[Screening of beta zero-thalassemia in cord blood from 2,423 newborns in Xilin county of Guangxi].

OBJECTIVE: To investigate the incidence of beta zero-thalassemia in newborns in Xilin county of Guangxi and identify the common type of the gene mutation. METHODS: Cord blood samples from successive 2,423 newborns were screened for beta zero-thalassemia by PAGE, and the gene mutation was assayed by PCR and dot blot hybridization. RESULTS: Seven cases (0.29%) of beta zero-thalassemia were revealed in the 2,423 newborns. The gene mutation types were as follows: three cases of CD17/CD17, two of CD17/CD41-42, one of CD41-42/CD41-42 and one of CD17/IVS-I-1. CONCLUSION: These data might be of help to prenatal diagnosis of beta zero-thalassemia in Guixi district.

[A preliminary study on the markers of pulmonary vascular endothelial cells in patients with ARDS].

OBJECTIVE: To explore the changes of the markers of pulmonary vascular endothelial cell (PVEC) in patients with ARDS. METHODS: The levels of plasma circulating endothelial cell(CEC), granule membrane protein-140(GMP-140), endothelin-1(ET-1) and von Willebrand factor (vWF) were determined respectively in 13cases of ARDS. The vWF on the surface of the PVEC were stained in 5 cases of ARDS. RESULTS: Circulating CEC, GMP-140, ET-1 and vWF levels were increased significantly (P < 0.01) but scores of vWF expression on the PVEC became weak (P < 0.01) in ARDS samples. The level of plasma GMP-140 elevated dramatically in 2nd day( > 4 times). CONCLUSIONS: PVEC are severely impaired during ARDS. GMP-140 released from PVEC might be a satisfactory criteria for early surveillance and for evaluation of prognosis of ARDS in clinical practice.

[Respiratory effects of beta-endorphin in chronic hypoxic rats].

OBJECTIVE: To assess the role of beta-EP in the physiological and pathophysiological process of respiratory regulation in chronic hypoxic rats. METHODS: The chronic hypoxic rat model was established by intravenous injections of papain 6 times, once a week. The concentration of beta-endorphin in medulla, pons, hypothalamus, central gray and plasma of chronic hypoxic rats were measured by radioimmunoassay. All animals were pretreated with sodium pentobartital (35 mg/kg) before experiment. 102 rats were randomly divided into three groups. Group 1 (n = 36). The respiratory rate (RR) and tidal volume (VT) were measured after intravenous injection of naloxone (NLX, 2 mg/kg), beta-EP (40 micrograms/kg, 160 micrograms/kg) or normal saline, Group 2 (n = 48). By the intracerebroventricular administration of NLX and beta-EP to the models, RR, VT and PaCO2 were observed after microinjection 5, 15, 30, 45, 60 min. Group 3 (n = 18). The respiratory effects of beta-EP after directly into the nucleus tractus solitari of the anaesthetised rats were investigated. RESULTS: The beta-EP contents in medulla, pons, hypothalamus, central gray and plasma of chronic hypoxic rats were significantly increased compared with control subjects (P < 0.01). It suggested that the pathophysiology of chronic hypoxic process infleuced the contents of beta-EP in the CNS and plasma in rats. Intracerebroventricular microinjection of beta-EP in normal rats, resulted in a significant decrease in RR (P < 0.05). No significant difference in RR and VT was observved after intravenous injection of naloxone (2 mg/kg) and beta-EP (40 micrograms/kg, 160 micrograms/kg) in treatment group compared with the control group (P > 0.05), Intracerebroventricular microinjection of NLX in chronic hypoxic rats, resulted in a marked increase in RR (P < 0.05) and central hypercapnic-sensitivity (t = 2.76, P < 0.05), Intracerebroventriculalr microinjection of beta-EP in chronic hypoxic models, resulted in severe respiratory depression after injection 15, 30, 45, 60 min (RR t = 3.41, 6.54, 6.97, 7.87, P < 0.01; VT t = 3.07, 7.27, 6.14, 6.08). CONCLUSIONS: These results indicate the beta-endorphin may be involved in central respiratory control of chronic hypoxic rats.

[Gastric cancer in the young].

OBJECTIVE: To determine the clinico-pathological characteristics of young patients with gastric cnacer. METHODS: From January 1968 through January 1993, among 6098 gastric cancer patients admitted for surgical treatment, 312 (5.1%) were < 35 years of age. The clinico-pathological data of these young patients were reviewed and compared to those of patients in the other age groups. RESULTS: In this series of young patients, female predominated. Histologically, there were more cases with poorly differentiated (49.2%) and muco-cellular (26.6%) adenocarcinoma. The most common gross morphology observed was of Borrmann type III (38.1%) and type IV (36.9%). The mojority of patients (83.3%) were in TNM stage III and stage IV. CONCLUSION: The prognosis of young patients with gastric cancer surgically treated is poor. This is due to the fact that most of them are in late stage and their cancers are poor in differentiation rather than their young age. To increase the awareness of cancer in apparently healthy young persons is important for early diagnosis and treatment, and better prognosis.

[Clinical and experimental studies on huodan tablet in treating pediatric viral myocarditis].

OBJECTIVE: To study the effect of Huodan Tablet in treating pediatric viral myocarditis. METHODS: Clinical manifestations and physical signs as well as laboratory examinations have been observed. RESULTS: The markedly effective rate was 68%, the total effective rate was 91.67% in therapeutic group, while in control group, the markedly effective rate was 30.83% and the total effective rate 70.84%. According to Ridit analysis, significant difference was shown between the two groups. r-therapeutic group = 0.5000, r-control group = 0.295 + 2 x 0.025, respectively. CONCLUSION: Huodan Tablet has no toxic and side-effect and can be taken safely and conveniently, it complies with the demands of WHO on new drug for prevention and cure of myocarditis.

[The effect of memory T lymphocytes on IgE production by B lymphocytes in patients with bronchial asthma].

OBJECTIVE: To study the role of CD(4)(+) CD(45)RO(+) T lymphocytes in the pathogenesis of bronchial asthma. METHODS: CD(4)(+) CD(45)RO(+) T lymphocyte subsets were isolated from peripheral blood of patients with bronchial asthma and healthy donors. Then they were cultured with self-B lymphocytes and designated as pokeweed mitogen (PWM)-stimulated and non-stimulated group. Secreted IgE was measured in the supernatant. RESULTS: CD(4)(+) CD(45)RO(+) T lymphocytes had a positive role on IgE production by B lymphocytes from patients of bronchial asthma in non-stimulated group, but the positive role did not exit in the presence of PWM. The effect of activated and non-activated CD(4)(+) CD(45)RO(+) T lymphocytes from patients with bronchial asthma on the secretion of IgE by B lymphocytes were further investigated. CONCLUSION: The function of activated CD(4)(+) CD(45)RO(+) T lymphocytes returned to normal level. It is suggested that CD(4)(+) CD(45)RO(+) T lymphocytes have adverse effects on IgE production in patients with bronchial asthma; which may be related to the pathogenesis of asthma.

[Changes of serum elastase, alpha 1-antitrypsin, procollagen III peptide and malonaldehyde in smokers with and without chronic obstructive pulmonary diseases].

OBJECTIVE: Serum elastase, alpha(1)-antitrypsin (alpha(1)-AT), procollagen III peptide (PIIIP) and malonaldehyde (MDA) were studied in smokers with and without chronic obstructive pulmonary disease (COPD) to explore the roles of these factors in COPD induced by smoking. METHODS: A design of 154-pair case-control study was used. alpha(1)-AT, MDA and PIIIP were determined with colorimetric method, fluo-spectrophotometric method and radio-immunological assay, respectively, in non-smoker control and smokers with and without COPD. RESULTS: Serum PIIIP content was increased in smokers and alpha(1)-AT decreased significantly in patients with COPD. CONCLUSION: Decreased alpha(1)-AT may be a susceptible factor in the development of COPD caused by smoking.

[Treatment of benign prostatic hyperplasia with laser through the endoscope].

OBJECTIVE: To evaluate the effect of transurethral laser prostatectomy (TULP) and its relation with the size of prostate. METHODS: 469 benign prostatic hyperplasia receiving TULP from 1988 to 1998 were reviewed retrospectively, and the methods of operation and preoperative and postoperative symptom were compared. The effect of 233 patients undergoing TULP was compared with the size of the prostate. RESULTS: The average value for IPSS of 469 patients decreased from 26.7 (preoperation) to 14.2 (postoperation). The mean value of maximum uroflow rate increased from 6.2 ml/s (preoperation) to 11.8 ml/s (postoperation). No difference was seen in postoperative effect between the transurethral contact and noncontact laser prostatectomy. If the estimating weight of prostate exceeded 50 g, the maximum flow rate after 3 years was decreased evidently, the value of IPSS increased to over 20. CONCLUSIONS: If the estimating weight of prostate is less than 50 g, TULP is indicated.

[Comparison of different alimentary reconstructions after total gastrectomy for cancer].

OBJECTIVE: To study the optimum reconstruction after total gastrectomy for malignant disease, especially the necessity of gastric substitute and duodenal passage. METHODS: Among 75 patients having total gastrectomy, five kinds of reconstruction were used: Braun, modified Braun (mBraun) I, II, III, and antigrade jejunal interposition (AJI). Postoperative complaints, body weight, food intake, serum nutritional parameters, complete blood count, half-emptying time of the gastric substitute, PNI, Visick index were evaluated. RESULTS: As compared with Braun group, the mBraun I and II groups showed less reflux esophagitis, higher nutritional parameters and PNI, while the operation time was similar. As compared with mBraun I and II groups, mBraun III and AJI groups showed better body weight, higher nutritional parameters, and PNI. CONCLUSIONS: It is essential to construct a gastric substitute and maintain the alimentary tract flowing through the duodenum after total gastrectomy for cancer, and the AJI may be a better choice in the operation of gastrectomy.

[Clinical protocols for the treatment of severe acute pancreatitis].

OBJECTIVE: To set up principles for the treatment of severe acute pancreatitis (SAP). METHODS: The therapeutic principles and results of SAP in three periods were analyzed. Period I (1980 - 1990) was characterized by early surgical intervention supplemented with integrated traditional and western medical therapy, period II (1991 - 1993) by integrated traditional and western medical therapy supplemented with surgery for early complications, and period III (1994 - 1997) by integrated traditional and western medical therapy supplemented with surgery for local infectious complications only. RESULTS: Because of the improvement of intensive care, the operation rates kept declining in the three periods. They were 77.59%, 54.55% and 29.55% with a mortality rate of 40.52%, 17.17% and 11.36%, respectively. CONCLUSIONS: Integrated traditional and western medicine has been effective in most SAP patients, and surgery is indicated only for late complications such as necrotizing infection. Early surgery should be restricted.

[Relationship between GSTM1 genotype and susceptibility to senile cataract].

OBJECTIVE: To study the relationship between the glutathione s-transferase gene deletion and cataract formation. METHODS: Blood cells of total of 77 cases with senile cataract and 76 controls were detected for GSTM1 gene, and the subcapsular epithelial cells of 22 cataract lenses were also detected for GSTM1 gene. RESULTS: The GSTM1 gene deletion rate in cataract group was 53.25% and that in the control group was 46.05%, they being not significantly different statistically (chi(2) = 0.750, P > 0.05, OR = 0.75). GSTM1 gene deletion rate in the subcapsular epithelial cells of 20 cases was basically consistent with that in blood cells. CONCLUSION: GSTM1 gene deletion is not related to senile cataract formation.

[Changes in expression of genes for insulin, glucagon and IGF-II in neonatal rats with malnutrition].

OBJECTIVE: To study the effects of malnutrition on the function of cells in fetal islet of pancreas. METHODS: A neonatal rat model with malnutrition was prepared by maternal food restriction (fed with less than half amount of the normal) during the 14th to 21st days of pregnancy. Expression of genes for insulin and glucagon in the pancreas, as well as expression of genes for insulin-like growth factor II (IGF-II) in the pancreas and liver of the rats were analyzed with Northern blotting technique. RESULTS: Pancreatic content of insulin mRNA was significantly lower in neonatal rats with malnutrition than that in the normal group (P < 0.05), but no obvious changes in glucagon mRNA in the pancreas was found. There was no significant difference in IGF-II mRNA content in the pancreas and liver between the neonatal rats with malnutrition and the normal control group. CONCLUSION: Malnutrition during rat fetal development could cause inhibition of expression of insulin gene, but no obvious effects on the expression of glucagon and IGF-II genes.

[Effects of tantalum and its oxide on exposed workers].

OBJECTIVE: To explore effects of tantalum and its oxide on the exposed workers. METHODS: Health examination for 113 workers exposed to tantalum and its oxide and a field survey for occupational hygiene in the workplace were carried out. RESULTS: Workplace air concentration of tantalum and its oxide averaged 0.1 - 7.6 mg/m(3). There was no significant difference in prevalence of symptoms and signs and biochemical, pulmonary function, chest X-ray and electrocardiograph examinations between the exposed and control workers. CONCLUSION: No adverse effect of tantalum and its oxide on the exposed workers was found.

[Effects of fasting on expression of insulin-like growth factor-I and IGF binding protein-1 genes in the liver of rats].

OBJECTIVE: To investigate the molecular mechanisms of body development and growth regulated by nutritional status. METHODS: Effects of fasting on the expression of insulin-like growth factor-I (IGF-I) and IGF binding protein-1 (IGFBP-1) genes were analyzed by Northern blotting technique in rats. RESULTS: Fasting for 48 hours, body weight of the rats decreased by 11.8%, level of blood glucose by 50.8% and content of IGF-I mRNA in the liver by 32.7% (P < 0.05). On the contrary, content of IGFBP-1 mRNA in the liver of fasted rats increased gradually, with a 2.5 time increase after 24 h fasting (P < 0.05). There was no significant change in the level of liver albumin mRNA after fasting. CONCLUSION: Fasting could inhibit the expression of IGF-I gene and enhance the expression of IGFBP-1 gene.

[Application of RF-II pedicle screw system in lumbar spondylolisthesis].

Thirty-eight patients with isthmic(31 cases), degenerative(6 cases), and post-traumatic(1 case) spondylolisthesis were treated with RF-II pedicle screw system. Eighteen patients were classified as Degree I lesion, 14 as Degree II lesion, and 6 as Degree III lesion. Postoperatively, 34 cases(90%) obtained an anatomical improvement obviously. The percentage of the patients who got excellent and good recovery was above 90%. We conclude that RF-II pedicle screw fixation system has the strongest three-dimensional rigidity and, attains complete and satisfactory decompression. The entire intervertebral disk in the level of the lesion is suggested to be removed. The system is ingeniously and originally designed, and is easy to obtain a simple and an effective reduction of the spondylolisthesis. Additionally, the application of the system can obtain a good interbody fusion by three steps: broadening the interbody space; stuffing the space with broken bones; and compressing the involved vertebral body. The authors considered that the RF-II screw system is one of the most ideal internal fixations for the treatment of lumbar spondylolisthesis.

[Studies on respiratory phase switching effects of Bötzinger complex].

The effects of electrical stimulation of Bötzinger complex (Bot.C) on respiratory rhythm were investigated in 40 urethane anesthetized adult rabbits. The results were as follows. (1) A short train stimulation delivered in the early inspiratory phase produced a transient inhibition of phrenic discharge. The stimulus, when delivered in the mid or late inspiratory phase, could cause a premature termination of the inspiratory phase ("inspiratory off-switch") and a switching to the expiratory phase, which was accompanied with the reduced duration of the consecutive expiratory phase. There was a negative linear correlation between the threshold intensity of inspiratory off-switching and delivery time of stimulation. (2) A short train stimulation delivery in the expiratory phase elicited a transient phrenic discharge. The discharge in the late expiratory phase was followed by a premature onset inspiration. This effect was also dependent on the strength and delivery time of the stimulus. The results suggest that the Bot.C is involved in the central control of respiratory phase-switching.

[Study on stable unified disinfectant capable of releasing chlorine dioxide].

Chlorine dioxide is a new type of forceful disinfectant, but it cannot be stored and transported as a gas and is very toxic. In this paper, a stable unified disinfectant capable of releasing chlorine dioxoide was introduced. This disinfectant was stable in dry state and could produce chlorine dioxide when it contacted with water. Its solution containing 3.3 mg.L-1 chlorine dioxide could kill 100% of B. subtilis var. niger spores after exposure for 5 minutes. Its solution containing 434 mg.L-1 chlorine dioxide could destroy 100% of HBsAg after exposure for 30 minutes. The content of chlorine dioxide in this disinfectant powder decreased by about 12% following storage at 56 degrees C for 14 days. It was essentially non-corrosive to stainless steel and moderately corrosive to carbon steel.

[Transposition of expanded superficial cervical artery skin flaps for the repair of cicatricial contracture of neck].

OBJECTIVE: In order to reduce the complication of the transposition of superficial cervical artery skin flap for the repair of neck defect, the method of pre-expanded skin flap was designed, and its clinical result was observed. METHODS: From March 1995 to October 1997, 12 cases with cicatricial contracture of the neck were treated by the following methods, preexpanded superficial cervical artery skin flap, and then transposed it for the reconstruction the cervical scar after burns. There were 8 males and 4 females and the age ranged from 6 to 32 years. A maximal size of flap was 35 cm x 14 cm and a minimal size was 16 cm x 7 cm. RESULTS: All the flaps were survived except one, which partial necrosis occurred in the tip. Postoperative follow-up for 6 months to 3 years showed that the physiological angle of cervico-mandibular angle was recovered and the appearance of flap was satisfactory without swelling and contracture. CONCLUSION: The preexpanded superficial cervical artery skin flap has many advantages, and it is particularly suitable for reconstruction of severe cervical contracture after extensive burns.
