Molecular epidemiology of Vibrio cholerae O139 in China: polymorphism of ribotypes and CTX elements.

Vibrio cholerae O139, the second etiological serogroup of cholera, triggered the first outbreak of O139 cholera in China in 1993. To analyze the clone polymorphism of O139 isolates in China, 117 strains of V. cholerae O139, isolated from different areas in China between 1993 and 1999, were selected to characterize the phylogenetic relationships by molecular techniques. Analysis of restriction fragment length polymorphism in the conserved 16S rRNA gene revealed seven different ribotypes within the 117 strains. Among these strains, there were eight that lacked the cholera toxin gene (ctxAB), zot, and the repetitive sequence (RS); these eight strains belonged to three individual ribotypes. Our results suggested that V. cholerae O139 strains in China had clone diversity in phylogeny. The results of our hybridization patterns for CTX genetic elements (ctxAB, zot, and RS) showed that CTXPhi genomes in most V. cholerae O139 strains had two or more copies and had extensive restriction patterns even for the strains which belong to the same ribotype. For 22 (20.1%) strains, the copies of ctxAB were different from those of zot, suggesting that a ctxAB-negative CTXPhi genome may exist in O139 strains. This ctxAB-negative CTXPhi genome may coexist with the intact CTXPhi genome in a strain. In addition, the dendrogram for I-CeuI-generated pulsed-field gel electrophoresis patterns showed that V. cholerae serogroup O139 has a closer relationship with one strain of serogroup O22 than with the strains of serogroup O1. The results of this study showed the clonal diversity and the distribution of O139 strains in China, suggesting multiple origins of the O139 cholera epidemic or sporadic events.

On-line separation and preconcentration of silver ion on amidino-thiourea immobilized glass beads packing in flow injection analysis with atomic absorption spectrometric detection.

Amidino-thiourea immobilized glass beads (AGB I) were prepared and used as the microcolumn packing for the flow injection (FI) on-line separation and preconcentration of Ag(I) coupled with atomic absorption spectrometry (AAS) determination. Base metal ions and anions with a concentration of 2.0 mg mL(-1) had no interference with the determination of Ag(I). The limit of detection (LOD) of Ag(I) for a preconcentration time of 60 s with a sampling flow rate of 5.0 mL min(-1) for 40.0 ng mL(-1) of Ag(I) were 0.50 ng mL(-1) with the peak-height absorbance mode and 1.26 ng mL(-1) with the peak-area absorbance mode, respectively. The relative standard deviations (RSD) of 7 replicate determinations were 0.9% and 0.7% for the peak-height absorbance (H) and the peak-area absorbance (A), respectively. The method was successfully applied to the determination of Ag(I) in ore samples.

[Effect of transfection of hEndostatin gene on CNE2 cell xenograft growth in nude mice].

BACKGROUND & OBJECTIVE: Tumor angiogenesis play an important role in growth and metastasis of cancer. Angiogenesis inhibitors induce apoptosis in cancer by inhibiting tumor angiogenesis and have strong inhibiting effect on both growth and metastasis of cancer. This study was designed to explore the effect of transfection of human endostatin gene on nasopharyngeal carcinoma CNE2 cells xenograft growth in nude mice. METHODS: The plasmids (pBlast-hIL-hEndostatin, pBlast-hEndostatin, and pBlast-MCS) were transfected and lipofectin-mediated into the CNE2 cell line. The biological activity of secreted hEndostain from gene-transferred cell lines was determined using MTT method in vitro. Then the transfected CNE2 cells were injected into the nude mice and tumorigenicity of CNE2 was observed in vivo. RESULTS: The supernatant of CNE2 cell transfected with pBlast-hIL-hEndostatin effectively inhibited the growth of endothelial cell (ECV304). The volume and the weight of tumor in pBlast-hIL -hEndostatin transfecting cells group were less than those in control group (P< 0.01). The growth speed of tumor in pBlast-hIL-hEndostatin transfecting cells group was slower than that in control group. CONCLUSION: Transfection of hEndostatin gene could inhibit CNE2 cell growth in nude mice.

[Kinetic study on the variety of lymphocytes and CD34+ cell subsets during autologous peripheral blood stem cell transplantation].

OBJECTIVE: To monitor the variety of lymphocytes and CD34(+) cell subsets in peripheral blood (PB) and peripheral blood progenitor cell (PBPC) during mobilization and collection, and determine the optimal time for PBPC collection. METHODS: Flow cytometry (FCM) was used to detect the concentration of lymphocyte subsets, such as CD3, CD4, CD8, NK and CD19, as well as CD34(+) cell subsets, such as CD34(+)CD38(-), CD34(+)Thy1(+) and AC133(+) in PB or PBPC during autologous peripheral blood stem cell transplantation (APBSCT) during high dose chemoradiotherapy (HDC) with recombinant human granulocyte colony-stimulating factor (rhG-CSF) in 47 consecutive patients with malignant solid tumor. Colony culture in vitro was performed when evaluating the clonogenic capacity of the PBPC. RESULTS: After mobilization, the concentration of lymphocyte subsets in PB decreased to the level below the baseline with CD3 (P = 0.007) and CD8 (P = 0.016) decreased significantly. On the contrary, the total CD34(+) cell and the other CD34 subsets in PB increased significantly (P < 0.05). CD34(+) cell peak of the blood was observed with the median time of 16 d (15-17 d) from the mobilization. The concentration of progenitor cell was the highest in the first apheresis. There was no significant change in PBPC lymphocyte subsets, except CD4 and CD4/CD8 which decreased below PB (P < 0.000 5). There was a significant linear correlation between PB CD34(+) cells and PBPC CD34(+) cells, CD34(+)CD38(-), CFU-GM or BFU-E (P < 0.05), but none between PB CD34(+) cells and PBPC MNC, CD34(+)Thy1(+) or AC133(+) cells. There was a significant correlation between PBPC CD34(+) cells and PBPC CD34(+)CD38(-), CD34(+)Thy1(+), MNC, CFU-GM or BFU-E (P < 0.05), but none between PBPC CD34(+) cells and AC133(+) cells. After transplantation, the CD4/CD8 ratio decreased markedly and CD19 cells returned to the normal range rapidly. CONCLUSION: PBPC increases significantly during mobilization by HDC and rhG-CSF. The immunosuppressed state exists after APBSCT. The peripheral blood CD34 cell count is able to predict autograft yield reliably and can be the best indicator for the clinical collection of PBPC.

[Expression of vascular endothelial growth factor and its receptors KDR and Flt1 in acute myeloid leukemia].

OBJECTIVE: To evaluate the expression of vascular endothelial growth factor (VEGF) and its receptors KDR and Flt1 in patients with acute myeloid leukemia (AML). METHODS: The expression of VEGF and its receptors mRNA was assayed by RT-PCR, the plasma of VEGF by ELISA. RESULTS: In 13 AML cell lines, the expression of VEGF, KDR and Flt1 mRNA were found in 13 (100%), 7 (53.8%) and 12 (92.3%), respectively. There were 21 (65.6%), 1 (3.1%), and 17 (53.1%) of 31 (AML) patients bone marrow mononuclear cells (BMMNCs) expressing VEGF, KDR and Flt1 mRNA, respectively. None of BMMNCs from 3 normal donor and CD(34)(+) cells from 2 normal donor was found to express VEGF, KDR and Flt1 mRNA. The plasma level of VEGF of 39 patients (new diagnosed, relapsed and secondary-AML) before treatment was (135.3 +/- 87.9) ng/L which was significantly higher than that of 15 complete remission (CR) patients (80.6 +/- 36.9) ng/L and 12 normal donors (80.6 +/- 33.1) micro g/L (P = 0.028, 0.007). The plasma level of VEGF of 15 non-responsive patients was (188.2 +/- 118.6) ng/L after two cycles of chemotherapy which was higher than that of 20 CR patients [(104.2 +/- 30.9) ng/L] (P = 0.004). CONCLUSION: VEGF and its receptors KDR and Flt1 mRNAs were expressed in BMMNCs of AML patients. The plasma level of VEGF directly affected the response to chemotherapy in AML patients.

[Efficient generation of recombinant adenoviruses expressing antiangiogenic fragment of human thrombospondin1].

OBJECTIVE: To construct recombinant adenoviruses expressing antiangiogenic fragment of human thrombospondin1 (TSP1f). METHODS: TSP1f cDNA was amplified by RT-PCR from normal human peripheral blood mononuclear cells and was subcloned into a shuttle vector pShuttle-CMV. After sequence confirmation, the resultant plasmid was linearized by the restriction endonuclease Pme I and cotransformed with the supercoiled adenoviral vector pAdEasy-1 into Escherichia coli strain BJ5183. Recombinants were selected by Kanamycin resistance and screened by restriction endonuclease digestion. Then, the recombinant adenoviral construct was cleaved with Pac I and transfected into the packaging cell line 293. The adenoviral vector ADV-TSP1f was propagated in 293 cells and purified by cesium chloride (CsCl) density centrifugation. PCR and Western blot analysis were performed to confirm TSP1f expression. RESULTS: Of 43 Kanamycin-resistant colonies obtained from cotransformation, all of the 10 smallest ones were the correct recombinants. TSP-1f was expressed efficiently by ADV-TSP1f. The virus stock titer after CsCl banding was 1.0 x 10(11) pfu/mL. CONCLUSIONS: Generating recombinant adenoviruses using AdEasy System results in highly efficient viral production and significantly decrease the time required to construct usable viruses. ADV-TSP1f can be further used in in vivo gene therapy studies.

[A clinical study of rhG-CSF in accelerating hematological recovery following autologous hematopoietic stem cells transplantation].

BACKGROUND & OBJECTIVE: High dose chemotherapy and radiotherapy supported by autologous hematopoietic stem cells transplantation(AHSCT) can improve the efficacy of some malignant solid tumors. Successful AHSCT should owe to rational usage of recombinant human granulocyte colony-stimulating factor (rhG-CSF) in some extent. The purpose of this study was to observe the role of rhG-CSF in hematological recovery after AHSCT for patients with solid tumors. METHODS: A total of 130 patients with solid tumors were divided into two groups. The rhG-CSF group received rhG-CSF at the dose of 250-300 microg daily from day 6 until WBC recovering to 5.0 x 10(9)/L. The control group did not receive rhG-CSF after AHSCT. RESULTS: A total of 132 person-times AHSCT were finished and 2 patients of them experienced twice transplantation. Twenty-four patients received autologous bone marrow transplantation (ABMT), 12 of them received rhG-CSF following AHSCT and the others did not. One hundred and six persons underwent autologous peripheral blood stem cells transplantation (APBSCT), 47 patients received rhG-CSF and 61 did not. (1)In rhG-CSF group and control group of ABMT,the median days of WBC up to 5.0 x 10(9)/L were 14 days and 24 days (P< 0.001),respectively. The median time of hospitalization was 33 days and 41 days (P< 0.05),respectively. There was no statistical difference in median time of PLT recovering to 20 x 10(9)/L and 50 x 10(9)/L in the two groups.(2)In rhG-CSF group and control group of APBSCT,the median time of WBC recovering to 5.0 x 10(9)/L were 10 days and 13 days (P< 0.01), respectively. The median time of hospitalization was 17 days and 20 days(P< 0.01),respectively. There was no significantly statistical difference in median time of PLT recovering to 50 x 10(9)/L in the two groups. (3)The frequency of infectious complication was no statistical difference between two groups (P=0.45). CONCLUSION: rhG-CSF administration after AHSCT can obviously accelerate the recovery of WBC and shorten the time of hospitalization, but there is no significant effect on the recovery of PLT. Further study is needed to investigate whether rhG-CSF can reduce the infectious complication.

[Autologous peripheral blood stem cell transplantation in the treatment of patients with primary large mediastinal B-cell lymphoma].

OBJECTIVE: To evaluate the therapeutic effectiveness and safety of high dose chemoradiotherapy (HDC) combined with autologous peripheral blood stem cell transplantation (APBSCT) in the treatment of patients with primary large mediastinal B-cell lymphoma (PMLBL). METHODS: Among nine patients with PMLBL treated with APBSCT, high dose chemotherapy combined with total body irradiation (TBI) or total lymph node irradiation (TLI)/subtotal lymph node irradiation (STLI) were given to 7 patients and high dose chemotherapy only as pre-transplantation regimen in 2 patients. All patients received supplementary irradiation at the primary mediastinum after transplantation. RESULTS: After a median follow-up of 24 (10 - 84) months, 5 patients achieved complete remission (CR) and 3 patients partial remission (PR) after induction chemotherapy. One patient developed progressive disease before transplantation. All 5 patients who achieved CR after induction chemotherapy became disease-free survivors (DFS). One of 3 patients who achieved PR after induction chemotherapy was DFS, the other two died in the third and fifth month, respectively. The patient who relapsed before transplantation died in the sixth month carrying the disease all along. According to the life table method, the cumulative probability of 7-year DFS and overall survival (OS) were both 66.7%. No transplant-related mortality was found. CONCLUSION: High dose chemoradiotherapy combined with autologous peripheral blood stem cell transplantation is a highly potential therapeutic treatment for poor prognostic primary mediastinal large B-cell lymphoma.

[Autologous peripheral blood stem cells mobilization with etoposide plus rhG-CSF versus cyclophosphamide plus rhG-CSF].

BACKGROUND & OBJECTIVE: It is important to get high quality autologous peripheral blood stem cell (APBSC) for successful peripheral blood stem cell transplantation. Cyclophosphamide (CTX) plus recombined human granulocyte colony-stimulating factor (rhG-CSF) is standard regimen for APBSC mobilization. Etoposide plus rhG-CSF is another regimen for mobilization in recent years. The purpose of this study was to observe and compare the effects of these two regimens in APBSC mobilization for the patients with malignant lymphoma and germ cell tumors. METHODS: Fifty-two patients were divided into two groups according to the regimen of mobilization. In CTX group, 26 patients were injected CTX 3.5 g/m(2) intravenously. The other 26 patients in VP-16 group, VP-16 1000 mg/m(2) or 1500 mg/m(2) were given intravenously at random. All patients received rhG-CSF once daily as subcutaneous injection at the dose of 5 microg x kg(-1) x day(-1) from the day of the nadir of white blood cell (WBC) till the day before the last APBSC harvest in both groups. APBSC harvest was performed daily when WBC recovered more than 2.5x10(9)/L in CTX group and 5.0x10(9)/L in VP-16 group. APBSC harvest finished when accumulated mononuclear cells (MNCs) were more than 5x10(8)/kg or CD34(+) cells were more than 2x10(6)/kg. Collected APBSC was infused following condition regimen. Blood parameters, account of harvested cells, the time of hematopoietic reconstitute, and adverse effects were compared between two groups. RESULTS: Following administration of CTX or VP-16, the nadirs of WBC and PLT in CTX group appeared significantly earlier than that in VP-16 group. The doses and times of rhG-CSF administration were not significantly different between the two groups. The start was later and the times of APBSC apheresis were less significantly in VP-16 group. The processed blood volume in single apheresis was more in VP-16 group. Speed and duration of single apheresis were similar in the two groups. Total MNCs were similar in the two groups, but the numbers of MNCs and CD34(+) cells collected in the first harvest and total number of CD34(+) cells collected were significantly more in VP-16 group. With equal quantity infusion of MNCs and CD34(+) cells infusion, hematopoietic reconstitution was achieved in both groups and the time of reconstitution was similar. Nausea and vomiting were more frequent in CTX group. Obvious PLT decrease was observed in VP-16 group. Other adverse effects were similar. CONCLUSION: CTX or VP-16 plus rhG-CSF both were safe and effective regimens for APBSC mobilization. More CD34(+) cells can be harvested by mobilization with VP-16 plus rhG-CSF than mobilization with CTX plus rhG-CSF. And the APBSC mobilized with VP-16 plus rhG-CSF can assure successful hematopoietic reconstitution.

Analysis of inhibitory action of modified U1 snRNAs on target gene expression: discrimination of two RNA targets differing by a 1 bp mismatch.

The modified U1 snRNA gene can suppress expression of a target transgene. In the present study, its potential utility to inhibit a dominant negative/gain of function mutation is explored. Using a green fluorescent protein (GFP) target gene, inhibition was achieved in all cells transduced with U1antiGFP directed at multiple sites within GFP. Using a chloramphenicol acetyltransferase (CAT) target gene, inhibition was not increased by increasing the hybridization domain from 10 to 16 bp or when a site in an upstream exon or intron was targeted. To determine if a U1 anti-target design could discriminate between two transcripts that differ by a 1-2 bp mismatch, GFPtpz and GFPsaph were chosen as targets because they share sequence homology except for three regions where a 1, 2 or 3 bp mismatch exists. The results demonstrated that U1antiGFP correctly reduced its cognate GFP expression by >90% and therefore U1 anti-target constructs are able to discriminate a 1 or 2 bp mismatch in their target mRNA. Thus, these U1 anti-target constructs may be effective in a strategy of somatic gene therapy for a dominant negative/gain of function mutation due to the discreteness of its discrimination. It may complement other anti-target strategies to reduce the cellular load of a mutant transcript.

[Mobilization of autologous peripheral blood stem cells by cytosine arabinoside combined with recombinant human granulocyte colony-stimulating factor].

OBJECTIVE: To observe the effect of cytosine arabinoside (Ara-C) combined with recombinant human granulocyte colony-stimulating factor (rhG-CSF) on mobilization of autologous peripheral blood stem cells (APBSCs) among malignant lymphoma patients and investigate appropriate dose of Ara-C. METHODS: Twenty-two patients with malignant lymphoma were randomly divided into two groups: in group A 3 g/m2.d-1of Ara-C was administered by intravenous drip in 2 divided doses for two days with the total dose of 6 g/m(2); in group B 5 g/m(2).d(-1) of Ara-C was administered by intravenous drip in two divided doses for two days with the total dose of 10 g/m(2). Blood routine examination was made every day. rhG-SCF at the dose of 300 microg.body(-1).d(-1) was injected subcutaneously once a day since the next day when the white blood cells reached the nadir until the end of APBCS harvest. APBSC harvest began when WBC >/= 5.0 x 10(9)/L and finished when the accumulated mononuclear cells >/= 5 x 10(5)/kg. RESULTS: The median of days when nadir of WBC appeared was 7 days in group A and 10 days in group B. The median of absolute neutrophil count (ANC) at ite nadir was 0.9X109/L in group A and 0.2 x 10(9)/L in group B. The median of days when ANC reached its nadir was 9 days in group A and 13 days in group B. The median of dose of rhG-CSF was 4.35 microg.kg(-1).d(-1) in group A and 4.35 microg.kg(-1).d(-1) in group B. The median of day when rhG-SCF administration began was the 11th day in group A and the 12th day in group B. The median of rhG-SCF administration days was 5 days in group A and 6 days in group B. The median of day when APBSC harvest began was the 15th day after Ara-C administration in group A and the 16th day in group B. The median of harvest time was 2 days in both groups. The volume, speed, and time of each apheresis were similar in the two groups. The time consumed and number of APBSCs collected in each harvest and the total number of APBSCs collected were similar in these two groups. CONCLUSION: Ara-C combined with rhG-CSF is safe and highly effective for APBSC mobilization. 6 g/m(2) is the suitable dose for APBSC mobilization.

Selective cooperation between fatty acid binding proteins and peroxisome proliferator-activated receptors in regulating transcription.

Lipophilic compounds such as retinoic acid and long-chain fatty acids regulate gene transcription by activating nuclear receptors such as retinoic acid receptors (RARs) and peroxisome proliferator-activated receptors (PPARs). These compounds also bind in cells to members of the family of intracellular lipid binding proteins, which includes cellular retinoic acid-binding proteins (CRABPs) and fatty acid binding proteins (FABPs). We previously reported that CRABP-II enhances the transcriptional activity of RAR by directly targeting retinoic acid to the receptor. Here, potential functional cooperation between FABPs and PPARs in regulating the transcriptional activities of their common ligands was investigated. We show that adipocyte FABP and keratinocyte FABP (A-FABP and K-FABP, respectively) selectively enhance the activities of PPARgamma and PPARbeta, respectively, and that these FABPs massively relocate to the nucleus in response to selective ligands for the PPAR isotype which they activate. We show further that A-FABP and K-FABP interact directly with PPARgamma and PPARbeta and that they do so in a receptor- and ligand-selective manner. Finally, the data demonstrate that the presence of high levels of K-FABP in keratinocytes is essential for PPARbeta-mediated induction of differentiation of these cells. Taken together, the data establish that A-FABP and K-FABP govern the transcriptional activities of their ligands by targeting them to cognate PPARs in the nucleus, thereby enabling PPARs to exert their biological functions.

[Clinical evaluation of effects from neo-adjuvant chemotherapy with epirubicin plus paclitaxel in cases of locally advanced breast cancer].

Neo-adjuvant chemotherapy of epirubicin plus paclitaxel was administered to 23 patients with locally advanced breast cancer (including 13 cases of stage IIb, 6 of stage IIIa, and 4 of stage IIIb). All patients were female. They were treated with epirubicin 60 mg/m2, on day 1, by i.v. followed paclitaxel 150 mg/m2 by 3 hours continuous infusion on day 2 and every 3 weeks repeatedly. Premedication with dexamethasone, ondansetron, diphenhydramine and cimetidine were administered to prevent gastroenteric and allergic reactions before chemotherapy. Two to 4 cycles were used. Ten out of 23 patients had a complete response, 10 had partial response, and 3 had no change. The response rate was 87% (20/23). Six out of 23 patients underwent breast conserving surgery as tumor size had become smaller and downstaging was realized after neo-adjuvant chemotherapy. The major toxicities included neutropenia, myalgia, arthralgia, nephrotoxicity, gastroenteric reactions, alopecia and flushing to the face. However, these were well tolerated in these patients.

[High dose chemoradiotherapy with autologous hemotopoietic stem cell transplantation for treatment of patients with advanced Hodgkin's lymphoma: a report of 11 cases].

BACKGROUND & OBJECTIVES: High dose chemoradiotherapy (HDT) with autologous hemotopoietic stem cell transplantation (ASCT) has become one of the important salvaged treatments for the Hodgkin's Lymphoma(HL) patients with relapsed or resistant disease, but its role as the primary treatment remains indefinite. This study was designed to further evaluate its status in the combined modality treatment, especially, to discuss its value in the primary treatment of the patients with advanced disease and poor prognosis. METHODS: Eleven patients who had advanced or relapsed disease with poor prognosis were enrolled in this study. Among them, 9 patients were primary treatment, and 2 patients were secondary treatment. One patient received autologous bone marrow transplantation (ABMT), and 10 patients received autologous peripheral blood stem cell transplantation (APBSCT). After induction treatment 4 cases achieved complete response (CR) and 7 cases achieved partial response (PR). High dose chemotherapy combined with total body irradiation (TBI) or total lymph node irradiation(TLI)/subtotal lymph node irradiation(STLI) were applied in 7 cases and high dose chemotherapy alone was used in 4 cases as preparative treatment before transplantion. Five cases received complementary irradiation in the primary sites after transplant. RESULTS: These cases who had been CR before transplantation were consolidative therapy, and among the rest with PR, 2 cases achieved CR, 1 cases PR, and 4 cases stable disease(SD). Furthermore all these patients who maintained SD had bone involvement. With a median follow-up of 13(1-84) months for all patients, all of them were alive at that time. Four cases were event-free survival (EFS); Four cases with bone involvement are progression-free survival (PFS). Three cases experienced relapse after transplant, one of them was EFS for 42 months again after a local relapsed site irradiation; the other two cases were being given further salvaged treatment now. According to the life tables method, the cumulative probability of 6-year PFS and OS was 55.68% and 100%, respectively. The major transplant-related toxicity was bone marrow suppression of grades IV. No obvious cardiac, hepatic, and nephritic toxicity was found, neither transplant relative mortality. CONCLUSIONS: HDT combined with ASCT is a method that worth to be further study to treat the patients with advanced or relapsed Hodgkin's Lymphoma with poor prognosis.

[Evaluation of two different regimens as neoadjuvant chemotherapy for breast cancer].

OBJECTIVE: To compare the efficacy and toxicity of two different regimens as neoadjuvant chemotherapy for breast cancer. METHODS: Forty-eight patients with stage II, III breast cancer as proved by cytology biopsy, were treated with either 5-Fu, epirubicin, cyclophosphamide (FEC) or epirubicin, paclitaxel (ET) regimens for 2 cycles every 3 - 4 weeks. Clinical responses in the breast and lymph nodes were assessed after 2 cycles of neoadjuvant chemotherapy. Patients in FEC arm received combination of 5-fluorouracil (5-Fu) 500 mg/m(2) by 4-hour continuous infusion on D1 and D8, epirubicin (EPI) 50 mg/m(2) by intravenous injection on D1, and cyclophosphamide (CTX) 500 mg/m(2) by intravenous injection on D1 and D8. Patients assigned to the ET arm received EPI 60 mg/m(2) by intravenous injection on D1, paclitaxel (TAX) 150 mg/m(2) by 3-hour continuous infusion on D2. All patients were treated by operation 2 weeks later and radiotherapy was added to some. RESULTS: For primary tumor in the breast, the overall response rate (RR) was 50.0% (12/24) in FEC arm and 79.2% (19/24) in ET arm. One patient showed clinical complete response (cCR), 11 partial response (PR), 12 no change (NC) after the FEC therapy, while 1 patient showed CR, 18 PR, 5 NC after ET therapy. There was no pathologic complete response or progressive disease, though a higher proportion of RR was observed in stage II than stage III patients in these two groups. Clinically palpable axillary lymph nodes which had been found in all 48 patients before 2 cycles of treatment, 50.0% (12/24) in the FEC patients and 66.7% (16/24) in the ET patients became in-palpable. The major toxicity, including leukopenia, gastroenteric reactions, were similar in both groups, but alopecia was more severe and arthralgia, myalgia, neurotoxicity and flushing of face were the unique features of the ET regimen. CONCLUSION: Neoadjuvant chemotherapy with two different regimens were effective to the primary tumor and axillary metastatic lymph nodes of breast cancer, and the side effects were tolerable. Higher efficacy and more side effects are observed in ET than in FEC regimen.
