OBJECTIVE: To investigate whether methylenetetrahydrofolate reductase (MTHFR) gene C677T polymorphism is linked with coronary heart disease (CHD). METHODS: Transmission/disequilibrium test(TDT), sib transmission/disequilibrium test(STDT), and sibship disequilibrium test(SDT) were used. Forty-five CHD pedigrees with at least one CHD patient in the first degree relatives of probands were recruited from Oct. 1998 to Feb. 1999. Among those recruited were 21, 2 and 22 pedigrees with the genotypes of both parents known, one parental genotype unknown and both unknown, respectively. MTHFR genotype was measured by PCR-RFLP technique. RESULTS: Neither the TDT for 23 nuclear families with at least one parental genotype known or the STDT and SDT for 40 sibships found significant difference between the transmitted and untransmitted MTHFR gene 677T allele distributions. CONCLUSION: The above results suggest that MTHFR gene 677T allele is probably not linked with CHD in Chinese population.
Oncogenic Ras induces cells to undergo apoptosis after inhibition of protein kinase C (PKC) activity. The integration of differential signaling pathways is required for full execution of apoptosis. In this study, we used Jurkat as well as Fas/FADD-defective cell lines expressing v-ras to determine the upstream elements required for activation of the caspase cascade in PKC/Ras-mediated apoptosis. During this Ras-induced apoptotic process, caspase-8 was activated, possibly through its binding to Fas-associated death domain (FADD), in Jurkat/ras and Jurkat/Fas(m)/ras cells but not in Jurkat/FADD(m)/ras cells. c-Jun NH(2)-terminal kinase (JNK) was activated in all three cell lines expressing ras in response to apoptotic stimulation. Suppression of JNK by dn-JNK1 blocked the interaction of FADD and caspase-8 and partially protected Jurkat/ras and Jurkat/Fas(m)/ras cells from apoptosis. However, dn-JNK1 had no effect on PKC/Ras-induced apoptosis in Jurkat/FADD(m)/ras cells. The results indicate that FADD/caspase-8 signaling is involved in PKC/Ras-mediated apoptosis, and JNK may be an upstream effector of caspase activation.
Pollution of heavy metal from industrial wastewater is a worldwide environmental problem. Biosorption is an effective technology for the treatment of low concentration industrial wastewater. The purpose of this study was to evaluate the heavy metal uptake capacities of the biomass of marine red tide Prorocentrum micans. All red tide biomass used in this study were obtained from cultivation in our lab. Batch experimental results showed that living and non-living biomass of P. micans killed by CH2O were similar setting property for Cu2+, Pb2+, Ni2+, Zn2+, Ag+ and Cd2+. This study indicated that biomass of P. micans has a high capacities for above six heavy metal and the kinetics under the conditions studied were relatively fast. About 90% of the biosorption occurred within 10 min and an equilibrium was reached in 30 min. The Biosorption capacities for heavy metal ions were strongly dependent on pH of the solution, and higher capacities were obtained at pH around 5 and higher. Bonding and FIR of P. micans polysaccharide obtained by the hot water method for Cu2+, Pb2+, Ni2+, Zn2+, Ag+ and Cd2+ was also studied. The study indicated that -OH and -CONH2 groups were active center of biosorption.
OBJECTIVE: The aim of this study is to measure the value of 2D-CT and 3D-CT in diagnosing mid-facial fractures. METHODS: 50 patients who suffered from facial trauma were scanned with both conventional axial-CT (3 mm thickness) and 3D reconstruction (surface shaded display, SSD). The scanning region was ranged from the upper teeth to superior edge of the orbit. Coronal-CT imaging was performed in 21 patients in order to compare with that of axial and 3D CT images. RESULTS: Axial 2D-CT can be better to identify facial fractures than 3D reconstruction in such sites that are medial wall of the maxilla (5/0 sides), lateroposterior wall of the maxilla (49/44 sides), upper alveoli (14/12 sides), lateral wall of the orbit (34/31 sides), and pterygoid process of the sphenoid bone (19/9 sides). Three-dimensional reconstruction was superior to 2D-CT in demonstrating fracture of anterior wall of the maxilla (60/57 sides) and shapes of the facial fracture. 2D-CT is similar to 3D-CT in diagnosing fractures of nasal bone (20 sides), zygomatic bone and arch (24 sides). Owing to the partial volume effect, the medial wall of normal or abnormal orbit could not be correctly evaluated on 3D reformation images. CONCLUSION: It is concluded that 2D-CT should be regarded as the foundations of diagnosing facial fractures, and 3D-reconstruction as a useful complementary tool.
OBJECTIVE: To observe the functional rehabilitation of injured peripheral nerve with electric acupuncture. METHODS: Sciatic nerve injury model was established by transection of left sciatic nerve in 60 Wistar rats, which were randomly divided into two groups. The experimental group was treated with electroacupuncture, no treatment in the control group. Change of nerve electrophysiological, power of muscle and sciatic functional index (SFI) were observed. RESULTS: Nerve muscle-action potential (MAP) and motor nerve conduction velocity (MNCV) in the experimental group were better than that of the control group (P < 0.01). The single muscle twitch and tetanization of gastrocnemius muscle were higher in the experimental group too (P < 0.05). SFI were significantly higher in the experimental group (P < 0.05). CONCLUSION: Electric acupuncture therapy can improve functional rehabilitation of injured peripheral nerve.
OBJECTIVE: To investigate early influence on anterior ocular segment of diabetic retinopathy (DR) by retinal photocoagulation. METHODS: Ultrasound biomicroscopy (UBM) was applied to obtain and analyze quantitatively the changes of anterior ocular segment, including the anterior chamber, anterior chamber angle, ciliary body and choroid before the retinal photocoagulation and on the 3rd and 7th day after the therapy in 36 eyes with DR. RESULTS: Significant elevating of intraocular pressure (IOP), narrowing of chamber angle and slight shallowing of anterior chamber were observed on the 3rd day after retinal photocoagulation. Ciliary detachment and choroidal detachment were observed simultaneously in 29 eyes (80.6%). Ciliary detachment or choroidal detachment occurred singly in 3 eyes (8.3%) on the 3rd day and most of them disappeared on the 7th day spontaneously The height of ciliochoroidal detachment in the superior quadrant is greater than that in other quadrants. The severity of ciliochoroidal detachment is negatively correlated to the age, not correlated to fast blood sugar, duration of diabetes and duration of DR. CONCLUSION: Temporary ciliochoroidal detachment with changes of anterior chamber angle and IOP may be a common complication after retinal photocoagulation that should be designed properly.
OBJECTIVE: To clarify the role of sexual related Y (SRY) gene detection in the diagnosis of gonadal dysgenesis. METHODS: Sixteen cases of gonadal dysgenesis were included in this study: 5 with androgen insensitivity syndrome, 1 with 17-alpha-hydroxylase deficiency, 4 with true hermaphrodite, 2 with 45, X/46, XY gonadal dysgenesis, 1 with 45, X gonadal dysgenesis, 1 with XY pure gonadal dysgenesis, 1 with testicular regression, and 1 XY female who gave birth to a normal baby. SRY gene was detected by using polymerase chain reaction (PCR) in blood and gonad samples and by direct sequencing of the SRY motif. RESULTS: Among the 16 cases, 15 were blood SRY positive, among which 13 (86.7%) showed the presence of testicular tissue, and 2 showed ovaries without testicular tissue. One SRY negative case showed the presence of testicular tissue. In 3 cases, SRY detection in gonadal tissue correlated with pathological findings but not with blood karyotype. The correlation between peripheral blood SRY and the pathology of the gonads was 81.25% and the correlation between the presence of peripheral blood Y chromosome and pathology of the gonads was 68.75%. Sequencing of the SRY motif in an XY female who gave birth to a normal baby showed no mutation. CONCLUSIONS: SRY detection is more sensitive and specific than blood karyotype in the prediction of the presence of testicular tissue. Peripheral blood karyotype does not necessarily reflect gonadal type. There may be testicular related factors other than the SRY gene.
OBJECTIVE: To evaluate the safety and efficacy of antegrade selective cerebral perfusion (ASCP) during aortic arch surgery as a means of extending the safe period of systemic circulatory arrest using multimodality neuromonitoring to objectively quantify the physiologic responses. METHODS: In twenty-two patients (all less than age 60) scheduled for repair of an aortic arch aneurysm, preoperative verification of effective collateral perfusion through both the carotid and vertebrobasilar arterial systems was documented with transcranial Doppler ultrasonography (TCD). During cardiopulmonary bypass, the sole arterial inflow from the pump was via the right subclavian artery. The magnitude of ASCP was quantified by TCD using peak middle cerebral artery velocity, while flow adequacy was measured by continuous regional cerebrovenous oxygen saturation (rSO2) using dual-wavelength spatially resolved near-infrared spectroscopy. RESULTS: All patients experienced an uneventful recovery. Flow in the middle cerebral artery became undetectable at ASCP < 5 ml.kg-1.min-1, so adjustments from a 15-20 ml.kg-1.min-1 baseline were used to maintain rSO2 above 50%. Furthermore, ASCP flow was highly correlated (P < 0.01) with both peak middle cerebral artery velocity and rSO2(r = 0.86 and 0.96, respectively). CONCLUSION: Neuromonitoring guided ASCP may be expected to extend the safe period and is at least partly responsible for the absence of neurologic complications in this patient cohort.
OBJECTIVE: To study the relationship between the quantities of HPV16 E6 gene in cervical tissues and the course of cervical disease. METHODS: The number of copies of HPV16 E6 gene was detected by competitive PCR in 44 positive samples including 20 cases of chronic cervicitis, 6 cases of cervical dysplasia and 18 cases of cervical cancer. RESULTS: The mean copies of HPV16 E6 gene in chronic cervicitis, cervical dysplasia and cervical cancer were 6.16, 5.33, and 6.45 copies in each microgram of cervical tissue respectively. The quantities of HPV16 E6 gene were not significantly different between cervical dysplasia and chronic cervicitis (P<0.01). CONCLUSIONS: This study underscores the importance of the relationship between the quantities of HPV16 E6 gene and the course of cervical disease. It also suggests that quantification of HPV16 E6 gene may be useful as a prognostic tool to identify women who are at increased risk of developing cervical cancer.
OBJECTIVE: To make better the RP-HPLC method for the determination of roxithromycin(RM) in human serum. METHODS: RM and clarithromycin (internal standard) were extracted from alkalinized serum sample (500 microliters) with methylene chloride. After evaporation of the organic layer, the residue was dissolved in 100 microliters of acetonitrile-ammonium phosphate (1:1, pH 6.0) and washed with n-hexane, then 20 microliters was injected onto a column (5 microns, 15 cm x 4.6 mm) of Penomenex luna C18. The mobile phase was acetonitrile-0.05 mol/L phosphoric acid (39:19:42, adjusted to pH 7.2 with ammonia water) pumped at 1.2 ml/min through the column. The variable wavelength UV detector operated at 0.01 aufs and the wavelength was set at 210 nm. RESULTS: The retention times for RM and clarithromycin were 4.4 min and 5.0 min, respectively. Standard curve was linear in the concentration range of 0.25 to 32 mg/L. The detection limit in serum was 0.06 mg/L; the average method recovery 97.4%; the inter-day RSD less than 3.0%. CONCLUSION: This method was found to be simple, rapid, sensitive and accurate for determination of RM in human serum.
OBJECTIVE: To study the state of proliferation and apoptotic cell death in cholesteatoma. METHOD: 20 cholesteatoma samples and 10 normal external ear canal skin samples were studied with immunohistochemical SABC technique and TUNEL technique. RESULT: Immunohistochemical analysis using a monoclonal antibody to proliferation cell nuclear antigen (PCNA) demonstrated the presence of proliferating cell in the basal cell layer of normal external ear canal skin, whereas in the cholesteatoma tissue samples larger numbers of proliferation cell nuclear antigen-positive cells were also observed in the prickle and granular cell layers. The PCNA labeling index and the mean optical density were (10.52 +/- 2.65)% and (0.1340 +/- 0.0363) in normal external ear canal skin samples and (36.91 +/- 22.77)% and (0.2427 +/- 0.0586) in cholesteatoma tissue samples. In cholesteatoma tissue samples and normal external ear canal skin samples, apoptotic cell were observed in the prickle and granular cell layers. The apoptotic indexers were (27.50 +/- 12.50)% in cholesteatoma tissue samples and (9.96 +/- 3.86)% in skin samples. CONCLUSION: The results demonstrate the rates of apoptotic cell death and cell proliferation in cholesteatoma are higher than those in normal skin. Increased cell proliferation and cell death will contribute to increased keratous debris.
OBJECTIVE: To elucidate possible functional subdivisions in the guinea pig accessory olfactory bulb. METHOD: The guinea pig accessory olfactory bulbs were cut in sagittal slice. Bipolar tungsten electrodes were inserted into anterior or posterior vomeronasal nerve layers and single square-pulses were delivered through the electrodes to activate afferent fibres. Glass microelectrodes were used to record extracellular field potentials of anterior or posterior external plexiform layers. RESULT: A single shook of the VNL provoked a characteristic damped oscillatory field potential and the oscillation in the pAOB was more distinct in wave form and longer in duration than those in the aAOB. The stimulation of anterior VNL elicited field potentials exelusively in the anterior region of EPL, whereas shocks to the posterior VNL provoked oscillatory responses only within the posterior EPL. CONCLUSION: The accessory olfactory bulb in the guinea-pig is distinctly segregated into the anterior and posterior subdivisions and an anatomical boundary exists in both regions. The results suggested that there are at least two different input-output pathways in vomeronasal systems.
OBJECTIVE: To elevate the effect of uvulopalatopharyngoplasty (UPPP) and minimize the postoperative complications. METHOD: The data of 70 patients with obstructive sleep apnea syndrome diagnosed with polysomnography were analyzed. RESULT: 15 cases (21%) reported unchanged snoring and apnea and hyponea index. 55 cases improved after UPPP for at least 6 months. The complications included hypertensive crisis (2 cases), respiratory embarrassment (4 cases) in period of paretic UPPP. After UPPP, The complications included local hemorrhage (1 case), wound dehiscence (2 cases), temporal velopalatal insufficiency (9 cases), one had suffocation after extraction of endotracheal tube. CONCLUSION: Our results indicate that the strategies of improving the effect and avoiding the complications include: Excision of the lateral pharyngeal wall in retropalatal region and fat tissue in soft palate. Protection of palatosalpingeus and palatostaphylinus. Part preservation of uvula. Confirmation of the obstructive region. Choice of the anesthesia and preventive tracheotomy. Modified tracheotomy were safe and convenient when exchanging trachea canula.
PURPOSE: To improve our diagnostic technique through the analysis of clinical features of Leber's hereditary optic neuropathy (LHON) harboring mtDNA point mutation at nt11778. METHODS: Detection of nt11778 mutation was performed on 38 patients clinically diagnosed as LHON in our ophthalmic center from year 1998 to 2000. Circumstances of onset and family history were obtained and ophthalmoscopy, fundus fluorescein angiography, visual field and visual evoked potential were performed on all 38 patients. RESULT: 30 In 38 patients (78.95%) harbor nt11778 mutation, including 28 male (93.33%) and 2 female (6.67%). The ratio of affected male to female is 14:1. Patients harboring nt11778 mutation display typical clinical manifestations. CONCLUSION: Identification of one of the three LHON specifically associated mtDNA mutations is essential to confirm the diagnosis.
PURPOSE: To study the change of visual acuity and related factors in idiopathic optic neuritis. METHODS: Retrospectively studying 136 cases (210 eyes) of idiopathic optic neuritis hospitalized in our department during last 5 years. The data was analyzed with the methods of multivariate stepwise regression analysis by SPSS statistic package. RESULTS: The final visual acuity was very strongly related positively to the initial visual acuity at the time point of starting treatment and negatively to the time span beginning the treatment to the time showing improvement, age of patient and the duration from onset of the disorder to the time point of treatment beginning. CONCLUSIONS: Promptly right treatment can significantly change the natural course and the final vision of optic neuritis. Severe visual loss, old age, longer time without right therapy and slow improvement after the treatment may indicate an unfavorable final visual recovery. The different dosages of steroid applied in this study was not significant in view of the final visual acuity.
PURPOSE: To report a case of idiopathic polypoidal choroidal vasculopathy (IPCV) and its angiographic characteristics. METHODS: General ophthalmic check up, analyses of fundus picture, funds fluorescein angiography (FFA), indocyanine green angiography (ICGA) as well as literature review. RESULTS: The lack of typical choroidal neovascularization (CNV) on FFA and characteristic polypoidal choroidal vasculopathy on ICGA as well as its unsymmetrical manifestation and discrepancy with age-related macular degeneratio (AMD) on the fundus picture are typical signs for the diagnosis of IPCV. CONCLUSION: The angiographic features on FFA and ICGA are specific for the diagnosis of IPCV. Differentiating IPCV and AMD has clinical significance in prognosing the visual acuity, progress and management of the diseases.
Telemedicine is a hot research focus. When we develop the telemedicine system, we find that in China the network condition can not fit the demands of the transregional telemedicine service. In order to improve the poor conditions, we have designed a set of telemedicine system based on distributed multilevel server network model. The paper expounds the structure and the working process of the system, then goes into the technology details in realizing it, finally discusses some relative questions.
