An investigation into smoking policy in Leeds' hospitals found official policy nebulous and its implementation ineffective.

Blood pressure and heart rate and withdrawal of antihypertensive drugs.

The immediate effects on heart rate and blood pressure of withdrawing antihypertensive drugs were studied over three-day periods in 26 patients. Four groups of drugs were studied. After withdrawal all patients taking clonidine showed a considerable increase in heart rate and blood pressure with intense ectopic activity. Patients taking postganglionic neurone-blocking drugs showed a similar but less pronounced reaction with increased ventricular ectopic activity. No alarming reactions were seen after withdrawal of methyldopa or beta-blocking drugs. Methyldopa and, especially, beta-blocking drugs are less likely to produce withdrawal reactions than clonidine or the postganglionic neurone-blocking drugs, and patients taking these drugs are therefore less likely to suffer violent reactions if they forget to take their tablets.

Regional planning of hospital facilities for children.

The quest for constraint of health care costs has combined with the quest for better distribution of specialized child health services to emphasize the need for facility planning that considers the needs and resources of an entire region. This article describes the guidelines derived through one such planning effort and the process through which they were derived. The process can, we believe, be replicated or modified for planning pediatric hospital facilities or other health care facilities in other regions of the country.

Population cytogenetics of the genus Caledia (Orthoptera: Acridinae). II. Variation in the pattern of C-banding.

The distribution of constitutive heterochromatin has been investigated in four chromosomal races of the grasshopper Caledia captiva (2n=23 male/24 female) by the C-banding technique. Each of the four races was found to have a distinctive banding pattern which is associated with the inter-racial differences in chromosomal rearrangements. The "Ancestral" race has a telocentric chromosome complements with large procentric C-bands which are structurally double on six pairs of chromosomes. The centromeres are unstained. The "General Purpose" race has a C-banding pattern very similar to that seen in other Acridine grasshoppers with the majority of its chromosomes showing a centromeric localisation of the bands. The two southern races, which show a complex polymorphism for presumed pericentric inversions on all twelve chromosomes, also show an unusually high level of interstitial and terminal C-bands. The different locations and numbers of these bands allow unambiguous identification of all the chromosome pairs within the complement. In two cases, there is good evidence to indicate that a C-band redistribution between acrocentric and metacentric chromosomes has occurred by pericentric inversion. Furthermore, C-band variation on the long arm of the metacentric X-chromosome indicates the presence of a large paracentric inversion. This double inversion system has involved over 95% of the X-chromosome. The interstitial and terminal C-bands probably have not resulted from heterochromatin movement within the complement but, more likely, have arisen by saltatory duplication of pre-existing sequences on the chromosome. A new nomenclature system for banded chromosomes is proposed which allows most kinds of chromosomal restructuring and rearrangement to be adequately enumerated.

Living and working in the United States of America. Some notes for the Australian graduate.

Medical licensing in the United State of America is dependent on examination in addition to education and training. Licenses are granted by each State, and each State has individual requirements. These requirements may include the Educational Council for Foreign Medical Graduates (ECFMG) certification, internship, visa status and the foreign licensing examination(FLEX) certificate. These factors are discussed in some detail. Temporary licensure for the physician who works in a training institution is also discussed. Ecomic pressures may force the physician on an Australian stipend to consider working outside his fellowhip or residency. Licensing for the foreign medical graduate who intends working in the United States is complex, and specific questioning of a potential employer regarding licensing is very important.

The spectrum of liver diseases in alcoholism.

Liver biopsies obtained from 100 alcoholic patient attending a clinic primarily for the management of their alcoholism, have been reviewed. The morphological appearances have been correlated with histories of alcohol consumption and with clinical and biochemical findings. There were 77 men and 23 women. Eight biopsies appeared normal, 62 showed fatty liver with or without fribrosis, 17 had alcoholic hepatitis with or without fibrosis and 13 had alcoholic hepatitis with established cirrhosis. Patients with fatty liver had drunk as long and as heavily as those with alcoholic hepatitis, suggesting that some other factor in addition to alcohol is of importance in the development of alcoholic hepatitis. Clinical and biochemical abnormalities showed no constant relationship to histological findings. Thus liver biopsy would seem to be an essential part of the full clinical assessment of the alcoholic patient.
