In smooth muscle cells, when they are contracted for long time by carbachol, after washing a relaxation occurs. Thereafter if they are stimulated again, a less intense and late contraction develops after a long period of latency. Several drugs used as inducers of the long contraction produce a crossed refractoriness; therefore it would seem that the refractoriness does not depend upon receptors. The block of Na-K pump, elicited by a digitalic agent, potentiates the dose ratio; conversely the latency reaches normal values if a saline solution, with a low NaCl content, is used. On the basis of our results, we hypothesize that during the first phase of the induction very high levels of Na accumulate into the smooth muscle cell; these high Na concentrations, present for few minutes also after the washing, could be responsible for the refractoriness. Since an increase of Ca in the saline solution potentiates the refractoriness, the usefulness of calcium salts, in treating the hyperexcitability of smooth muscle cells could be due to this strengthened effect.
A reduction of the thirst was observed one hour after intramuscolar injection of 0.3 mg/kg propranolol in the rat; this effect was not observed with higher doses. According to work hypotheses, small doses could act blocking renal beta-adrenergic receptors: the stopped renin emission reduces angiotensin production that is the basic factor of thirst. The AA hypothesize that the lack of the effect in response to higher doses of propranolol can be explained through a different action of this drug which antagonizes the first one.
Out of 1260 biopsies performed on neoplasias of the breast in 11 years' surgical practice, 463 (36,7%) malignant tumours were encountered. The surgical strategy in the latter cases was based on two basic parameters: a) the histological report on the intraoperative biopsy; b) the clinical stage (TNM). After surgery oncological treatment followed the now universal standard practice: --T1, T2, T3, N+, M0 and T4 independent of N or M: multiple chemotherapy for 6-12 months then periodic check ups as in N- cases. --M1: multiple chemotherapy, hormone and radiation treatment combined in various ways. The results obtained in terms of trouble free periods and survival are in line with reports in the literature including those describing a larger number of cases.
The authors detected the possibility, during the phase of low tone contraction of isolated intestine, to develop a further contraction, a supertone (S), simply washing the preparation. It has been evidenced that S appears when the tonic phase is low; it does not disappear if the tone is increased lowering NaCl concentration in the saline plasma; is absent in the KCl induced contraction; is induced also by low doses of atropine; is decreased by digitalic agents. The results obtained lead the authors to conclude that: the reduced tonic phase is due to a positive factor, the Ca++ of the cytoplasm, and to a negative factor, the high level of cytoplasmic Na+. This high level of Na+ is maintained by a dynamic equilibrium with ion input through channels opened by muscarinic agent and output by a Na-K-ATP asi pump. The Na+ output is predominant and fraction of Ca++, no longer counterbalanced by Na+, causes the rise of S.

The authors relate their experience about Chronic Cystic Disease. They emphasize the frequency of relapses and the risks that such a pathology involves. They finally suggest a surgical treatment plan that, in their opinion, should be applied whenever the patient is a peri-menopausal aged woman who has undergone, at least twice, an operation for a Chronic Mastopathy with a histologically proved epiteliosis. The operation suggests is a total glandulectomy with simultaneous breast reconstruction by means of a prosthesis.
AA. are reporting their experience on the subject of the surgical treatment of the rectal prolapse in all his clinical forms, during 12 years of activity that has taken place in the clinical surgery of the University of Ferrara. From their casuistry it is shown that usually this pathology is joined with other morbid forms of the small pelvis which vary according to the degree of the prolapse. They emphasize the excellent results obtained and support the surgical way of laparotomy adopted in the treatment of prolapse of II type (incomplete) and III type (complete).

It has been analyzed the speed of contraction (measured as Vmax) of guinea-pig intestine in vitro, after stimulation with carbachol. High doses of carbachol induce an high Vmax; but repeating the dose at short interval (4 min) the speed of contraction is reduced until it reaches values of Vmax 4,3. Low doses of carbachol determine a low Vmax that with repeating doses increases until Vmax 4,2. On the base of this tendency at the same Vmax, and from the anomalous behaviour of the intestines to one dose in front to different doses in carbachol. The excludes that the process could be due to a passive adaptation and hypothesizes a model of behaviour inside the cell. A model to which the myocells balance the stimulus in the way to make constant their velocity of contraction.
The effects of a block of Na-K-ATPase pump on the contraction of intestinal smooth muscle (ileum of guinea pig) induced by carbachol has been analysed. Two parameters have been evaluated: a) maximum velocity of contraction (Vmax.C); b) maximum velocity of relaxation (Vmax.D) after washing. The block of pump is produced by a digitalic agent (desacetyl-lanatoside C). The application of 0.3 micrograms/ml of lanatoside for 1 min causes an increase of Vmax.C and a decrease of Vmax.D; whereas it does not influence the height of contraction. This effect is in agreement with the mechanism of action of Na on the contraction curve.

The contraction-curve of guinea-pig ileum, in response to carbamylcholine has been analysed. The curve is characterized by three phases: a rapid contraction, the clonus (C), immediately followed by a partial relaxation; thereafter the curve goes up again and finally, there is a tonic phase (T) characterized by a low and steady contraction (fade). The effects due to different experimental conditions have been investigated: increasing doses of carbamylcholine, ageing of the specimen, atropine application on the tonic phase; moreover the most notable experimental condition was Na reduction in the medium. This decrement, produces a decrease of C and an increases of T. On the basis of the results it has been concluded that: a) Na+ which enters the cell under muscarinic stimulation mobilizes Ca++ from deposits and causes contractile effects; thereafter, Na++ also moves Ca++ from actomyosine and produces a relaxing effect; b) spare receptors complete the action of the drug; when these receptors are activated by high doses of the muscarinic agent, they increase the amount of Na+ which enters the cell to negatively modulate the contraction.
This paper presents a case of malignant fibrous histiocytoma localized in the sigmoid colon with multiple metastases in the right lobe of the liver and describes the pathological and microscopic aspects, clinical course and characteristics. The primary localization in the sigmoid colon is extremely rare, this being only the second case reported.
The AA. report their experience on 22 cases of aneurysm involving under renal abdominal aorta as mortality rate in elective surgery is about 16% and in emergency operation it rises up to 60%. The AA. remark the opportunity of early surgical treatment. Ti decrease mortality rate.
