The biochemical testing of Enterobacteriaceae can be rapidly and automatically performed with commercial identification systems. There are three groups of identification systems: the kits, the microtiter plates and laboratory apparatus. The ratios of agreement between these new systems and the conventional tests are equivalent. The two first categories of systems are inexpensive unlike apparatus which must be utilized only in important laboratories.
The authors describe the unusual cardiac malformation in a young girl with Turner's syndrome: right ventricular hypoplasia and atrial septal defect, resulting in central cyanosis. A trial of temporary occlusion of the atrial septal defect at cardiac catheterisation before surgical closure suppressed the right-to-left shunt and the cyanosis without significantly changing the hemodynamic parameters. The authors review the types of cardiovascular abnormalities associated with Turner's syndrome and emphasize the value of a temporary occlusion test of defects in certain cyanotic cardiac malformations with atrial right-to-left shunts; it may lead to a relatively simple surgical procedure, as in the case described.
This epidemiological survey was conducted in the Northern country of France, on 753 communes. Nitrate concentration in drinking water was measured from 1974 to 1976. The mean concentration rates (M.C.R.) calculated for the three years, were under 95 mg/l, and 93% were under 43 mg/l. The association between the M.R.C. and digestive and urinary cancer mortality rates observed in the same area (1968-1975) was studied, using different statistical techniques. No systematic relationship was observed taking into account the site of the tumor and the age and the sex of the subjects. The role of other variables is considered in the discussion: duration of exposure, use of other beverages such alcohol or mineral water. The duration of exposure, use of other beverages such alcohol or mineral water. The authors conclude that drinking water does not seem to play a determinant role in the causation of cancer; relationship, if it exists, it remains to be demonstrated.
Antibiotic treatment of infections due to opportunistic organisms often involves an aminoglycoside/bêta lactamine combination. In order to help in the selection of the aminoglycoside used, a statistical study was made of 1041 strains of organisms obtained from an intensive care unit to 4 aminoglycosides: gentamicin, amikacin, tobramycin and dibekacin. These included Gram negative bacilli (enterobacteria, pseudomonas, acinetobacter) and staphylococci. The study showed amikacin to be the most suitable aminoglycoside for prescription in an infection threatening the vital prognosis in the short term and where the organism has not been identified. In non-threatening infections when the organism has not been identified, gentamicin, tobramycin or dibekacin should be used in preference to amikacin. For any type of infection, once the organism has been identified, amikacin should only be used if it is the only effective agent. When the organism is equally sensitive to gentamicin, tobramycin and dibekacin the choice of drug should take into account the known potential toxicities of each drug.
Mechanism of aminoglycoside nephrotoxicity is yet unclear. Reduction of 20 p. cent of glomerular filtration initial value is observed in 10 p. cent of cases. Aminoglycoside nephrotoxicity appears to be related to high doses, to duration of treatment and also to individual factors such as age and preexisting renal impairment. No mean of predicting aminoglycoside nephrotoxicity without any error is available. Meanwhile, accurate measurement of glomerular filtration and of beta 2-microglobulin urinary elimination seems clinically more usefull for detection of impaired renal function than uneasy measurement of urinary enzyme excretion and than monitoring the aminoglycosides serum concentrations.

A sample of 45 algerian women and a sample of 125 french women of the same age and marital status are compared on their state of psychological well, or illbeing. All the probands were extracted from the general population. Our measurement of "impairment in daily life" appears to be a reliable detector of light or more consequential depression. This syndrome covers almost the whole range of the psychopathology we have found, and its structure is essentially the same in the French and Algerian samples. The one-year prevalence of depressive syndrome for the two samples combined (n = 170) is 15% for the mildly impaired and 3% for the more markedly impaired. The intercultural comparison shows a statistically significant difference between the two samples, the algerian women being relatively more depressed. When the entire sample is divided into sub-groups, the results show a close relationship between global life conditions and mental health. In particular, the women who go out to work and like their activity are the most symptom-free.

A 24 year old female presented with paroxysmal pseudo-ventricular tachycardia with wide QRS complexes and complete atrioventricular dissociation. Electrophysiological studies showed a junctional tachycardia with an antidromic reciprocating rhythm. The activation involved Mahaïm nodo-ventricular fibres in the anterograde direction, the nodo-hisian pathway being the retrograde limb of the reentry pathway. The main point of interest of this case was the presence of complete atrioventricular dissociation during tachycardia, the atria remaining in sinus rhythm with the possibility of sinus capture formally excluding the diagnosis of a paraseptal Kent bundle. This reentry mechanism has been previously described and constitutes a differential diagnosis of ventricular tachycardia.
