Nonsecretory myelomatosis without intracellular immunoglobulin. Immunofluorescent and ultramicroscopic studies.

A patient with multiple myeloma is reported with severe hypogammaglobulinaemia but without monoclonal immunoglobulin (M-component) in the serum or Bence Jones protein in the urine. In May-Grünwald-Giemsa stained bone marrow film 45% of the cells resembled plasma cells. Marrow immunofluorescent studies showed no increase of immunoglobulin containing cells. Ultrastructural studies showed a large number of cells with a well developed rough endoplasmic reticulum and other morphological features characteristic for plasma cells.

Bone marrow metastases from prostatic cancer-marked cytolytic effect after only a few days of treatment with diethylstilbestrol.

A 42-year-old male patient became hospitalized with severe back pain and marked bleeding tendency from disseminated intravascular coagulation. The bone marrow aspirate showed numerous nests of cancer cells presumably from a prostatic carcinoma. After only 4 days of treatment with diethylstilbestrol his condition was markedly improved, and a new bone marrow aspirate showed extensive necrosis of the cancer cells.

Abnormal lymphocyte populations in pure red cell aplasia.

Studies of blood lymphocytes from 4 patients with pure red cell aplasia were performed with lymphocyte surface markers, and with various in vitro tests for lymphocyte functions. Pathologically low B-lymphocyte values were found. In contrast, no marked deviation from normals were seen for T-lymphocytes and Fc-receptor-bearing lymphocytes thought largely to represent non-B, non-T-lymphocytes. In 3 patients normal lymphocyte transformation was found with unspecific and specific mitogens, while the DNA-synthesis induced by unspecific mitogens was subnormal in the fourth patients. The lymphocyte-mediated PHA-induced cytotoxicity against target cells in vitro was subnormal in 2 patients, while no depression was seen in antibody-dependent cytotoxicity mediated in vitro by lymphocytes (K-cells). It is concluded that considerable immunological abnormalities are associated with pure red cell aplasia, and the possible significance of this is discussed.

Confirmation of the existence of human serum leukaemia-associated antigens (LAA).

We have obtained antisera from rabbits which react with serum from several leukaemia patients after absorption with normal human serum. The specificities of these rabbit antisera have been shown to be closely related to those of the original anti-LAA antisera of Viza et al (1970) and Harris et al (1971). Thus, the existence of leukaemia associated (but probably not leukaemia specific) serum antigen in man has been confirmed. One of the animals was immunized with normal amniotic fluid obtained at 15-16 weeks of gestation. Thus, LAA seems to be a normal constituent of amniotic fluid. This suggests that LAA is another onco-fetal component.

Needle-like crystals in plasma cells in a patient with a plasma cell proliferative disorder.

A patient with a plasma cell proliferative disorder and a monoclonal IgG-kappa protein in the serum was observed over a period of four years. Two unusual features were seen in this patient: (1) An apparently benign course in spite of relatively large and slowly increasing amounts of Bence Jones protein (monoclonal kappa light chains) in the urine, and (2) crystalline structures located outside the cisternae of the endoplasmic reticulum in a large number of bone marrow plasma cells. The crystals, which ultrastructurally appeared to be composed of protein material, were not stained by immunofluorescent antisera to immunoglobulins. They were, however, observed only in cells staining for gamma heavy and kappa light chains. This suggest an association between the crystalline structures and the synthesis of monoclonal immunoglobulin proteins.

Morphological features in the peripheral blood film indicating the presence of cryoproteins.

Light gray, irregular threads in a May-Grünwald & Giemsa stained blood film made of EDTA-blood some time after drawing may sometimes call attention to a previously unsuspected cryoproteinaemia. The cryoproteinaemia discovered in this way in our patient consisted of IgG complexes resulting in proteinuria. After treatment with prednisone and cyclophosphamide the cryoproteinaemia and proteinuria disappeared.
