[Nailing for alignment combined with the Sarmiento method. An interesting combination in the treatment of unstable diaphyseal fractures of the tibia].

The authors have reviewed 51 cases of unstable fracture of the tibia treated by nailing followed 3 weeks later by bracing using the Sarmiento technique. The results confirmed the basis of Kuntscher nailing with reaming. This combination of two techniques can rival more sophisticated techniques such as locked nailing. Several complications were noted - one sepsis, one non-union, two malunions and one shortening of 2 cms.

[The risk non-union following closed-focus nailing and reaming. Results of 1059 interventions using the Kunstcher method].

The authors have studied a series of 1059 compound and closed fractures of the diaphysis of long bones treated by intramedullary nailing according to the technique of Kuntscher, to assess the incidence of non-union. Between January 1st 1967 and December 31st 1980, 503 fractures of the tibia, 440 of the femur and 116 of the humerus were so treated in both adolescents and adults. The fractures were mostly sustained in road traffic accidents. The risk of aseptic non-union was small, but occurred more readily when the skin was damaged and when the site of the fracture was at the junction of the lower two quarters of the tibia; the incidence was highest when the technique of nailing was incorrect or when the going was undertaken too late. The incidence of non-union and infection are compared with those of other series using the same closed technique and with reports describing insertion of the nail of the plate by opening the fracture. It is concluded that intramedullary nailing using the Kuntscher technique is the most reliable treatment for this type of injury for routine use in an acute fracture service. In this series the incidence of aseptic non-union was approximately 1%, of infection with non-union less than 0.5% and of early infection with eventual union less than 1.5%.

Electroencephalographical-anatomical correlation of brain tumours in infancy and childhood.

EEG correlated with anatomical aspects of 198 brain tumours in children gave the following results in hemispheric neoplasms: a rather precise location in 40% of the cases; lateralized in 42%; 14% showed only generalized anomalies and 4% were normal tracings. The findings depended on location and ICP condition, but not on neurological signs nor on histological nature. Basal-midline location (88 tumors) displayed 82.4% abnormal EEGs; 50% pointed to the more invaded hemisphere, mainly from the upper brain stem, ventricles and basal ganglia regions. Hypophyseal tumours gave 50% abnormalities with proved compression of the third ventricle, hypothalamus and cerebral peduncles. In posterior fossa tumours (60 cases), abnormalities are frequent (86.7%), but the localizing value of EEG is uncertain due to bisynchronous bursts mainly in the occipital region; 15% were lateralized with correlating asymmetrical location and/or growth of tumours. Differences compared to adult tumours are mentioned as specially influenced by maturational processes of the brain.

[A new orthopedic apparatus for the treatment of congenital equinovarus clubfoot. The active-passive articulated splint. Critical study propos of 72 cases treated in our department].

The authors present a new equipment for orthopaedic assesment of club-foot. This "dynamic splint" aims to produce a self-correction of the deformity by the infant at time of each movement of his lower limb. With regard to 72 such deformities, concerning 47 children, they record the results after an average follow-up of more than 2 years. Criterions of results, clinical and chiefly radiological, more objectives, are classified into 3 parts : correction of varus of the foot, correction of adduction of the hind-foot, correction of equinus.

[Course of psychopathologic and extrapyramidal motor symptoms during long-term treatment of schizophrenic patients with psycholeptic drugs (author's transl)].

65 chronic-schizophrenic outpatients were treated with fluphenazinedecanoate for 24 months. Hallucinations and delusions remitted between week 24 and 36, formal disturbances of thinking occurred in 50% of the patients up to the 24th week, mood disorders and disorders and schizophrenic changes in personality could only gradually be controlled. In the first 3 months of treatment the incidence and intensity of rigor grew up to the critical point after 24 weeks of treatment; by the 60th week of treatment it had completely vanished. Akathisia reached its peak after the first and the 36th week and two different populations could be distinguished. 8% of the patients showed dyskinetic reactions up to the 12th week; 20% of the patients showed hyper- and dyskinesia after 72 weeks of treatment. 30 to 40% of the patients required anticholinergics between the 12th and 48th weeks; after the 84th week this medication was no longer necessary.

Isocitrate dehydrogenase of Tetrahymena pyriformis.

We have studied the isocitrate dehydrogenase of Tetrahymena pyriformis. This enzyme is able to utilize both NAD and NADP, but kinetic studies suggest that the enzymatic activity with NAD is not of physiological signifance. Some of the factors that might regualte the NADP-dependent isocitrate dehydrogenase were also studied. This enzyme has an absolute requirement for divalent cations; Mg,+ and Mn2+ will serve as cofactors but the latter is more effective than the former. It is known that this enzyme is subject to a concerted inhibition by oxaloacetate and glyoxylate. Either glyoxylate or oxaloacetate alone also are capable of inhibiting the enzyme although higher concentrations are required. We have found concerted inhibition also for the NAD-dependent isocitrate dehydrogenase from rat liver and yeast. The activity of the Tetrahymena pyriformis enzyme is inhibited by NADPH. This inhibition is competitive with NADP. The Ki and Km values are, respectively, 20 micrometers and 18 micrometers.

Solid-phase synthesis and cellular localization of a C- and/or N-terminal labelled peptide.

We report the solid-phase synthesis by the Fmoc strategy of a peptide containing a cysteamide group at its C-terminus. This peptide was subjected to further modifications including the linkage of fluorophores, namely lucifer yellow and coumarin respectively, at the C- and/or N-terminals. After incubation with living cultured cells these two probes were localized and it is concluded that the post-synthesis modifications can strongly modify the localization of the peptide.

[New 5'-phosphonates, modified through the nucleoside sugar residue, as inhibitors of HIV replication].

A number of nucleoside 5'-phosphonates and nucleoside 5'-methylphosphonates were synthesised, to study their ability to inhibit reproduction of HIV-1. Three compounds, 5'-hydrogen phosphonates of 3'-azido-2',3'-dideoxythymidine (AZT-HP, IVc), 3'-fluoro-2',3'-dideoxythymidine (FLT-HP, IVa) and 2',3'-dideoxyadenosine (ddA-HP, I), exhibit potent anti-HIV-1 activity with selectivity indices similar to or better of those of their parent nucleosides.
