Of the 97 throat smear-positive cases of diphtheria treated, 16 developed evidence of myocarditis (16.5%). Age, duration of illness prior to admission, anatomical extent of membrane formation, total leukocyte count and serum glutamic oxalo-acetic transaminase (SGOT) levels were analysed in fatal and non-fatal cases and the differences between the two groups found to be statistically significant with regard to anatomical extent of membrane formation, total leukocyte count and SGOT levels. The extension of membrane formation to two or more sites was a highly sensitive predictor of mortality. A total leukocyte count > 25,000 cells/mm3 had a high specificity and positive predictive value while SGOT levels of > 80 IU/l had high sensitivity and a negative predictive value.

Eighteen children diagnosed as having laryngeal diphtheria with various degrees of respiratory obstruction were treated with intravenous dexamethasone in addition to penicillin and anti-diphtheritic serum. Reduction in the severity of laryngeal obstruction was assessed 12-hourly. Eleven of 12 children with mild-to-moderate obstruction responded within 24 hours. The 12th child died of toxaemia and renal failure following tracheostomy. Four of six with severe obstruction also responded within 24 hours. The remaining two underwent tracheostomy, one of whom died. Dexamethasone appears to have a role to play in obviating tracheostomy in laryngeal diphtheria.

Twenty cases of clinically diagnosed congenital hypothyroidism during a 7-year period at a District Hospital in South India are analysed. A maximum of nine cases were diagnosed in the age group 1-5 years, only two before 3 months of age and five after 10 years of age. Two siblings of one family had the disease. Three children were associated with muscular hypertrophy, i.e. Kocher-Debre-Semelaigne Syndrome. Constipation and delayed milestones were the commonest symptoms (60 per cent). Classical facies (100 per cent), wide open anterior fontanella (65 per cent), coarse skin (60 per cent), and stunted growth (60 per cent) were the common physical findings. IQ was in between 30 and 40 in 60 per cent of cases. Only 50 per cent of them were followed up for a period of 1-5 years. Though physical growth was good on thyroxine IQ of above 80 was achieved in only three (30 per cent).

Epidemiological and clinical analysis of 228 cases of diphtheria treated during a 10-year period from 1980-1989 is presented. No statistically significant change in the incidence over a decade was noticed. A high percentage of cases (44.74%) were noted in the age group of 5 years and above. Bilateral tonsillar and pharyngeal membranes together were seen in 60% cases. All patients were treated with diphtheria antitoxin and benzyl penicillin. Severe myocarditis (9 cases), fulminant haemorrhagic complications (2 cases), palatal palsies (11 cases) and polyneuritis and cranial nerve paralysis (2 cases) were the significant complications. Case fatality rate was 13.21%. Statistically significant increase in the mortality (p less than 0.05) was noticed in the year 1987 and onwards. In this period, myocarditis (8 out of 16 deaths) is gradually replacing laryngeal obstruction, the major cause of death in early eighties or post-tracheostomy complications (6 out of 16 deaths). Only 2 patients died of haemorrhagic diphtheria in this period.

Three girls and one boy out of six siblings born to parents of consanguineous marriage presented with pituitary dwarfism. Their parents were of normal height. All the affected children had features of classical isolated growth hormone deficiency. No hypoglycaemic attacks were noted. Three of them attained puberty at the age of 16 years.
