[Urokinase-type plasminogen activator (uPA), uPA receptor (uPA-R) and inhibitors (PA I -1) expression in hepatocellular carcinoma in relation to cancer invasion/metastasis and prognosis].

OBJECTIVE: To study the relevance of uPA, uPA-R and PA I -1 to hepatocellular cancer (HCC). METHODS: The expression at protein level for uPA, uPA-R and PA I -1 was examined in 48 cases of HCC and 12 cases of benign tumors of the liver (as control) immunohistochemically. RESULTS: When compared to HCC-adjacent normal liver tissue and the control, positive rates of immune staining for uPA, uPA-R and PA I -1 on cell membrane were significantly higher in HCC cells (P < 0.05). Positive staining of uPA and uPA-R was seen in 16 of 22 and 19 of 22 cases of HCC with invasion, respectively (P < 0.01 and < 0.001). In 8 of 8 cases with cancer emboli, and in 6 of 6 cases with lymph node metastasis was the expression of uPA-R positive. Compared with 2 of 17 cases without recurrence, uPA-R was positive in 15 of 17 recurrent cases (P < 0.01). In 36 cases who survived, 17 was positive for uPA-R and 15 for PA I -1, while in 12 cases who died 2 years after surgery, 12 and 9 cases were positive for uPA-R and PA I -1, respectively (P < 0.01 and < 0.05). In 15 cases positive for all three parameters, 11 had cancer invasion and 7 died in 2 years, while in the controls with negative staining 2 had invasion and none died in 2 years (P < 0.05). CONCLUSION: Expression of uPA, uPA-R and PA I -1 is increased in HCC, and uPA and uPA-R may contribute significantly to HCC invasion and metastasis. uPA-R and PA I -1 are associated with poor prognosis.

Surgical treatment of atrial fibrillation with maze procedure by radiofrequency ablation.

OBJECTIVE: To compare the results of treatment of atrial fibrillation (Af) with maze procedure by radiofrequency in 25 patients undergoing mitral valve replacement operation (MVRO) (treatment group) with those in 25 patients undergoing MVRO only (control group). METHODS: The maze procedure modified was completed by radiofrequency ablation with macroscopy during MVRO in the treatment group, whereas the patients in the control group underwent MVRO only. No one in the two groups took antiarrhythmic drugs before and after operation. RESULTS: The heart rhythm became stable sinus rhythm in 22 of 25 patients of the treatment group after operation and the success rate was 88.0%. Af recurred in none of the 22 patients during the follow-up for 3-24 months. In the control group, 5 patients showed transient sinus rhythm, but Af recurred when they left hospital. CONCLUSIONS: Maze procedure by radiofrequency ablation is an effective method to cure Af and its success rate was the same as that of surgical incision and cryolesion. But the former took 10-15 minutes only with light damage of atrial muscle and no bleeding, whereas the latter took 1-1.5 hours.

[Clinical study on Zishen Decoction in chronic uric acid nephrosis].

OBJECTIVE: To observe the ameliorative effect of Zishen Decoction (ZSD) on chronic uric acid nephrosis (CUAN). METHODS: The 72 CUAN patients were divided into 2 groups: The group with ZSD treatment, the dose of which was 400 ml/d and group with zyloric as the control, the dose of which was 200 mg/d. The treatment lasted for 8 weeks. RESULTS: The general effective rate of the ZSD group was 92.86%; and that of the control group Was 66.67%. There was significant difference between the two groups (P < 0.01). ZSD treatment reduced the levels of blood uric acid, serum creatinine and blood urea nitrogen and the levels of albumin, beta 2-microglobulin and the activity of N-acetyl-D-glucosaminidase in CUAN (P < 0.01). The levels of triglyceride and total cholesterol decreased and the levels of high density lipoprotein cholesterol increased in the serum of ZSD treated CUAN (P < 0.01). CONCLUSIONS: ZSD exerted obviously ameliorative effect on renal function in CUAN.

[Inhibitor of plasminogen activator 1 (PAI-1) in hepatocellular carcinoma].

OBJECTIVE: To study the expression of the type-1 inhibitor of plasminogen activator (PAI-1) on protein and mRNA in hepatocellular carcinoma (HCC), and the relation between PAI-1 and biological behaviour of HCC. METHOD: The tumor specimens of HCC (n = 48) and benign liver tumor (contorl n = 12) were tested by immunohistochemistry for the expression of PAI-1 protein. The tumor specimens of HCC (n = 20) and normal liver tissue (contorl n = 5) were evaluated by Northern blot analysis for the expression of PAI-1 mRNA. RESULT: PAI-1 protein and PAI-1 mRNA were higher in tumor cell than in the marginal tissue of tumor and in control (P < 0.01 and P < 0.05). In deaths 2 years after surgery, PAI-1 was increased as compared to the survival group (P < 0.05). When negative PAI-1 conupared with positive urokinase-type plasminogen activator (uPA) and positive uPA receptor (uPAR). The nambeigs invasion case increased, the difference was more significant than that in the group with negative PAI-1, uPA and uPAR (P = 0.039). CONCLUSION: The expression of PAI-1 protein and PAI-1 mRNA is increased in HCC. PAI-1 contributes to the invasion, metastasis and prognosis of HCC.

[Changes of PAI-1 and biological behaviour of human hepatocellular carcinoma in metastatic model of nude mice].

OBJECTIVE: To study the relationship between plasminogen activator inhibitor type-1 (PAI-1) and biological behaviour of hepatocellular carcinoma (HCC). METHOD: 40 nude mice with LCI-D20 metastatic modes of human HCC (MMHCC) were established. The change of PAI-1 was determined by PAI-1 kit and immunohistochemistry with monoclonal antibody against PAI-1 in MMHCC progression. RESULT: Following MMHCC progression from early to advanced stage, PAI-1 increased from 6.2 +/- 1.8 Au/ml to 15.4 +/- 0.7 Au/ml in plasma (P < 0.05), from 0.4 +/- 0.1 Au/mg to 0.8 +/- 0.3 Au/mg in extract (P < 0.05). The correlation between PAI-1 and tumor size and AFP level was strong (r = 0.9648 and r = 0.9544, P < 0.05 and P < 0.05). CONCLUSION: PAI-1 was increased gradually following tumor progression in MMHCC. PAI-1 correlated with tumor size and AFP level. PAI-1 related well with the invasiveness and prognosis of HCC.

[X-ray diffraction Fourier Pattern Analysis of the Chinese materia medica "shannai" (Kaempferia Galanga L.)].

Every Chinese materia medica consists of dozens of ingredients. It is a multiingredient system. X-ray diffraction provides a way to determine Chinese materia medica in diffraction space which can reflect the general structural characteristics of Chinese materia medica and quantitative changes of partial ingredients. Considering the influence of varieties, growing places, growing periods, collecting seasons and other phenological factors, we selected 6 samples of Kaempferia galanga L. and Kaempferia marginata Carey, collected in different seasons from different growing places for X-ray diffraction Fourier Pattern Analysis. We have obtained characteristic peaks and diffraction fuzzy figures for distinguishing purposes. The result shows that X-ray diffraction Fourier Pattern Analysis can be used for distinguishing Chinese materia medica with similar shape and properties, similar tissue texture or similar chemical composition.

[Studies on the fine stereostructure of taxoids].

The anticancer drug taxol is a new type antimicrotubular drug. The structure-activity relationship study on taxol and its analogs has indicated the importance of the C13 side chain and the C4-C5-C20 epoxypropane group. The crystal structures of 4 taxoids having the basic skeleton with 5/7/6 membered ring and three types of 10 taxoids having the basic skeleton with 6/8/6 membered ring have been analyzed. The effects of the induction by different substructures at C4 on the stereostructures of taxoids were studied, and the relationship between the 6/8/6/4 skeleton and the anticancer activity was discussed from the crystallographic point.

To use or not to use? Backward equations in stochastic carcinogenesis models.

The method based on the Kolmogorov backward equations of Little (1995, Biometrics 51, 1278-1291) for computing hazard functions for the multistage carcinogenesis models fails when model parameters are time-dependent. In addition to suggesting an alternative method based on the Kolmogorov forward equation, this note highlights the interplay of the forward equation, the backward equation, and the characteristic method. Advantages and disadvantages of the forward and backward equations are discussed.

Effect and clinical value of splanchnic nerve block of hemodynamics in ACST.

The condition of acute cholangitis of severe type (ACST) develops very rapidly and the prognosis is poor. The main clinical feature is that there is an obvious dynamic variation, which is the principal factor for the early occurrence of shock and death. In this study the Japanese long ear rabbits were used and biliary tract pressure increasing and splanchnic nervous plexus blocking experiments were conducted. Our results indicate that 0.6% of lidocaine can be used to block the right celiac plexus. It can avoid the decrease of blood pressure due to the pressure increase of the biliary tract, and the decrease of blood pressure due to the pressure increase of the biliary tract can be corrected by local anesthesia. 8 cases in conformity with the ACST diagnostic standards received the right renal capsule block injection for the purpose of stopping the celiac plexus, 6 cases of whom had a return of blood pressure to various extent, suggesting that the splanchnic nervous activity in the occurrence of ACST is of great significance. The technique may provide a new approach for the clinical treatment of ACST.

Determination of CAG repeat length in the androgen-receptor gene using frozen serum.

OBJECTIVES: To determine CAG repeat length in exon one of the androgen-receptor gene, using frozen serum as a source of deoxyribonucleic acid (DNA). METHODS: Samples from previously frozen serum samples were prepared for polymerase chain reaction (PCR) by heating thawed serum to 100 degrees C and subsequent centrifugation at 16,000 g. Three microliters of supernatant were added to the PCR in combination with primers flanking the CAG repeat in exon one of the human androgen-receptor gene. After 35 cycles, the PCR amplicons were electrophoresed on an agarose gel, excised, purified, and sequenced. CAG repeat length was directly determined from DNA sequencing gels. RESULTS: Amplified DNA fragments were obtained after PCR from 140 of the 200 specimens examined to date. The DNA amplicon was successfully sequenced in 111 samples derived from 104 individuals. Within this group, the androgen receptor CAG repeat length varied from 11 to 27. The median CAG repeat length was 21; the lower and upper quartiles were 18 or less and 23 or more, respectively. Results were consistent in each case that CAG repeat length was repetitively determined. CONCLUSIONS: The establishment of a methodology to determine androgen-receptor gene CAG repeat length from frozen serum samples opens multiple new opportunities to explore the potential clinical significance of this genetic polymorphism.

[Comparison of changes of bone mass, parathyroid hormone and calcitonin between two animal models of bone loss].

The changes in bone mass, blood parathyroid hormone (PTH) and calcitonin (CT) between rats suspended for 14 and 21 days, simulated weightlessness, and rats ovariectomized (OVX) for 30 and 60 days were observed. The results revealed that mineral density of T6 and L3 was significantly increased in rats suspended for 14 days. T6 mineral density was also significantly increased in rats suspended for 21 days, but L3 mineral density was significantly decreased. T6 mineral density had no change in OVX rats, but L3 mineral density was significantly decreased in them both for 30 and 60 days. Mineral content at proximal 1/3 of femur shaft was significantly decreased in rats suspended for 14 and 21 days, and in rats OVX for 60 days. Bone biomechanical properties of suspended rats were more seriously deteriorated than that in OVX rats. There was no change in blood PTH, but CT was significantly increased in rats suspended for 14 and 21 days. Blood PTH and CT were significantly decreased in rats OVX for 30 and 60 days. The results indicate that the mineral redistribution and bone loss are presented in trabecular bone of suspended rats, but not in OVX rats. The deterioration of bone quality was more seriously in suspended rats than that in OVX rats. Both the cortical and trabecular bone in the two models were affected.
