLs), but the impact on cortical development is less well defined. Gamma-aminobutyric acid(GABA)ergic neurons destined for the cerebral cortex migrate through the developing white matter and form the subplate during late gestation. The authors hypothesized that GABAergic neurons are vulnerable to perinatal systemic insults in premature infants, and that damage to these neurons contributes to impaired cortical development. METHODS: An immunohistochemical analysis involving markers for oligodendrocytes, GABAergic neurons, axons, and apoptosis was performed on a consecutive series of 15 human neonatal telencephalon samples obtained postmortem from infants born at 25 to 32 weeks of gestation. The tissue samples were divided into two groups based on the presence or absence of WMLs by performing routine histological analyses. The expression of GABAergic neurons was compared between the two groups by using age-matched samples. Two-tailed t-tests were used for statistical analyses. Ten infants had WMLs and five did not. Significant losses of oligodendrocytes and axons and markedly increased apoptosis were appreciated in tissue samples from the infants with WMLs. Samples from infants with WMLs also showed significant losses of glutamic acid decarboxylase-67-positive cells and calretinin-positive cells, shorter neuropeptide Y-positive neurite lengths, and losses of cells expressing GABA(A)alpha1, GABA(B)R1, and N-acetylaspartate diethylamide NR1 receptors when these factors were compared with those in samples from infants without WMLs (all p < 0.02). CONCLUSIONS: In addition to oligodendrocyte loss, axonal disruption, and excess apoptosis, a significant loss of telencephalon GABAergic neuron expression was found in neonatal brains with WMLs, compared with neonates' brains without WMLs. The loss of GABAergic subplate neurons in infants with WMLs may contribute to the pathogenesis of neurological deficits in children.

[Effect of chronic transauricular kindled seizures on passive-avoidance test memory retention in rats].

OBJECTIVE: To investigate effect of chronic transauricular kindled seizures on passive-avoidance test memory retention in rats. METHODS: Chronic transauricular kindled seizures was induced by repeated application of initially subconvulsive electrical stimulation through ear-clip electrodes once every 24 h until the occurrence of 3 consecutive clonic-tonic seizures. A passive avoidance test was used to measure memory retention ability. Morphological changes in neurons of hippocampal CA1 region was examined after HE staining. Histamine, gamma-aminobutyric acid (GABA) and glutamate levels in the hippocampus were measured by high performance liquid chromatography (HPLC). RESULT: Chronic transauricular kindled seizures impaired passive-avoidance test memory retention in rats. The damaged CA1 neurons were observed and histamine content in the hippocampus markedly decreased 24 h after the end of kindling in the chronic transauricular kindled rats. CONCLUSION: Chronic transauricular kindled seizure impaired passive-avoidance test memory retention, and it might be due to the damaged CA1 neurons and a decrease of histamine in the hippocampus induced by epilepsy.

[Effects of endogenous histamine on memory impairment induced by pentylenetetrazole-kindled epilepsy in rats].

OBJECTIVE: To investigate the mechanisms of memory impairment induced by pentylenetetrazole (PTZ)-kindled epilepsy in rats and the effects of endogenous histamine. METHODS: Rats were injected i. p with a subconvulsive dose of PTZ every 48 h until fully kindled. Memory was tested by shuttle box with passive avoidance. Brain histamine was measured spectrofluorometrically. Neurons of hippocampus were investigated with HE stain. RESULT: PTZ-kindled epilepsy caused memory impairment in rats, i .e. latency of passive avoidance was shortened in shuttle box. Pretreatment of histidine, the precursor of histamine, showed an ameliorating effect on memory impairment induced by epilepsy. Decreased histamine contents in the hippocampus, thalamus and hypothalamus were observed after fully kindled in rat. In addition, intact neurons of the CA1 and CA3 regions in hippocampus decreased to 72.7 % and 78.9 % compared with those in control group. CONCLUSION: PTZ-kindled epilepsy causes memory impairment, and it might be due to a decrease of brain histamine and loss of hippocampal neurons induced by epilepsy.

[Effect of exogenous RGS16 gene transfection on growth of glioma cell line C6].

BACKGROUND & OBJECTIVE: Wild type p53 could induce RGS16 mRNA expression, and RGS16 protein is related with carcinogenesis of glioma. This study was designed to investigate the effects of exogenous RGS16 gene transfection on the growth of rat glioma cell line C6. METHODS: A eukaryotic expression plasmid pIRES2-EGFP-RGS16 was constructed, and transfected into C6 cells. The cells were selected by G418. The expression of enhanced green fluorescent protein (EGFP) was observed under fluorescent microscope, and the expression of RGS16 mRNA and protein was detected by reverse transcription-polymerase chain reaction (RT-PCR) and immunocytochemistry. The effects of RGS16 gene on cell cycle progression, cell growth, and proliferation of C6 cells were measured by flow cytometry, cell growth curve, and clonogenic assay in plate. RESULTS: C6-RGS16 and C6-GFP cells, which separately expressed RGS16 and pIRES2-EGFP vector, were constructed successfully. S phase proportion was significantly higher in C6-RGS16 cells than in C6-GFP and C6 cells (28.5% vs. 18.9% and 14.3%, P<0.05); C6-RGS16 cells grew faster than C6-GFP and C6 cells. The clone formation rate was significantly lower in C6-RGS16 cells than in C6-GFP and C6 cells (12% vs. 25% and 25%, P<0.05). CONCLUSION: Exogenous RGS16 gene could promote cell cycle progression of glioma C6 cells from G1 phase to S phase, and accelerate cell growth, but can't increase the clone formation.

[Construction of the prokaryotic expression vector and expression of human CIDE-3 gene].

AIM: To construct a prokaryotic expression vector for human CIDE-3 gene, and to express the gene in E.coli BL21(DE3). METHODS: The total RNA was extracted from human hepatocellular carcinoma cell line HepG2. CIDE-3 gene fragment was amplified by RT-PCR and was cloned into the pET28a(+) vector. The recombinant vector was identified by restriction endonuclease digestion analysis and DNA sequencing, and then it was transformed into E.coli BL21(DE3) through IPTG induction to express the target protein bearing His tag. RESULTS: A 516 bp of CIDE-3 gene fragment was obtained. After E.coli BL21(DE3) was transformed with recombinant vector pET28a(+)-CIDE-3 and through IPTG induction, the recombinant protein with relative molecular masse about 23,000 was obtained. SDS-PAGE analysis showed that the expressed product was mainly inclusion bodies, accounting for 32% of the total bacterial proteins. CONCLUSION: Recombinant expression vector pET28a(+)-CIDE-3 is constructed successfully. The expressed CIDE-3 protein will be helpful to our further research.

[Research advances in the diagnosis and treatment of inconspicuous penis of children].

Inconspicuous penis is a group of symptoms that can make penis body not be showed normally instead of a kind of simple disease. They are presented with conclude concealed penis, buried penis, webbed penis and trapped penis. Most of them are resulting from congenital abnormality except the trapped penis. The diminutive penis caused by hypospadia and intersex and micropenis resulted from endocrine disease does not belong to this category. Pathological changes of concealed penis, buried penis and webbed penis are congenital but trapped penis is caused by acquired reasons. It's not difficult to make the diagnosis by the pathological and clinical manifestations. According to different age's indication, type and severity of the disease, corresponding surgery method ought to be chosen and the curative effect can be obtained.

[Study on decomposed products of trichlorfon in process of gas chromatographic analysis by mass spectrometry].

The decomposed products of trichlorfon in gas chromatographic analysis were identified by mass spectrometry (MS). After MS interpretation, three decomposed products, trichloroacetaldehyde, dimethyl phosphite and dichlorvos were identified. The effects of gas chromatographic conditions on decomposed products of trichlorfon, e. g. injection temperature, injection mode and oven ramp, were studied. The experiments showed that all of the factors have effects on decomposed products of trichlorfon, however, the injection temperature is the key factor to cause trichlorfon being decomposed. The higher the injection temperature is, the bigger the amount of trichlorfon being decomposed. When the injection temperature was raised from 150 degrees C to 250 degrees C, the remaining trichlorfon fell from 86% to 20%. Therefore, on-cold column injection mode gas chromatography or high performance liquid chromatography was recommended for exact quantification of trace trichlorfon.

The mechanism of organophosphorus pesticide-induced inhibition of cytolytic activity of killer cells.

The main toxicity of organophosphorus pesticides (OPs) is neurotoxicity, which is caused by the inhibition of acetylcholinesterase. OPs also affect immune responses including effects on antibody production, IL-2 production, T cell proliferation, decrease of CD5 cells, and increase of CD26 cells and autoantibodies. However, there have been few papers investigating the mechanism of OP-induced inhibition of cytolytic activity of killer cells. This study reviews the new mechanism of OP-induced inhibition of activities of natural killer (NK), lymphokine-activated killer (LAK) and cytotoxic T lymphocytes (CTL). NK, LAK and CTL induce cell death in tumor or virus-infected target cells by two main mechanisms. The first mechanism is direct release of cytolytic granules that contain perforin, granzymes, and granulysin by exocytosis to kill target cells, which is called the granule exocytosis pathway. The second mechanism is mediated by the Fas ligand (Fas-L)/Fas pathway. To date, it has been reported that OPs inhibit NK, LAK and CTL activities by at least the following three mechanisms: 1) OPs impair the granule exocytosis pathway of NK, LAK and CTL cells by inhibiting the activity of granzymes, and by decreasing the intracellular level of perforin, granzyme A and granulysin, which was mediated by inducing degranulation of NK cells and by inhibiting the transcript of mRNA of perforin, granzyme A and granulysin; 2) OPs impair the FasL/Fas pathway of NK, LAK and CTL cells, as investigated by using perforin-knockout mice, in which the granule exocytosis pathway of NK cells does not function and only the FasL/Fas pathway remains functional; 3) OPs induce apoptosis of immune cells.

[Clinical features and prognosis in breast cancer patients over 70 years of age].

OBJECTIVE: To investigate the clinical characteristics and prognostic factors in breast cancer patients over 70 years of age. METHODS: From 1980 to 2003, 280 female breast cancer patients over 70 years old were treated and the data were retrospectively reviewed. The clinical features including age, comorbidity, initial symptom, tumor size and location, pathological type, lymph node status, hormonal receptor status, treatment approaches and overall survival were analyzed. RESULTS: These 280 patients accounted for 2.9% of all breast cancer patients registered in our institution during the same period. Presentation of breast lump as initial symptom accounted for 92.5% of the patients. The median time from the presentation of initial symptom to initial diagnosis was 4 months. Major pathological type was invasive ductal carcinoma (74.3%). Estrogen or progesterone receptor was found to be positive in 72.9% by immunohistochemical staining. 165 patients (58.9%) had comorbidity such as coronary heart disease, hypertension, diabetes, etc. 256 patients underwent surgery consisting of 162 modified mastectomies, 46 mastectomies, 38 lumpectomies, 7 lumpectomies plus lymph node dissection, 2 lymph node resection and 1 with unavailable surgery record. The cumulative 5- and 10-year overall survival was 69.9% and 40.6%, respectively. Factors affecting the prognosis were tumor size, lymph node status, pathological stage, vascular invasion and endocrine therapy by univariate analysis. The lymph node status and vascular invasion were found to be two independent prognostic factors affecting significantly the prognosis by multivariate analysis. CONCLUSION: Female breast cancer patients over 70 years of age exhibit distinctive clinical and pathological characteristics. Surgery and endocrine therapy are important to achieve good clinical outcome. Lymph node status and vascular invasion are two independent prognostic factors.

[Simultaneous determination of five isoflavonoids in commercial Radix Astragali by high performance liquid chromatography].

A reversed-phase high performance liquid chromatographic method (RP-HPLC) was developed for the simultaneous analysis of five isoflavonoids of Huangqi, the roots of Astragalus mongholicus. HPLC determination was performed on a Diamonsil C18 column (200 mm x 4.6 mm i.d., 5 microm) and detected at 230 nm. The mobile phase consisted of (A) acetonitrile and (B) water. Gradient program was adopted as follows: 0-25 min from 17% A to 31% A, 25 -45 min 35% A. The flow rate was maintained at 1.0 mL/min. The method was proved to be linear in the ranges of 20.12-201.2 mg/L (r = 0.9992), 4.62-46.2 mg/L (r = 0.9997), 4.86 -48.6 mg/L (r = 0.9997), 9.24-92.4 mg/L (r = 0.9995) and 6.92-69.2 mg/L (r = 0. 999 5) for the five isoflavonoids, calycosin-7-O-beta-D-glucoside, formononetin-7-O-beta-D-glucoside,9,10-dimethoxypterocarpan-3-O-beta-D-glucoside, calycosin and formononetin, respectively. The recoveries were greater than 94%, and relative standard deviations were less than 3.2%. The method has been successfully applied to the simultaneous determination of the five isoflavonoids of radix astragali.

Characterization of the transdermal transport of flurbiprofen and indomethacin.

Transdermal permeation of two types of NSAIDs, [(3)H] flurbiprofen and [(14)C] indomethacin, was examined by use of the Ussing-type chamber method. We found that the transdermal permeability in the absorptive direction (P(abs)) of [(3)H] flurbiprofen was significantly higher than that of [(14)C] indomethacin. A lower pH (5.0) on the epidermal side increased the accumulation and the P(abs) of [(3)H] flurbiprofen (18-fold and 50-fold, respectively) and [(14)C] indomethacin (18-fold and 22-fold, respectively), compared with pH 7.4. Coadministration of unlabeled flurbiprofen and indomethacin increased P(abs) of [(3)H] flurbiprofen and [(14)C] indomethacin, respectively, in a concentration-dependent manner. Similar high-affinity transport was also observed in the uptake of [(3)H] flurbiprofen by human epidermal keratinocytes (HEK001 cells). RT-PCR analysis revealed the expression of mRNA of numerous transporters including MRP, OATP, MCT and OCTN family members in hairless mouse skin, human skin and HEK001 cells. These findings support the novel hypothesis that transdermal permeation of NSAIDs is mediated by saturable transport mechanisms, which may be candidates as targets for transdermal delivery of drugs.
