Platelet alpha-granule release in 22 patients affected by chronic myeloproliferative disorders with thrombocytosis and seven subjects with thrombocytosis secondary to splenectomy were studied. We found elevated beta-thromboglobulin (BTG) and platelet factor 4 (PF4) plasma levels in all patients. Intraplatelet content of BTG and PF4 was decreased in patients with idiopathic thrombocythaemia (IT) and idiopathic myelofibrosis (IMF). The BTG and PF4 results were also expressed as the ratio plasma BTG and PF4: whole blood platelet count. In patients with IT, BTG: whole blood platelet count ratio was low, conversely, the same ratio was high in patients with IMF. In conclusion, our results suggest the presence of an abnormal, BTG-deficient clone in IT and a peripheral platelet activation in IMF.

18 patients (14 men, 4 women) with a mean age of 64.3 years (range 41-75) and with chronic cardiac failure, NYHA functional class 1-2 (12 cases) or 2-3 (6 cases), insufficiently responsive to digitalis and diuretics, were treated for 4 weeks with Ro 12-4713, a minoxidil-like drug. The effects were assessed at rest and during maximal exercise, each patient was his own control. All patients completed the study. The treatment did not affect the body weight, the heart rate or the blood pressure, but it should be stressed that all patients were normotensive. The ejection fraction (by 99mTc pertechnetate scintigraphy) increased from 42.0 +/- 10.0 (resting means +/- SD) to 48.9 +/- 8.7%, and from 44.7 +/- 10.1 to 57.9 +/- 8.7% (end-exercise values), both changes being significant (p less than 0.0001). The ejection rate increased in a similar manner. The resting stroke volume increase from 53.5 +/- 13.6 to 60.2 +/- 16.5 ml (p less than 0.005), but the exercise stroke volume did not increase significantly. The cardiac output increased both at rest and during exercise, but the changes did not reach statistical significance. The end-diastolic volume decreased from 132.6 +/- 126.9 +/- 47.0 to 126.9 +/- 42.3 ml (resting values), and from 151.5 +/- 44.4 to 120.6 +/- 36.6 ml (exercise), the changes being significant (p less than 0.005). The physical work capacity increased, i.e. the duration of exercise increased from 7.8 +/- 3.2 to 9.9 +/- 1.9 min (p less than 0.05) and the 'quality of life' (symptoms related to heart failure) improved (fewer symptoms) in 14/18 patients.(ABSTRACT TRUNCATED AT 250 WORDS)
Pulsed extremely low-frequency electromagnetic fields interact with rat liver regeneration following partial hepatectomy when delivered to the rats immediately after the operation and every 12 hr thereafter. This interaction results first in an increased ornithine decarboxylase activity, an enzyme used as an early marker of cell growth. The rate of labeled thymidine incorporation into DNA is also increased by the treatments with magnetic fields during the early phases of liver regeneration. Glycogen depletion and lipid accumulation, two well-known early peculiar phenomena of liver regeneration following partial hepatectomy, are quantitatively decreased by the treatments with electromagnetic fields. The recovery to normal glycogen and lipid contents is completed within 5 days after surgery, instead of 7 days as found in control rats.

1 The angiotensin-converting-enzyme inhibitor captopril is known to produce beneficial haemodynamic effects in patients with chronic congestive heart failure. 2 Twelve patients with chronic congestive heart failure were conventionally treated with digitalis and diuretic therapy plus oral captopril (75-150 mg/day), and 14 patients were used as a control group. 3 There was no improvement in functional and haemodynamic values in the controls, but the patients treated with captopril showed a significant functional improvement (increase of exercise time and improvement in New York Heart Association functional class), a definitive decrease in systemic vascular resistance (20%), and a significant increase in the cardiac index and ejection fraction (18% and 28% respectively).
In the last years, total bone scan has gained an important place in the staging of the neoplasms of the lung. Out of 271 patients with lung cancer who underwent a bone scan in the period october 1979-october 1980. 10 showed an increased fixation of the isotope with a linear pattern along the diaphysis of the limbs. These findings, clinically silent, suggested the diagnosis of hypertrophic osteoarthropathy, which has been subsequently confirmed radiologically. The different pathogenetic theories and the evolution of the scintigraphic picture after therapy of the primary disease are briefly discussed.

The authors explored the gastric function of 20 volunteers by classical methods and by the assessment of gastric 99Tc pertechnetate clearance. From a comparison of results obtained with the various methods they come to the conclusion that the pertechnetate clearance method is dependable, easy to perform, and noninvasive; accordingly, they recommend its use as the method of choice for monitoring the treatment of duodenal ulcers and evaluating its results.
