Popliteal venous aneurysms with or without pulmonary embolism.

Popliteal venous aneurysms are rare and are most often found in patients with pulmonary embolism suggesting that such aneurysms are thrombogenic. Only 16 cases have been found in the past and only a few of these patients were without thrombus or pulmonary embolism. In this paper three patients are presented of whom two showed signs of thrombus. Surgical resection was performed to prevent thrombosis with a successful result. The literature is reviewed and according to this and our own experience surgical resection under cover of anti-coagulation is recommended. Our surgical technique is presented.

Popliteal venous aneurysm.

A case of popliteal venous aneurysm in a 69-year-old male is presented. Although rare these aneurysms are potential sources of thromboembolism indicated by the fact that they are most often discovered in patients with a history of recurrent pulmonary embolism. The aneurysm was successfully treated by simple surgical resection and a short course of oral anticoagulants.

Perineal rupture following vaginal delivery. Long-term consequences.

Twenty-five women with complete perineal rupture were compared with a control group of 25 women, with regard to risk factors and possible long-term sequelae as a consequence of perineal damage. Following an average period of 78 months (52-123), all women received a questionnaire and were invited to a clinical follow-up, which included perineal inspection and measurement of the anal pressure profile. Vacuum extraction was more frequent in the rupture group than among controls (p less than 0.002). Eighty percent of the women in the rupture group were primiparae, compared with an overall rate of 40% (p less than 0.02). Forty-two percent of the women in the rupture group reported anal incontinence, compared with none in the control group (p less than 0.01). Most of these women reported stress-provoked incontinence regarding flatus and loose stools. Measurement of the anal pressure profile showed markedly reduced sphincter pressure, with maximum squeeze in the rupture group, but no differences were found regarding maximum anal pressure at rest. Sphincter length was reduced both at rest and with maximal squeeze in the rupture group. It is concluded that complete perineal rupture is a condition with possible long-term consequences such as reduced sphincter strength and partial anal incontinence.

Tissue/blood partition coefficients for xenon in various adipose tissue depots in man.

Tissue/blood partition coefficients (lambda) for xenon were calculated for subcutaneous adipose tissue from the abdominal wall and the thigh, and for the perirenal adipose tissue after chemical analysis of the tissues for lipid, water and protein content. The lambda in the perirenal tissue was found to correlate linearly to the relative body weight (RBW) in per cent with the regression equation lambda = 0.045 . RBW + 0.99. The subcutaneous lambda on the abdomen correlated linearly to the local skinfold thickness (SFT) with the equation lambda = 0.22 SFT + 2.99. Similarly lambda on the thigh correlated to SFT with the equation lambda = 0.20 . SFT + 4.63. It is concluded that the previously accepted lambda value of 10 is generally too high in perirenal as well as in subcutaneous tissue. Thus, by application of the present regression equations, it is possible to obtain more exact estimates of the adipose tissue blood flow measured with the 133Xe wash-out method.

The reliability of transabdominal ultrasound scanning in the determination of prostatic volume. An autopsy study.

The prostatic volume as determined by transabdominal ultrasound scanning using the ellipsoid method was compared to the prostatic volume determined by autopsy in 26 post-mortem men. The regression line was nearly optimal (Y = 1.06 chi + 0.57) although it seems that ultrasound underestimates prostatic volume slightly. The parametrical correlation coefficient was 0.86 and the non-parametrical Spearmans rho was 0.76. It is concluded that transabdominal ultrasound scanning has a definite value in the assessment of prostatic size.

Pyogenic granuloma: evaluation of oral conditions.

A consecutive series of 43 pyogenic granulomata in the oral cavity is presented. Most were located to the marginal vestibular gingivae and only a minority (12%) of these had a history of trauma whereas 70% in extragingival locations had a previous history of injury. On the basis of an evaluation of oral conditions the aetiology and the pathogenesis of the pyogenic granuloma are discussed. Trauma, microtrauma due to toothbrushing and gingival inflammation seem to be pathogenetic elements. It is suggested that the granuloma is a localised tissue response to a non-specific irritant.

A long-term gastrointestinal follow-up in patients operated on for congenital hypertrophic pyloric stenosis.

A gastrointestinal follow-up of 91 patients operated on for congenital hypertrophic pyloric stenosis (CHPS) between 1942 and 1964, using the Fredet-Ramstedt method, is presented. Among 85 patients available for follow-up, 72 responded to a request for reexamination. These patients were interviewed personally by one of the authors. Thirty-four patients (47%) consented to have a Roentgen examination done, performed as a double contrast study, investigating the structure and function of the stomach and duodenum. Forty-two patients (58%) had varying minor gastrointestinal complaints. Only two patients had severe symptoms. X-ray examination in 34 patients (47%) showed only three with pathological changes, i.e. one with mild deformation of the duodenal bulb, another with slight duodenal reflux. The third X-ray showed massive gastric retention with duodenal bulb deformation. The patient suffered from severe dyspepsia and vomitus. No correlation was found between the severity of the CHPS and the presence of later gastrointestinal symptoms. It is concluded that the Fredet-Ramstedt's operation for CHPS is an efficious operation with few perioperative complications and good long-term results.

Surgical treatment of ureteric stones.

638 patients with ureteric stones have been evaluated. 174 patients (28%) were selected for surgical treatment. 129 out of the 174 patients initially underwent open surgery. In the other 45 patients the initial treatment was stone extraction by the Dormia technique after which 27 stones were removed successfully. In the remaining 18 patients catheterisation of the ureter was performed, after which stones passed spontaneously in 15 patients. In one patient the stone was removed by another attempt with the Dormia procedure and two patients underwent operation. At follow-up 4-6 weeks after the diagnosis of ureteric stones was made, no signs of stones were found by urography in either the 464 patients (72%) who never underwent any surgical procedure or in the patients treated by open surgery or other procedures. It is concluded that parameters such as degree of ureteric obstruction, infection, pain and stones not passed within 6 weeks are important factors in selection of patients for operation.
