Preclinical laboratory course in dental behavioral science: changing human behavior.

Dental health professionals are frequently frustrated when they present preventive or therapeutic regimens to patients. This paper describes an approach to the teaching of behavioral science designed to familiarize the dental student with applying behavioral change techniques to dental problems. Ninety-nine second-year dental students participated in the two-credit course. Students were taught in six groups, each led by a clinician-behavioral scientist team. The behavioral science materials were presented in five slide-tape programs developed by the authors. The key to the course was a behavior change project in which each student identified a problem, designed an intervention strategy, and attempted to manage the problem using the behavioral techniques presented.

Dentists' self-evaluations: relationship to clinical performance.

Today's dental practice standards are set by the individual dentist. Quality is assured, in large part, by the accuracy of self-evaluations by dental practitioners. As a result, dental educators have attempted to improve the accuracy of self-reported evaluations by dental students by teaching such skills in the laboratory and clinic. This investigation examines multiple self-evaluations of the quality of restorative practice of 1,196 general practitioners, and investigates the relationship between these ratings and actual quality of treatment by 102 dentists for 2,753 patients.

[Synthesis of glaucoma problems (author's transl)].

Author intends to unite the part results of the glaucoma research (viz. blood vessel system, blood chemism, chamber angle, vegetative reflexes, psychosomatic factors, central effects) in a synthesis. The outcome of this synthesis is displayed in a schematic table.

[Recent aspects in the pathomechanism of glaucoma (author's transl)].

The NEFA-content of the blood serum increases in cases of acute glaucoma. The Cortisol content of the plasma of glaucomatous persons is above 20 microng% and the free cortisol-content of the plasma shows an increase in glaucoma cases. Macrofibrils can be observed by electron microscopic observation of acute glaucoma cases. The combination of diabetes and glaucoma appears more and more frequently. The pseudoglaucomatous excavation has a vasogenic origin. The use of contraceptives may provoke glaucoma.

A system for coding the presenting requests of ambulatory patients.

Effective methods developed to review and study the care of patients in hospital have not been applicable to ambulatory care, in which definitive diagnosis is the exception rather than the rule. A reasonable alternative to using diagnosis as the basis for assessing ambulatory care is to use the problems or requests presented by the patients themselves. A system has been developed for classifying and coding this information for flexible computer retrieval. Testing indicates that the system is simple in design, easily mastered by nonphysicians and provides reliable, useful data at a low cost.

Unusual cardiac reaction to chemotherapy following mediastinal irradiation in a patient with Hodgkin's disease.

A 27 year old man with Hodgkin's disease experienced three separate episodes of chest pain, each occurring on the sixth day of a cycle of mustargen, oncovin, procarbazine, prednisone (MOPP) combination chermotherapy. The first episode appeared to represent a myocardial infarction, whereas the next two were less serious. Numerous studies were performed including coronary angiography, cardiac catheterization and open pericardial biopsy. It is suggested that the patient represents an example of a previously undescribed syndrome due to chemotherapy administered after cardiac irradiation.

Effects of neoplasms on inflammation: depression of macrophage accumulation after tumor implantation.

The local accumulation of macrophages at sites of neoplasms may be a critical event in immunologically mediated tumor killing. Individuals with neoplasms, however, have been noted to have depressed monocyte chemotactic responsiveness in vitro. To determine the effect of neoplasms on macrophage migration, mice were implanted subcutaneously with either sarcoma or hepatoma cells and their macrophage migratory function quantified in vivo and in vitro. The ability of tumor-bearing animals to mobilize macrophages to an inflammatory site in vivo was depressed by as much as 61% by 6 days after tumor implantation. The in vitro chemotactic responsiveness of macrophages recovered from the peritoneal cavities of tumor-bearing animals was also markedly depressed. Macrophage migration was not affected by implantation of normal syngeneic or allogeneic tissues. In addition, the accumulation of polymorphonuclear leukocytes in vivo was not depressed in tumor-bearing animals. These findings suggest that neoplasms themselves may depress the host's ability to localize macrophages at inflammatory sites in vivo and thereby hinder immunologically mediated tumor destruction.

[The psychosomatic factors in ophthalmology (author's transl)].

The importance of the psychosomatic factors is gradually increasing in the ophthalmological practice. In cases with hysterical symptoms they cause amaurosis and changes of refraction. Emotional factors lead to strabism and photophoby. Anxiety conditions and stress effect lead to glaucoma. In a short review the principles of psychosomatic treatment are dealt with.
