Model-based design and implementation of secure, interoperable EHR systems.

For designing and implementing secure, interoperable, portable, and future-proof EHR systems, a comprehensive and standardized methodology supported by appropriate tools has to be established and applied. Based on the component paradigm, the ISO Reference Model - Open Distributed Processing has been used to describe the different views on information systems deploying the appropriate vocabulary for each single model view. The concepts considered rank from legal, organizational, and functional up to technical aspects of systems. The harmonization of vocabularies can be performed by meta-languages. The approach has been demonstrated for the model-based design, implementation and maintenance of a clinical study distributed over the Internet.

Public key infrastructures for health.

Introducing the technological step into this new 3rd millennium, advanced communication means like global networks including the Internet become more and more important for a fast and convenient information exchange across regional and even national borders. Concerning the sector of public and private healthcare and welfare, new health information system, or citizens' information systems in general, are coming up to meet the needs of the whole information society. Thus, developing and implementing those systems is one of the most important aims of the present and the near future. Access to, and communication of, relevant patient-related administrative and medical information items always means a secure and trustworthy way of dealing with data. Concerning the main aspects of specific legal, social, ethical, technical, organisational, and even political requirements for secure access and secure communication of health data in terms of data protection, data security, privacy, safety and quality using unprotected networks as, e.g., the Internet, there is a strong and even growing need for the fundamental technology of Trusted Third Parties to meet the whole range of the security categories as integrity, confidentiality, availability, accountability, and access control.

Time stamp services for trustworthy health communications.

Trustworthy communication and co-operation requires in general static TTP services describing status and relation of communicating principals as well as their corresponding keys and attributes. Additional TTP services are needed to provide trustworthy information about dynamic issues of communication and cooperation such as message identifier, time and location of processes, workflow relations and system behaviour. Certified time stamps are important notary's functions dealing with the services mentioned above. Within the European RESHEN project, the ONCONET regional health network for improving cancer care has been moving from system-related to certified time stamps using a local secure time stamp server or deploying remote time stamp services provided by an accredited CA. After implementing such services at the beginning of 2002, an evaluation of the operation modes will be performed during 2002 to derive recommendations for optimisation of these important services in national and international health networks.
