Effects of different manipulations of acupuncture on electrical activity of stomach in humans.

The author observed effects of 4 different acupuncture manipulations (lifting-thrusting, twisting-twirling, uniform reinforcing-reducing, and needling with sensation propagating along channels) on electrical activity of the stomach, using amplitude and frequency of human electrogastrogram (EGG) as the indexes. The experiment proved that both the twisting-twirling and the uniform reinforcing-reducing methods inhibited electrical activity of the stomach to reduce the frequency of EGG (P < 0.05), and at the same time the uniform reinforcing-reducing method decreased the amplitude (P < 0.001); while both the lifting-thrusting method and the manipulation for needling sensation propagating along channels excited electrical activity of the stomach to raise the amplitude of EGG (P < 0.01), and the lifting-thrusting method increased the frequency (P < 0.001). Accordingly, the lifting-thrusting method or the manipulation for needling sensation propagating along channels is suitable for chronic superficial gastritis, chronic atrophic gastritis, carcinoma of stomach with lower frequency and amplitude of EGG; whereas the uniform reinforcing-reducing or twisting-twirling method is good for patients with gastric and duodenal ulcer and hyperhydrochloria with higher frequence and amplitude of EGG.

A clinical investigation on massage for prevention and treatment of recurrent respiratory tract infection in children.

The TCM therapeutic principle of "eliminating the pathogenic factors by supporting the healthy energy" was adopted in massage to treat and prevent recurrent respiratory tract infection of children. Susceptible and healthy children of the same age were used as controls. As a result, the therapeutic effect of the treatment group is significantly better than that of the controls, with all of the immunologic indexes being approximately normal when the patients were reexamined 3 and 6 months after the massage. Statistically, the difference was significant (P < 0.01). Massage has been proved very helpful in improving the general constitution, enhancing the immune functions, preventing and treating the condition, and in health care as well.

[Surgical treatment of sacral tumor].

From 1990 to 1996, 21 patients with sacral tumor were surgically, including 8 cases with giant tumor of bone, 7 cases of spinal cord tumor, each 2 cases of neurofibroma and adenoma, 1 case of myeloma and 1 case of lipoma with low grade of malignancy. A total of 22 operations involving one for recurrent tumors in 21 cases were performed. Sacral resection and curettage plus resection were the surgical ways. 19 patients were followed up with an average period of 2.5 years. 15 patients showed good results, 3 patients occurred urinary incontinence and constipation, one of 3 cases occurred weakness of ankles and feet. Authors conclude that surgery should be advised and actively adopted for sacral tumors.

[The study of autoantibodies in women with habitual abortion of unknown etiology].

OBJECTIVE: To evaluate the relationship between habitual abortion of unknown etiology and autoantibodies. METHODS: IgG and IgA antibodies against cardiolipin, specific antinuclear double-stranded DNA and single-stranded DNA were measured by enzyme linked immunosorbent assay (ELISA). Antinuclear antibody, smooth muscle antibody, antimitochondrial antibody and antiheart muscle antibody were performed by indirect immunofluorescence assay. Antiextractable nuclear antigen's antibodies were determined by immunodiffusion assay. RESULTS: Most women with habitual abortion of unknown etiology showed higher incidence of antibodies to cardiolipin, nuclear, smooth muscle compared with normal multigravida control (P < 0.05). CONCLUSIONS: These results indicate that most women with habitual abortion of unknown etiology have a higher level of autoreactivity, which may affect the course of pregnancy. The incidence of antismooth muscle antibody in women with habitual abortion of unknown etiology is also increased compared with normal multigravida control.

[Cervical ripening score by transperineal ultrasonography and its predictive effect for induction of labor by prostaglandin E2].

OBJECTIVE: To establish a cervical ripening score by transperineal ultrasonography for induction of labor by Prostaglandin E2, and evaluate its predictive effect. METHODS: 105 primiparae underwent cervical assessment by digital and transperineal ultrasonography examination. The five indices of cervical status, other clinical parameters and latency period (from induction to the onset of labor) were analysed by Cox multivariate model. RESULTS: Transperineal ultrasonography was simple, visualable satisfactorily, noninvasive and less uncomfortable. The width of cervical internal os, the length of the canal and position of the presenting part were the main factors that influenced the latency period (P < 0.05). Primiparae with score > or = -4.5 were easy to initiate the onset of labor within 12 hours (P < 0.001). The predictive results were in good agreement with clinical outcomes, and had less misdiagnosis (the Kappa value, specificity and the positive predictive value were 0.7409, 0.7917 and 0.9254, respectively), and were better than the Bishop score (the corresponding value were 0.5680, 0.6667 and 0.8806, respectively). CONCLUSIONS: Transperineal ultrasonography is safe and effective. In predicting the effect on induction of labor of prostaglandin E2 in primiparae, the transperineal ultrasonographic cervical ripening score is a valuable method.

[Prenatal gene diagnosis of alpha-thalassemias].

OBJECTIVE: To study the value of polymerase chain reaction (PCR) in prenatal diagnosis of alpha thalassemias. METHODS: Amniotic fluid prenatal gene diagnosis with polymerase chain reaction was carried out on eleven fetuses whose parents are both heterozygotes with alpha-globin gene deficiency. A DNA fragment of 224bp in the production means normal alpha-globin gene sequence, while a 630bp fragment indicated the alpha-globin gene deficiency. Both 224bp and 630bp fragments in the same sample means heterozygote. RESULTS: Three of the 11 fetuses (one pregnancy was twin) were with normal alpha-globin gene sequence, while 4 were homozygotes and the other 4 were heterozygotes. For the 3 fetuses with ascitic fluid under ultrasound examination, 2 were homozygotes and the other one was heterozygote by gene diagnosis. Two of the 4 homozygotes from induced abortion were typical Bart's syndrome, one was edema in the whole body and the other one with short limbs and abdominal hernia. CONCLUSION: The method of PCR in prenatal diagnoses for detection of alpha-thalassemias is simple, accurate and rapid.

[A pharmacokinetic study of 125I-VIP in rat].

To investigate the pharmacokinetics of 125I-VIP in normal rats, a single dose of 125I-VIP was given intravenoualy to rats and from 0.5 to 240 min after the administration, serial blood samples were taken and measuredp by gamma scintillation counter and finally expressed as mu Ci/ml blood. The data were analyzed by computer for the estimation of pharmacokinetic parameters and the judgment of compartment model with a program of 3P87. The results demonstrated that the pharmacokinetics of 125I-VIP in rat fitted with the three-compartment model(Wi = 1) based on either the comparison of the calculated theoretic value with measured concentration, the values of AIC and r2 or F test for the model judgment, and that the average distributive half-life of 125I-VIPk in blood was 1.36 min, the elimination half-life(T1/2 beta was 67.3 min, the K12(3.589 min-1) was markedly higher than that of K12(0.045 min-1), and the K21:K12(0.0127) was obviously lower than that of K31:K13(0.215). The present study indicates that 125I-VIP has a rapid uptake and slow elimination in the tissues with high density and affinity VIP receptors (the second compartment).

[Observations on rat's muscle at various postmortem intervals by scanning electron microscopy].

The aim of this study was to observe the morphological changes of muscle in the process of rigor mortis. The quadriceps of 40 rats at various postmortem intervals were observed under the scanning electron microscope (SEM) and the light microscope by phosphtungstic acid-haematoxylin staining. The results showed that the striations of muscle were blurred within 4 hours, but they became apparent from 6 hours to 24 hours after death. The authors suggest that this phenomenon be associated with the increased resistance of muscle against the postmortal changes. The observations by scanning electron microscopy and light microscopy have revealed that the muscles do contract in the process of rigor mortis because the distance between two Z lines shortens and the I band narrows, compared with those in anaesthetised animals. The basic biochemical process for the formation of rigor mortis is the same as that of muscle contraction except that the former happens postmortem and the latter antemortem.

[Ultrasonographic diagnosis of peripartum cardiomyopathy].

OBJECTIVE: Analysing the characteristics of ultrasonographic manifestation for peripartum cardiomyopathy (PPCM), and discussing its diagnostic criteria. METHOD: Echocardiographic examination in 20 PPCM and 20 normal volunteers (NC) was completed with Acuson 128 computed sonography. RESULTS: All 20 PPCM showed an enlargement in the left ventricle and left atrium. Left ventricular end diastolic anterio-posterior diameter was 65.4 +/- 4.5 mm, left atrium end systolic anterio-posterior diameter was 43.1 +/- 6.1 mm. There was a very significant difference in left atrium and ventricle size between PPCM and NC (P < 0.001). Among 20 PPCM, there were enlargements of the right atrium in 6 cases, of the right ventricle in 8 cases and 20% cardiomegaly. All patients showed decreased left ventricular wall movement. Ejection fraction (0.33 +/- 0.08) and fraction shorting (16.2 +/- 5.0%) were significant different with that of NC (P < 0.001). The regurgitation rate in the heart valvae was MR 100%, TR 70%, PR 70% and AR 15% respectively. CONCLUSION: The ultrasonographic manifestation of PPCM was not specific, so the diagnosis of PPCM depended on peripartum history, left atrium and ventricle enlargements or cardiomegaly, decreased left ventricular function based on ruling out of other heart diseases.

[Construction and expression of ScFv from anti-human gastric cancer McAb 3H11].

OBJECTIVE: To construct single chain antibody (ScFv) from anti-gastric cancer McAb 3H11. METHODS: Phage display technology was used to construct ScFv library from 3H11 hybridoma cells. Site-directed mutagenesis was used to resume the N-terminal sequences of 3H11 ScFv. RESULTS: The library was screened against human gastric cancer cells MGC803 but no positive clone was obtained. Then, a random mutated ScFv library was constructed by error-prone PCR method, and panning selection was performed. Again the identification of positive clone was failed. Finally the N-terminal sequences of ScFv variable region was resumed to 3H11 original sequences by site-directed mutagenesis via PCR, and the expressed ScFv in bacterial culture supernatant showed binding activity to gastric cancer cells. CONCLUSION: The N-terminal of the variable region introduced by PCR primers may seriously affect binding activity of the antibody.

[Application of pectoralis major myocutaneous flap in the reconstruction of tongue: an analysis of 86 cases].

OBJECTIVE: To analyze the results of the use of pectoralis major myocutaneous (PMM) flap in the reconstruction of tongue. METHODS: From 1984 to 1995, PMM flap was applied in 86 cases with cancer of tongue after radical resection for the immediate reconstruction of the lingual defect. RESULTS: Notwithstanding the advanced lesions in stages T3 and T4, patients had good survival and functional results. The 1-, 3- and 5-year survival rate was 86.0%, 51.2% and 23.3%, respectively. There was complete flap necrosis in one case and partial necrosis in fifteen cases, but the latter spontaneously regressed. The swallowing and speech rehabilitations following surgical treatment for tongue cancer were acceptable, considering the advanced disease status. CONCLUSION: The PMM flap with its abundant tissue and excellent blood supply and anatomic proximity provides a reliable method of primary reconstruction of the substance of the tongue. The surgical procedure is simple, easy to perform. Reconstruction of the tongue with PMM flap meets the clinical requirements in the shape, communication and swallowing rehabilitations. The PMM flap is a reliable material for the reconstruction of tongue with major defect following subtotal glossectomy.

[Arterial infusion chemotherapy for patients with advanced and moderately advanced cancer of urinary bladder].

OBJECTIVE: To study the effect of arterial infusion chemotherapy in patients with carcinoma of urinary bladder. METHODS: Eleven patients with advanced or moderately advanced cancer of the urinary bladder (10 transitional cell carcinoma, 1 squamous-cell carcinoma) who were for various reasons not suitable for surgical treatment were treated by arterial infusion chemotherapy according to Seldinger's percutaneous approach. RESULTS: The treatment results were satisfactory with an overall response rate of 90.9% (CR = 2, PR = 8). The average survival time of the 11 patients was 26.2 +/- 13.1 months. The 1-year survival rate was 81.8%. Side effects were mild. CONCLUSION: Arterial infusion chemotherapy is of use in patients with advanced or moderately advanced bladder cancer.

[The role of hematopoietic growth factors on extensive apoptosis of myelodysplastic hematopoietic cells and its clinical significance].

OBJECTIVE: To investigate the effects of hematopoietic growth factors (HGFs) on apoptosis of hematopoietic cells in myelodysplastic syndromes (MDS) patients. METHODS: CD34+ cells in bone marrow from 12 MDS patients were purified by an immunomagnetic beads sorting system. Apoptosis of hematopoietic precursors was assayed by propidium iodine staining and flow cytometric analysis. RESULTS AND CONCLUSION: High dose rhEpo partly suppressed apoptosis of hematopoietic cells in MDS patients. At the 7th, 10th, 14th, cultured day, the apoptotic percentages of the high dose Epo group (rhEpo 200 U/ml or 100 U/ml) were substantially lower than that of the low dose group (20 U/ml, 2 U/ml and 0.2 U/ml) and showed a dose-dependent effect with rhEpo. rhGM-CSF and rhIL-3 could also suppress hematopoietic cell apoptosis in vitro, the differences between the high dose group (rhIL-3 200 U/ml or rhGM-CSF 500 U/ml) and the low dose group (rhIL-3 50 U/ml or rhGM-CSF 50 U/ml) were significant. Extensive apoptosis of myelodysplastic hematopoietic cells were markely suppressed when rhEpo and rhIL-3 or rhEpo and rhGM-CSF were given together.

[Analysis of CD34+ cell amounts in normal and abnormal bone marrow cells of patients with paroxysmal nocturnal hemoglobinuria].

OBJECTIVE: To explore the contributive role of CD34+ cell amount in pathogenesis of clonal dominance in paroxysmal nocturnal hemoglobinuria (PNH). METHODS: Bone marrow nuclear cells (BMNCs) were doubly labeled by PE-conjugated anti-CD59 monoclonal antibody (McAb) and FITC-conjugated anti-CD34 McAb, and CD34+ cells contained in different cell populations were determined by flow cytometry. RESULTS: 1. CD34+ cells mainly appeared in window A (containing mainly of lymphocytes) and window D (composed of blasts, other immature cells and monocytes) in both normal controls and PNH patients. 2. The numbers of CD34+ cells in normal controls were (3,701 +/- 896)/10(5) cells and (11,373 +/- 1,574)/10(5) cells in window A and window D, respectively. Compared with normal controls, PNH patients possessed significantly decreased number of CD34+ cells [(1,215 +/- 749)/10(5) cells and (6,420 +/- 2,337)/10(5) cells in window A and window D, respectively]. 3. In normal controls, nearly all CD34+ cells in windows A and D expressed CD59 antigen (99.2% +/- 1.0% in A and 98.7% +/- 0.8% in D), whereas in PNH patients, the CD34+ cells clearly showed two immunophenotypes of CD34+CD59+ and CD34+CD59- with the latter being predominant in both window A and window D(CD59- cells constituting 79.3% +/- 15.5% and 85.9% +/- 7.8% of CD34+ cells, respectively). 4. The amounts of CD34+ cells in CD59+ populations [(463 +/- 276)/10(5) cells in window A and (3,841 +/- 1,188)/10(5) cells in window D] reduced to a greater extent than that in CD59- population [(2,603 +/- 2,084)/10(5) cells in window A and (7,105 +/- 2,739)/10(5) cells in window D]. CONCLUSION: Hematopoietic stem/progenitor cells in BMNCs of PNH patients are markedly decreased. Reduction of CD34+ cells in CD59+ cell population is even more obvious than that in CD59- population. The dominance of the abnormal clone in hematopoiesis might be a relative one in the setting of severely impaired normal hematopoiesis.

[Comparative study on efficacy of regimens including streptomycin or ethambutol].

OBJECTIVE: To evaluate the advantages and disadvantages of regimens of 2E3H3R3Z3/4H3R3(EMB regimen) and 2S3H3R3Z3/4H3R3(SM regimen) in tuberculosis control program. METHOD: Retrospective, cross-sectional and prospective studies were carried out in Shijiazhuang city, Hebei province from January 1994 to June 1996. RESULT: There was no significant difference between the two regimens in efficacy, relapse rate and full course supervision. The EMB regimen was found more applicable than the SM regimen, and the SM regimen caused more side effects than the EMB regimen. Streptomycin skin test had a 4.5% positive rate, and using SM costs 84% more than using EMB. One of the drawbacks found in the SM regimen was that only in 42.9% of the rural sanitation units the disinfection standard could be fulfilled, and the patients preferred the EMB regimen to the SM regimen. CONCLUSION: The EMB regimen is more applicable than the SM regimen in the tuberculosis control program.
