Oral-TPN-induced bacterial translocation and impaired immune defenses are reversed by refeeding.

Although certain defined diets have been shown to promote bacterial translocation (BT), the ability to reverse diet-induced BT has not previously been investigated. Furthermore, little is known about the effects of defined diets on host immune defenses. To address these questions, we measured BT and immune reactivity in rats fed a normal diet or enteral elemental (ORAL-TPN) diet. After 7 days on the elemental or normal diet, the rats were killed, and BT and mitogen responsiveness to concanavalin A and phytohemagglutinin were measured. In separate experiments, the effects of these diets on in vivo host defenses was measured with a Staphylococcus aureus abscess model. Additional experiments were performed to determine the time required to reverse ORAL-TPN-induced BT and impairment of host immune defenses by reinstituting normal feedings. Administration of the ORAL-TPN diet for 7 days was associated with BT to the mesenteric lymph node complex of all animals, decreased blastogenic response of blood and splenic lymphocytes to mitogens, and decreased ability to control an in vivo infectious challenge with S. aureus. Each of the derangements was reversed by the reinstitution of normal feedings. In summary, the enteral administration of an elemental diet for 7 days is associated with disruption of the gut microflora, BT, and the development of an immunocompromised state, all of which can be reversed by refeeding the animals a normal diet.

Effect of local and systemic burn microenvironment on neutrophil activation as assessed by complement receptor expression and morphology.

Previously, we documented that humoral factors, especially complement products, contained in burn wound blister fluid (BF) modulate normal neutrophil (PMN) function and metabolism. The goal of the current study was to examine the effects of the local (BF) and systemic (burn serum or plasma) burn microenvironment on PMN activation as assessed by complement receptor expression and morphology. Induction of CR1 (C3b) and CR3 (iC3b) receptor expression of normal PMNs after incubation in medium, BF, or plasma (serum) from healthy volunteers or burned patients was measured using monoclonal antibodies and flow cytometry. BF (10% v/v, 37 degrees C) induced 25% more CR1 receptor expression than control or burn plasma (p less than 0.05), while the levels of CR3 expression of PMNs incubated in BF (10% or 80% v/v at 37 degrees C) was more than 300% that found when the PMNs were incubated in medium, burn, or patient plasma (p less than 0.05). The electron microscopic appearance of PMNs incubated in these fluids documented that degranulation was greater when cells were incubated in BF than medium, control, or patient plasma. These results indicate that PMN activation (CR1 and CR3 expression) is greater in cells exposed to the local (BF) than the systemic (plasma) humoral microenvironment shortly after thermal injury.

[Comparative studies on antifertility mechanism and toxicology of Tripterygium wilfordii monomer T4 and gossypol].

Inhibition of spermatogenesis and turnover of basic nuclear protein in late elongated spermatids of rat testis by Tripterygium wilfordii monomer T4 was more obvious than that by gossypol. Clustered positive deposits of gamma-GT, ALP reaction were found in the inner edges of epididymal epithelium. Head and tail separation, mid-piece coiling and axial fiber dislocation of epididymal spermatozoa in T4 treated rats were more obvious than those of gossypol-treated rats. Mitochondrial LDH, LDH-X and MDH were also found to be inhibited by T4. T4 did not significantly affect enzyme activities in kidney and liver, though it did cause slight changes. Effects of T4 on the chief rat organs were milder than those of gossypol, except for a more obvious antifertility effect. The authors suggest that T4 may be developed into an ideal male contraceptive.

[A prospective serological epidemiological investigation of hepatitis B virus infection in a prison].

2,443 male prisoners were under investigation, most of them were youth, and followed up for 12-36 (mean 23.7 +/- 11.4) months. Sera HBV markers were detected every year. The positive rate of HBsAg was 25.1%, in which transient HBV carriers were 14.7% and chronic asymptomatic HBV carriers (AsC) were 85.3%. Seroconversion rate of HBsAg in AsC was 1.4% per year. The positive rate of anti-HBs was 46.3%, anti-HBs diminished in 6.0% per year. It was suggested that the antibody response to frequent exposure to HBV persisted for a long time. HBeAg seroconversion was 22.3% per year, and its reoccurrence was 7.9% per year, indicating the fluctuation and stability of infection status. The prevalence rate of HBV infection (with one or more than one of HBsAg, anti-HBs or anti-HBc) was 81.1%. Incidence of HBV infection among those susceptible persons found at first investigation was 22.3% per year.

[An experimental study on preparing monoclonal antibodies of rough Brucella spp. and identification species Brucella].

A report of preparation of monoclonal antibodies of B. canis and B. ovis and the use of identification for rough species of Brucella was described. The results showed that the above two monoclonal antibodies were specific for rough Brucella Spp. The use of the above described monoclonal antibodies gave us the possibility to differentiate rough and smooth Brucella species. The results also showed that the ELISA using monoclonal antibodies of rough Brucella Spp. was more specific than the agglutination test using routine antibodies of rough Brucella.

Identification and localization of major cortical proteins in the ciliated protozoan, Euplotes eurystomus.

Shape-preserving cortical residues have been isolated from Euplotes eurystomus cells by the application of Triton X-100 at high ionic strength. These integrated structures consist of articulated plates and widely interspersed cages that formerly contained basal bodies associated with the clusters of cilia characteristic of this cell type. Using SDS-PAGE and immunolocalization procedures, we have identified major subunit proteins of both the plates (116, 110 (X10(3] Mr, and the basal body cages (86 X 10(3) Mr). The potential for studies of these proteins in contributing to our understanding of cortical development and evolution in Euplotes is discussed.

Molecular cloning of multiple bovine aspartyl protease genes.

Eight bovine genomic clones have been identified as members of the aspartyl protease gene family. The clones were prepared in phage vector lambda EMBL3 from Sau3AI partial digests of DNA from a single animal. Restriction maps show that seven of these clones are related and comprise at least five non-overlapping sequences. Allowing for allelic variation these probably represent three or more different genes. The nucleotide sequences show open reading frames (ORFs) corresponding closely to exons 6, 7 and 8 of human and porcine pepsin A. Comparison with other aspartyl proteases shows that these are multiple bovine pepsin A genes. The seven clones would encode at least two different but closely related forms of pepsin A. The 5' splice site at the end of exon 7 in all seven clones is the unusual sequence GC. The eighth clone contains an ORF homologous to exon 2 of the mammalian aspartyl proteases. The corresponding amino acid sequence is more closely related to bovine chymosin than to any of the other known sequences; it may be functionally homologous to chymosin but could be a novel mammalian aspartyl protease. The intron/exon boundaries seen in both this clone and in the bovine pepsin A clones are at the same positions as found in human pepsin A, bovine chymosin and human and mouse renins, further evidence that the general structure of mammalian aspartyl protease genes has been strongly conserved.

[Histochemical changes in endometrial fibrolytic activity with different IUDs].

The histochemical changes of endometrial fibrinolytic activity were observed in rabbits. 1 horn of the uterus was put in IUDs made of different materials (steel, copper, magnet, and EACA-copper). It was found that the endometrial fibrinolytic activity in the experimental horns was significantly higher than the controls, and the effect was the highest with the copper IUD. In the contrasted groups (copper versus steel, copper versus magnet, steel versus magnet), the endometrial fibrinolytic activity induced by the copper IUD was the highest (P0.05), while there was no significant diffeence between that of steel and magnet (P0.05). In the EACA-copper IUD groups, the results were also observed on days 7, 14, and 28 after insertion of the devices. There was no difference in fibrinolytic activity between the experimental horns and the controls in 7 and 14 days groups, but it was significantly higher than the controls in 28 days group. In the contrasted groups of the EACA-copper IUD and copper IUD the endometrial fibrinolytic activity of the former was lower than that of the latter in 7 days group (0.05 P 0.1). This suggests that fibrinolytic activity may be inhibited by EACA, which, however, may be released from IUD at such a rapid rate that it loses its effect in 28 days.

Mutations in the 'a' determinant of hepatitis B surface antigen among Chinese infants receiving active postexposure hepatitis B immunization.

Twenty-four infants who became positive to the surface antigen of hepatitis B virus (HBsAg) despite a complete course of active postexposure immunization with plasma derived hepatitis B vaccine were studied. The polymerase chain reaction amplified products of the common neutralizing epitope 'a' determinant of HBsAg (Nucleotide 419-598) from serum samples were sequenced and analyzed for nucleotide mutations. Four cases (16.7%) had mutations that led to amino acid substitutions between codons 124 and 147. Only one case (N1) showed a substitution at codon 145 (from glycine to arginine, 145R), the other three were at codons 126-129. The mother of N1 was co-infected with the wild type and the mutant virus. Five years later, serum of N1 showed only the wild type virus. There was no significant relationship between the mutation rate and the anti-HBs response to hepatitis B vaccination. Results suggest that without immune selective pressure, 145R variant was not frequently observed, and was not stable. Mutation in the 'a' determinant was not an important cause of failure to prevent maternal-infant transmission of HBV by active postexposure hepatitis B immunization in Chinese children.

Aromatase and its inhibitors.

Inhibitors of aromatase (estrogen synthetase) have been developed as treatment for postmenopausal breast cancer. Both steroidal substrate analogs, type I inhibitors, which inactivate the enzyme and non-steroidal competitive reversible, type II inhibitors, are now available. 4-hydroxyandrostenedione (4-OHA), the first selective aromatase inhibitor, has been shown to reduce serum estrogen concentrations and cause complete and partial responses in approximately 25% of patients with hormone responsive disease who have relapsed from previous endocrine treatment. Letrozole (CGS 20, 269) and anastrozole (ZN 1033) have been recently approved for treatment. Both suppress serum estrogen levels to the limit of assay detection. Letrozole has been shown to be significantly superior to megace in overall response rates and time to treatment failure, whereas anastrozole was found to improve survival in comparison to megace. Both were better tolerated than the latter. The potential of aromatase within the breast as a significant source of estrogen mediating tumor proliferation and which might determine the outcome of inhibitor treatment was explored. Using immunocytochemistry and in situ hybridization, aromatase and mRNAarom was detected mainly in the epithelial cells of the terminal ductal lobular units (TDLU) of the normal breast and also in breast tumor epithelial cells as well as some stromal cells. Increase in proliferation, measured by increased thymidine incorporation into DNA and by PCNA immunostaining in response to testosterone was observed in histocultures of breast cancer samples. This effect could be inhibited by 4-OHA and implies that intratumoral aromatase has functional significance. An intratumoral aromatase model in the ovariectomized nude mouse was developed which simulated the hormone responsive postmenopausal breast cancer patient. This model also allows evaluation of the efficacy of aromatase inhibitors and antiestrogens in tumors of estrogen receptor positive, human breast carcinoma cells transfected with the human aromatase gene. Thus, the cells synthesized estrogen which stimulated tumor formation. Both aromatase inhibitors and antiestrogens were effective in suppressing tumor growth in this model. However, letrozole was more effective than tamoxifen. When the aromatase inhibitors were combined with tamoxifen, tumor growth was suppressed to about the same extent as with the aromatase inhibitors alone. Thus, there was no additive or synergistic effects of combining tamoxifen with aromatase inhibitors. This suggests that sequential treatment with these agents is likely to be more beneficial to the patient in terms of longer response to treatment.
