[Epidemiological notes on nasal bleeding].

The incidence of epistaxis is difficult to calculate because only a 10% of them look for medical attention. In this study the nasal bleeding means the 13.3% of the ENT emergencies (279 patients). Its incidence was 6 cases/10,000/year. The majority were over 50 years old (57.3%). There was season variations. The anterior epistaxis were more frequent but he posterior ones increased over the 40. An 18% had rebleeding more significatively with age and localization. We discuss the mechanisms that can explain the male dominance, the season incidence, and the old ages of the patients with hospital epistaxis.

[Surgical treatment of laryngotracheal stenoses. Our experience].

Laryngo-tracheal stenoses (LTS) treatment is a problem of difficult management for the otolaryngologist. There are many and varied techniques in order to solve the conundrum. The aim of this article is to analyze retrospectively the outcomes of surgical treatment of LTS in our Institution. We have checked 11 cases (9 men and 2 women, arithmetic mean 30.7 years) operated through surgical procedures during a 6 years lapse of time. All patients were endotracheally intubated prior the stenosis appearance. Stenosis location most encountered were subglottic-tracheal combination. To the end of the study 44 percent of the series was successfully cured and the 33 percent only partially recovered. The employed techniques (endoscopic or open surgery) are revised at the light of today's literature.

[Computerized analysis of craniography: new limits of the norm for static and dynamic forms].

Literature about normal parameters of Craniocorpography (CCG) is scant and poorly documented. Its results are variable according to the series. Our aim is to reach a criteria of normality for CCG in our reference population. We have prospectively made Romberg and Unterberger tests to 72 healthy volunteers (mean age 34.7 years old, SD 17.07, 55.6% men) using a computerized method of measurement and making its estimation by means of a statistical model widely accepted. All the studied variables observe the conditions of Gaussian distribution. The normal limits for lateral and antero-posterior oscillation were 9 centimeters, 12 centimeters for lateral sway and 59 degrees to the right and 46 degrees to the left for angular deviation. The computerized method of analysis has facilitated the detection of the limits of the light tracings, which together with the careful design of our study guarantees the validity of the results.

[Prognostic factors influencing anatomic and functional outcome in myringoplasty].

The objectives of myringoplasty are essentially two: restore the eardrum integrity and improve hearing level. We have carried out a retrospective study of myringoplasties performed in our Department, between 1996 to 2000, corresponding to 197 surgical interventions, in 162 patients. The characteristics more frequently seen for operating ears were central localization of tympanic membrane perforation, dry ear, good eustaquian tube function and underlay technique placing fascia graft. Endaural or retroauricular approach were similar in number. We obtained 73.6% of anatomic success, though in some cases we needed to reoperate the patient. When we analysed risk factors of anatomic success, we noticed than only the tympanic perforation site had influence, with better rate in posterior perforations and lower rate in subtotal perforations. We analysed functional results in ears with anatomic success, finding 75.6% of hearing level improvement with a gap of less than 20 dB. Factors influencing hearing improvement were surgeon experience and the presence of middle ear alterations.

[Results of tympanoplasty with antrum exclusion in the treatment of middle ear cholesteatoma].

INTRODUCTION: Exclusion of the antrum was designed to avoid the problem of mastoid ventilation by means of insufficient eustachian tube. MATERIAL AND METHODS: We retrospectively reviewed 72 ears with cholesteatoma who underwent antrum exclusion tympanoplasty (57% in men, mean age 40.9 years). Mean follow-up was 31.2 months (range, 12-64 mo). RESULTS: Exclusively epitympanic-antral cholesteatoma location (28.4%), and the necrosis of the long branch of the incus (54%) were the most common. The most used material to reconstruct the ossicular chain was Torp prosthesis (73.6%), followed by incus autograft (11.1%). We reported the following complications: postoperatory otorrhea (5.5%), perforation of neotympanum (16.7%), prosthesis extrusion (23.6%, time average of 15.6 months), and recurrence of cholesteatoma (4.2%). DISCUSSION/CONCLUSION: The technique with antrum exclusion has reported acceptable results in the control cholesteatoma recidivism. Although the functional results are similar to those of the open techniques, the resulting cavity with the antroexclusion is more anatomic.

[The usefulness of craniocorpography in the diagnosis of patients with dizziness and increasing muscle tension in the neck].

By the moment, craniocorpographic record of the Romberg test has been scarcely used in clinical practice. Since craniocorpography is a method for recording oscillations of the head and shoulders, it can show characteristic patterns in patients with vertigo caused by increased muscle tension in the neck. In order to confirm this hypothesis, a prospective comparative study was performed with 329 patients. They were included in three different groups: patients with cervical vertigo, patients with vestibular pathology and a healthy control group. Movements of the head (with static shoulders) are more usual in patients with cervical vertigo (67%) than in those with vestibular pathology (27%) and healthy subjects (31%). With regard to the kind of head oscillations, front-back flexions and oscillations in all directions were the most frequent (36% and 34%, respectively). Nevertheless, the only movements previously reported (cervical rotation or torsion) only were 3% of total head movements in our study. We concluded that static craniocorpography is an useful method in diagnosing vertigo secondary to increased muscle tension in the neck.

[Asymptomatic acoustic neurinoma associated with hypophyseal macroadenoma].

UNLABELLED: The widespread use of gadolinium-enhanced MRI (Gd-MRI) in our hospitals has supposed an increase in detection of asymptomatic acoustic neuromas (AN) as incidental finding. Concurrent primary intracranial tumors of different cell types are extremely uncommon. CLINICAL CASE: A 23 year-old woman complained of irregular menstrual cycles and galactorrhea for two years. A Gd-MRI scan showed a pituitary macroadenoma and an AN of 1.4 cm. with intracanalicular and cisternal extension. The patient did not have any otologic or neuro-otologic symptoms or signs. Audiograms revealed bilateral normal hearing. The results of acoustic reflex test and electronystagmographic caloric tests were normal. The AN underwent radiosurgery treatment. DISCUSSION: The literature about atypical AN is reviewed, giving a particular concern on asymptomatic cases. We only have found one case of association between AN and pituitary adenoma. The real prevalence of AN in general population is discussed.

[Unifocal eosinophilic granuloma of the temporal bone (Langerhans cell histiocytosis].

Eosinophilic granuloma is one of the diseases of unknown etiology traditionally known as histiocytosis X and now called histiocytosis of the Langerhans cells since it has been found to originate by proliferation of this cell line. We report a clinical case of unifocal eosinophilic granuloma of the temporal bone in an adult. We reviewed the literature on the diagnosis, prognosis, treatment, and assessment of these patients. The importance of making a thorough analysis of all the anatomical structures in imaging is emphasized as a way of making a precise early diagnosis. The treatment of choice is surgical removal of the tumor. Irradiation is used for inaccessible or incompletely resected lesions. The prognosis of unifocal eosinophilic granuloma is very good, with successful disease control being achieved in 95% of cases.

[Epistaxis: prospective study on emergency care at the hospital level].

We prospectively studied 279 patients with epistaxis referred to ENT specialist from emergency room in our hospital in one year. Masculine sex (62%), medium or old ages (median 56 year), and associated diseases (HBP 22.9%, anticoagulants or antiplatelet drugs 11.1%), were the most common. Predominant local etiologic factors were trauma (12.9%) and inflammation (14%). Epistaxis had been essential in 36.9%. The most frequent location in all ages was the anterior one, especially located in the Kiesselbach's area, although the incidence of the posterior epistaxis increases in characteristic way from fourth decade, affecting more to men. Most of epistaxis (> 99%) were treated successfully by conservative approach and only two patients required surgical or interventional therapy. Patients between 30-59 year and the posterior epistaxis was the most refractory to the treatment. Hospitalized patients were older than ambulatory ones and had more associated diseases. Their average hospital stay was 9.2 days.
