[Epidemiology of trachoma and vision disorders in central Viet Nam].

Cataract is the first blinding disease in VIETNAM. Its frequency was studied in Central VIETNAM. Trachoma and xerophthalmia are very common. The evaluation of the prevention program against blindness in Central Areas of VIETNAM is set up. The authors point out an important improvement of the situation. Nevertheless they stress the necessity of carrying on the anti-trachomatous and anti-xerophthalmic campaign. A higher number of cataract operations could be performed since a sufficient number of operating kits was allowed.

[Cataract in Senegal].

1000 cataracts were cured thanks to surgery at the Ophthalmological Centre of BOPP in Dakar. The authors analyzed them according to several aspects (age, sex, aetiology and anatomical shape of the cataracts), according to the therapeutical results and complications. When having compared extracapsular and intracapsular, they stress the double problem of prevention and treatment of endophthalmia and of optical correction of aphakia at a health centre.

[Direct detection of toxoplasma in the aqueous humor by gene amplification].

For immunocompetent patients, ocular toxoplasmosis is the most frequent infectious cause of chorioretinal inflammation. Nowadays, the laboratory diagnosis of ocular toxoplasmosis requires serological tests and anterior chamber puncture to detect the local production of specific antibodies. The authors describe a new technique to detect toxoplasma in aqueous humor by a polymerase chain reaction in which the target is a 88 bp specific rDNA fragment. 31 patients were concerned (23 highly suspect of ocular toxoplasmosis and 8 controls). The presence of the parasite in aqueous humor was found in 7 cases. No false positive was detected. The sensitivity of the test is reduced by the poor volume of the sample. The combination of this technique with Desmont's coefficient gives a better positive predictive value. We emphasize the pathophysiological value of this technique by suggesting the presence of tachizoites in the anterior chamber. According to our knowledge, this finding has never been demonstrated. In the future, this should be a very promising technique for the diagnosis of ocular toxoplasmosis.

[Permanence of chlamydia in trachomatous tissue].

Electron-microscopic examination of conjunctival and corneal biopsies from six apparently healed trachomatous patients demonstrated chlamydiae in situ. Nine previously trachomatous patients were submitted to the same biopsies; all biopsies were positives. After two months specific anti-chlamydial treatment, we observed the same result before anterior segment surgery. Nevertheless surgery was performed without consequences except for the two corneal grafts which failed. We must ask for two questions: how long chlamydial particles persist in trachomatous tissue? What is the real efficacy of topical anti-chlamydial drugs?

Chlamydia trachomatis↗

Frequency of chlamydia in patients with vernal keratoconjunctivitis.

Ten years ago, we surmised that VKC could be provoked by chlamydia. 34 patients without any contamination of trachomatous origin were recently studied. We performed examinations of biopsies (histology and ultrastructure), serology, tissue group and specific allergen skin tests. Our findings indicate that Chlamydia could be an allergen provoking VKC in atopic patients.

Exclusive use of calcium channel blockers in preoperative and intraoperative control of pheochromocytomas: hemodynamics and free catecholamine assays in ten consecutive patients.

This study investigates the role of nicardipine hydrochloride in preoperative and intraoperative blood pressure control and intraoperative catecholamines (norepinephrine and epinephrine) release in 10 patients undergoing pheochromocytoma resection. Nicardipine was used orally in the preoperative period for either 1 or 8 days (60 to 120 mg/24 hr) and then was infused during anesthesia until tumor removal, continuously at a rate of 2.5 to 7.5 micrograms/kg/min, depending on systolic arterial pressure level. All patients were successfully operated on. No severe hypertensive crisis occurred during tumor manipulation, although several patients had a 3- to 85-fold (norepinephrine) and 3- to 40-fold (epinephrine) increase of catecholamines from baseline levels. Hemodynamics data suggest that nicardipine caused significant inhibition of vascular smooth muscle contraction (42% decrease in systemic vascular resistance); calcium-dependent catecholamines release was not inhibited by nicardipine infused as mentioned. Use of nicardipine may be recommended for perioperative and intraoperative control of pheochromocytomas and might substitute for routine alpha-adrenergic blockade.

[Immunoscintigraphy or intraoperative isotopic detection of colorectal neoplastic lesions using labelled monoclonal antibodies. Preliminary study. 20 cases].

20 patients submitted to surgery for colo-rectal malignancies, had I.V. injection of 500 microcuries of 125 I F (ab')2 anti CEA, 3 to 5 days preoperatively, after previous thyroid blockade by iodine. At the time of laparotomy, any obvious neoplastic growth or suspicious area was checked by "intra-operative scintigraphy" according to the technique already developed in thyroid surgery by Sten Lennquist. Result was considered as positive when the uptake ratio suspicious tissue/normal tissue was greater normal tissue than 1.5. 18 out of 19 pathologically proven carcinomas, 1 out of 3 liver metastasis, 3 out of 3 extra-hepatic deposits, and 3 out of 9 metastatic nodes only were positive at scintigraphy. These results are not correlated with preoperative plasma level of CEA. Perhaps some colo-rectal carcinomas might be able to secrete CEA but not to excrete it. Improvement of intraoperative decision-making by this method needs further experience to be appreciated. It was helpful in 2 of our 20 cases.

[Pheochromocytoma. Experience of 60 operations].

Between 1971 and 1987, 60 patients presenting 68 pheochromocytomas were operated on in the University Teaching Hospital of Lille. Diagnosis has been transformed by the use of CT scan, which for the last 10 years has enabled the lesion to be more reliably localized than by MIBG scintigraphic scanning. Combination of the two investigations enables the diagnosis to be confirmed in 100% of cases. The following topics are covered: preparation for surgery with the indications for alpha and beta blockers, route of approach--preferably median laparotomy--and operative sequelae as a function of the benign or malignant nature of the tumor.

[Etiology of childhood visual impairment in 1989].

We studied the etiologies of visual impairment in the 171 children managed at the Alfred Peyrelongue Center, Ambarès France. This center is open to visually impaired children aged 3 to 20 years and free of significant motor or cognitive handicaps. Etiologies of visual impairment in the decreasing order of frequency were as follows: cataract (31 cases), tapetoretinal degeneration (28 cases), optic nerve lesion (22 cases), retinopathy of prematurity (13 cases), coloboma malformations (13 cases), severe congenital myopia (12 cases), congenital glaucoma (10 cases), congenital nystagmus (8 cases), congenital achromatopsia (6 cases), retinal abnormalities, and ocular injuries. We discuss these results, compare them with those of a similar study carried out in the same center in 1978, and briefly review the literature.

[Mortality in a general surgery department of a University Hospital Center. A study during 1985: 1409 surgically treated cases, 45 postoperative deaths].

In 1985, 1409 consecutive patients underwent surgery in one surgical Professorial Unit of Lille (France). 45 (3.2%) died post-operatively: 28 (62%) were more than 70 years of age, 23 (50%) died after emergency procedure and 18 (40%) died in sepsis. Nothwerthy in retrospect, 20 (44%) died after surgical indication or procedure of questionable legitimacy. Preoperative appraisal of the surgical risk is still challenging nowadays but conclusions drawn from those charts should avoid identical outcome in similar patients.
