Is There Potential Clinical Utility in Reporting Variants of Uncertain Significance From Prenatal Sequencing?

OBJECTIVE: Prenatal sequencing of fetuses with abnormalities detected on imaging is expanding globally. Debate continues over whether variants of uncertain significance (VUS) should be reported prenatally, with some recent national position statements opposing this. In England, VUS that fit the fetal phenotype and require little further evidence for upgrade are discussed at multidisciplinary team (MDT) meetings to determine whether to report. We review the VUS reported by one English laboratory that provides prenatal sequencing to half of England. METHOD: The laboratory's database was searched from 01/10/2020 to 30/04/2025 to ascertain all cases where a VUS was reported. Pregnancy outcomes were obtained from local clinical teams to determine whether the VUS status had been resolved. RESULTS: VUS were reported in 41/881 fetuses sequenced. Follow-up data were available for 38/41 cases, of which 23 were subsequently upgraded to likely pathogenic and 1 downgraded to likely benign. The most common reason for upgrade was new information from post-mortem or postnatal review (17/23). CONCLUSION: There is clinical utility in reporting VUS from prenatal sequencing following MDT discussion. Although reporting VUS leaves parents with uncertainty, follow up (including post-mortem when applicable) resolves this in over 50% of cases (79% of cases re-examined pre- or postnatally).

Antibacterial effect of caprylic acid and monocaprylin on major bacterial mastitis pathogens.

Bovine mastitis is the most significant economic drain on the worldwide dairy industry. Concerns regarding poor cure rates, emergence of bacterial resistance, and residues in milk necessitate development of alternative therapeutic approaches to antibiotics for treatment of mastitis. A variety of free fatty acids and their monoglycerides have been reported to exert antimicrobial activity against a wide range of microorganisms. The objective of our study was to examine the efficacy of caprylic acid, a short-chain fatty acid, and its monoglyceride, monocaprylin, to inactivate common mastitis pathogens, including Streptococcus agalactiae, Streptococcus dysgalactiae, Streptococcus uberis, Staphylococcus aureus, and Escherichia coli. Milk samples containing 50 mM or 100 mM caprylic acid, and 25 mM or 50 mM monocaprylin were inoculated separately with a 3-isolate mixture of each of the 5 pathogens, and incubated at 39 degrees C. Populations of surviving bacteria were determined at 0 min, 1 min, 6 h, 12 h, and 24 h of incubation. Both caprylic acid and monocaprylin reduced all 5 pathogens by >5.0 log cfu/mL after 6 h of incubation. Among the bacterial species tested, Strep. agalactiae, Strep. dysgalactiae, and Strep. uberis were most sensitive, and E. coli was most tolerant to caprylic acid and monocaprylin. Results of this study indicate that caprylic acid and monocaprylin should be evaluated as alternatives or adjuncts to antibiotics as intra-mammary infusion to treat bovine mastitis.

Larvicidal activity of Tagetes patula essential oil against three mosquito species.

Larvicidal activity of Tagetes patula essential oil was tested against the fourth instar larvae of Aedes aegypti, Anopheles stephensi, and Culex quinquefaciatus. Five different concentrations of essential oil were studied and the results were compared with that of synthetic insecticide, malathion. A. aegypti (LC(50) 13.57, LC(90) 37.91) was most susceptible followed by An. stephensi (LC(50) 12.08, LC(90) 57.62) and C. quinquefaciatus (LC(50) 22.33, LC(90) 71.89).

Biology and applications of mycorrhizal fungi.

Mycorrhizae have been shown to increase growth and yield of plants. They have been identified with both nutrient mobilization and nutrient cycling. Arbuscular (or endo-) mycorrhizae play a significant role in agriculture and most natural ecosystems, whereas ectomycorrhizae have a great potential in forestry and wasteland regeneration. The use of mycorrhizal fungi would reduce dependence on chemical fertilizers besides minimizing environmental pollution. The present review addresses the progress that there has been in the area of the ecto- and endomycorrhizae. It also examines the potential of field applications of mycorrhizal biotechnology in agriculture and forestry.

Novel mutations and polymorphisms in the Fanconi anemia group C gene.

Fanconi anemia (FA) is an autosomal recessive disorder associated with hypersensitivity to DNA cross-linking agents and bone marrow failure. At least four complementation groups have been defined, and the FA group C gene (FAC) has been cloned. We have screened 76 unrelated FA patients of diverse ethnic and geographic origins and from unknown complementation groups for mutations in the FAC gene either by chemical cleavage mismatch analysis or by single-strand conformational polymorphism (SSCP). Five mutations were detected in four patients (5.3%), including two novel mutations (W22X and L496R). Nine polymorphisms were detected, seven of which have not been described previously (663A-->G, L190F, IVS6 + 30C-->T, I312V, V449M, Q465R, and 1974G-->A). Six of the nine polymorphisms occurred in patients or controls from the Tswana or Sotho chiefdoms of South Africa and were not found in 50 unrelated European controls. Restriction site assays were established for all 8 pathogenic mutations identified in the FAC gene to date and used to screen a total of 94 unrelated FA patients. This identified only one other group C patient, who was homozygons for the mutation IVS4 + 4A-->T. This study indicates that the proportion of FA patients from complementation group C is generally likely to be less than 10%. Guidelines for the selection of FA patients for FAC mutation screening are proposed.

Avoiding medication mixups. Identifiable imprint codes.

This study was done to determine if current imprinting of solid medication forms permits health care professionals to identify the manufacturers involved so as to be able to activate the hierarchic identification system mandated by the Food and Drug Administration. We tested 15 representatives of 6 groups of health professionals for their ability to identify the manufacturer after having examined 30 solid-dosage forms drawn from a pseudo-random sample of stock hospital formulary products. The correct identification of the manufacturer was the sole criterion. Of the 2,700 opportunities, the manufacturer was able to be identified for only 43%. Nurses and medical students had a 35% success rate, pharmacists and poison center specialists a 55% success rate, and residents and attending physicians a 40% rate. None approached 95% accuracy. Currently employed imprints fail in their objective to permit health care professionals--or the general public--to rapidly identify prescription drugs. The manufacturers' logotypes need to be modified if this identification system is to be implemented. We propose a simple voluntary collaborative effort by the pharmaceutical industry to solve the problem.

Biosoluble surgical material from 2,3-dialdehyde cellulose.

Dialdehyde cellulose (DAC) was prepared by periodate oxidation of various cellulosic materials in the form of sutures and bandages. The suitability of using DAC for making bioabsorbable sutures and antimicrobial wound dressing bandages was studied. The investigations indicated that DAC combines the requisite mechanical strength with an inherent antimicrobial nature. It was found that the oxidation of cellulose to yield DAC also rendered it bioabsorbable. The DAC was coated with different concentrations of urea formaldehyde in order to improve the mechanical properties for making sutures.

Effect of anionic polymeric hydrogels on spermatozoa motility.

The effects of a few synthetic polymers on the motility of human spermatozoa in vitro have been studied. An alternate copolymer of styrene and maleic anhydride, poly(S-MA), poly (styrene-maleic acid), poly(S-MC), poly(hydroxy-ethyl methacrylate-methacrylic acid) copolymer, poly(HEMA-MAC), poly(HEMA) homopolymer and poly(MAC) homopolymer were chosen for this purpose. It was found that all the carboxylic acid containing polymers are strong inhibitors of the motility of spermatozoa. Poly(HEMA) did not have any inhibitory effect on the motility of spermatozoa.

Novel mode of contraception using polymeric hydrogels. I.

A new method of fertility control has been proposed and tested. The technique involves the use of the polymer, which when injected into the vas deferens lowers the pH sufficiently so as to kill the spermatozoa passing through. The polymer provides an acidic environment for a prolonged time and slowly erodes. The fertility may be restored after complete solubility of the polymer itself over a period of time or by flushing it with a suitable solvent. This method of male contraception is nonsurgical, nonocclusive, and reversible.

pH-sensitive microcapsules for drug release.

Microcapsules were designed for a sustained drug release, where the external medium controls the rate of release of the drug. As a model, secretin was encapsulated in acryloyl chloride-lysine capsules, and the conditions of formation are described. The scanning electron micrographs show the formation of good spherical microcapsules in the size range of 5-10 microns. The release of secretin was studied in four media having different pH. Polymer dissolution was pH sensitive, and the capsules placed in different media eroded at a constant rate, depending on the pH of the medium. Dissolution of the microcapsules was limited to the polymer buffer media and the drug was released by zero-order kinetics. The possible use of such a system is in the treatment of duodenal ulcers and the diagnosis of pancreatic diseases.

p-Amino salicylic acid - oxidized cellulose system: a model for long term drug delivery.

The mobilization of p-amino salicylic acid (PASA) on periodic oxidized cellulose (O.C) as a biocompatible carrier was investigated. The immobilization of the PASA is based on Schiff's base formation between the amino group of PASA and the aldehyde group of O.C. The in vivo and in vitro release of p-amino salicylic acid was studied. Such a system may be useful for the sustained delivery of the drugs in the body, since O.C. itself is a biosoluble carrier.

Biocompatibility screening of poly (vinyl chloride) implants.

It was proposed to test the biocompatibility of a polymeric implant by assaying enzyme activity in the tissue homogenate. Medical grade poly(vinyl chloride) was implanted in rat gluteal muscle and muscle specimens taken from the implanted and non-implanted sites after 7, 14 and 21 days. In the muscle homogenate taken in suitable solvents, the activities of alkaline phosphatase, acid phosphatase, Ca2+ ATPase, leucine aminopeptidase (LAP) and succinic dehydrogenase (SDH) were tested and compared at implanted and non-implanted sites. Increased activity of acid phosphatase, alkaline phosphatase, LAP and Ca2+ ATPase were seen at the implanted site but the SDH activity was reduced. The enzyme activity at both sites was maximum around 14 days and the highest increase in activity at the implant site compared to the control site was observed for alkaline phosphatase and acid phosphatase. After 21 days, the activity decreased gradually. In case of SDH, the inhibition was more after 14 days and gradually decreased after 21 days.

An insulin delivery system from oxidized cellulose.

Cellulose oxidized with periodate (OC) has been shown to be a biocompatible, biodegradable matrix suitable for immobilization and sustained release of drugs. In the present work, insulin was immobilized on the OC-matrix and the release profile in vivo and in vitro was examined. The hormone is bound to the matrix by Schiff's bond formation and is released by hydrolysis of this bond as well by degradation of the OC-matrix. The release can be sustained over longer periods by reducing the Schiff's base with NaBH4. The insulin depots could be conveniently injected as suspensions and are able to maintain glucose levels in diabetized rabbits at normal values over several days.

Primary amyloidosis of urethra.

Primary amyloidosis is a rare disease. Only 10 cases of amyloid tumor of the urethra have been reported previously, with this being the eleventh case. Because of its rarity and because it stimulates carcinoma, biopsy is essential for definitive diagnosis and treatment. The literature is reviewed, and symptomatology, pathology, and management are discussed.

Injectable non-occlusive chemical contraception in the male-I.

A new contraceptive technique has been proposed and tested. The method uses a novel polymer which when injected into the vas deferens, lowers the pH sufficiently kill the spermatozoa passing through. The polymer itself does not degrade in the process but can be flushed out to regain fertility. Experiments carried out on rats demonstrate the efficacy of the technique so as to have non-surgical, non-occlusive and reversible male contraception.

Potential biosoluble carriers: biocompatibility and biodegradability of oxidized cellulose.

The biocompatibility and biodegradability of periodate oxidized cellulose is assessed in vivo. Morophological and histopathological studies over a period of six months indicate that oxidized cellulose degrades slowly without producing excessive tissue reaction. Preliminary investigations on the kinetics of degradation of oxidized cellulose in vitro, immobilization of alpha-chymotrypsin on it and the rate of the enzyme release in a solution of pH 7.4 are also briefly reported.
