[Clinical investigation on 42 cases of pulmonary tuberculoma].

OBJECTIVE: For the purpose to find the possible reasons causing pulmonary tuberculoma been misdiagnosed as lung cancer. METHODS: Clinical symptoms, chest X ray films, tomographic and CT manifestations as well as laboratory data of 42 cases of pulmonary tuberculoma pathologically proved were thoroughly reviewed. All the radiological appearances were compared with pathological findings. RESULTS: Shallow lobulated contour, spurlike shadow formation, vacoule sign, satellite lesions, pleural involvement, calcified spots were found very important for differential diagnosis of these 2 diseases. CONCLUSIONS: It is difficult to differentiate pulmonary tuberculoma from lung cancer. By paying special attention to the above mentioned appearance, evaluating them very carefully and considering them with positive laboratory findings, it might be possible to avoid misdiagnosis before surgical operation.

Binding conformers searching method for ligands according to the structures of their receptors and its application to thrombin inhibitors.

AIM: To develop a method of finding binding conformers for ligands according to the three-dimensional structures of their receptors. METHODS: Combining the systematic search method of ligand with the molecular docking approach of ligand fitting into its receptor, we developed a binding conformer searching method for ligands. RESULTS: The binding conformers of phosphonopeptidyl thrombin inhibitors were recognized. The binding (interaction) energies between these inhibitors and thrombin were calculated with molecular mechanical method. CONCLUSION: Both of the total binding energies and steric binding energies have good correlations with the inhibitory activities of these thrombin inhibitors, demonstrating that our approach is reasonable. It can also be used to explain the inhibition mechanism of thrombin interacting with these inhibitors.

[One-stage reconstruction with the pectoralis major myocutaneous flap for the pharyngectomy].

18 patients with the advanced oropharyngeal cancer from October 1983 to July 1992 were retrospectively studied in our hospital. The blood supply of the pectoralis major myocutaneous flap (PMMF). PMMF came mainly from the pectoral branch of the acromiothoracic artery. The flap area ranged from 15 cm x 10 cm to 3 cm x 3 cm. 3 cases had partial necrosis and 4 cases received prophylactic tracheotomy. The cosmetic and functional restoration was satisfactory in most of the patients. The factors which affected the wound healing were also discussed. The 3 year and 5 year survival rates were 61.5% and 40%, respectively. This paper revealed that the PMMF was the first choice for the defect of the pharyngectomy.

[Experimental studies on the therapeutic effects of lung lavage with large volume of saline on silicosis].

Sterile saline was instilled into and aspirated from the lung of dust-exposed rabbits in imitation of clinical method of the whole-lung lavage. The changes of the biochemical and cellular components in alveolar fluid were observed before and after lavage with a view to providing evidence for the applicability of the method in the treatment of silicosis. The results showed that the number of cells, the protein content and the activities of both LDH and AKP were significantly lower than those of control group, except the total phospholipid and dipalmityl phosphatidyl choline (DPPC) in alveolar fluid after lavage. A certain amount of dust was also removed from lung with lavage. The wash-out of SiO2 from the first lavage was higher than that of the second lavage. It is suggested that lavage might delay the development of silicosis and be more effective in early stages of silicosis.

[Clinical analysis in seventy-two patients with chronic thromboembolic pulmonary hypertension].

To improve the identification of chronic thromboembolic pulmonary hypertension. We restrospectively analysed clinical data of 72 patients with chronic thromboembolic pulmonary hypertension (CTEPH). The levels of arterial blood gases appeared to be decrease in PaO2 and PaCO2, and increase in P(A-a)O2. There were 80.3% of unsymmetrical pulmonary hyperlucencies on chest radiograph and 76.4% of right ventricular hypertrophy on electrocardiograph, 92.5% and 97.1% of right atrial and right ventricular enlargement, respectively. 11.1% of pericardial effusion was noted on echocardiograph, and 98.5% of rise in pulmonary arterial pressure calculated by Doppler. The mean pulmonary arterial pressure was 6.50 +/- 1.80 kPa (48.75 +/- 13.50 mmHg), in part, by right cardiac catheterazation. Both the incidences of pulmonary embolism were 100.0% on pulmonary angiography and on radionuclide lung perfusion scan. There were 43.1% of the history of deep venous disorders and 75.0% of the positive findings by radionuclide venography in the lower extremities, respectively. The misdiagnostic and the lost diagnosis rate of prehospitalization accounted for 90.3%. The understanding of clinical manifestations and laboratory findings of CTEPH is important to promote diagnostic sense and level of CTEPH.

[Renal subcapsular effusion cured by using greater omentum packed].

The fistula of proximal convoluted tubule which causes the persistent leakage of the glomerular filtrate and brings about renal subcapsular effusion is very seldom seen. There has been no similar report about it. Up to now, there has been no definite diagnosis and treatment for this disease. We made the advantage of the great absorption power of the greater omentum blood vessels networks to make internal drainage. In this way, the glomerular filtrate was reabsorbed and thus the disease was cured.

[Comparison between magnetic resonance imaging and CT in detection and localization of non-magnetic intraocular foreign bodies].

OBJECTIVE: To approach the characteristics of magnetic resonance imaging (MRI) and CT in detection and localization of intraocular foreign bodies. METHODS: Detection and localization of 28 cases (42 foreign bodies) of non-magnetic intraocular foreign body were compared in MRI and CT with criteria of operative results. RESULTS: The positive rates of MRI and CT in detection of 42 non-magnetic intraocular foreign bodies were respectively 90.5% and 95.2% (chi 2 test: P > 0.5). The positive rates of MRI and CT in localization were respectively 89.5% and 60.0% (chi 2 test: P < 0.05). CONCLUSION: MRI could be used in detection and localization of non-magnetic intraocular foreign bodies with high positive rates in detection and superior to CT in demonstrating non-magnetic intraocular foreign bodies with low density and the relationship between foreign bodies and intraocular tissues. MRI was not as believable as CT in detection of foreign bodies in eye wall and not an appropriate modality in examinations of magnetic intraocular foreign body.

[Differentiation of lens epithelial cell and effects of serum and ocular tissue on its proliferation in vitro].

OBJECTIVE: To study the pathogenesis of lens posterior capsular opacification (PCO) and the effects of the breakdown of blood-aqueous barrier and remain of lens cortex on PCO in cell culture level. METHODS: The differentiation of cultured bovine lens epithelial cells (BLEC) was observed by Coomassie BB staining, light and electron microscope. The effects of fetal bovine serum, aqueous humor and lens cortex, etc., on the proliferation of BLEC were tested by Giemsa-staining colorimetry. RESULTS: BLEC in vitro could gradually differentiate into lens fiber cells or lens fibers in 1-7 passages. The characteristics of differentiation were that the shape of cells changed from polygon into fusiform and fibriform, the bulk enlarged, and cytoskeleton gradually increased. Fetal bovine serum promoted the proliferation of BLEC depending on its concentration (P < 0.05). High concentration of aqueous humor inhibited the proliferation (P < 0.01). Supernatant and suspension of lens cortex, supernatant of lens nucleus, vitreous humor could promote the proliferation (P < 0.01). CONCLUSION: The differentiation of lens epithelial cells plays important roles in PCO, the breakdown of blood-aqueous barrier, the remain of lens cortex and the prolapse of vitreous may promote PCO by stimulating the proliferation of lens epithelial cells.

[Vitrectomy for extraction of intraocular foreign bodies].

OBJECTIVE: To evaluate the effect of treatment of intraocular foreign bodies (IOFBs) with vitrectomy and its influential factors. METHODS: A retrospective research was done on 517 in-patients having undergone extraction of IOFBs with vitrectomy. RESULTS: Postoperatively, the successful rate of the extraction of IOFBs was 97.6%, the rate of cases with useful vision (0.05 to 0.4) increased from preoperative 12.5% to 23.2% and that with good vision (0.5 and better) from 7.2% to 17.0% (P < 0.005). When IOFBs were located in anterior segment, vitreous and retina, the increasing rates of cases with useful vision respectively were 44.4%, 25.1% and 10.7%, and that of good vision were 26.3%, 12.7% and 2.7%. CONCLUSIONS: Vitrectomy is favorable for the extraction of IOFBs, the increase of postoperative vision and the management of complications.

[Clinical features and treatment of congenital inferior oblique paresis].

OBJECTIVE: To discuss the diagnosis and differential diagnosis of isolated congenital inferior oblique paresis. METHODS: 13 patients with isolated congenital inferior oblique paresis examined and operated between 1984 and 1994 in our hospital were retrospectively reviewed. RESULTS: After a follow-up of 6-75 months, six patients had excellent surgical results, and the rest had improvement. CONCLUSIONS: Isolated congenital inferior oblique paresis occurs infrequently. Its diagnosis is based on the following: (1) presence of vertical deviation in primary position, (2) the results of duction and version examination, (3) negative forced duction test that is different from the test in Brown's syndrome. Surgery is usually indicated in most patients. It consists of weakening superior oblique muscle and recession of inferior rectus muscle in the homolateral eye and recession of superior rectus in the contralateral eye.

[Studies on the analgesic and antiinflammatory action of radix Linderae extract].

Both of aqueous extract and alconol extract of Radix Linderae at a dose 5 g/kg and 10 g/kg could length obviously pain threshold in mice in hot plate test. The samples at a dose 20 g/kg could inhibit significantly the writhing frequency induced by potassium antimony tartate in mice, also antagonize the swelling of ear induced by inflammatory agent and decrease the swelling rate. The component further isolated from the plant could antagonize the swelling of rat toes induced by carrageenin.

[Detection of human papillomavirus types 16, 18 DNA related sequences in bronchogenic carcinoma].

In order to study the relationship between human papillomavirus (HPV) and bronchogenic carcinoma, "high-risk" HPV 16, 18 DNA sequences were detected in samples from 50 lung cancer patients, 18 patients with benign pulmonary diseases and 4 fetal lung tissues by polymerase chain reaction (PCR) and dot-blot hybridization using biotin-labelled probes. The results showed that HPV 16, 18 DNA related sequences were found in 32% of lung cancer specimens, including 10 cases of HPV 16, 5 cases of HPV 18, 1 case of both types. 48.2% (13/27) of the squamous cell carcinomas were shown to be positive for HPV 16, 18 DNA; in addition, two adenocarcinomas and one small cell carcinoma were positive for HPV 16 DNA. No specimen from benign diseases and fetal lung tissue showed positive result. Most of HPV DNA positive cases had a history of heavy smoking. These results implicated that primary bronchogenic carcinomas may be related to HPV infection.

[The preliminary investigation on proliferative and survival capabilities of normal and abnormal mononuclear cells in bone marrow of patients with paroxysmal nocturnal hemoglobinuria].

OBJECTIVE: To unfold the proliferation and survival behavior of normal and abnormal early hematopoietic cells in paroxysmal nocturnal hemoglobinuria(PNH). METHODS: The bone marrow mononuclear cells (BMMNC) from 6 patients with PNH were separated into two subpopulations using CD59 monoclonal antibody combined with goat-anti-mouse IgG immunomagnetic beads, and then cultured in vitro. RESULTS: The BMMNCs from normal control had better proliferation capacity and stronger potential of suvival than both CD59(+) and CD59(-) BMMNCs from PNH patients. The CD59(+) BMMNCs from PNH patients, which should be expected "normal" in view of the CD59 expression, were surprisingly found to have less capabilities to form CFU-GM and BFU-E in semisolid cultures and less abilities of expansion and survival in liquid cultures than their CD59(-) counterparts. The CD59 expression of CD59(+) BMMNC in PNH patients diminished to some extent after a period of liquid culture while the normal control remained unchanged indicating that some of CD59 antigen on the cells were gradually lost during cultivation. CONCLUSION: In all cases of PNH studied, CD59(-) BMMNCs dominated hemopoiesis, and CD59(+) BMMNCs seems to have less potential to proliferate. The emergence of abnormal CD59(-) cells could not only result from PIG-A gene mutation but might also arise as the consequence of loss of GPI-anchored protein during the course of hematopoietic development and differentiation.

[Study of the photoluminescence spectra of laser dye impregnated in porous silicon].

We studied the photoluminescence (PL) of the compound films, which were made by implanting some laser dyes into the porous silicon films, and compared it with the PL of the laser dyes in other states. We also measured the PL of the compound films with different substrates. It was found that the spectra symmetry of the implanted laser dyes is better than that of the dyes in other states. The experimental results suggest that the PL of the compound films mainly comes from the monomers of the implanted laser dyes.

[The change of trace elements content in hair during the period of gestation].

Study on trace elements content change in hair during the period of gestation for 52 lying-in women was performed. The quantitative results for 7 elements have been obtained and the change tendencies for other 7 elements without standard value have been observed on the basis of a self comparison method for each individual.
