Self-assembled pattern formation of block copolymers on the surface of the sphere using self-consistent field theory.

The spherical surface is spatially discretized with triangular lattices to numerically calculate the Laplace-Beltrami operator contained in the self-consistent field theory (SCFT) equations using a finite volume method. Based on this method we have developed a spherical alternating-direction implicit (ADI) scheme for the first time to help extend real-space implementation of SCFT in 2D flat space to the surface of the sphere. By using this method, we simulate the equilibrium microphase separation morphology of block copolymers including AB diblocks, ABC linear triblocks and ABC star triblock copolymers occurred on the spherical surface. In general, two classes of microphase separation morphologies such as striped patterns for compositionally symmetric block copolymers and spotted patterns for asymmetric compositions have been found. In contrast to microphase separation morphology in 2D flat space, the geometrical characteristics of a sphere has a large influence on the self-assembled morphology. For striped patterns, several of spiral-form and ring-form patterns are found by changing the ratio of the radius of a sphere to the averaging width of the stripes. The specific pattern such as the striped and spotted pattern with intrinsic dislocations or defects stems from formed periodic patterns due to microphase separation of block copolymers arranged on the curved surface.

A phase II study of carboplatin and chronic high-dose tamoxifen in patients with recurrent malignant glioma.

PURPOSE: To determine the response rate, time to disease progression, survival, and toxicity of intravenous carboplatin and chronic oral high-dose tamoxifen in patients with recurrent malignant gliomas. PATIENTS AND METHODS: Patients with histological confirmation of recurrent malignant gliomas were eligible for this multicenter phase II trial. Treatment consisted of 400 mg/m2 carboplatin intravenously every 4 weeks and oral high dose chronic tamoxifen (80 mg bid in women and 100 mg bid in men). RESULTS: Twenty seven patients met the eligibility criteria and were evaluable for response. The histological subtypes were: 16 (59%) glioblastoma multiforme (GBM), malignant astrocytoma (5 patients), malignant mixed glioma (5 patients), and glioblastoma/gliosarcoma (1 patient). Twenty-two patients (82%) had an ECOG performance status of 0 or 1. No complete responses were observed, 4 patients (15%) achieved a partial response, and 14 patients (52%) had stable disease. Median time to progression was 3.65 months (95%CI 2.56, 4.83). Median overall survival was 14.09 months (95%CI 7.06, 19.91). One patient with a recurrent GBM had a sustained partial response and is progression free 81 months since starting treatment. Another patient with mixed malignant oligoastrocytoma also had a prolonged partial response (lasting 63 months) and is alive 84 months after treatment for recurrence. The most frequently reported grade 3 or 4 toxicities were fatigue (19%), nausea (11%) and anorexia (11%). CONCLUSIONS: Carboplatin and high dose tamoxifen has similar response rates to other regimens for recurrent malignant gliomas and are probably equivalent to those found using tamoxifen as monotherapy. Long-lasting periods of disease free survival in some patients (particularly those with malignant mixed oligo astrocytomas) were found.

Factors contributing to guidance performance when using a camera-based guidance aid.

A guidance aid is a device that provides guidance information to the driver rather than replacing the driver. With a camera-based guidance aid, the view seen by a forward-looking video camera is displayed on a monitor situated within the operator station of the vehicle. As the vehicle moves forward, images of the ground scroll vertically across the monitor. The rate at which the image scrolls, the image velocity, is related to the forward velocity of the vehicle, the placement of the camera (height and tilt angle), and the optical characteristics of the guidance camera. When tested with a tractor at forward velocities between 1.6 and 12.8 km/h, lateral error increased linearly as image velocity increased. Driver self-confidence decreased linearly as image velocity increased. Based on subjective feedback, drivers preferred a camera tilt angle of 20 degrees (over either 30 degrees or 40 degrees) because it yielded the greatest look-ahead distance. Statistically, a tilt angle of 30 degrees was best for a camera with a narrow field of view (narrow FOV, 20 degrees in the lateral direction). For a camera with a wide field of view (wide FOV, 39 degrees in the lateral direction), there was no statistical difference. For the narrow FOV camera, a camera height of 1.1 m yielded statistically smaller lateral errors than a camera height of 1.5 m. There was no statistical difference for the wide FOV camera. Overall, the lateral error was statistically smaller for the narrow FOV camera than for the wide FOV camera due to the difference in the lateral ratio for each camera, where the lateral ratio is the ratio of the lateral field of view of the camera to the fixed monitor width.

A model to describe the settling behavior of fractal aggregates.

A model to predict fractal dimension from sedimentating fractal aggregates has been successfully developed. This model was developed using the settling rate and size data of fractal aggregates. In order to test the validity of the model, a purpose-built settling rig, equipped with lens with magnification of 1200x, which can capture images of particles/flocs down to 2 microm in diameter was used. The performance and technique of the settling rig were validated by comparing the measured settling rates of 30- and 50.7-microm standard particles with their theoretical settling rates calculated using Stokes' law. The measured settling rates were within 10% agreement with the calculated Stokes' velocities. The settling rates and sizes of the particles/flocs were analyzed using image analysis software called WiT 5.3. The maximum temperature gradient across the settling column was 0.1 degrees C, which effectively eliminated convective currents due to temperature differences in the settling column. A total of 1000 calcium phosphate flocs were analyzed. Calcium phosphate flocs with fractal dimensions varying from 2.3 to 2.8 were generated via orthokinetic aggregation. Measurements of fractal dimensions, using light scattering, were done simultaneously with the settling experiments and they were found to be constant. The fractal dimensions calculated using the model agreed with those obtained by light scattering to within 12%.

[The management of laryngotracheal invasion by well-differentiated thyroid carcinoma].

OBJECTIVE: To investigate the clinical characteristics and treatment of well-differentiated thyroid carcinoma with laryngotracheal resection. METHODS: Forty-nine cases of well-differentiated thyroid carcinoma with laryngotracheal invasion were treated by different kinds of surgical modalities including local shaving-off, partial laryngotracheal resection with sternocleidomastoid myoperiosteal flap reconstruction, sleeve tracheal resection, total laryngectomy and incomplete tumor resection. Some patients received postoperative radiotherapy. The follow-up periods were 3-20 years with an average of 8 years. RESULTS: Twenty-two patients with local invasion were treated with shaving-off procedure, among which fifteen had their lesions removed completely with naked eyes and two relapsed postoperatively. Among the 8 cases with minor residual diseases four cases shoured postoperative relapse. Sixteen patients were treated with sleeve tracheal resection or partial laryngotracheal resection with sternocleidomastoid myoperiosteal flap reconstruction. Two patients were treated with total laryngectomy among which only one relapse was found. The only postoperative complication was intratracheal granuloma formation which was found in three patients. The survival rate of the incomplete resection group was significantly lower than that of patients above mentioned. No local relapse was found in postoperative radiotherapy patients except in the patients of the incomplete resection group. CONCLUSION: According to the degree of invasion, adequate surgical procedure can obtain both cancer resection and organ preservation. Postoperative radiation may decrease relapse and prolong survival.

Characterization of Salmonella-induced filaments (Sifs) reveals a delayed interaction between Salmonella-containing vacuoles and late endocytic compartments.

Salmonella typhimurium is a facultative intracellular pathogen that colonizes host cells throughout the course of infection. A unique feature of this pathogen is its ability to enter into (invade) epithelial cells and elongate the vacuole within which it resides into tubular structures called Salmonella-induced filaments (Sifs). In this study we sought to characterize the mechanism of Sif formation by immunofluorescence analysis using subcellular markers. The late endosomal lipid lysobisphosphatidic acid associated in a punctate pattern with the Salmonella-containing vacuole, starting 90 min after infection and increasing thereafter. Lysobisphosphatidic acid-rich vesicles were also found to interact with Sifs, at numerous sites along the tubules. Similarly, cholesterol-rich vesicles were also found in association with intracellular bacteria and Sifs. The lysosomal hydrolase cathepsin D was present in Sifs, both in a punctate pattern and, at later times, predominantly in an uninterrupted linear pattern. Rab7 associated with Sifs and expression of the N125I dominant negative mutant of this GTPase inhibited Sif formation. Transfection of HeLa cells with a vector encoding SifA fused to the green fluorescent protein caused swelling and aggregation of lysobisphosphatidic acid-containing compartments, suggesting that this virulence factor directs membrane fusion events involving late endosomes. Our findings demonstrate that Sif formation involves fusion of late endocytic compartments with the Salmonella-containing vacuole, and suggest that SifA modulates this event.

Axonal regeneration stimulated by the combination of nerve growth factor and ciliary neurotrophic factor in an end-to-side model.

The aim of this study was to investigate the potential for stimulating axonal regeneration in the context of end-to-side coaptation using a combination of nerve growth factor and ciliary neurotrophic factor in the rat sciatic nerve model. Four experimental groups (n = 8) were used: end-to-side coaptation only, end-to-side coaptation plus growth factor injection, primary repair, and nontransferred gap control. Twenty weeks after surgery histologic analysis showed that the ratio of axon density was significantly increased for the growth factor injection group. Histologic evidence suggested contamination from the proximal peroneal stump. Electrical stimulation and muscle weights showed that the target muscles had been reinnervated in all groups except the nontransferred gap control group. These data support the conclusion that the use of nerve growth factor and ciliary neurotrophic factor in combination may enhance regeneration in the peripheral nervous system. This is consistent with previous reports on the central nervous system and suggests a potential application in future studies aimed at improving peripheral nerve regeneration. Another conclusion is that contamination from the proximal peroneal stump may explain the regeneration observed in the end-to-side model. Further study using retrograde labeling is needed to establish the origin of the regenerating axons. Finally, evidence suggests that regenerating axons can use the epineurium of an intact nerve to bridge a gap in continuity.

Regional dependence of cerebral reperfusion after circulatory arrest in rats.

The severity of neurologic dysfunction after circulatory arrest depends on cerebral reperfusion during and after resuscitation. The objective of current study was to investigate the temporal and spatial patterns of the cerebral perfusion immediately after resuscitation. Precise control of circulatory arrest was achieved in rats by combination of asphyxia and transient blockage of cardiac-specific beta-adrenergic receptors with esmolol, an ultra-short-acting beta-blocker. Animals were randomized into 3 groups with resuscitation starting 0.5 (sham group, no asphyxia, n = 5), 4 (Group 2, n = 5), or 12 minutes (Group 3, n = 8) later by retrograde intraarterial infusion of donor blood along with a resuscitation mixture. Cerebral perfusion was measured by magnetic resonance imaging (MRI) using arterial spin labeling. The average perfusion before arrest was 163 +/- 27 mL 100 g(-1) min(-1) under isoflurane anesthesia. Resuscitation led to transient perfusion increase, which started from thalamus and hypothalamus and later shifted to the cortex. Severe hypoperfusion to as low as 6% to 20% of the normal level developed in the first 10 to 20 minutes of reperfusion and lasted for at least 2 hours. On the fifth day after circulatory arrest, all animals showed a normal level of perfusion (159 +/- 57 mL 100 g(-1) min(-1) ) and minimal neurologic deficit. Nevertheless, histologic examination revealed extensive changes in the CA1 region of the hippocampus consistent with global ischemia and reperfusion damage. The combination of an improved circulatory arrest model and noninvasive MRI cerebral perfusion measurements provides a powerful tool for investigations of circulatory arrest and resuscitation, allowing for evaluation of therapies aimed at modulating cerebral reperfusion.

Ten-year review of hysterectomy morbidity and mortality: can we change direction?

The medical records of all women who underwent hysterectomy for benign disease performed between 1986 and 1995 were reviewed to ascertain the incidence of morbidity and mortality of abdominal, vaginal, and laparoscopically assisted vaginal hysterectomy at a university teaching hospital. A total of 1940 hysterectomies were performed during this period; 74% of hysterectomies were performed abdominally, 24% vaginally and 2% were laparoscopically assisted. In 80% of the patients uterine leiomyomas, adenomyosis, dysfunctional uterine bleeding or uterine prolapse were the indications for hysterectomy The overall complication rate was 44% for abdominal hysterectomy (AH) and 27.3% for vaginal hysterectomy (VH). An unintended major surgical procedure was required in 3% and 1% of women undergoing AH and VH respectively The rate of return to the operating room for haemostasis was 0.6% for AH and 0.2% for VH. The AH group was four times more likely than the VH group to require surgical intervention (36% versus 9%) at readmission. Vaginal hysterectomy was associated with a lower febrile morbidity and minor complication rate. Prophylactic antibiotics reduced the febrile morbidity for VH and AH by 50% (Student's t-test, p = 0.02) and 40% (Student's t-test, p < 0.001) respectively The overall mortality rate was 1.5 per 1000.

Expression of the chaperonin 10 gene during zebrafish development.

We have isolated a cDNA encoding chaperonin 10 (cpn10) from the zebrafish. Using northern, western, and in situ hybridization analysis, we observed that the cpn10 gene is expressed uniformly and ubiquitously throughout embryonic development of the zebrafish. Upregulation of cpn10 expression was observed following exposure of zebrafish embryos to a heat shock of 1 hour at 37 degrees C compared to control embryos raised at 27 degrees C. The extracellular form of Cpn10 called early pregnancy factor (EPF), found in the serum of pregnant mammals, was not detected in the serum of either male or female zebrafish. These expression studies suggest that Cpn10 plays a general role in zebrafish development as well as being consistent with the hypothesis that EPF is involved in the embryo implantation process in mammals.

Pulmonary adenocarcinoma simulating malignant mesothelioma.

Adenocarcinoma of the lung with pleural involvement frequently resembles pleural epithelioid mesothelioma clinically as well as macro- and microscopically. Special stains, immunohistochemical studies, and electron microscopic studies are needed to differentiate these 2 tumors. We report a case of pleural involvement by adenocarcinoma, mimicking in the hematoxylin-eosin stain an epithelioid mesothelioma, correctly identified only after immunohistochemical and electron microscopic examinations.

Ultrastructure of the periductal area of comedo carcinoma in situ of the breast.

We have previously shown that the different biological natures of comedo ductal carcinoma in situ (DCIS) and non-comedo DCIS may, in part, be explained by the different expression patterns of tenascin, a large extracellular matrix protein, as observed by immunohistochemical studies. In the present study, we compared 8 cases of comedo DCIS with 5 cases of non-comedo DCIS by ultrastructural analysis, focusing on the myoepithelium, basal lamina, and tenascin-positive extracellular periductal stromal matrix. Our observations show that the comedo type DCIS frequently has an altered basal lamina, a looser and more disorganized collagenous matrix, and a general increase in stromal cellularity, including fibroblasts, lymphocytes, histiocytes and small blood vessels. In addition, in comedo DCIS, the lateral intercellular spaces between large myoepithelial cells that border the basal lamina are often expanded, compared to those of non-comedo DCIS. These results identify structural characteristics of comedo DCIS that may play a role in its greater preinvasive potential. They may also provide a structural basis for the different strategies that are needed for for clinical management of comedo DCIS, compared to non-comedo DCIS.

Case report: primary isolated angiomyolipoma of the spleen.

A primary isolated angiomyolipoma presenting as a splenic mass is described. This is the first reported case of primary splenic angiomyolipoma without angiomyolipoma in the kidney or evidence of tuberous sclerosis, and it coexists with an adenocarcinoid of the appendix.

[Refraction shift after intraocular lens implantation in children].

OBJECTIVE: To evaluate the refraction shift after posterior chamber intraocular lens (PC-IOL) implantation in children. METHODS: A long term retrospective study involved 65 eyes of 65 patients aged from 5 to 12 years who underwent PC-IOL implantation between December 1988 and December 1994. The mean follow-up was 7.1 years (range 5 to 10 years). Refraction shift, keratometric readings and axial length were periodically measured. Sixty-five healthy eyes were in the control group. RESULTS: The mean preoperative axial length of the surgical group was (22.48 +/- 0.44) mm, and of the control group, (22.43 +/- 0.41) mm. There was no statistically significant difference in axial length after 10 years in both groups, (23.45 +/- 0.53) mm in surgical group and (23.41 +/- 0.50) mm in control group. All the eyes showed a myopic drift with 3.29 D in surgical group and 1.75 D in control group (F = 2.318, P < 0.05). CONCLUSION: The ocular growth of children is found to be unaffected by the PC-IOL implantation, the choice of IOL power should be retained in physiological hyperopia in pseudophakic children.

[Clinico-pathological significance of microvessel density and vascular endothelial growth factor expression in adenoid cystic carcinoma of salivary glands].

OBJECTIVE: To evaluate the clinical significance of microvessel density(MVD) and vascular endothelial growth factor(VEGF) expression in adenoid cystic carcinoma of salivary glands. METHODS: Immunohistochemical staining(SP) was used to quantify MVD and VEGF expression in specimens from 55 patients with adenoid cystic carcinoma of salivary glands. SPSS statistical software was used. RESULTS: High VEGF expression was correlated with tumor site and stage (P = 0.0398, P = 0.0175), but not with local recurrence, distant metastasis, regional lymph node metastasis, histological classification and survival rate. A significant correlation was not noted between MVD and the above variables. The mean MVD in cases of high VEGF expression was significantly higher than those in cases of low VEGF expression (P = 0.0202). CONCLUSIONS: In adenoid cystic carcinoma of salivary glands, MVD and VEGF may be served as a prognostic factor which needs further study, but there is a significant positive correlation between MVD and VEGF expression.

[Titanium miniplate fixation in head and neck cancer surgery].

OBJECTIVE: To evaluate the rigid fixation with miniplates in head and neck cancer surgery. METHOD: A consecutive series of 40 patients with head and neck tumor underwent rigid fixation with the Martin titanium miniplates system in a 6 years period. The miniplates were used to fix bone graft reconstruction following mandible resection (n = 14), mandibulotomies for oral and oropharyngeal surgical access (n = 12), maxilla swinging for nasopharyngeal access (n = 11) and other purposes (n = 3). Twenty-three of all cases(57.5%) received perioperative radiotherapy with average doses of 60Gy at the osteotomy site. RESULTS: Eleven of the 40 patient (22%) developed plate related complication which included 5 infections, 3 mandibular osteonecroses, 1 plate exposure, 1 screw loose and 1 nonunion. Complications were more likely to occur in patients with perioperative radiation than patients without (39.1% versus 11.8%, P = 0.079). Except one case, All the uncomplicated dentulous patients demonstrated stable and good occlusion. Oral feeding usually started on the 3-4th day after surgery. There was no need for intermaxillary fixation. CONCLUSION: The miniplate provided stable mandibular or maxillary fixation and accurate repositioning, eliminated the need for intermaxillary fixation. The malleability and versatility of miniplate make it unique in bone graft shaping for mandibular reconstruction. Complication seems to be related to perioperative radiotherapy. Median or paramedian mandibular osteotomy, which is non-radiated region in most radiotherapy cases, is advocated.

[Loss of heterozygosity on chromosome 9p13-23 in microdissected laryngeal squamous cell carcinoma by microsatellite analysis].

OBJECTIVE: Loss of heterozygosity (LOH) studies indicate that allelic loss associated with the development of head and neck of squamous cell carcinoma occurs most frequently at 9p21-22. However, the target of chromosome 9p21-22 loss has been the source of significant debate. A putative tumor suppressor gene, p16, has been identified at the 9p21 location, but genetic alterations of p16 located in this region are unusual. To refine the hot spots of LOH on chromosome 9p13-23 will be helpful to find other putative tumor suppressor genes in laryngeal squamous cell carcinoma. METHODS: The sections of paraffin-embedded tumor tissues were microdissected to enrich for neoplastic cells. LOH on 9p13-23 was analyzed in a set of 42 paired blood and tumor samples using polymerase chain reaction (PCR) with 13 highly polymorphic microsatellite markers. RESULTS: Of the 42 total tumors, 41(97.6%) showed LOH in at least one of the microsatellite markers tested at the chromosome 9p13-23 region. The most frequently deleted marker was D9S162 with in 17 of 19 (89.5%) informative samples. The marker D9S171, which is located in 9p21, had LOH detected in 12 of 15 informative cases (80.0%). LOH at the D9S1748 marker (closest to the p16 gene locus) was detected in 18 of 36 informative cases (50.0%). Fine deletion mapping also revealed two minimal regions of LOH encompassing markers D9S161-D9S171 at 9p21 and IFNA-D9S162 at 9p22-23. CONCLUSION: These findings imply the high frequency of LOH at 9p13-23 in laryngeal squamous cell carcinoma and the presence of at least two as yet unidentified tumor suppressor genes in 9p13-23 region. Those putative tumor suppressor genes may become inactivated during the progression of the laryngeal squamous cell carcinoma.
