[Evaluation of pulmonary function in children of preschool age treated in the neonatal period with distension methods].

Within the framework of a long-term follow up of the development of neonates with low birth-weight the authors investigated pulmonary functions in a group of 20 children aged 5.5 years treated during the neonatal period on account of early asphyctic syndrome, hyaline membrane syndrome and bronchopulmonary dysplasia by the distension method, using a continual positive pressure. The birth-weight of the investigated children varied between 600 and 1500 g. While the authors observed a significant reduction of the forced vital capacity, as compared with the appropriate values for the given age, sex and anthropometric data in almost 40% of the children, the values indicating obstructive changes (one-second vital capacity - FEV1 and mean flow rate - FEV25-75%) were reduced only in 15% of the investigated children. The revealed changes detected during the symptom-free period with a normal clinical picture are the functional background for aimed prevention of chronic bronchopulmonary disease and effective rehabilitation.

[Perspectives on reducing morbidity and mortality in Prague].

From data obtained by means of a questionnaire from Prague neonatologists from seven maternity hospitals and four intensiv care units in Prague in 1980-1984 a detailed analysis was made of the specific morbidity and mortality of neonates during the first seven days after birth in a large neonatal population, incl. causes of morbidity and mortality. The analysis was focused on the morbidity of those nosological non-inflammatory and inflammatory units which threaten the life of neonates, which cause morbidity and where mortality is the extreme pole. Data and causes were compared with analogous data from countries with the highest standard of neonatal care--Sweden and Finland. The authors outline the procedure used for analysis of the necessary data in order to differentiate inevitable and evitable risks which damage the foetus and neonate and they analyzed the causes of different data from obstetric practice focused on prevention of hypoxia, immaturity, lethal congenital defects and factors from differentiated neonatalogical care.

Development of flexor spastic signs in preterm infants.

Fifty-five preterm infants with birthweights between 800 and 2000g and gestational ages between 25 and 33 weeks were repeatedly examined for the Rossolimo and Mendel-Bechterew lower-limb reflexes. Up to 34 weeks conceptional age (CA) an abduction-extension pattern was characteristic. Between 34 and 36 weeks CA the response changed to one implying flexor spasticity, which in some cases lasted throughout the first postnatal trimester. It is thought that this response occurring around 34 to 38 weeks CA is a sign of normal developmental maturation. On the other hand, an extension-abduction response after 38 weeks CA and a flexor response after the first trimester is indicative of delayed motor development and might suggest an upper motor neuron lesion.

[Experience in studying flexor pyramidal signs in premature infants].

Fifty-five premature newborns weighing from 800 to 2000 g were observed for several weeks. The subject of examination was flexory pyramidal signs--the Rossolimo and Mendel-Bekhterev symptoms. The presence of these reflexes after 12 weeks of life may signal impairment of the upper motoneuron in children with perinatal pathology.
