[Traumatic hemipelvectomy and postoperative quality of life. Report of 2 cases 18 years after trauma].

Report and progress of 2 traumatic hemipelvectomies in motor-cyclists with excluding extraperitoneal tearing of half of the pelvis and disrupture of the pelvic vessels for 18 years. The purpose of this case study was the analysis of the quality of life, the clinical complications and the reintegration of the 2 patients 18 years after traumatic hemipelvectomy.

[The multi-point contact (MPC) osteosynthesis plate. 2: Initial clinical results of therapy of forearm fracture with MPC plates].

Plate osteosynthesis is a method of choice in the treatment of forearm fractures with senso-motorical impairment, poorly or non-reduceable, retained, fragmented and open fractures. Dislocated fractures of the forearm frequently cause problems because of their poor reduction and retention. In such cases, exclusively conservative treatment requires reduction and change of therapeutical approach to avoid functional limitations resulting from axial malpositioning. The sincere contact of bone fragments, accompanied by sufficient anatomical axial and conservative reduction techniques that maintain parossal perfusion as well as a limited osteosynthesis plate contact should be the aim of the operative strategy. The use of a new titanium osteosynthesis plate system with multi-point contact between bone and plate promotes the incorporation of vessels and callus formation at the bone-plate interface and leads to the realization of the aforementioned aim, which was revealed in animal studies before. In order to examine the results, 18 patients with forearm fractures were studied, which underwent reduction and internal fixation using this new titanium osteosynthesis plate, the multi-point contact plate (MPC) system. In these patients, functional and radiological results after plate removal were studied. Our initial clinical experiences with the MPC-plate for internal fixation confirm the animal study results. The in-growth of parossal vessels into the multiple inter-point spaces enhances the subimplant (bio-logical) formation of callus, which can be visualized radiographically and intraoperatively after plate removal. These results indicate that the MPC plate has marked advantages over conventional plates.

[The value of digital luminescence radiography within the scope of traumatologic follow-up examinations].

At the present time we cannot unhesitatingly recommend the general use of digital luminescence radiography in traumatological follow-up examinations. Marked drawbacks of this method are, for example, sudden changes in contrast in the marginal areas of osteosynthesis material, occasional limitations in the detailed assessment of spongious structures and problems in respect of imaging geometry. By modifying the image processing parameters, increasing the image matrix and enlarging the format spectrum, however, these problems should be capable of being resolved in the future. Positive features, on the other hand, are even now the possibility of reducing the dosage to a marked degree in many traumatological follow-up examinations, especially in children and adolescents, and in case of conservatively treated fractures. In the long run we can foresee the routine use of digitalised examination methods on traumatology coupled with the possibility of storage in an image filing and communication system, although this is at present not yet feasible due to lack of requisite experience and the cost of the necessary equipment.

[Epilepsy and Dupuytren's contracture--a syntropy of 2 diseases?].

Dupuytren's contracture was observed in 21,6 percent of 524 patients with epilepsy. This combination is characterised by a steadily progression by relatively benign course with a consequenly low frequency of operation and rate of recurrence. Possible genetic association has to be taken into consideration, because idiopathic epilepsy is the type most frequently found with Dupuytren's contracture. In this material no correlation has been found between Dupuytren's contracture and liver disease. Diabetes mellitus and other aetiologic factor which have been suggested. The duration of epilepsy and the medication given have not been analysed.
