[Prophylaxis of pyogenic complications in total replacement of large joints].

Results are studied of management of 378 patients having undergone total knee and hip arthroplasty over the period 1995-2001. Surgical interventions were undertaken in those patients presenting with osteoarthrosis deformans, rheumatoid arthritis, for complicated posttraumatic fractures, pathological fractures secondary to an oncological process. Pyo-inflammatory complications which necessitated removal of endoprothesis developed in nine (2.32%) patients. A strict keeping to the system of the purulent complications prophylaxis in total arthroplasty is an important component of a successful management of those patients presenting with grave forms of affections of the great joints.

[The influence of contrykal on intraoperative blood loss during endoprosthesis implantation of the hip joint].

Application of the proteases inhibitors constitutes one of possible pharmacologic methods of the blood loss compensation during performance of operative intervention in orthopedics, especially for endoprosthesis of the main joints. Application of contrykalum for endoprosthesis of the hip joint is expedient, if prognosticated intraoperative blood loss exceeds 1000 ml. Efficacy of preparation was not established for the lesser blood loss cases.

[Experience in the correction of postoperative shivering and tremor in the main joints endoprosthesis].

In 74 patients, to whom total endoprosthesis of the hip and knee joints was performed, suffering postoperative shivering and trembling, the temperature homeostasis was investigated. In 66.6% of observations negative signs of fever with hypercapnia and saturation indexes lowering were revealed. The tramadol application constitutes secure prophylactic measure in occurring postoperative complications, connected with shivering and trembling.

[Preoperative patient blood preservation as an alternative to donor blood transfusion during joint endoprosthesis surgery].

In 79 patients, to whom total endoprosthesis of the knee and hip joints were performed, the efficacy of preoperative blood preparation was studied up. The method is effective and secure alternative to the donor blood transfusion in orthopedic surgery. Taking into account the ability of patient with rheumathoid arthritis to restore the hemoglobin level, the expediency of preoperative blood preparation in them must be determined individually.

[Impact of anesthesia methods on the course of perioperative period in endoprosthesis of the hip joint].

The impact of methods of anesthesia on the course of perioperative period was studied up in 132 patients, to whom the total endoprosthetics of the hip joint was performed. The occupation in the operation room term and analysis of the postoperative pain syndrome occurrence with its cupping served as comparative categories for general and regional methods of anesthesia. Central neuronal blockade, epidural and spinal anesthesia have predominant significance for enhancing of the intraoperative and postoperative analgetic defense.

[Anesthesia using laryngeal mask in orthopedic surgery].

Experience of application of laryngeal mask (LM) for realization of artificial pulmonary ventilation in patients in complicated conditions for trachea intubation in orthopedic surgery allows to recommend it as an alternative method. Simplicity of establishment, absence of complications after application of LM were noted.

[Application of the apparatus autohemotransfusion in endoprosthesis of big joints].

We observed 16 patients in whom the method of camera intraoperation reinfusion of washed erythrocytes was used during endoprosthetics of joints. For reinfusion we used the system Elmd-500 (Electromedics Inc., USA). The concentration of hemoglobin in peripheral blood remained stable in early and late post-operation period. No complications after transfusion of washed erythrocytes were observed.

[A comparative evaluation of the subfractional composition of the blood plasma by using laser correlation spectroscopy in orthopedic-traumatological diseases].

Analysis presented by the authors confirms that laser correlative spectroscopy may be in principle a method for a study of plasma homeostasis on subfractional level in various orthopedic and traumatological diseases. The method allows to assess changes in the inner environment of the body with regard to diversity of structural and functional reorganization induced by various pathological processes.

[Postoperative nausea and vomiting prophylaxis after total hip and knee arthroplasty].

A study was made in two groups of those patients (n = 113) having undergone planned total coxofemoral and knee arthroplasty. 56.2 percent of patients in the control group experienced postoperative nausea and vomiting. The institution of standardized measures permitted reducing the incidence of postoperative nausea and vomiting down to 4%.

[Hemodynamic features of operations with tourniquet in orthopedic surgery].

The investigation was carried out in group of 38 patients where total arthroplasty knee joints operations were planned and performed. The operations have been performed under general or regional anesthesia. Usage of a knee joints arthroplasty technique under a compression with tourniquet in conditions of a general anaesthesia, is accompanied by a hypertensive syndrome requiring for a hemodynamic correction. The decompression after prosthetic repair is integrated to a recirculation hypotension especially expressed with usage of hypotensive drugs. The obtained data prove the regional methods of an anaesthesia as more reasonable for total arthroplasty of knee joints.

[The diagnostic problem of arthrosis deformans].

Plasma homeostasis was studied in patients with degenerative diseases of joints with the aid of laser correlative spectroscopy. The above instrumental method is distinguished by high resolving power permitting very small bodily internal shifts to be accurately detected. Integrated parameters were studied characterizing homeostasis at the subfractional level in a series of 43 patients.
