Carbohydrate content of apolipoprotein B-48 from rat chylomicrons of varying density.

Monosaccharide composition was determined in apolipoprotein B-48 (apoB) of chylomicrons of rat mesenteric lymph. Chylomicrons were separated into three fractions based on density. Triglyceride and apolipoprotein content were determined in each. ApoB was isolated and quantified using precipitation with isopropanol. Chylomicrons were collected in lymph under normal conditions, and with Poloxalene 2930 when chylomicron secretion was inhibited. Most of the triglyceride was carried in the least dense fraction, while the highest apoB content was in the most dense fraction under normal conditions. Mannose and galactosamine contents of apoB were similar in all fractions while contents of both glucosamine and galactose were highest in the least dense fraction. When chylomicron secretion was inhibited by Poloxalene, the amount of triglyceride recovered in the least dense fraction was significantly reduced. Despite the inhibition of lipid transport in the least dense fraction of chylomicrons by Poloxalene, there was little change in apoB recoveries and in the relative content of various monosaccharides in the apoB from each of the three fractions as compared to results obtained during lipid absorption under normal conditions. In conclusion, carbohydrate composition of apoB of chylomicrons is heterogeneous and varies with chylomicron density.

Treatment of severe reactive hypoglycemia with a somatostatin analogue (SMS 201-995).

Reactive (or postprandial) hypoglycemia can sometimes represent a severe disorder refractory to conventional therapeutic measures. We present in this first individual trial, to our knowledge, that the administration of a somatostatin analogue (SMS 201-995) may alleviate the severity of complaints and does not appear to be diabetogenic. The effects of the somatostatin analogue were documented in a 5-hour oral glucose tolerance test, where not only the glucose-induced and C-peptide rise was clearly attenuated, but also the blood glucose concentration did not fall low enough to induce hypoglycemic symptoms.

[The clinical picture and development of aortic aneurysms in older persons].

The authors discuss 11 patients with aortal aneurysms (AA) which they diagnosed in the course of 1983-1988. In seven instances the abdominal aorta was affected; once the ascending part and once the arch; twice the descending part and once the whole aorta. In four patients the aneurysm had a dissecting character. Patients of advanced age were involved (73.9 +/- 9.7 years) with multiple morbidity and frequently also general advanced atherosclerosis which was the main cause of development of aneurysms. The authors analyze the clinical picture and course of AA in different patients and draw attention to their great variability. In the discussion they deal with the position of diagnostic methods, analyze critically the causes, course and prognosis of AA incl. interpretation of their own group of patients. In view of the frequent (and rising) incidence of AA in old age, the authors consider it important to pay attention to this problem also from the aspect of clinical gerontology.

[In vitro comparative effect of cefixime and 7 other beta-lactamines against Streptococcus pneumoniae].

The comparative in vitro activities of cefixime, penicillin G, amoxicillin, cefotaxime, cefaclor, cefadroxyl, cefotiam and cefuroxime against 83 recent clinical isolates of S. pneumoniae were determined. In addition, the susceptibilities of 17 relatively or fully penicillin-resistant strains of S. pneumoniae were evaluated. Cefixime was more active than the first generation cephalosporins cefaclor and cefadroxyl against penicillin-susceptible strains. The MIC50 and MIC90 obtained by the agar dilution method were 0.25 and 0.5 mg/l respectively. The MICs obtained by a microdilution technique were one dilution lower. Cefixime exhibited little or no activity against relatively or fully penicillin-resistant strains.

[Cheilitis granulomatosa as the first manifestation of Crohn's disease].

A 39-year-old man developed a painless swelling of the lower lip, which histologically was a granulomatous inflammation of the corium as in granulomatous cheilitis. Four weeks after onset of the disease recurrent diarrhoea set in, the result of Crohn's disease of the jejunum, terminal ileum and caecum. Granulomatous cheilitis, occurring as sole sign of the Melkersson-Rosenthal syndrome, is a manifestation of Crohn's disease. Even if there are no intestinal symptoms. Crohn's disease must be excluded in the differential diagnosis.

Metastases to pineal gland.

Metastases to the pineal gland is an uncommon event. Bronchogenic carcinomas are the most frequent source of the primary lesion. A case of small cell carcinoma of the lung with solitary metastases to the pineal gland is reported; the literature is reviewed.
