Long-term expression of the 35,000 mol. wt fos-related antigen in rat brain after kainic acid treatment.

Systemic injection of kainic acid, a rigid analogue of glutamate, induces both the short-term and the long-term expression of activator protein-1 transcription factors. The short-term responses of activator protein-1 factors such as c-fos and fos-related antigens have been well studied. However, the long-term expression of activator protein-1 factor(s) induced by kainic acid is poorly understood. The present study was designed to document the long-term expression (up to seven months) of the fos-related antigens and to map their distributions in the rat brain after systemic treatment with kainic acid. A single dose of kainic acid (8 mg/kg) was injected i.p. into Fischer 344 rats and their epileptic seizure behaviour was monitored. The rats with full limbic seizures were chosen for long-term study. By using immunocytochemistry with an antibody that cross-reacts with all known fos-related antigens, western blot analysis and a gel mobility-shift assay, we have now shown that a 35,000 mol. wt fos-related antigen was induced by kainic acid treatment and expressed at high levels for up to five months. This fos-related antigen still maintains the activator protein-1 DNA binding activity in the rat brain seven months after kainic acid treatment. The fos-related antigens and activator protein-1 binding activity were continuously expressed at high levels throughout the experimental period in the dentate granule cells where mossy fibre collateral sprouting occurred after kainic acid treatment. Our results suggested that long-term expression of fos-related antigen may reflect the pathophysiological changes after kainic acid administration.

[Surgical treatment of primary mediastinal tumor: a report of 90 cases].

In the past 22 years 90 cases of pathologically confirmed primary mediastinal tumors (PMT) were surgically treated. Of these 90 cases, there were 38 thymomas (42.3%), 19 teratoblastomas (21.2%), 9 neurofibromas (10.0%) and 24 others (26.5%). Radical excision was performed in 70 (77.8%), palliative excision in 6 (6.7%), exploratory thoracotomy in 12 (13.3%). Postoperative death occurred in 2 (2.2%). No relapse was reported in patients who had received radical excision during the follow-up period of 3 months to 13 years. Besides careful history taking and physical examination, X-ray examinations of the chest, particularly CT scan were valuable in the diagnosis and differential diagnosis of PMT. Difficulties in diagnosis were due to unusual pathological changes and clinical manifestations. Due precautionary measures should be taken to avoid injury to the heart, major blood vessels and the spinal cord. Cord injury was the consequence of direct operative trauma, compression due to intraspinal hemorrhage, and/or operative interruption of blood supply.

[Distribution and ischemia-induced changes of nitric oxide synthase-positive neurons in the cingulate areas in rats].

Distribution of nitric oxide synthase (NOS)-positive neurons of cingulate areas and its changes following bilateral common carotid artery occlusion were investigated with nicotinamide adenosine dinucleotide phosphate diaphoras (NADPH-d) reaction. Positively stained NOS-neurons were observed in the areas 24, 32, and 25 of the anterior cingulate cortex, and in the areas 23 and 29 of the posterior cingulate cortex. After bilateral common carotid artery occlusion, the results showed that the NOS-positive neurons increased markedly in the areas 24, 32, 25, 23 and 29 of the cingulate cortex (P < 0.01).

Predictive value of serum parathyroid hormone levels for bone turnover in patients on chronic maintenance dialysis.

With the increasing occurrence of adynamic bone disease, it is essential to determine the level of bone turnover in chronically dialyzed patients before instituting vitamin D therapy. To assess the value of serum parathyroid hormone (PTH) levels for prediction of bone turnover, we determined sensitivity, specificity, and predictive value positive of serum PTH, alone or in combination with other variables, in 79 patients who underwent one or two bone biopsies. Serum PTH levels were determined by a radioimmunometric assay and were obtained at the time of bone biopsies. Patients were classified into (1) low or normal and (2) high bone turnover according to the value of activation frequency of bone. There were 57 biopsy specimens taken from hemodialysis patients and 39 specimens from continuous ambulatory peritoneal dialysis patients (CAPD). All patients with serum PTH levels within or below the normal range had low or normal bone turnover. Values of serum PTH above 450 pg/mL were 100% and 95.5% specific for high bone turnover in hemodialysis and CAPD patients, respectively. Values of serum PTH between 65 and 450 pg/mL had worse predictive value positive in CAPD patients (48.6% to 78.6%) than in hemodialysis patients (67.3% to 87.1%). When other characteristics of the patients were taken into consideration, only age in hemodialysis patients and serum ionized calcium in CAPD patients improved the predictive value of serum PTH. All hemodialysis patients younger than 45 years of age with serum PTH levels above 65 pg/mL (n = 15) had high bone turnover, and CAPD patients with low or normal bone turnover had higher serum ionized calcium. However, overall, bone turnover could not be predicted by serum PTH measurements in 30% of hemodialysis and 51.3% of CAPD patients. The data suggest that for patients with serum PTH levels between 65 and 450 pg/mL, bone biopsies are indicated to precisely assess bone turnover prior to initiation of vitamin D therapy.

[Influence of age on severe pancreatitis].

Eighty-four patients of severe pancreatitis were divided by different line of age and the mortality of patients with severe pancreatitis above and below the line was comared. The results showed that the 60-year of age was the age with the lowest mortality. The analysis of 35 patients with severe pancreatitis who were older than 60 years of age indicated that multiple gallstones was the most common cause initiating the disease. The common severe concurrent diseases were hypertension, coronary disease, cerebrovascular disease, and respiratory disease. Common organ failures were ARDS, shock and heart failure. The number of failed organs in elderly patients in the fifth and seventh day after admission was obviously more than that in young patients.

[Laser ablation of intervertebral disc: animal experiment].

The lumbar intervertebral discs (L3-6) were ablated through a transperitoneal approach in 12 adult dogs by using Nd: YAG laser (1.06 microns) with a 600 microns quartz fiber. The status of limbs motion and sphincter (bladder, bowel) was observed for evaluating the safety of laser irradiation. After irradiation, the animals were sacrificed at prescribed intervals of up to 40 weeks (2, 4, 8, 12 and 40 weeks after operation). The lumbar intervertebral discs were harvested and subjected to light microscopic observation. No dog had suffered from neurogenic dysfunction of limb motion and sphincter. Histological findings immediately after the irradiation showed the disc was vaporized and a cavity was made. After 2 and 4 weeks, fibrous tissues began to proliferate, but cartilaginous tissues replaced the fibrous tissues 12 weeks after the laser irradiation. No new bone formation was found within 40 weeks after operation. On the basis of this study and our previous cadaveric study, percutaneous laser disc decompression (PLDD) was applied in clinical practice in march of 1993. 10 patients underwent PLDD utilizing the same laser equipment. The average follow-up was 3 months. According to the Macnab's criteria, there was an excellent response in 7 patients and a good response in 3.

[Percutaneous laser disc decompression: an experimental study].

The discs of 15 fresh human cadaveric spines (L1-S1) were radiated with Nd: YAG laser energies varying from 1000 to 3000J in 5 groups. The volume ablation rates, changes in intradisc pressure, and changes, in temperature at four important sites around the disc were measured during the laser radiation. The ablated volume was significantly greater at higher radiating energies. There was a significant decrease in the intradisc pressure after ablation of nucleus pulposus tissue. The increase in temperature of adjacent tissue was very low, not exceeding 3 degrees C. The results of this study provide a basis for the use of lasers in percutaneous lumbar discectomy, and in clinical practice in the future.

[Research on stomach yin deficiency syndrome due to abdominal surgery or severe acute abdominal diseases (I)].

The Spleen-Stomach theory is an important component of the theories of TCM. Exploration of pathophysiologic changes in patients with Stomach Yin Deficiency Syndrome (SYDS) is of help in explaining the Spleen-Stomach theory. Hence patients with SYDS due to abdominal operation or severe acute abdominal diseases were selected as objects to be observed. The endotoxin in plasma, immune function of the body, the mean temperature and blood perfusion rate at the centre of the tongue, area representing the Spleen and Stomach, the surface pH and index of electric conduction of the tongue and rheologic changes were determined. The results showed that the content of plasma endotoxin in patients with SYDS was 346 +/- 216 ng/ml (mean +/- SD). The value in all patients with SYDS was beyond normal (50 ng/ml), the highest being 1050 ng/ml. ANAE and LTT were decreased, as compared to the control (P less than 0.001). The content of IgG, IgA, CIC in blood increased (P less than 0.05, P less than 0.001, and P less than 0.001 respectively), and that of IgM decreased (P less than 0.002). The consistency of C3 in patients with SYDS was 83.6% of the control (P less than 0.001), and that of C4 and C5 increased (P less than 0.01 and P less than 0.001 respectively). The mean temperature of the tongue with SYDS was 0.64 degrees C lower than the control (P less than 0.05) and the blood perfusion rate was decreased 16.4% than the control (P less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

[Endocrine function in stomach yin deficiency syndrome (SYDS): research on patients with SYDS following abdominal surgery or severe acute abdominal diseases (II)].

Patients with SYDS following abdominal operation or with severe acute abdominal diseases were observed on changes of the thyroid, adrenal function and digestive hormones. The results showed that the serum concentration of T3 in patients with SYDS decreased by about 50% with the control (P less than 0.001). The concentration of T4 decreased too (P less than 0.001). The rT3 content increased and there were no obvious changes in the content of TSH and T3RUR. The manifestations conformed to "low T3 syndrome" in nonthyroid diseases. The amount of urine 17-KS and 17-OHCS per 24 hours decreased (P less than 0.001 and P less than 0.05 respectively). The serum gastrin level decreased by 60.4% with the control (P less than 0.002) and that of VIP increased (P less than 0.05). The results suggested that the patients had decreased thyroid and adrenal function and the secretion of hormones of digestive tract decreased.
