Muscle strength as a predictor of bone mineral density in young women.

It is widely accepted that physical activity is beneficial to bone. However, the specific relationships of muscle strength to bone mineral density (BMD) are poorly understood. We examined strength and BMD in 59 women aged 18-31 years who ranged in exercise patterns from sedentary to active. Mineral density of the right proximal femur (hip) and spine (L2-4) was evaluated by dual-energy x-ray absorptiometry. BMD at the midradius was measured by single-photon absorptiometry. Dynamic strength (one repetition maximum) was measured for the following muscle groups: back, elbow flexors (biceps), leg extensors (quadriceps), and the hip flexors, extensors, adductors, and abductors. Isometric grip strength was assessed by dynamometry. Mineral density at the hip correlated independently with muscle strength and body weight, but not with age. Specifically, femoral neck BMD was significantly correlated with back strength and weight, whereas trochanter and overall hip mineral density were significantly related to biceps, back, and hip adductor strength. Hip mineral density was not related to strength of the quadriceps groups or to that of the hip flexors, extensors, or abductors. In addition, muscle strength was an independent predictor of lumbar spine and midradius mineral density. In stepwise multiple regression analysis, biceps strength proved the most robust predictor of hip BMD and grip strength best predicted bone density at the lumbar spine and radius. We conclude that muscle strength is an independent predictor of bone mineral density, accounting for 15-20% of the total variance in bone density of young women.(ABSTRACT TRUNCATED AT 250 WORDS)

Oral hygiene instruction and health risk assessment in dental practice.

We studied oral hygiene instruction given to 109 patients in 19 Washington State dental practices to investigate the extent to which therapists targeted their efforts toward patients with high disease risk. Patients were examined prior to instruction and prophylaxes. Therapists' instructions were tape-recorded and their content analyzed: therapists' expectations were scored. There were no statistically significant associations between patients' initial plaque levels and the process/content of the oral hygiene instructions delivered. On average, therapists spent 9.4 minutes of each prophylaxis session discussing oral hygiene. Therapists were judged more genuine with those patients for whom they had higher expectations of compliance, i.e., those with less plaque and low disease risk. We conclude that dental practitioners were not employing effective risk assessment strategies in selection of patients most in need of intensive instructional efforts.

How effective is oral hygiene instruction? Results after 6 and 24 weeks.

We studied the clinical effectiveness of oral hygiene instruction given to 71 "high" plaque patients in 19 Washington State dental practices to determine the extent of plaque reduction. Patients were examined prior to instruction and prophylaxis. Therapist instructions were tape-recorded and content analyzed, and therapists' expectations were scored. Patients were surveyed as to oral health behaviors and general health habit characteristics. Only 20 of 71 (28%) initially "high" plaque patients had reduced mean plaque scores to 1.0 or less at six weeks. An additional 13 percent had deteriorated plaque levels at 24 weeks. We concluded that therapists did not consistently follow principles of effective instruction and often overestimated their effectiveness. Determinants of relapse involve the educational process, the patients' oral status, and patients' life situations.

Bilateral keratitis as a manifestation of Lyme disease.

An 11-year-old girl developed bilateral keratitis, which we believe was a manifestation of Lyme disease. She had had several attacks of Lyme arthritis and was twice treated with parenteral penicillin. The keratitis developed five years after the initial episode of Lyme arthritis at a time when there were no other manifestations of Lyme disease. It cleared completely in both eyes after topical corticosteroid therapy.

Results of a dental fear survey in Japan: implications for dental public health in Asia.

The purpose of this investigation was to initiate the study of dental fear in Japan. 415 college students, aged 18-22 yr were surveyed. A standardized questionnaire which has been used in the United States was translated into Japanese and was administered to the students. More than 80% of those surveyed reported some dental fear. Six to 14% of the students reported extreme fear of the dentist. The majority of the subjects admitted that they delayed making dental appointments due to fear. Muscle tension was the most common physiological symptom reported. The dental drill and needle were the most fear-provoking stimuli.

The prevalence and practice management consequences of dental fear in a major US city.

In 1986, 1,019 residents of Seattle were surveyed about their dental fears, dental experiences, and perceived oral health status. High dental fear in Seattle was found to affect 204 per 1,000 people. More than 66% acquired their fear in early childhood. Females were 1.8 times more likely than males to report high fear (P less than .001). An individual was 1.6 times as likely to have high levels of dental fear if he or she had at least one oral problem such as bleeding gingiva (P = .004).

Neuronal changes in fetal cortex transplanted to ischemic adult rat cortex.

Fetal cortex from 16- and 17-day-old embryonic rats was transplanted into the parietal cortex of 12 adult rats rendered ischemic by temporary intraluminal occlusion of the middle cerebral artery. Ischemic injury in the host cortex adjacent to all nine surviving transplants was demonstrated with hematoxylin and eosin and cresyl violet strains. Nicotidamide adenine dinucleotide phosphate-diaphorase (NADPH-d) histochemical studies revealed a normal number of NADPH-d-positive neurons, whereas acetylcholinesterase (AChE) staining revealed many more AChE-positive neurons in the transplants compared to the host parietal cortex. This could be due to: 1) selective survival of AChE neurons in the transplants compared to the host cortex; 2) increased expression of AChE in transplanted neurons; 3) induction of AChE in normally AChE-negative neurons; or 4) decreased transport of the AChE enzyme from the perikarya to fibers in surviving transplanted neurons. Many fibers positive for AChE and NADPH-d crossed between the host and transplant, although fiber density in the transplants was less than in normal host cortex. These results should encourage future investigation of whether similar transplants improve neurological function following experimental stroke.

Recognizing and treating fears in general practice.

Recognizing and treating dental fears requires attention to the behavioral, physiologic, and cognitive manifestations of anxiety. This article provides questions to ask and strategies to employ to identify and use in assisting the fearful patient in general practice.

The challenge of fearful and phobic children.

Many children are fearful of going to the dentist. Professionals have the opportunity to avoid causing long-term anxiety disorders by providing a safe environment and offering children opportunities to overcome their normal childhood fears during early visits. It is more important to focus on a positive interaction between dentist and child rather than the completion of the dental procedure.

Effect of topical beta APN application on evoked potential conduction in rat sciatic nerve and spinal cord.

The purpose of this study is to evaluate possible toxic effects of beta-aminopropionitrile fumarate (beta APN), a lathyrogenic agent that inhibits fibrosis. This drug has been considered for use as an adjunct to surgical repair after topical application upon peripheral and central nervous system structures. In vivo and in vitro studies were done using rats to study the dose dependent neurotoxicity of this water soluble chemical. The results indicate that when the neural sheaths are removed the amplitude of the evoked sciatic nerve potential is irreversibly suppressed from 1 to 10 mM concentrations of beta APN. Nerve conduction velocities are relatively less affected with reduction from 43 to 35 m/sec by beta APN immersion. Similarly, the spinal cord studies show that when the dura and arachnoid are opened and damaged, 0.1 mM beta APN causes increased latency (from 9.9 to 14.5 msec) and decreased amplitude (from 79.4 to 56.8 microV) of cortical somatosensory evoked potentials. Possible mechanisms for the neurotoxic effects of beta APN are discussed.

A new regimen of amsacrine with high-dose cytarabine is safe and effective therapy for acute leukemia.

Amsacrine and high-dose cytarabine (HiDAc), when administered as single agents, are effective treatment of acute leukemia. When used in combination, a high remission rate is also possible. We treated 47 patients with acute myelogenous leukemia (AML), acute lymphoblastic leukemia (ALL), and blastic phase of chronic myelogenous leukemia (CML) with a combination of amsacrine and HiDAc. The patients received amsacrine 200 mg/m2 daily for three days and, concurrently, HiDAc 3 g/m2 over three hours once daily for five days. Of 20 evaluable patients with AML in relapse, there were 12 remissions; of seven additional patients with primary refractory AML, there were two remissions, and of 12 patients with ALL in relapse, there were eight remissions. The three patients with blastic phase CML and the three patients with biphenotypic leukemia did not respond. Nausea, vomiting, stomatitis, hepatic dysfunction, and diarrhea were common, but cutaneous, conjunctival, and significant cerebellar and cerebral side effects were absent. We conclude that this regimen is highly effective therapy for AML and ALL and is also safe, eliminating the major toxicities encountered with HiDAc.

Suppression of luteal function in dogs by luteinizing hormone antiserum and by bromocriptine.

Beagle bitches were treated with equine anti-LH serum (ALHS) or the dopamine agonist bromocriptine at selected times during the 2-month luteal phase of the ovarian cycle or pregnancy. After a single injection of ALHS (10 ml, i.m.) at Day 42 of pregnancy (N = 2) or the ovarian cycle (N = 3), progesterone was reduced (P less than 0.05) to 7-24% of preinjection values within 1-2 days, and by 4-8 days returned to levels not different from those in control bitches treated with normal horse serum. Injections of bromocriptine (0.1 mg/kg, i.m.) daily for 6 days caused abrupt declines in progesterone which lasted 6-8 days in bitches treated at Day 8 or 22 of pregnancy (N = 5). In bitches treated at Day 42 of pregnancy (N = 3) or in non-pregnant cycles (N = 4) the bromocriptine treatment caused declines (P less than 0.05) in progesterone which were permanent, extensive (less than 2 ng/ml), and therefore abortive. The declines in progesterone in response to immunoneutralization of LH and to prolactin-lowering doses of a dopamine agonist demonstrate that normal luteal function in dogs requires both LH and prolactin.

The use of nitrous oxide in the treatment of children: results of a controlled study.

The sequential analysis of behavior has begun to show that the behavior of the dentist is a major influence on the fear-related behaviors of children, even when N2O is used. The moment-to-moment behaviors of the dentist influence the child's behaviors. Certain behaviors of dentists, such as distraction, appear to be effective especially when the N2O is being administered.
