Serological mass survey for early detection of nasopharyngeal carcinoma in Wuzhou City, China.

A serological mass survey was carried out in Wuzhou City of the Guangxi Autonomous Region, China. Sera were collected from 12,932 persons between the ages of 40 and 59. The positive rate of VCA/IgA antibody-positive person was 5.3%, but no EA/IgA antibody was found in sera from VCA/Iga-negative persons. Thirteen and nine nasopharyngeal carcinoma (NPC) patients were detected from the VCA/Iga and EA/IgA antibody-positive persons, respectively. With the present combination method the detection rate of NPC for 12,932 persons was 100.5/100,000 and for 680 VCA/IgA antibody-positive persons it was 1,900/100,000. Thus, the rate was twice and 37 times higher, respectively, than the annual incidence rate of NPC in persons of the same age group from 1975-1978 in Wuzhou City. Of 13 NPC patients, 9 were in stage I (70%) and 4 in stage II (30%). Therefore, it is possible to reduce the mortality rate of NPC in Wuzhou City by radiotherapy of NPC patients in the early stage of the disease. The present results further suggest that EB virus is closely associated with NPC.

A method for simultaneous RF ablation and MRI.

Radiofrequency (RF) energy has many advantages in thermal tumor ablation protocols. With the recent development of open MRI systems, interventional MRI procedures, including thermal ablation, have become the focus of great research interest. However, the significant interference between RF generators and MR imagers has prevented simultaneous imaging and RF ablation and, until now, has limited the role of RF-based thermal therapy in interventional MRI. Here, a simple switching circuit designed with consideration of patient safety provides compatibility between open MRI systems and RF thermal lesion generators. The experimental results show that the switching circuit allows imaging during RF ablation and opens new opportunity for MR-guided thermal therapy.

Shifted rotated keyhole imaging and active tip-tracking for interventional procedure guidance.

The tools and devices used for patient treatment during interventional procedures must be accurately and safely localized. Recently, procedure guidance has been performed increasingly with MRI, but tool localization has been performed primarily by analyzing the lack of signal caused by displaced excited tissue and/or susceptibility artifacts. In this investigation, a new technique was developed to actively detect and visualize interventional tools using receive coils mounted on the tip of the interventional devices. Unlike earlier methods, in which images from small device-mounted coils are super-imposed on a previously acquired image data set, our localization method uses similar tip-mounted tools in combination with a radiofrequency (RF) switching circuit, standard imaging coils, and specially modified sequences to toggle between the standard and tip-mounted receive coil within a single fluoroscopic mode sequence. With this technique, the misregistration between the reported anatomic location and true location of interventional devices is minimized.

Chemical and microphysical characterization of ambient aerosols with the aerodyne aerosol mass spectrometer.

The application of mass spectrometric techniques to the real-time measurement and characterization of aerosols represents a significant advance in the field of atmospheric science. This review focuses on the aerosol mass spectrometer (AMS), an instrument designed and developed at Aerodyne Research, Inc. (ARI) that is the most widely used thermal vaporization AMS. The AMS uses aerodynamic lens inlet technology together with thermal vaporization and electron-impact mass spectrometry to measure the real-time non-refractory (NR) chemical speciation and mass loading as a function of particle size of fine aerosol particles with aerodynamic diameters between approximately 50 and 1,000 nm. The original AMS utilizes a quadrupole mass spectrometer (Q) with electron impact (EI) ionization and produces ensemble average data of particle properties. Later versions employ time-of-flight (ToF) mass spectrometers and can produce full mass spectral data for single particles. This manuscript presents a detailed discussion of the strengths and limitations of the AMS measurement approach and reviews how the measurements are used to characterize particle properties. Results from selected laboratory experiments and field measurement campaigns are also presented to highlight the different applications of this instrument. Recent instrumental developments, such as the incorporation of softer ionization techniques (vacuum ultraviolet (VUV) photo-ionization, Li+ ion, and electron attachment) and high-resolution ToF mass spectrometers, that yield more detailed information about the organic aerosol component are also described.

Drug switching patterns among patients taking non-steroidal anti-inflammatory drugs: a retrospective cohort study of a general practitioners database in the United Kingdom.

OBJECTIVE: To examine the frequency and determinants of switching between different non-steroidal anti-inflammatory drugs (NSAIDs) and the relationship with co-prescription of gastro-protective drugs (GPDs). DESIGN: This was an analysis of 30,654 patients receiving a total of 209,140 NSAID prescriptions in the UK from 1 January 1997 to 31 December 1998 identified through the MediPlus database. Analyses examined switching, repeat, termination and GPD co-prescription rates in new and continuing takers according to age and sex. RESULTS: Each patient received an average of 6.8 prescriptions in the year of study. Of the prescriptions 72.2% were for one of three NSAIDs, ibuprofen, diclofenac, or naproxen, and 7.2% of prescriptions were for fixed combination products of an NSAID plus a gastroprotective drug. At least 16.0% of continuing takers, and 28.5% of new takers switched to another NSAID in the review period. On average, new patients switched more frequently than continuing patients (0.39 switches/patient/year versus 0.23 switches/patient/year, p < 0.001). Switching between NSAIDs decreased with age and was less common in women (p < 0.05). Switching was associated with a 24% and 33% increased probability of GPD prescription in new and continuing takers, respectively. DISCUSSION: The frequency of switching, and of GPD co-prescription at switching, suggest that dissatisfaction with NSAIDs is frequent, and that gastrointestinal intolerance is a common feature of this dissatisfaction.

Harmonic distortion from nonlinear systems with broadband inputs: applications to lung mechanics.

We present a simple index, extended harmonic distortion (kd), to represent the degree of system nonlinearity under sparse pseudorandom noise inputs (SPRN). The frequencies in a SPRN waveform are neither harmonics nor sums or differences of the other component frequencies. Expressed by percentage, the kd is the square root of the ratio of output power at non-input frequencies to the total output power. We evoke three simple corrections to recover the true kd under imperfect SPRN inputs. Simulations on two block-structured nonlinear models (Wiener and Hammerstein) demonstrate the necessity and effectiveness of these corrections especially for the Wiener-type nonlinearity. By applying kd to pressure-flow data of dog lungs, we found that the nonlinear harmonic interactions from a lung arise primarily from its tissues most likely the processes governing the tissue stiffness. This nonlinearity, assessed from kd, is stronger at higher tidal volumes and enhanced (but to a lesser degree) during bronchoconstriction. We conclude that since the kd approach avoids the necessity for multiple-input measurements and lengthy data records, it may be useful for tracking the dynamic variations in nonlinearities of a physiological system.
