High bifurcation of median nerve at the wrist causing common digital nerve injury in endoscopic carpal tunnel release.

A 45-year-old right-handed woman was treated by endoscopic carpal tunnel release under local anaesthetic. When cutting the proximal half of the flexor retinaculum with the retrograde blade, she complained of an electric shock-like pain in her middle and ring fingers. Open exploration of the carpal tunnel was performed and a laceration of the middle/ring common digital nerve, which bifurcated at the unusually high level of the wrist crease, was found.

The use of a surgical grade calcium sulfate as a bone graft substitute: results of a multicenter trial.

In a prospective, nonrandomized, multicenter study, 109 patients with bone defects were treated with a surgical grade calcium sulfate preparation as a bone graft substitute. The calcium sulfate pellets were used in place of morselized cancellous bone graft for the treatment of patients with bone defects who usually would require grafting secondary to trauma, periprosthetic bone loss, tumor, or fusion. The calcium sulfate was used alone or mixed with other materials such as bone marrow aspirate, demineralized bone matrix, or autograft. The defects that were treated were contained and were not necessary for the stability of the bony structure. Radiographic and clinical data were collected at predetermined intervals for 12 months. At 6 months postoperatively, radiographic results for all patients showed that 99% of the calcium sulfate had been resorbed and 88% of the defect was filled with trabeculated bone. There were 13 complications; however, only four (3.6%) were attributable to the product. The results of a subgroup of 46 patients with benign bone lesions treated in the same manner are identical to the results of the overall study population. Surgical grade calcium sulfate pellets are considered a convenient, safe, and readily available bone graft substitute that yield consistent successful results.

Empirical Bayes estimation for combinations of multivariate bioassays.

This article presents a new empirical Bayes estimator (EBE) and a shrinkage estimator for determining the relative potency from several multivariate bioassays by incorporating prior information on the model parameters based on Jeffreys' rules. The EBE can account for any extra variability among the bioassays, and if this extra variability is 0, then the EBE reduces to the maximum likelihood estimator for combinations of multivariate bioassays. The shrinkage estimator turns out to be a compromise of the prior information and the estimator from each multivariate bioassay, with the weights depending on the prior variance.

[Unification of Oriental and Western medicine with study on Oriental and Western medicine].

In Korea the system of medical management has been bifurcated into two parts since the restoration of nation from Japanese after World War II. One is 'Western Medicine' and the other is 'Oriental Medicine' like Chinese but not like Japanese. The authors attempted to study on the unification of both medicines to prevent the confusion of medicare for people. The major part of medical care is accomplished by Western medicine among people in Korea and China. However, Oriental medicine is still present as well as college of Oriental medicine in small number. There have been long struggles between two parts of medicine because of each assertions for the theories. The ancient medicine has been likely to have its characteristics either in Western or Oriental medicine although in many countries were succeeded by Western medicine except a few Asian countries such as Korea and China. In Japan since the license of Oriental(herb) medicine was ceased about one hundred years ago, the Western medicine has been authorized by law until today and the herb is the secondary medicine as a kind of folk treatment as well as continuous and enormous study for scientification. In only China and Korea this herb medicine has been kept to use as a part of medicine by law though China has developed combined medicine to use both Western and Oriental medicine by one physician since previous prime minister Chou En-Lai around the year 1950 who made a recommendation to the two medical societies, Western and Oriental. This fact has a big sense to establish the unification of two medicines in near future in Korea as well as China. For this accomplishment of medical unification both parts of medicine require sincere and enormous efforts to study the theory and practice of herb medicine even from now on. This unification of two medicines will provide the best medicare in most convenient way to the people in the world creating new world medicine like the third medicine including alternative medicine. The acupuncture is changing to a new method in Western medicine especially applying into anesthesia and pain clinic in even USA and Western countries at this time. In conclusion the unification of the Western and Oriental medicine will provide the most reasonable and practical medicare including alternative medicine to people in the world. Because the Oriental medicine will be used at least separately to supplement the Western medicine by one physician.

Tensile strength of cross-stitch epitenon suture.

Using a computerized tensiometer, the mechanical properties of two types of repair in the canine flexor digitorum superficialis tendon were examined. The slope and ultimate tensile strength of a cross-stitch epitenon suture were compared to those of a simple over-and-over running epitenon suture. Both epitenon sutures were combined with a modified Kessler core suture. Suture material was 6/0 monofilament nylon for the epitenon suture, and 4/0 braided polyester for core suture. Average relative values of individual ultimate tensile strength for the cross-stitch group was 245% compared to that for the running suture group. There was no discrepancy in slope between the two groups.

SECReT of the eukaryotic large ribosomal subunit.

The large ribosomal subunit of the thermophilic fungus Thermomyces lanuginosus was reconstructed in three dimensions from electron micrographs. This is the first reported reconstruction of the eukaryotic complex and demonstrates specific structural features such as an intersubunit canyon and a potential channel for the nascent peptide chain.

General estimation of relative potency using prior information.

A general point and interval estimator of the log relative potency is exhibited in closed form, which incorporates prior information on the log relative potency for symmetrical as well as asymmetrical parallel line bioassay. The point estimator turns out to be a weighted average of the usual estimator and the prior mean where the weights are determined by the prior variance.
