[Side effects and contrast medium resorption of iopamidol and iocarmate in lumbar myelography].

Lumbar myelographies were carried out with lopamidol and locarmat of identical concentrations of iodine in 2 groups of 20 patients each. In about one-third of the cases, clinical side effects and EEG changes were found after the examination without difference with the exception of one seizure and one state of confusion which occurred only after locarmat. Resorption of the contrast media was judged by CT examinations of the lumbar spine and the head. There was a more rapid decrease of lumbar contrast density in patients with a narrow lumbar sac which was accompanied by a more rapid intracranial accumulation of the contrast medium. This may be due to a more active transport of cerebrospinal fluid in this group of patients.

[The diagnostic significance of spinal computer tomography with special reference to perpendicular projection (author's transl)].

The computer tomographic examination of the spine and of the cord by third generation apparatus is of increasing diagnostic value because of improved spacial resolution and density discrimination. By means of a localisation scan or topogram, it is possible to choose an accurate position for the section through the area of interest and to tilt the gantry to produce a ray parallel to the neural arch. All abnormalities affecting the spine can easily by localised and their extent determined. Intraspinal tumours and disc prolapse in the cervical and lumbar region, where the spinal canal is relatively wide, can be differentiated from the cord by a simple scan. In the thoracic region, on the other hand, it is necessary to use intrathecal contrast injections for the diagnosis of purely intraspinal processes.

[Neuroradiology of embryonal brain development. Comparative studies on the prenatal development of brain vessels and liquor spaces in mammals and humans].

In more than 400 mammalian and human embryos and fetuses of an age of gestation of 4 weeks up to term, the prenatal development of the cerebral vessels and ventricles was studied by radiological techniques. Postmortal arterio- and phlebographies demonstrated the complex transformations of the leptomeningeal arteries and veins. The capillary growth within the brain itself was examined by a combination of microangiographic and histological techniques, and a clear morphological correlation between the differentiation of the nervous tissue and the invasion of the capillaries was shown. By ventriculography, the changes of the size and shape of the ventricular system were documented, and quantitative development diagrams of the cerebral ventricles could be established. On the basis of these morphological findings, in vivo, angiographies were carried out in fetal cats (in utero) and also in chicken embryos of an age of gestation of 5--14 days. It could be shown, that the average circulatory speed of the blood stream within the carotid arteries increases parallel to the enlargement of the embryo and its arteries, whereas the circulatory time and the number of heart beats during the period of carotid opacification does not change to a great extent.

[Angiographic examinations of the circulatory development of living chick embryos (author's transl)].

In chick embryos of an age of incubation of 5-14 days, the physiological development of the circulation and the morphological differentiation of the arterial system were studied by intravital and postmortal angiography. For the examinations of the living embryos, a special radiographic and injection technique had to be developed. The contrast medium was injected into the umbilical veins and transported by the actions of the embryonic heart. Continuous ECG recordings showed no marked interference of the injections with the cardiac activity. According to the angiographic findings, the circulation is relatively fast within the main arteries, but the capillary perfusion is prolonged and lasts up to several minutes. The average circulatory velocity of the blood stream within the carotid artery increases parallel to the arterial enlargement, whereas the circulatory time decreases and the number of heart beats during the period of carotid opacification does not change to a great extent. By this, a steady transport of gas and nutritional material may be achieved in the growing arterial system.

Embryonal and fetal development of capillaries: microangiographic investigations. I. The brain stem.

The prenatal development of the intracerebral capillaries within the brain stem was systematically examined by a special microangiographic technique in 69 embryos and fetuses of cattle and pigs between 4 and 17 weeks of gestation. The microradiograms were compared with the corresponding histological slices. The different developmental stages and morphological patterns of vascularization are related to the local stage of maturation, histological structure and metabolic activity of the nervous tissue.

[Diagnosis and differential diagnosis of cerebro-vascular malformations by CT (author's transl)].

In 38 patients, the diagnosis of a cerebrovascular malformation (17 arteriovenous agniomas including one low-flow- and two venous angiomas; 10 aneurysms; 4 arteriovenous fistulae of the cavernous sinus, the tentorium and one of the Great Vein of Galen; 6 megadolical basilar arteries) was initially made by computertomographic (CT) examination, including contrast enhancement. The characteristic and pathognomonic CT findings are described and compared with those of cerebral angiography also done in these cases. The problems of differential diagnosis and the reasons for a false CT diagnosis in 5 other patients with a cerebro-vascular malformation are investigated; and the diagnostic value of cerebral angiography and CT is discussed and their complementary functions are being pointed out.

Embryonal and fetal development of capillaries: microangiographic investigations. II. The telencephalon.

The development of the intracerebral capillaries of the telencephalon was studied by the combined techniques of microangiography and histology in embryos and fetuses of cattle and pigs. In the telencephalic colliculi ganglionares, vascularization starts during the 4th week of gestation and leads to the formation of a peripheral and a central arterial system separated by the internal capsule. Capillary development within the pallium occurs 3 weeks later and consists in the formation of longitudinal arteries, concentrically arranged rings of deviations and bifurcations, and a periventricular capillary network.

[Cerebral angiography with ioglicinate (Rayvist). A double blind study of image quality and clinical side effects (author's transl)].

The image contrast quality and the patient tolerance of two osmotically active water-soluble contrast agents were examined and compared in double blind study cerebral angiography: Rayvist (an Ioglicinate developed recently) and Telebrix (AN Ioxithalamate). In 100 direct carotid or retrograde brachial angiograms the tolerance of Rayvist was significantly better, primarily because of less subjective patient side effects and less frequent cardiovascular reactions. However, there was no significant difference in neurological complications or deterioriation of EEG-findings, both quite rare anyway.

Prenatal neuroradiology: comparative radiological investigations of the embryonal and fetal brain.

In 400 embryos and fetuses of domestic animals and men, prenatal development of the cerebral vessels and ventricles was examined by postmortem arteriography, phlebography, microradiography, and ventriculography. In the ventricular system, qualitative and quantitative changes could be demonstrated and developmental diagrams of the growing ventricular spaces and angles were elaborated. The differentiation of the aortic arches into the major cranial arteries and the complex transformations of the cerebral venous system were documented by angiography within a period of gestation of 4 weeks up to term. Intrauterine angiography was also carried out in living fetuses of cats as preliminary studies of the development of cerebrovascular malformations.

Angiographic, operative, and histologic findings after embolization of craniofacial angiomas.

After pre- or intraoperative angiography and Gelfoam embolization in patients with deforming vascular malformations in the craniofacial region, the angiographic findings and the results of embolization were controlled by operation and histology. In arteriovenous hemangiomas the disfigurations of the craniofacial region originate from the shunt vessels themselves, in capillary hemangiomas exclusively from neoplastic, pathologically dilated and convoluted veins which could be demonstrated only by percutaneous punctures. Lymphatic hemangiomas also could be radiologically shown only after direct punctures and without any feeding or draining vessels. During operation vast necrosis of the fatty tissue in the region of embolization and massive phlebolithiasis in the draining veins were found. Histologically, arterial obliterations could be shown in the former shunt area. The reconstructive operations could be done in bloodless surgical regions and always showed good cosmetic results, which were regarded as constant because of the embolization of the shunt area.

Comparative microangiographic and histologic studies of the embryonal development of intracerebral capillaries.

In 69 embryos and fetuses of cattle and pigs with an age of gestation of 4 weeks to 4 months the development of intracerebral capillaries was studied comparatively by microangiography and controlled by histologic slices. The first invasion of blood vessels into the wall of the neural tube takes place in the brain stem and colliculus ganglionaris at an age of gestation of 4 weeks. After that, specific and different angioarchitectonic patterns of capillary development can be demonstrated within the telencephalon, basal ganglia, and brain stem. The telencephalic vessels show concentrically arranged areas of ramification which interrupt their longitudinal course towards the ventricular cavity. The branchings are related topographically to the projection fibers from the internal capsule and to the subependymal matrix layer. In the basal ganglia, the developing internal capsule interrupts the blood supply of the caudate nucleus from the periphery, and a new central system is established mainly running radially towards the ventricular canal. Thus, it could be shown in consecutive stages of cerebral development that the angioarchitectonic pattern of a certain embryonal stage and brain area depends on the histologic structure of the neural tissue and its specific metabolic demands.

Diagnostic value and limitations of selective spinal angiography in different lesions of the vertebral bones.

The technique, diagnostic results and significance, differential diagnostic considerations and limitations of selective spinal angiographies in chronic inflammatory processes (Pott's disease 11 cases). hemangiomas (21 cases) and aneurysmal bone cysts (9 cases) as well as benign and malignant tumors (26 cases) of the vertebral bones and adjacent tissues are described and demonstrated with regard to radiological findings.

[The radiological demonstration of vessels in embryos and foetuses. Comparative radiological studies of the venous system of the neck, head and brain in domestic animals. Paper 3 (author's transl)].

A method was developed for prenatal, post-mortem angiography, post-mortem angiography of the venous system of the brain, head and neck; its development was studied in the embryos and foetuses of 80 cattle, 20 sheep and 20 pigs with a crown-rump lenght of 19 to 155 mm. Radiological and corrosion preparations were studied. The embryonic and foetal venous systems were filled with thin barium sulphate through the umbilical and omphalic veins. The development and transformation of the cerebral veins and sinuses are compared and are related to the corresponding vascular anatomy of the adult animal and to man. The radio-embryological method for demonstrating cerebral vessels is also applicable to live foetuses.
