Esophageal reconstruction by jejunal transfer.

Forty-five esophageal reconstructions were carried out using a free or pedicled transfer of jejunal segments. Thirty were performed for benign stenosis or atresia, and 13 were performed for malignancy. The remaining two were used to repair anastomotic leakage or fistulization resulting from prior esophagogastrostomy. Average anastomotic arterial and venous diameters were 1.2 mm and 3.0 mm respectively. Of the 45 reconstructions, 44 were successful; the single failure was the result of tearing of the mesenteric arcade. There were four fatalities. Jejunal transfer is an effective method of esophageal reconstruction.

Molecular characterization of the maize Rp1-D rust resistance haplotype and its mutants.

The Rp1-D gene for resistance to maize common rust (Puccinia sorghi) is a member of a complex locus (haplotype) composed of Rp1-D and approximately eight other gene homologs. The identity of Rp1-D was demonstrated by using two independent gene-tagging approaches with the transposons Mutator and Dissociation. PIC20, a disease resistance (R) gene analog probe previously mapped to the rp1 locus, detected insertion of Dissociation in an Rp1-D mutation and excision in three revertants. Independent libraries probed with the PIC20 or Mutator probes resulted in isolation of the same gene sequence. Rp1-D belongs to the nucleotide binding site, leucine-rich repeat class of R genes. However, unlike the rust resistance genes M and L6 from flax, the maize Rp1-D gene does not encode an N-terminal domain with similarity to the signal transduction domains of the Drosophila Toll protein and mammalian interleukin-1 receptor. Although the abundance of transcripts of genes from the rp1 complex changed with leaf age, there was no evidence of any change due to inoculation with avirulent or virulent rust biotypes. A set of 27 Rp1-D mutants displayed at least nine different deletions of Rp1-D gene family members that were consistent with unequal crossing-over events. One mutation (Rp1-D*-24) resulted in deletion of all but one gene family member. Other unique deletions were observed in the disease lesion mimic Rp1-D*-21 and the partially susceptible mutant Rp1-D*-5. Different rp1 specificities have distinct DNA fingerprints (haplotypes). Analysis of recombinants between rp1 specificities indicated that recombination had occurred within the rp1 gene complex. Similar analyses indicated that the rust R genes at the rp5 locus, 2 centimorgans distal to rp1, are not closely related to Rp1-D.

Efficiency of leukocyte removal by filters made of superfine glass fiber membranes.

BACKGROUND AND OBJECTIVES: To demonstrate the application of leukocyte removal filters made of a new type of filter material - superfine glass fiber - for depleting leukocytes in SAGM red cell suspensions and preventing nonhemolytic transfusion reactions. MATERIALS AND METHODS: The extent of leukocyte depletion and red cell recovery was based on cell counts. Trace leukocytes were counted in a 50-microl Nageotte counting chamber or by using a flow cytometer. The chemical stability of the glass fiber membranes was studied by plasma emission spectrometer and by measuring the ion content and weighing nonvolatile matter in water extract. The structural stability of the glass fiber membranes was studied by a micropore-filter membrane method. RESULTS: Leukocyte removal filters made of superfine glass fiber membranes removed more than 99.0% of leukocytes in SAGM red cell suspensions prepared from 400 ml whole blood. Red cell recovery exceeded 90%, and the total number of residual leukocytes was less than 5x10(6). A water extract of the glass fiber membranes contained only traces of Si4+ and Ca2+ and less than 2 mg/100 ml of nonvolatile matter. No broken or loose fibers were found in the filters. Scanning electron microscopy showed that the web structure of the glass fiber membranes was instrumental in trapping and holding leukocytes. CONCLUSION: A filter made of glass fiber membranes is effective in leukocyte depletion.

[Cancer mortality in high background radiation area of Yangjiang, China, 1979-1995].

OBJECTIVE: To estimate cancer risk associated with the low level radiation exposure of average annual effective dose of 6.4 mSv in HBRA the high background radiation area (HBRA) of Yangjiang, China. METHODS: The cancer mortality data of 1979-1986 were collected from a dynamic cohort by prospective survey. The data of 1987-1995 were obtained from a fixed cohort by retrospective and/or prospective survey. The mortality investigation on the spot consisted of two steps, i.e. the follow-up of the members in the cohort and the ascertainment of the death causes. The estimate of cumulative individual dose of the cohort members included that of the exposure from natural external and internal sources. Both direct (TLD measurement) and indirect (environmental measurement and occupancy pattern) approaches were used for individual external dose estimate. On the basis of the hamlet-specific average annual external dose, the cohort members were classified into four groups for internal comparison: high, medial and low dose groups from HBRA and control group from control area (CA). Relative risk (RR) and excess relative risk coefficient (ERR per sievert) and its 95% confidence interval (CI) was estimated using AMFIT program in Epicure. RESULTS: During the period 1979-1995, there were 10,415 total deaths and 1003 cancer deaths among 1,698,350 person-years at risk in the cohort of 125,079 subjects. The adjusted RR (95% CI) with sex and age group for all cancers of whole HBRA was 0.99 (0.87-1.14). As for the site-specific cancer of whole HBRA, the RRs of cancers of stomach, colon, liver, lungs, bone, female breast, and thyroid were less than one, while the RRs of cancers of nasopharynx, esophagus, rectum, pancreas, skin, cervix uterus, brain and central nervous system, leukemia and lymphoma were larger than one. However, all of them except for esophagus cancer were not different statistically from one (P > 0.05). The homogeneity tests of RRs for all cancers and for site-specific cancer among the three dose groups in HBRA revealed that the RRs in these dose groups were not different statistically (P > 0.05) for all. The ERR (95% CI)/Sv of all cancers for both sexes and all ages was -0.10 (-0.67, 0.69). CONCLUSION: An increased cancer risk associated with the high levels of natural radiation in HBRA was not found. On the contrary, the mortality of all cancers in HBRA was generally lower than that in CA, but not significant statistically.

[The role of NF-kappa B in the TNF-alpha-induced endothelial cell apoptosis].

OBJECTIVE: To investigate whether TNF-alpha could activate the signaling pathway-NF-kappa B/I-kappa B alpha required for the expression of inducible nitric oxide synthase (iNOS) to induce endothelial cell apoptosis by nitric oxide (NO). METHODS: Aminoguanidine (AG, an inhibitor of iNOS) and Pyrrolidine dithiocarbamate (PDTC, an inhibitor of NF-kappa B) were evaluated for their influence on the apoptosis of cultured bovine pulmonary artery endothelial cells (BPAEC) induced by TNF-alpha. Apoptosis was confirmed by morphology, ultrastructural observation, agarose gel electrophoresis of DNA, and percentage of DNA fragmentation. Western blot analyses were used to detect the expression of iNOS and the levels of I-kappa B alpha in BPAEC exposed to TNF-alpha. Electrophoretic mobility shift assays (EMSA) were used to determinate the activity of NF-kappa B in BPAEC. RESULTS: TNF-alpha induced BPAEC apoptosis (18.0 +/- 4.3)% in a concentration- and time-dependent manner. Both AG and PDTC attenuated the apoptosis of BPAEC induced by TNF-alpha, with peventages of DNA fragmentation in 2, 4, 10 mmol/L AG being (10.0 +/- 2.2)%, (7.8 +/- 1.2)% and (8.2 +/- 1.3)% vespectively. In BPAEC after TNF-alpha exposure, Western blot analyses revealed the expression of iNOS and the decrease in I-kappa B alpha. EMSA demonstrated an impermanency increase in NF-kappa B binding activity. CONCLUSIONS: TNF-alpha can induce endothelial cell apoptosis by NO, which is produced by increasing iNOS expression and activating the signal pathway-NF-kappa B.

[Extensive transbasal approach for removal of tumours in the nasal, sphenoid and clival regions].

OBJECTIVE: To improve operative results of removal of the tumors in midline cranial base and the reconstruction of defect at frontal base through modification of the operative approach and techniques. METHODS: Extensive transbasal approach was used in 15 patients with tumours in the nasal, sphenoid and clival regions. RESULTS: The tumor was removed in 10 patients, near totally removed in 4 and partial removed in 1. The operative results of all patients were excellent except an elderly patient with mild operative morbidity. CONCLUSIONS: The procedure we used expand the operative indications for the tumors situated at the midline cranial base and improve the operative results. The procedure has advantages such as wide operative space, reliable reconstruction of the frontal base, and no severe complications.

[Experimental study of osseointegrated dental implants placed into fresh extraction sockets in dogs].

OBJECTIVE: To investigate the better way in treating bone defects around implants. METHODS: 12 mongrel dogs, pure titanium implants were immediately inserted in the mesial roots of the second, third and fourth mandibular premolar sockets. The defects around implants were separately filled with demineralized freeze-dried bone allograft (DFDBA), demineralized freeze-dried allocancellous in combination with bBMP (bBMP compound bone) and nothing. RESULTS: Histologic observation showed that bone defects around implants were substituted by new bone in DFDBA group at 12 weeks and in bBMP compound bone group at 8 weeks, and also the osseointegration were obtained between implants and bone, but bone defects were only filled about 3/5 in no graft group at 12 weeks. CONCLUSION: Immediate implants could obtain good osseointegration if the defects around implants were treated properly and the right technique was used, and also the use of bBMP compound bone could promote obviously bone formation in defects around implants.

[The effects of transforming growth factor alpha and beta 1 on the proliferation of alveolar type II cells in vitro].

OBJECTIVE: To investigate the effect of transforming growth factor alpha and beta(1) (TGFalpha and beta(1)) on the proliferation of alveolar type II cells in vitro and the molecular mechanism related. METHODS: (3)H-TdR incorporation and cell counting for the assay of cell growth; dot blot, in situ hybridization, and immunohistochemical methods for analysing the expression of cyclin D1 and cycle-dependent kinase 4 (CDK4) genes. RESULTS: With increasing of the concentration of TGFalpha and beta(1) (0.01 ng-100 ng/ml), TGFalpha enabled to increase (3)H-TdR incorporation and the number of alveolar type II cells, while the effect of TGFbeta(1) was opposite and both effects were dose-dependent (P < 0.01). TGFalpha-treated cells decreased the expression of CDK4 mRNA, and the relevant in comparing with the control (P < 0.01). On the contrary, TGFbeta(1)-treated cells decreased the expression of CDK4 mRNA), CDK4 as well as the cyclin D1 proteins (P < 0.05). CONCLUSION: Both TGFalpha and beta(1) were considered implicated in the regulation of proliferation of cultured alveolar type II cells isolated from the adult rats and CDK4 might be a common target against TGFalpha and beta(1) signals.

[An anatomical investigation relating to the stapes elevation surgery].

OBJECTIVE: For the purpose of improving patient's hearing in stapes elevation surgery. METHOD: The anatomical features relating to this operation were observed and measured in 30 ears of fresh cadavers, which included the length of lenticular process, the highness of stapes head and neck, the thickness of lower border of footplate, the angle of incudostapecial joint and the gradients of upper and lower wall of vestibular niche. The relation between two types of the present operation and the investigation were discussed. RESULT: The two types of present operations were in accord with the physiologicoanatomical features of the stapes and incus. CONCLUSION: In order to improve the patient's hearing, making use of biobinder for strengthening the incudostapedial connexion as a routine is suggested.

[The prospects of research on VR rehabilitation engineering].

In this paper, we present the recent development of Vitual Reality (VR) technology and rehabilitation engineering and discuss the rationality and feasibility of the application of VR to rehabilitation. With VR technology, psychological induction can be better introduced in rehabilitating exercises and patients can be tremendously released from fret during traditional physical training for rehabilitation. The VR is expected to bring about a technological revolution in rehabilitation engineering.

[Some basic techniques of virtual reality body healthiness vehicles for rehabilitation].

Some know-how techniques of virtual reality rehabilitation vehicles are discussed in this paper after a brief introduction of the application background and the structure and functions of the vehicles. This kind of vehicles can be used for psychological treatment during rehabilitation. Pateints can select different modes of physical exercises according to their individual situations, and therefore they can get rehabilitation much faster and more easily than those under the traditional treatments.

[Dexamethasone inhibits IL-5-induced eosinophil survival].

OBJECTIVE: To assess the effect of dexamethasone (Dex) on interleukine-5-induced survival of eosinophils (EOS) in vitro. METHODS: EOS were purified from guinea pigs by Percoll density gradient and incubated for 4 days in the presence of IL-5 with or without Dex. Cell viabilities were determined by staining cells with fluorescein diacetate and propidium iodide. RESULTS: In the absence of Dex, IL-5 enhanced EOS survival was inhibited in a dosedependent manner. Inhibition was time-dependent and required at least 2 days exposure of EOS to Dex. When EOS were incubated with varying concentrations of IL-5 and 1,000 nmol/l Dex, survival inhibition was reduced at higher concentration of IL-5, and completely abolished by IL-5 1,000 pg/ml. CONCLUSIONS: Dex acts directly on mature EOS to decrease their viabilities, implying the Dex may be important in the treatment of allergic and other inflammatory eosinophilic disorders.

SUR-8, a conserved Ras-binding protein with leucine-rich repeats, positively regulates Ras-mediated signaling in C. elegans.

We describe the identification and characterization of a novel gene, sur-8, that positively regulates Ras-mediated signal transduction during C. elegans vulval development. Reduction of sur-8 function suppresses an activated ras mutation and dramatically enhances phenotypes of mpk-1 MAP kinase and ksr-1 mutations, while increase of sur-8 dosage enhances an activated ras mutation. sur-8 appears to act downstream of or in parallel to ras but upstream of raf. sur-8 encodes a conserved protein that is composed predominantly of leucine-rich repeats. The SUR-8 protein interacts directly with Ras but not with the Ras(P34G) mutant protein, suggesting that SUR-8 may mediate its effects through Ras binding. A structural and functional SUR-8 homolog in humans specifically binds K-Ras and N-Ras but not H-Ras in vitro.

AP30, a differential protein marker for perilymph and cerebrospinal fluid in middle ear fluid, has been purified and identified as human apolipoprotein D.

Using two-dimensional (2-D) gel electrophoresis, human perilymph and cerebrospinal fluid have been shown to be highly enriched for an acidic protein with MR 30,000, we designated it as AP30. The protein exhibits charge heterogeneity, with at least eight isoforms visible between pI 4.5 to 5.5 on 2-D gels. Purification of the protein was carried out by ammonium sulfate precipitation, polybuffer exchanger column chromatofocusing, and acetone fractional precipitation. The resulting preparation also contains eight spots in the acidic area of 2-D gels, and one broad band located at Mr 30,000 by SDS-PAGE. Digestion of AP30 with neuraminidase causes the isoforms to shift to a more basic position and to consolidate into two primary spots, indicating that AP30 is a variably sialylated glycoprotein. Amino acid analysis of AP30 revealed an amino acid content very similar to that of human apolipoprotein D. Attempts to determine the amino acid sequence demonstrated that the N-terminus is blocked. Edman sequencing of two peptide fragments, generated by cyanogen bromide cleavage of AP30, both revealed sequences having 100% identity to human apolipoprotein D. Western blot analysis of AP30 with the antibody against authentic human apolipoprotein D demonstrated a high degree of cross-reactivity. These studies indicate that AP30 from human perilymph and cerebrospinal fluid is a member of the apolipoprotein D family.

Destabilized PCNA trimers suppress defective Rfc1 proteins in vivo and in vitro.

Replication factor C (RFC) and the proliferating cell nuclear antigen (PCNA) are two essential DNA polymerase accessory proteins that are required for numerous aspects of DNA metabolism including DNA replication, DNA repair, and telomere metabolism. PCNA is a homotrimeric ring-shaped sliding DNA clamp that can facilitate DNA replication by tethering DNA polymerase delta or DNA polymerase epsilon to the DNA template. RFC is the 5-subunit multiprotein complex that loads PCNA onto DNA at primer-template junctions in an ATP-dependent reaction. All five of the RFC subunits share a set of related sequences (RFC boxes) that include nucleotide-binding consensus sequences. We report here that a mutation in the gene encoding the large subunit of yeast RFC gives rise to DNA metabolism defects that can be observed in vivo and in vitro. The rfc1-1 substitution (D513N) lies within the widely conserved RFC box VIII consensus sequence and results in phenotypes including DNA replication defects, increased sensitivity to DNA damaging agents, and elongated telomeres. Mutant Rfc1-1 complexes exhibit in vitro DNA replication defects that are sensitive to ATP concentrations, and these defects can be suppressed by mutant PCNA proteins which contain substitutions that destabilize the homotrimeric sliding DNA clamp.
