[Parasitic helminths infection in Coho salmón, Oncorhynchus kisutch, during their return to Simpson river, Chile].

Between may and july 1994, 17 adult returning salmons, Oncorhynchus kisutch, were collected in the River Simpson, Chile. All fishes showed infection by plerocercoids of Diphyllobothrium sp. in different locations: stomach, spleen, liver, mesenteries and gonads. Infection with larval cestodes of an unidentified species of Phillobothriidae was determined in the intestine of seven (41.2%) salmons and its prevalences of infection showed significant differences between female and male salmons. The 94.4% of total plerocercoids of Diphyllobothrium were isolated from the stomach wall. Prevalence and mean intensity of infection by Diphyllobothrium sp. did not show significant differences between fishes of different sex.

[A case of massive ascariasis in the riverside sector of the Riñihue lake, Chile].

A case of massive ascariasis in a three and a half years old girl who eliminated 104 worms after treatment with mebendazole is reported from a riverside sector of Riñihue Lake, Chile. Chilean literature about intestinal massive ascariasis is reviewed. Worms showed 12-29 cm of length and 0.2-5.5 g of weight. Total worms weighted 155.8 g.

[Chronic caustic esophagitis in childhood. A functional study].

The purpose of this study was to investigate the motor function of the esophagus in patients with chronic caustic esophagitis. We evaluated 7 patients between 2 and 10 years of age, who had ingested caustics between 15 months and 3 years of age. The method we used was radiology with fluoroscopy vision and manometry. There was no correlation between radiologic and manometric findings. Esophageal transit was normal in all patients, even in those that had small stenosis while manometry showed motor disfunction in patients with severe esophageal damage.

[Blastocystosis and other intestinal protozoan infections in human riverside communities of the Valdivia River basin, Chile].

Between March and October 1987, the prevalence of infection by Blastocystis hominis and other intestinal protozoan, their relationship with the age and sex of the hosts, and the percentage of infected persons in family groups were determined in riverside communities of Valdivia River Basin, Chile. One or more intestinal protozoan species were determined in 72.5% of the examined persons. The prevalence was greater for B. hominis (61.8%). The prevalences of B. hominis, Endolimax nana and Entamoeba coli were greater in relation to the age of the host. The sex of the host and prevalence of infections by B. hominis and other species of intestinal protozoans did not show association. Prevalence of B. hominis was greater in persons from houses with no sanitary faeces disposal. Over 60% of the members of family groups showed infection by B. hominis in 53.1% of the groups compared to 2.4%-21.8% of infections by other protozoan species. Faecal samples of 45 pigs revealed 22.2% of infection by Blastocystis.

[The usefulness of endoscopic polypectomy of rectosigmoid and colon in childhood].

Rectal polyps are relatively frequent in infancy and constitute one of the main causes of hematoquezia during this time of life. They are usually treated with multiple drugs before the correct diagnosis is made. We report our experiences with the use of rectoscopy and colonoscopy for the diagnosis and treatment of 214 patients with rectosigmoid polyps, including other sites in the colon, studied between 1982-1988. The highest incidence was between 3 and 6 years of age (63.8%), and the most frequent location in 253 rectosigmoid polyps (91%) and histologically (99.7%) were juvenile polyps. Endoscopic polypectomy proved to be a valuable method, easy to perform and with few complications to treat colon polyps, thus eliminating the surgical interventions.

[Predictive value of ultrasonography in portal hypertension].

Portal hypertension is a common pathology in childhood and one of its most common causes is cavernomatosis of the portal vein. This obstruction causes hemodynamic changes which lead to splenomegaly and collateral circulation. Esophageal varices are one of the most important sequelae, which endanger the patient's life because of a bleeding tendency. Ecosonography helps to detect the thickening of the lesser omentum vis a vis the aortic diameter, caused by the collateral circulation. We studied 15 children presenting with portal hypertension resulting from portal vein cavernomatosis; we performed an upper GI endoscopy and abdominal ecosonography. The endoscopy revealed grade II esophageal varices in 20% of cases, the remaining 80% had grade III and grade IV. Ecosonography revealed an increased lesser omentum/aorta ratio in children with portal hypertension, compared to controls (p < 0.001). Our results suggest that the lesser omentum/aorta ratio has diagnostic value in pediatric portal hypertension.

[Achalasia in children].

Achalasia is a pathology which was first described more than 300 years ago, it is relatively rare in children. Four cases of childhood achalasia were studied in children admitted to the J. M. de los Ríos Hospital during the period January, 1971 to June, 1990. The clinical presentation was characterized by progressive dysphagia to solids and liquids, regurgitations, weight loss and in certain cases, a moist cough, predominantly nocturnal. The diagnostic workup carried out included radiological, endoscopic and manometric studies. They were helpful to diagnose the following positive signs: esophageal dilation with alteration of motility and a small caliber cardio-esophageal junction, in addition to an elevation of the inferior esophageal sphincteric pressure with non-peristaltic esophageal contractions. The Starck dilator was used to create a mechanical disruption of the muscle fibers of the inferior esophageal sphincter. The procedure improved the clinical symptoms, lowered the inferior esophageal sphincteric pressure leading to adequate pondostatural progress and a satisfactory evolution.

[Parasitism in fishes and human riverside communities of the Huillinco and Natri lakes (Great Island of Chiloé), Chile].

In April and December 1989, 35 fish from Lake Huillinco (42 degrees 48'S, 74 degrees 02'W) and 36 fish from Lake Natri (42 degrees 48'S, 73 degrees 50'W), in the Great Island of Chiloé (Chile) were examined. Coprological samples from 159 persons, 17 dogs, 19 pigs and 4 cats from around both lakes were examined for Diphyllobothrium spp. infection. In the Lake Huillinco the following helminths of fishes were determined: Contracaecum sp. and Hysterothylacium sp. in Salmo trutta, Cauque mauleanum and Eleginops maclovinus; Dichelyne (Cucullanellus) dichelyneformis in S. trutta and E. maclovinus and Scolex pleuronectis in S. trutta. One specimen of Mugil cephalus did not show helminth parasites. Prevalence of infection were greater for Contracaecum sp. in S. trutta (75.0%) and C. mauleanum (76.0%); and Hysterothylacium sp. in E. maclovinus (75.0%). Mean intensity was higher for D. (C.) dichelyneformis in E. maclovinus. Contracaecum sp. in S. trutta, Oncorhynchus mykiss, Oncorhynchus kisutch and Galaxias maculatus; Acanthocephalus sp. in S. trutta and G. maculatus, S. pleuronectis in O. mykiss and Cystidicoloides sp. in G. maculatus were determined at Lake Natri. Prevalence and intensity of infection were higher for Contracaecum sp. in S. trutta and O. kisutch. Infection by Diphyllobothrium sp. was determined in one domestic cat. Prevalence of infection by intestinal protozoan and helminths in human population only showed significative differences for Ascaris lumbricoides and Trichuris trichiura that were higher in the Lake Huillinco. Importance of natural infection by helminth parasites for fish in cultured condition and possible mechanisms of infections in relation to the diet of fishes are discussed.

[Distribution of the infection by Camallanus corderoi (Nemata: Spiruroidea) in different autochthonous hosts and localities of the Valdivia river basin, Chile].

Six hundred and thirty nine autochthonous fish were examined in 1986-1987 in search for Camallanus corderoi infection on different biotopes from Valdivia River basin (between 39 degrees 30'-40 degrees 00'S, 73 degrees 30'-71 degrees 45'' W). Brachigalaxias bullocki, Galaxias maculatus, Galaxias platei, Aplochiton taeniatus, Cheirodon australe, Basilichthys australis and Cauque mauleanum are new hosts for C. corderoi. Infection by C. corderoi in fish is acquired by ingestion of infected copepods and prevalence and intensity of infection showed differences between host fish and biotopes. In general, infrapopulations of C. corderoi showed overdispersion in different hosts.

[New cases of human diphyllobothriasis in Panguipulli, Chile].

A retrospective laboratory survey on 10758 patients at the Hospital of Panguipulli showed the existence of 11 cases of diphyllobothriasis during a ten year (1979-1988) period. Age and sex as well as data of biannual prevalence of diphyllobothriasis are given. Some factors that determine the existence of diphyllobothriasis focus at lakes from Valdivia River Basin are briefly reviewed.

[Usefulness of endoscopic retrograde cholangiopancreatography in diagnosis of choledochal cysts in children].

We report 11 infants with choledochal cyst confirmed during surgery. In none of the patients an abdominal mass was palpated during physical examination. In 10 (91%) of the patients, ERCP showed the cyst. Ultrasound was positive in 82% (9/11) of the cases. Two patients had acute relapsing pancreatitis with severe anatomical abnormalities at the pancreatobiliary junction. Multiple biliary lithiasis were seen in four cysts. ERCP is of significant value in establishing the diagnosis, defining anatomic abnormalities and complications. ERCP should be the procedure of choice in patients with suspected choledochal cyst.

[Liaison psychiatry: a new perspective on mental health in Latin America].

The situation of Mental Health in Latin America is analyzed. Urgently creating a system able at handling the psychiatric disease increase in the years to come is a conclusion this paper strongly aims at. The beginning of Psychiatry is reviewed, and the importance of Psychiatry at the General Hospital is outlined as well as its rapprochement toward the community, which led to a better control and prevention in the field of Mental Health. A definition of Psychiatry is proposed, as a branch of Medicine which stands in between mental hospitals and communities. Active teamwork, coupled with the physician's work and the tasks other members of the health personnel are engaged in, has led to a more integrated approach onto the ill thus benefiting not only patients but also the whole community.
