Total arterial myocardial revascularization without cardiopulmonary bypass.

The risks associated with cardiopulmonary bypass have led to an interest in coronary surgery without the use of such a bypass. Six patients of mean(s.d.) age 62.0(8.0) (range 52-71) years were selected for elective coronary surgery without cardiopulmonary bypass. In five cases a midline sternotomy and in one case a small anterolateral thoracotomy were performed; in the latter case the harvesting of the proximal end of the left internal mammary artery was video-assisted by thoracoscopy. The left internal mammary artery was used in all cases; the right internal mammary artery was used in one case, the radial artery in four, the inferior epigastric artery in two and the right gastroepiploic artery inn one. No patient died or had a stroke. There were no postoperative episodes of low cardiac output syndrome or perioperative myocardial infarction. All patients were extubated within a few hours after surgery. The mean(s.d.) intensive care unit and hospital stays were 1.3(0.5) and 5.0(0.9) days, respectively. Total arterial myocardial revascularization without cardiopulmonary bypass using composite grafts, is a new and promising technique that is feasible with low risks and good early results in selected cases.

Sleep suppression of ventricular arrhythmias: a predictor of beta-blocker efficacy.

It has been reported that the frequency of premature ventricular contractions in some patients tend to decrease during the hours of sleep when modifications in autonomic tone and bradycardia occur. The aim of this study was to evaluate whether the phenomenon of sleep suppression may be a sensitive and specific parameter for predicting the antiarrhythmic effect of beta-blockers on premature ventricular contractions. The presence of sleep suppression was evaluated in 45 patients (mean age 50 +/- 17 years) with frequent premature ventricular contractions at two baseline Holter recordings. Sleep suppression was defined as > 50% reduction in the number of nighttime as opposed to day-time premature ventricular contractions. Three groups of patients were identified: those with sleep suppression at both Holter recordings (group 1); those with sleep suppression at only one Holter recording (group 2); and those without sleep suppression at either Holter recording (group 3). A third Holter was performed 5 days after nadolol administration. In group 1, nadolol led to a mean reduction in the number of premature ventricular contractions of 90% (> 70% in 21/23 patients). In group 2, the mean reduction was 76% (> 70% in three out of six patients). In group 3, there was a mean increase in the number of premature ventricular contractions of 33%. The positive predictive accuracy of sleep suppression in relation to the antiarrhythmic efficacy of nadolol is very high (88%) when sleep suppression is present during two baseline Holter recordings. Sleep suppression is a sensitive parameter for identifying the premature ventricular contractions likely to benefit from beta-blocker administration.

The effect of nadolol on heart rate and the standard deviation of the RR intervals.

The aim of this study was to evaluate the effect of nadolol in modifying the standard deviation of RR intervals (SDRR) and the coefficient of variance (CV) in 47 patients characterized by symptomatic, frequent (> 30 h-1) and reproducible premature ventricular contractions (PVCs). Sixteen patients had suffered a previous myocardial infarction (Group 1), 22 had various non-ischaemic cardiac diseases (Group 2) and nine had no heart disease (Group 3). All patients underwent two 24 h Holter recordings during the washout period, and a third Holter recording was taken 5 days after the administration of nadolol (80 mg.day-1). The RR cycle length (RR), SDRR and CV were evaluated for each 24 h period, as well as for six daytime (1000-1600h) and six night-time hours (0000-0600h). In all three groups, nadolol was highly effective in lengthening RR. However, the effect on SDRR was different in the three groups; 24-h SDRR tended to be reduced in all three groups (but the reduction was significant only in Group 1) due to a certain RR homogeneity. On the other hand, daytime SDRR tended to increase (but the increase was significant only in Group 3). CV is concomitantly affected by heart rate and SDRR, therefore it may be more sensitive in evaluating the effect of beta-blockers. The reduction in CV post-nadolol means that the bradycardia-induced effect is associated with an inadequate increase, or even a reduction in SDRR, whereas the absence of any CV changes (found only in Group 3) is the result of a concomitant increase in both RR cycle length and SDRR.(ABSTRACT TRUNCATED AT 250 WORDS)

[Is it possible to predict the anti-arrhythmic effect of beta blockers based on various relations between premature ventricular beat and the preceding cardiac cycle?].

The aim of our study was to correlate betablocker effects on premature ventricular beats (PVBs) with different patterns between PVB frequency and cardiac cycle. The computer program we used correlates each PVB to the preceding cycle length in a 24-hour electrocardiogram recording. The arrhythmic patterns obtained were defined as tachycardia-dependent, indifferent and bradycardia-dependent. We selected 51 patients (39 males, 12 females, mean age 52 years) with different cardiac diseases, all in sinus rhythm, with > 25 PVBs/h, characterized by PVB stability and reproducible pattern in 2 Holter monitoring recorded at a 3-5 day interval. A tachycardia-dependent pattern (TDP) was demonstrated in 20 patients; 22 patients showed an indifferent pattern (IP) and 9 a bradycardia-dependent pattern (BDP). A third Holter monitoring was performed 4-5 days after nadolol administration (80 mg/day). Nadolol caused a 87.7% PVB suppression in TDP patients (p < 0.001), a 34.8% PVB reduction in IP patients (p < 0.01) and a 36.3% increase in BDP patients (NS). The different effect of nadolol on these groups was highly significant (chi 2 30.9; p < 0.0001). These results indicate that pattern definitions is useful in identifying PVB subsets which are likely to be improved, or not, by betablockers.

[Transesophageal study in the diagnostic evaluation of pre-excitation].

Electrophysiologic non-invasive transesophageal testing is compared to intracardiac study in the management of patients with Wolff-Parkinson-White (WPW) syndrome. Transesophageal study can be reliably used to identify the participation of the accessory pathway in reciprocating supraventricular tachycardia and to determine the anterograde conduction properties of the accessory pathway. Using the shortest pre-excited interval during induced atrial fibrillation, or programmed and continuous atrial transesophageal stimulation can markedly reduce the need of intracardiac evaluation. The greater safety and economy of transesophageal compared to the intracardiac technique justify its wider use in preliminary screening of all WPW patients, unless ablative treatment has been clinically indicated, and in evaluating long-term drug protection against a potential deleterious ventricular response during atrial tachyarrhythmias.

[Incidence of ectopic pregnancy at the 1st Clinic of Obstetrics and Gynecology of Bari during the past 10 years. Clinico-statistical considerations].

The Authors, in the period 1979-1988, discovered 175 ectopic pregnancies out of a total of 16.641 pregnancies: a rate of incidence of 1 in 95 (1.05%); 28 patients were fitted with an IUD. In accordance with published data, they recognize an increase in the incidence of ectopic pregnancies and emphasize the importance of a precocious clinical diagnostic, to reconcile timeliness of operation with the need of conservative surgery, the indication for which should take into account many factors (the age, the general condition, the parity, the wish for a pregnancy, the state and condition of the tube on each side) since it seems to predispose to recurrent ectopic pregnancies. They indicate the negative effects on the tube for patients fitted with an IUD.

Continuous suture technique and impairment of the atrioventricular conduction after aortic valve replacement.

OBJECTIVE: Atrioventricular (AV) conduction impairment represents an infrequent complication following aortic valve replacement (AVR). Although several conditions have been considered as potential risk factors for such a postoperative event, the role of the surgical technique has not been evaluated. The aim of this study was to investigate if the suture technique for implantation of the prosthetic valve is a potential risk factor need of pacemaker (PM) implantation after AVR. METHODS: One hundred twenty-four patients undergoing AVR were enrolled in this study. A "continuous" suture technique was performed in 72 patients (58%; Group A), whereas an "interrupted" suture technique was used in 52 patients (42%; Group B). The incidence of "major," requiring PM implantation, as well as "minor," without PM implantation, impairment of the AV conduction was evaluated and compared in the two groups by means of univariate and multivariate analyses. RESULTS: The patients of the two groups were homogenous for all preoperative parameters except patient age. A longer clamp time was present in the patients of group B than those of group A (73 +/- 24 minutes and 60 +/- 24 minutes, respectively; p < 0.01). Postoperatively, hospital mortality consisted of four patients (3.2%) with no statistical importance between the two groups. Eleven patients of group A (17.5%) and one patient of group B1 (2.2%) required PM implantation (p < 0.05). Prolonged P-R interval was recorded in 15 patients of group A and in 5 patients of group B (p < 0.05). CONCLUSIONS: We showed that the continuous suture technique increases the need for postoperative PM implantation after AVR. No major differences have been observed, however, regarding in-hospital mortality and length of hospital stay, regardless of the type of prosthetic suture technique.

[Acute postoperative mediastinitis: retrospective analysis of various treatment methods].

Infective acute mediastinitis is a postoperative complication reported in 0.5-1% of patients undergoing open chest operations. The treatment of choice for this life-threatening complication is still a matter of debate. The aim of this study was to retrospectively analyse the efficacy of different therapeutic approaches in the treatment of postoperative infective mediastinitis. In the 2nd Division of Cardiac Surgery, from October 1986 to May 2000, 10,234 patients underwent cardiac surgery operations. In 42 patients (0.4%) the operation was complicated by acute infective mediastinitis requiring surgical treatment. On the basis of the treatment opted for, these patients were subdivided into 5 groups: 23 patients underwent continuous iodopovidone (Betadine) mediastinal irrigation (GL) associated with surgical omentoplasty in 8 patients (GLO); 5 patients underwent isolated omentoplasty (GO), and 4 patients were treated with a pectoral muscle flap (GF). In 8 patients other different procedures were performed (GS). There were no deaths in GF and GS despite 24% and 20% mortality reported among patients who underwent mediastinal irrigation (GL) and isolated omentoplasty (GO), respectively. The mean hospital stay was 15 +/- 1 days in GF, 16 +/- 1 days in GS, 25 +/- 11 in patients who underwent omentoplasty and 27 +/- 14 in patients who underwent mediastinal irrigation. Predictors of death were low cardiac output syndrome (P < or = 0.009) and respiratory insufficiency (P < or = 0.032) when found before treatment. Our study suggests that surgical omentoplasty should be the treatment of choice in deep mediastinal infections, whereas wound sterilisation, associated with surgical chest wall reconstruction, seems to be a better procedure in superficial infective disease. A more extended clinical series would be needed to confirm these preliminary data.
