The authors considered basic methodology concerning evaluation of occupationally related morbidity: terminology, legal and methodologic items. Suggestions included new single-numerical index for occupationally related morbidity, considering categories of risk, severity and occupational connection.
The article deals with essentials of evidence-based medicine, its principles in occupational medicine and patterns of evaluation of occupation and disease relatedness. The authors suggest an algorithm of medical examinations considering evidence-based medicine principles.
Loader machine operators in Volzhskiĭ Automobile Works, who suffer from cervical osteochondrosis, were subjected to X-ray screening with special applied methods. The screening revealed various compensatory and dystrophic changes involving locomotion apparatus of hands and cervical part of spine. The studied occupational factors proved to enhance the locomotion disorders in the examinees.
The paper is concerned with the results of neurological examination of 113 patients who developed acute radiation sickness of various degrees of severity during the elimination of the effects of the accident at the Chernobyl Nuclear Power Station. A clinical picture of neurological disorders at different periods of ARS was described. CNS specific and nonspecific lesions were defined.
Under neurological investigation were 113 patients with different degrees of severity of acute radiation disease (ARD). The patients were relatively evenly irradiated by beta- and gamma-rays as a result of CPP accident. The onset time and markedness of the signs of primary reaction were related to ARD severity. Early appearance of the signs of disturbances in the central nervous system functional state suggested a stronger affliction, malignant course and fatal outcome of the disease. Ii ARD patients with IV-th and III-rd degree of severity the syndromes observed were these: acute radiation encephalopathy immediately caused by the ionizing irradiation of the CNS, and a secondary radiation-toxic encephalopathy. The hemorrhagic lesioning of CNS and cerebral hemispheres were noted in terminal stages. In rehabilitation phases, neurasthenic and neurovascular syndromes appeared irrespective of the ARD severity.
